| 2 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 3 | Soluble intercellular adhesion molecule-1,D-lactate and diamine oxidase in patients with inflammatory bowel disease显示文摘AIM:To study the levels of serum soluble intercellular adhesion molecule-1 (sICAM-1), plasma D-lactate and diamine oxidase (DAO) in patients with inflammatory bowel disease (IBD), and the potential clinical signif icance.METHODS: Sixty-nine patients with IBD and 30 healthy controls were included in this study. The concentration of sICAM-1 was detected with enzyme-linked immu-nosorbent assay, the level of D-lactate and DAO was measured by spectroscopic analysis, and the number of white blood cells (WBC) was determined by routine procedure.RESULTS:The levels of sICAM-l, DAO, and WBC in IBD patients were significantly higher than those in the control group (P<0.01). sICAM-l in IBD patients was found to be closely related to the levels of DAO and D-lactate (212.94±69.89 vs 6.35±2.35, P=0.000), DAO 212.94±69.89 vs 8.65±3.54, P=0.000) and WBC (212.94±69.89 vs 7.40±2.61, P=0.000), but no signif icant difference was observed between patients with ulcerative colitis and patients with Crohn's disease. | 6 | Histological Subtypes of Lung Cancer in Chinese Males from 2000 to 2012显示文摘Objective To characterize the histological and epidemiological features of male lung cancer patients in China. Methods The demographic and histological information about male lung cancer patients identified from 2000-01-01 to 2012-12-31,was collected from the Cancer Hospital of the Chinese Academy of Medical Sciences. Relative frequencies(RF) were estimated for major histological subtypes and compared according to the years of diagnosis and birth. Results The RF of adenocarcinoma(ADC) increased from 21.96% to 43.36% and the RF of squamous cell carcinoma(SCC) decreased from 39.11% to 32.23% from 2000 to 2012 in the 15 427 male lung cancer patients included in this study(Z=17.909,P<0.0001; Z=-6.117,P<0.0001). The RF of ADC increased from 28.72% in 2000-2004,36.88% in 2005-2008 to 48.61% in 2009-2012 in patients born after 1960. The age-adjusted RF of ADC in 2007-2012 increased consistently in all the investigated areas. Conclusion The increased RF of ADC in male lung cancer patients highlights the need for further investigation of the etiologic factors of these tumors. | 12 | Serum N-glycan markers for diagnosing liver fibrosis induced by hepatitis B virus显示文摘BACKGROUND Hepatitis B virus (HBV) infection is the primary cause of hepatitis with chronic HBV infection,which may develop into liver fibrosis,cirrhosis and hepatocellular carcinoma.Detection of early-stage fibrosis related to HBV infection is of great clinical significance to block the progression of liver lesion.Direct liver biopsy is regarded as the gold standard to detect and assess fibrosis;however,this method is invasive and prone to clinical sampling error.In order to address these issues,we attempted to find more convenient and effective serum markers for detecting HBV-induced early-stage liver fibrosis.AIM To investigate serum N-glycan profiling related to HBV-induced liver fibrosis and verify multiparameter diagnostic models related to serum N-glycan changes.METHODS N-glycan profiles from the sera of 432 HBV-infected patients with liver fibrosis were analyzed.Significant changed N-glycan levels (peaks)(P <0.05) in differentfibrosis stages were selected in the modeling group,and multiparameter diagnostic models were established based on changed N-glycan levels by logistic regression analysis.The receiver operating characteristic (ROC) curve analysis was performed to evaluate diagnostic efficacy of N-glycans models.These models were then compared with the aspartate aminotransferase to platelet ratio index (APRI),fibrosis index based on the four factors (FIB-4),glutamyltranspeptidase platelet albumin index (S index),GlycoCirrho-test,and GlycoFibro-test.Furthermore,we combined multiparameter diagnostic models with alanine aminotransferase (ALT) and platelet (PLT) tests and compared their diagnostic power.In addition,the diagnostic accuracy of N-glycan models was also verified in the validation group of patients.RESULTS Multiparameter diagnostic models constructed based on N-glycan peak 1,3,4and 8 could distinguish between different stages of liver fibrosis.The area under ROC curves (AUROCs) of Model A and Model B were 0.890 and 0.752,respectively differentiating fibrosis F0-F1 from F2-F4,and F0-F2 from F3-F4,and surpassing other serum panels.However,AUROC (0.747) in Model C used for the diagnosis of F4 from F0-F3 was lower than AUROC (0.795) in FIB-4.In combination with ALT and PLT,the multiparameter models showed better diagnostic power (AUROC=0.912,0.829,0.885,respectively) when compared with other models.In the validation group,the AUROCs of the three combined models (0.929,0.858,and 0.867,respectively) were still satisfactory.We also applied the combined models to distinguish adjacent fibrosis stages of 432patients (F0-F1/F2/F3/F4),and the AUROCs were 0.917,0.720 and 0.785.CONCLUSION Multiparameter models based on serum N-glycans are effective supplementary markers to distinguish between adjacent fibrosis stages of patients caused by HBV,especially in combination with ALT and PLT. | Xi Cao Qing-Hua Shang Xiao-Ling Chi Wei Zhang Huan-Ming Xiao Mi-Mi Sun Gang Chen Yong An Chun-Lei Lv Lin Wang Yue-Min Nan Cui-Ying Chen Zong-Nan Tan Xue-En Liu Hui Zhuang | 2020 | World Journal of Gastroenterology2020,26,10: | 12 |

| 15 | Visual Fixation Assessment in Patients with Disorders of Consciousness Based on Brain-Computer Interface显示文摘Visual fixation is an item in the visual function subscale of the Coma Recovery Scale-Revised(CRS-R).Sometimes clinicians using the behavioral scales find it difficult to detect because of the motor impairment in patients with disorders of consciousness(DOCs). Braincomputer interface(BCI) can be used to improve clinical assessment because it directly detects the brain response to an external stimulus in the absence of behavioral expression. In this study, we designed a BCI system to assist the visual fixation assessment of DOC patients. The results from 15 patients indicated that three showed visual fixation in both CRS-R and BCI assessments and one did not show such behavior in the CRS-R assessment but achieved significant online accuracy in the BCI assessment. The results revealed that electroencephalography-based BCI can detect the brain response for visual fixation. | 18 | Pathophysiological mechanisms and therapeutic approaches in obstructive sleep apnea syndrome显示文摘Obstructive sleep apnea syndrome(OSAS)is a common breathing disorder in sleep in which the airways narrow or collapse during sleep,causing obstructive sleep apnea.The prevalence of OSAS continues to rise worldwide,particularly in middle-aged and elderly individuals.The mechanism of upper airway collapse is incompletely understood but is associated with several factors,including obesity,craniofacial changes,altered muscle function in the upper airway,pharyngeal neuropathy,and fluid shifts to the neck.The main characteristics of OSAS are recurrent pauses in respiration,which lead to intermittent hypoxia(IH)and hypercapnia,accompanied by blood oxygen desaturation and arousal during sleep,which sharply increases the risk of several diseases.This paper first briefly describes the epidemiology,incidence,and pathophysiological mechanisms of OSAS.Next,the alterations in relevant signaling pathways induced by IH are systematically reviewed and discussed.For example,IH can induce gut microbiota(GM)dysbiosis,impair the intestinal barrier,and alter intestinal metabolites.These mechanisms ultimately lead to secondary oxidative stress,systemic inflammation,and sympathetic activation.We then summarize the effects of IH on disease pathogenesis,including cardiocerebrovascular disorders,neurological disorders,metabolic diseases,cancer,reproductive disorders,and COVID-19.Finally,different therapeutic strategies for OSAS caused by different causes are proposed.Multidisciplinary approaches and shared decision-making are necessary for the successful treatment of OSAS in the future,but more randomized controlled trials are needed for further evaluation to define what treatments are best for specific OSAS patients. | Renjun Lv Xueying Liu Yue Zhang Na Dong Xiao Wang Yao He Hongmei Yue Qingqing Yin | 2023 | Signal Transduction and Targeted Therapy2023,8,6: | 8 |
| 19 | Expression level of beta-catenin is associated with prognosis of esophageal carcinoma显示文摘AIM: To examine the association of beta-catenin with the clinicopathologic features and prognosis of esophageal squamous cell carcinoma (ESCC). METHODS: Beta-catenin mRNA expression level in 40 ESCC patients (28 males and 12 females, age range 38-82 years, median 60 years) was analyzed by real- time PCR. Beta-catenin mRNA expression levels in tumor cells were categorized as weaker (level 1) or equal to or stronger (level 2) than those in endothelial cells. We examined the correlation between the beta-catenin expression and the clinicopathological factors and prognosis of ESCC patients. RESULTS: Level 2 beta-catenin expression was found in 29 patients. ESCC with level 2 expression had a higher rate of lymphnode metastasis (0.0776 ± 0.0369 vs 0.3413 ± 0.1803, P < 0.001) and deeper tumor invasion (0.0751 ± 0.0356 vs 0.3667 ± 0.1928, P < 0.001), and a poorer survival rate (P = 0.0024) than ESCC with level 1 expression.
