| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 2 | Systemic immune-inflammation index for predicting prognosis of colorectal cancer显示文摘AIM To investigate the clinical significance of preoperative systemic immune-inflammation index(SII) in patients with colorectal cancer(CRC). METHODS A retrospective analysis of 1383 cases with CRC was performed following radical surgery. SII was calculated with the formula SII =(P × N)/L, where P, N, and L refer to peripheral platelet, neutrophil, and lymphocyte counts, respectively. The clinicopathological features and follow-up data were evaluated to compare SII with other systemic inflammation-based prognostic indices such as the neutrophil-lymphocyte ratio(NLR) and platelet-lymphocyte ratio(PLR) in patients with CRC.RESULTS The optimal cut-off point for SII was defined as 340. The overall survival(OS) and disease-free survival(DFS) were better in patients with low NLR, PLR, and SII(P < 0.05). The SII was an independent predictor of OS and DFS in multivariate analysis. The area under the receiver-operating characteristics(ROC) curve for SII(0.707) was larger than those for NLR(0.602) and PLR(0.566). In contrast to NLR and PLR, SII could effectively discriminate between the TNM subgroups. CONCLUSION SII is a more powerful tool for predicting survival outcome in patients with CRC. | 3 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 4 | Cardiotoxicity of 5-fluorouracil and capecitabine in Chinese patients: a prospective study显示文摘Background:5-Fluorouracil(5-FU)and capecitabine-associated cardiotoxicity ranging from asymptomatic electro-cardiography(ECG)abnormalities to severe myocardial infarction has been reported in a number of studies,but such cardiotoxicity in Chinese patients with malignant diseases has not been investigated to date.In the present study,we aimed to prospectively evaluate the incidence rates and clinical manifestations of 5-FU-and capecitabine-associated cardiotoxicity in cancer patients recruited from multiple centers in China.Methods:Among the 527 patients who completed the study,196 received 5-FU-based chemotherapy and 331 received capecitabine-based chemotherapy as either first-line or adjuvant therapy.Adverse events were reported during the treatment and up to 28 days of follow-up.Outcome measures included ECG,myocardial enzymes,cardiac troponin,brain natriuretic peptide and echocardiography.Univariate analysis and logistic regression were performed for subgroup analysis and identification of significant independent variables that are associated with cardiotoxicity of both agents.Results:In total,161 of 527 patients(30.6%)experienced cardiotoxicity.The incidence rate of cardiotoxicity was 33.8%(112/331)in the capecitabine group,which was significantly higher than the rate of 25%(49/196)in the 5-FU group(P=0.0042).110/527 patients(20.9%)suffered arrhythmia,105/527(19.9%)developed ischemic changes,while only 20/527 patients(3.8%)presented heart failure and 6/527 patients(1.1%)had myocardial infarction.Pre-existing cardiac disease,hypertension,capecitabine-based chemotherapy and duration of treatment were identified as sig-nificant risk factors associated with cardiotoxicity.The odds ratio were 15.7(prior history of cardiac disease versus no history),1.86(capecitabine versus 5-FU),1.06(5-8 versus 1-4 chemotherapy cycles)and 1.58(hypertension versus no hypertension),respectively.Conclusions:Cardiotoxicity induced by fluoropyrimidines in the Chinese population may be underestimated in clini-cal practice.Close monitoring of patients is recommended,especially for those patients at high risk for cardiotoxicity.Possible risk factors are duration of treatment,capecitabine-based chemotherapy,pre-existing cardiac diseases and hypertension.Trial registration This study was initiated on January 22,2014 and has been retrospectively registered with the registra-tion number ChiCTR1800015434 . | Jianjun Peng Chao Dong Chang Wang Weihua Li Hao Yu Min Zhang Qun Zhao Bo Zhu Jun Zhang Wenliang Li Fenghua Wang Qiong Wu Wenhao Zhou Ying Yuan Meng Qiu Gong Chen | 2018 | Cancer Communications2018,38,1: | 23 |

| 7 | Novel immunological and nutritional-based prognostic index for gastric cancer显示文摘AIM: To assess the prognostic significance of immunological and nutritional-based indices, including the prognostic nutritional index(PNI), neutrophillymphocyte ratio(NLR), and platelet-lymphocyte ratio in gastric cancer.METHODS: We retrospectively reviewed 632 gastric cancer patients who underwent gastrectomy between1998 and 2008. Areas under the receiver operating characteristic curve were calculated to compare the predictive ability of the indices, together with estimating the sensitivity, specificity and agreement rate.Univariate and multivariate analyses were performed to identify risk factors for overall survival(OS). Propensity score analysis was performed to adjust variables to control for selection bias.RESULTS: Each index could predict OS in gastric cancer patients in univariate analysis, but only PNI had independent prognostic significance in multivariate analysis before and after adjustment with propensity scoring(hazard ratio, 1.668; 95% confidence interval:1.368-2.035). In subgroup analysis, a low PNI predicted a significantly shorter OS in patients with stage Ⅱ-Ⅲ disease(P = 0.019, P < 0.001), T3-T4 tumors(P <0.001), or lymph node metastasis(P < 0.001). Canton score, a combination of PNI, NLR, and platelet, was a better indicator for OS than PNI, with the largest area under the curve for 12-, 36-, 60-mo OS and overall OS(P = 0.022, P = 0.030, P < 0.001, and P = 0.024,respectively). The maximum sensitivity, specificity, and agreement rate of Canton score for predicting prognosis were 84.6%, 34.9%, and 70.1%, respectively.CONCLUSION: PNI is an independent prognostic factor for OS in gastric cancer. | 8 | Renewed profile of the Mesozoic magmatism in Korean Peninsula: Regional correlation and broader implication for cratonic destruction in the North China Craton显示文摘Widespread Mesozoic magmatism occurs in the Korean Peninsula(KP). The status quo is poles apart between the northern and southern parts in characterizing its distribution and nature, with the nearly absence of any related information in North Korea. We have the opportunity to have conducted geological investigations in North Korea and South Korea during the past ten years through international cooperation programs. This led to the revelation of a number of granitoids and related volcanic rocks and thus facilitates the comparison with those in East China and Japan. Mesozoic granitoids in the KP can be divisible into three age groups: the Triassic group with a peak age of ~220 Ma, the Jurassic one of ~190–170 Ma and the late Early Cretaceous one of ~110 Ma. The Triassic intrusions include syenite, calc-alkaline to alkaline granite and minor kimberlite in the Pyeongnam Basin of North Korea. They have been considered to form in post-orogenic settings related to the Central Asian Orogenic Belt(CAOB) or the Dabie-Sulu Orogenic Belt(DSOB). The Jurassic granitoids constitute extensive occurrence in the KP and are termed as the Daebo-period magmatism. They correlate well with coeval counterparts in NE China encompassing the northeastern part of the North China Craton(NCC) and the eastern segment of the CAOB. They commonly consist of biotite or two-mica granites and granodiorites, with some containing small dark diorite enclaves. On one hand, Early Jurassic to early Middle Jurassic magmatic rocks are rare in most areas of the NCC, whilst Middle-Late Jurassic ones are not developed in the KP. On the other hand, both NCC and KP host abundant Cretaceous granites. However, the present data revealed contrasting age peaks, with ~130–125 Ma in the NCC and ~110–105 Ma in the KP. Cretaceous granites in the KP comprise the dominant biotite granites and a few amphibole granites. The former exhibit mildly fractionated REE patterns and zircon εHf(t) values from ?15 to ?25, whereas the latter feature strongly fractionated REE patterns and zircon εHf(t) values from ?10 to ?1. Both granites contain inherited zircons of ~1.8–1.9 or ~2.5 Ga. These geochemical characters testify to their derivation from re-melting distinct protoliths in ancient basement. Another Cretaceous magmatic sub-event has been entitled as the Gyeongsang volcanism, which is composed of bimodal calc-alkaline volcanic rocks of 94–55 Ma and granitic-hypabyssal granitic bodies of 72–70 Ma. Synthesizing the Mesozoic magmatic rocks across the KP, NCC and Japan can lead to the following highlights:(1) All Triassic granites in the NCC, KP and Japan have similar characteristics in petrology, chronology and geochemistry. Therefore, the NCC, KP and Japan tend to share the same tectonic setting during the Triassic, seemingly within the context of Indosinian orogensis.(2) Jurassic to earliest Cretaceous magmatic rocks in the NCC seem to define two episodes: episode A from 175 to 157 Ma and episode B from 157 to 135 Ma. Jurassic magmatic rocks in the KP span in age mainly from 190 to 170 Ma, whereas 160–135 Ma ones are rare. With the exception of ~197 Ma Funatsu granite, Jurassic magmatic rocks are absent in Japan.(3) Cretaceous granites in the KP have a peak age of ~110, ~20 Ma younger than those in the NCC, while Japan is exempt from ~130–100 Ma granites.(4) The spatial-temporal distribution and migratory characteristics of theJurassic-Cretaceous magmatic rocks in Japan, KP, and NE China-North China indicate that the subduction of the Paleo-Pacific plate might not be operative before Late Cretaceous(~130–120 Ma).(5) Late Cretaceous magmatic rocks(~90–60 Ma) occur in the southwestern corner of the KP and also in Japan, coinciding with the metamorphic age of ~90–70 Ma in the Sanbagawa metamorphic belt of Japan. | 9 | The causes and impacts for heat stress in spring maize during grain filling in the North China Plain--A review显示文摘High-temperature stress(HTS) at the grain-filling stage in spring maize(Zea mays L.) is the main obstacle to increasing productivity in the North China Plain(NCP). To solve this problem, the physiological mechanisms of HTS, and its causes and impacts, must be understood. The HTS threshold of the duration and rate in grain filling, photosynthetic characteristics(e.g., the thermal stability of thylakoid membrane, chlorophyll and electron transfer, photosynthetic carbon assimilation), water status(e.g., leaf water potential, turgor and leaf relative water content) and signal transduction in maize are reviewed. The HTS threshold for spring maize is highly desirable to be appraised to prevent damages by unfavorable temperatures during grain filling in this region. HTS has negative impacts on maize photosynthesis by damaging the stability of the thylakoid membrane structure and degrading chlorophyll, which reduces light energy absorption, transfer and photosynthetic carbon assimilation. In addition, photosynthesis can be deleteriously affected due to inhibited root growth under HTS in which plants decrease their water-absorbing capacity, leaf water potential, turgor, leaf relative water content, and stomatal conductance. Inhibited photosynthesis decrease the supply of photosynthates to the grain, leading to falling of kernel weight and even grain yield. However, maize does not respond passively to HTS. The plant transduces the abscisic acid(ABA) signal to express heat shock proteins(HSPs), which are molecular chaperones that participate in protein refolding and degradation caused by HTS. HSPs stabilize target protein configurations and indirectly improve thylakoid membrane structure stability, light energy absorption and passing, electron transport, and fixed carbon assimilation, leading to improved photosynthesis. ABA also induces stomatal closure to maintain a good water status for photosynthesis. Based on understanding of such mechanisms, strategies for alleviating HTS at the grain-filling stage in spring maize are summarized. Eight strategies have the potential to improve the ability of spring maize to avoid or tolerate HTS in this study, e.g., adjusting sowing date to avoidHTS, breeding heat-tolerance varieties, and tillage methods, optimizing irrigation, heat acclimation, regulating chemicals, nutritional management, and planting geometric design to tolerate HTS. | 11 | Effect of postoperative early enteral nutrition on clinical outcomes and immune function of cholangiocarcinoma patients with malignant obstructive jaundice显示文摘BACKGROUND Most cholangiocarcinoma patients with malignant obstructive jaundice(MOJ)have varying degrees of malnutrition and immunodeficiency preoperatively.Therefore,perioperative nutritional support has important clinical significance in the treatment of cholangiocarcinoma.AIM To investigate the effects of postoperative early enteral nutrition(EEN)on immunity function and clinical outcomes of cholangiocarcinoma patients with MOJ.METHODS This prospective clinical study included 60 cholangiocarcinoma patients with MOJ who underwent surgery.The patients were randomly divided into an experimental group and a control group according to the nutrition support modes.The control group received postoperative total parenteral nutrition(TPN),whereas the experimental group received postoperative EEN and parenteral nutrition(PN;EEN+PN).The clinical outcomes,postoperative immune function,incidences of surgical site infection and bile leakage,intestinal function recovery time,average hospitalization days,and hospitalization expenses of the two groups were assessed on postoperative days(PODs)1,3,and 7.RESULTS The CD3+T,CD4+T,CD8+T,and CD4+T/CD8+T cell count and the immunoglobulin(Ig)G,IgM,and IgA levels in the EEN+PN group were significantly higher than those in the TPN group on PODs 3 and 7(P<0.05),whereas no significant differences in the CD3+T,CD4+T,CD8+T,and CD4+T/CD8+T cell counts and IgG,IgM,and IgA levels before operation and on POD 1 were found between the two groups(P>0.05).The intestinal function recovery time and postoperative hospital stay were shorter(P<0.001 for both)in the EEN+PN group than in the TPN group.The hospitalization expenses of the EEN+PN group were lower than those of the TPN group(P<0.001).However,the incidence of abdominal distension was higher than in the EEN+PN group than in the TPN group(P<0.05).The incidence rates of biliary leakage and surgical site infection were not significantly different between the two groups(P>0.05).CONCLUSION A postoperative EEN program could reduce the incidence of postoperative complications and improve the clinical outcomes and immune functions of cholangiocarcinoma patients with MOJ and is thus beneficial to patient recovery. | Bing-Qiang Ma Shi-Yong Chen Ze-Bin Jiang Biao Wu Yu He Xin-Xin Wang Yuan Li Peng Gao Xiao-Jun Yang | 2020 | World Journal of Gastroenterology2020,26,46: | 14 |
| 12 | Reproductive management through integration of PGD and MPS-based noninvasive prenatal screening/diagnosis for a family with GJB2-associated hearing impairment显示文摘A couple with a proband child of GJB2(encoding the gap junction protein connexin 26)-associated hearing impairment and a previous pregnancy miscarriage sought for a reproductive solution to bear a healthy child. Our study aimed to develop a customized preconception-to-neonate care trajectory to fulfill this clinical demand by integrating preimplantation genetic diagnosis(PGD), noninvasive prenatal testing(NIPT), and noninvasive prenatal diagnosis(NIPD) into the strategy. Auditory and genetic diagnosis of the proband child was carried out to identify the disease causative mutations. The couple then received in-vitro-fertilization treatment, and eight embryos were obtained for day 5 biopsy. PGD was performed by short-tandem-repeat linkage analysis and Sanger sequencing of GJB2 gene. Transfer of a GJB2 c.235del C heterozygous embryo resulted in a singleton pregnancy. At the 13 th week of gestation, genomic DNA(g DNA) from the trio family and cell-free DNA(cf DNA) from maternal plasma were obtained for assessment of fetal chromosomal aneuploidy and GJB2 mutations. NIPT and NIPD showed the absence of chromosomal aneuploidy and GJB2-associated disease in the fetus, which was later confirmed by invasive procedures and postnatal genetic/auditory diagnosis. | 14 | A two-step lineage reprogramming strategy to generate functionally competent human hepatocytes from fibroblasts显示文摘Terminally differentiated cells can be generated by lineage reprogramming,which is,however,hindered by incomplete conversion with residual initial cell identity and partial functionality. Here, we demonstrate a new reprogramming strategy by mimicking the natural regeneration route, which permits generating expandable hepatic progenitor cells and functionally competent human hepatocytes. Fibroblasts were first induced into human hepatic progenitor-like cells (hHPLCs), which could robustly expand in vitro and efficiently engraft in vivo. Moreover, hHPLCs could be efficiently induced into mature human hepatocytes (hiHeps) in vitro, whose molecular identity highly resembles primary human hepatocytes (PHHs). Most importantly, hiHeps could be generated in large quantity and were functionally competent to replace PHHs for drug-metabolism estimation, toxicity prediction and hepatitis B virus infection modeling. Our results highlight the advantages of the progenitor stage for successful lineage reprogramming. | 15 | Epidemiology,clinical and laboratory characteristics of currently alive HIV-1 infected former blood donors naive to antiretroviral therapy in Anhui Province,China显示文摘Background Unregulated commercial blood/plasma collection among farmers occurred between 1992 and 1995 in central China and caused the second major epidemic of human immunodeficiency virus type 1 (HIV-1) infection in China. It is important to characterize HIV-1-infected former blood donors and to study characteristics associated with disease progression for future clinical intervention and vaccine development. Methods A cross-sectional study was performed on HIV-1-infected former blood donors (FBDs) and age-matched HIV-seronegative local residents. Demographic, epidemiologic, clinical and key laboratory data were collected from all study participants. Both unadjusted and adjusted multivariate linear regressions were employed to analyze the association of the decrease of CD4+ T-cell counts with other characteristics. Results Two hundred and ninety-four HIV-1-infected FBDs and 59 age-matched HIV-seronegative local residents were enrolled in this study. The unregulated blood/plasma collection occurred more than a decade (10.8 -12.8 years) ago, which caused the rapid spread of HIV-1 infection and the high prevalence of co-infection with hepatitis C virus (HCV, 89.5%); hepatitis B virus (HBV) co-infection was observed in only 11 HIV+participants (3.7%). Deterioration in both clinical manifestation and laboratory parameters and increase of viral loads were observed in parallel with the decrease of CD4+ T-cell counts. The decrease of total lymphocyte counts (P<0.001) and hemoglobin levels (P<0.001) and the appearance of dermatosis (P=0.03) were observed in parallel with the decrease of CD4+ T-cell counts whereas viral loads (P<0.001) and CD8+ T-cell counts (P=0.01) were inverselyassociated with CD4+ T-cell counts. Conclusions Co-infection with HCV but not HBV is highly prevalent among HIV-1-infected FBDs. CD4+ T-cell counts is a reliable indicator for disease progression among FBDs. | 17 | GCH1 plays a role in the high-altitude adaptation of Tibetans显示文摘Tibetans are well adapted to high-altitude hypoxia. Previous genome-wide scans have reported many candidate genes for this adaptation, but only a few have been studied. Here we report on a hypoxia gene(GCH1, GTP-cyclohydrolase I), involved in maintaining nitric oxide synthetase(NOS) function and normal blood pressure, that harbors many potentially adaptive variants in Tibetans. We resequenced an 80.8 kb fragment covering the entire gene region of GCH1 in 50 unrelated Tibetans. Combined with previously published data, we demonstrated many GCH1 variants showing deep divergence between highlander Tibetans and lowlander Han Chinese. Neutrality tests confirmed a signal of positive Darwinian selection on GCH1 in Tibetans. Moreover, association analysis indicated that the Tibetan version of GCH1 was significantly associated with multiple physiological traits in Tibetans, including blood nitric oxide concentration, blood oxygen saturation, and hemoglobin concentration. | 18 | Association of genotypes of rs671 within ALDH2 with risk for gastric cardia adenocarcinoma in the Chinese Han population in high-and low-incidence areas显示文摘Objective: This study aimed to determine if gastric cardia adenocarcinoma(GCA) risk was associated with the lys(A or *2) allele at the rs671(glu504lys) polymorphism within the aldehyde dehydrogenase 2(ALDH2) gene in a Chinese Han population. We also aimed to investigate ALDH2 genotypic distributions between subjects from high- and low-incidence areas for both GCA and esophageal squamous cell carcinoma(ESCC).Methods: We designed a case-control study including 2,686 patients with GCA and 3,675 control subjects from high- and lowincidence areas for both GCA and ESCC in China. Taq Man allele discrimination assay was used to genotype the rs671 polymorphism. χ~2 test and binary logistic regression analysis were used to estimate the odds ratios for the development of GCA,and multivariate ordinal logistic regression was used to analyze ALDH2 genotypic distributions among different groups.Results: Compared with ALDH2*1/*1 homozygotes, ALDH2*1/*2 and ALDH2*2/*2 carriers did not increase the risk for GCA in the Chinese Han population(P>0.05). Interestingly, the ratio of homozygous or heterozygous ALDH2 *2 carriers in highincidence areas for both GCA and ESCC was lower than that in low-incidence areas(P<0.001).Conclusions: Genotypes of rs671 at ALDH2 may not increase GCA susceptibility in Chinese Han populations. In addition, the ALDH2 genotypic distribution differs between Chinese Han populations from high- and low-incidence areas for both GCA and ESCC. | 20 | A draft sequence of the rice (Oryza sativa ssp. indica) genome显示文摘The sequence of the rice genome holds fundamental information for its biology, including physiology, genetics, development, and evolution, as well as information on many beneficial phenotypes of economic significance. Using a 'whole genome shotgun' approach, we have pro-duced a draft rice genome sequence of Oryza sativa ssp. in-dica, the major crop rice subspecies in China and many other regions of Asia. The draft genome sequence is constructed from over 4.3 million successful sequencing traces with an accumulative total length of 2214.9 Mb. The initial assembly of the non-redundant sequences reached 409.76 Mb in length, based on 3.30 million successful sequencing traces with a total length of 1797.4 Mb from an indica variant cultivar 93-11, giving an estimated coverage of 95.29% of the rice genome with an average base accuracy of higher than 99%.
