12018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 3Histological Subtypes of Lung Cancer in Chinese Males from 2000 to 2012显示文摘Objective To characterize the histological and epidemiological features of male lung cancer patients in China. Methods The demographic and histological information about male lung cancer patients identified from 2000-01-01 to 2012-12-31,was collected from the Cancer Hospital of the Chinese Academy of Medical Sciences. Relative frequencies(RF) were estimated for major histological subtypes and compared according to the years of diagnosis and birth. Results The RF of adenocarcinoma(ADC) increased from 21.96% to 43.36% and the RF of squamous cell carcinoma(SCC) decreased from 39.11% to 32.23% from 2000 to 2012 in the 15 427 male lung cancer patients included in this study(Z=17.909,P<0.0001; Z=-6.117,P<0.0001). The RF of ADC increased from 28.72% in 2000-2004,36.88% in 2005-2008 to 48.61% in 2009-2012 in patients born after 1960. The age-adjusted RF of ADC in 2007-2012 increased consistently in all the investigated areas. Conclusion The increased RF of ADC in male lung cancer patients highlights the need for further investigation of the etiologic factors of these tumors. 5Migration and accumulation of natural gas in Kela-2 gas field显示文摘With the guidance of petroleum system theory,the dynamic filling history of natural gas in the Kela-2 gasfield is analyzed by using a large suite of oil and gas geo-chemistry evidence in combination with the tectonic evolu-tion history and reservoir evolution history. It concludes thatthe Kela-2 gas field was formed by capturing the gas gener-ated during the main gas generation period, while the latekerogen cracking gas contributed a little to the gas field. Itsuggests that the gas generated during the main gas genera-tion accumulated in the early-formed wide-gentle anticline,which is the necessary condition for natural gas to re-migrateand enrich late to form the large-scale gas reservoir. 9Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. 10Analyzing Influencing Factors of Rural Poverty in Typical Poverty Areas of Hainan Province: A Case Study of Lingao County显示文摘Rural poverty and poverty reduction are not only the focal issues that have attracted worldwide attention, but also the vital issues on people's livelihood that has attached great importance and aimed to be solved by the central and local governments of China. Based on the survey data of 354 farming households, this paper, taking the national poverty county of Lingao County, Hainan Province for an example, examined the characteristics of rural poverty of the county. Moreover, this paper established the spatial lag model(SLM) from five dimensions, namely, status of the household head, household structure, health status, income composition and traffic accessibility, to analyze the main influencing factors of rural poverty according to the values of Moran's I and the diagnosis of spatial dependence of the OLS model. It is found that the poor farming households gathered mainly in five towns in the north and southwest of the county, and the rural poverty have the characteristics of low educational level of the heads, more minor children, high population of farming peasants, high incidence of disease and low proportion of household wage-equivalent income. The results also showed that the variables such as the number of minor children, the number of migrant worker, the number of farming peasants and the proportion of wage-equivalent income have significant effectiveness on rural poverty, while the status of the household head, health status and traffic accessibility have little influence. 11Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. 13Serum Gamma-glutamyl Transferase Levels Predict Functional Outcomes after Aneurysmal Subarachnoid Hemorrhage显示文摘Objective We aim to explore the potential association between serum gamma-glutamyl transferase levels and functional outcome after aneurysmal subarachnoid hemorrhage in a Chinese population. Methods A total of 386 aneurysmal subarachnoid hemorrhage patients were included in the study from September 2007 to February 2015. Baseline serum gamma-glutamyl transferase levels and 6-month follow-up functional outcomes were determined. A poor outcome was defined as a modified ranking scale score of ≥ 3. The multivariable logistic model was used to analyze the relationship between serum gamma-glutamyl transferase and clinical outcomes after aneurysmal subarachnoid hemorrhage. Results The adjusted poor outcome rates of patients with gamma-glutamyl transferase levels of < 30 U/L, 30-50 U/L and ≥ 50 U/L were 16.7%, 19.6%, and 34.4%, respectively(P < 0.01). The age-sex and multivariable adjusted odds ratios(95% confidence intervals) of poor prognosis comparing the top group(≥ 50 U/L) with the lowest group(< 30 U/L) were 5.76(2.74-12.13), 6.64(2.05-21.52), and 6.36(1.92-21.02). A significant linear trend existed between gamma-glutamyl transferase level and aneurysmal subarachnoid hemorrhage prognosis. This association was also observed among nondrinkers. Conclusion Patients with higher gamma-glutamyl transferase levels were more likely to have a poor prognosis. 14Clinical features, survival and prognostic factors of primary testicular diffuse large B-cell lymphoma显示文摘Objective: To assess the clinical features, survival and prognostic factors of primary testicular diffuse large B-cell lymphoma(DLBCL).Methods: A retrospective study of 37 patients with primary testicular DLBCL was carried out from November 2003 to May 2012. Their clinical features, survival and prognostic factors were analyzed.Results: During a median follow-up period of 39.8 months(5.4-93.0 months), the median progression-free survival(PFS) was 26.2 months(95% CI: 0-65 months) and the 3-year overall survival(OS) rate was 78.4%. Within the whole cohort, the factors significantly associated with a superior PFS were limited stage(stage I/II), lactate dehydrogenase(LDH) ≤245 U/L, international prognostic index(IPI) ≤1, primary tumor diameter <7.5 cm, and patients who had complete response(CR) and received doxorubicin-contained chemotherapy(P<0.05). There was a trend toward superior outcome for patients who received combined therapy(surgery/chemotherapy/radiotherapy)(P=0.055). Patients who had CR, primary tumor diameter <7.5 cm and IPI score ≤1 were significantly associated with longer PFS at multivariate analysis.Conclusions: Primary testicular DLBCL had poorer survival. CR, primary tumor diameter and IPI were independent prognostic factors. 15Environmental and Psycho-social Factors Related to Prostate Cancer Risk in the Chinese Population:a Case-control Study显示文摘Objective To study the risk environmental and psycho-social factors associated to prostate cancer(PCa) in Chinese population. Methods 250 PCa patients and 500 controls were enrolled in this case-control study. Information was collected and logistic regression analysis was used to estimate the odds ratios(OR) and 95% confidence intervals(95% CI) for relationship between lifestyle, eating habits and psycho-social factors with PCa risk. Results Green vegetables and green tea were associated with a decreased risk of PCa(OR=0.39, 95% CI: 0.28-0.53; OR=0.59, 95% CI: 0.40-0.87, respectively). Family history of PCa(OR=7.16, 95% CI: 2.01-25.49), history of prostate diseases(OR=2.28, 95% CI: 1.53-3.41), alcohol consumption(OR=1.97, 95% CI: 1.33-2.90), red meat consumption(OR=1.74, 95% CI: 1.20-2.52), barbecued(OR=2.29, 95% CI: 1.11-4.73) or fried(OR=2.35, 95% CI: 1.24-4.43) foods were related with increased PCa risk. Negative psycho-social factors including occupational setbacks(OR=1.61, 95% CI: 1.00-2.59), marital separation(OR=1.94, 95% CI: 1.29-2.91), self-contained suffering(OR=2.37, 95% CI: 1.58-3.55), and high sensitivity to the personal comments(OR=1.73, 95% CI: 1.18-2.54) were related to PCa. Conclusion Regular consumption of green vegetables and green tea may suggest protective effects on PCa. Alcohol consumption, red meat consumption and barbecued or fried foods were associated with PCa. 16Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Gastric Cancer Patients显示文摘Purpose Malnutrition is severe among gastric cancer patients in China. While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for Chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. 20Ultrasonic diagnosis of patients with clonorchiasis and preliminary study of pathogenic mechanism显示文摘Objective:To discuss the liver function damage mechanism of patients with clonorchiasis by analyzing the ultrasound characteristics,liver function,change of the serum inflammatory factors and cell apoptosis factors.Methods:Color Doppler ultrasound technique was adopted to detect the portal vein and blood flow change of patients with clonorchiasis;ELISA was used to determine the level of different serum inflammatory factors.The levels of serum total bilirubin,serum albumin and glutamic-pyruvic transaminase were detected by automatic biochemical analyzer.Western blot was used to determine the expression of proteins relevant to apoptosis.Results:Compared with the health control group,the trunk diameter of portal vein and the thickness of spleen,as well as the hepatic artery pulsation index of clonorchiasis patients increased obviously,the mean blood flow velocity of portal vein(P<0.05 or P<0.01) decreased.The content of total bilirubin and transaminase in plasma increased significantly,but albumin decreased(P<0.05).Levels of TNF-α,IL-6 and IFN-γ increased remarkably,and the level of every factor was significantly different among patients with Child-Pugh Ⅰ,Child-Pugh Ⅱ and Child-Pugh Ⅲ classification of liver function(P<0.05 or P<0.01).With the exacerbation of liver dysfunction,levels of TNF-α,IL-6 and IFN-γ gradually increased(P<0.05).Compared with the healthy control group,the expression quantity of apoptosis protein Fas,FasL,Bax and Caspase-3 increased significantly(P<0.05 or P<0.01),but Bcl-2 decreased(P<0.05).Conclusions:Changes of ultrasonic characteristics and liver dysfunction,caused by liver fluke infection,may be related to that both inflammatory response and apoptosis response have participated in the pathogenic process and liver damage course of clonorchiasis.Mei Fan Lin Lu Chun Su Mei Xue Ji-Mei Dou Pei Li Han-Qi Feng Yan-Bing Fan 2016Asian Pacific Journal of Tropical Medicine2016,9,7:3
