| 1 | Tourette syndrome associated with attention deficit hyperactivity disorder: The impact of tics and psychopharmacological treatment options显示文摘Tourette syndrome(TS) is a neurodevelopmental disorder characterized by multiple chronic motor and vocal tics beginning in childhood. Several studies describe the association between TS and attention deficit hyperactivity disorder(ADHD). Fifty percent of children diagnosed with ADHD have comorbid tic disorder. ADHD related symptoms have been reported in 35% to 90% of children with TS. Since ADHD is the most prevalent comorbid condition with TS and those with concomitant TS and ADHD present with considerable psychosocial and behavioral impairments, it is essential for clinicians to be familiar with these diagnoses and their management. This paper highlights the association between treating ADHD with stimulants and the development of tic disorders. The two cases discussed underscore the fact that children with TS may present with ADHD symptomatology prior to the appearance of any TS related symptoms. | 3 | Co-expression of CDX2 and MUC2 in gastric carcinomas: Correlations with clinico-pathological parameters and prognosis显示文摘AIM: To evaluate the role of CDX2 homeobox protein as a predictor for cancer progression and prognosis as well as its correlation with MUC2 expression. CDX2 represents a transcription factor for various intestinal genes (including MUC2) and thus an important regulator of intestinal differentiation, which could previously be identified in gastric carcinomas and intestinal metaplasia.METHODS: Formalin-fixed and paraffin-embedded tissues from 190 gastric carcinoma patients were stained with monodonal antibodies recognizing CDX2 and MUC2, respectively. Immunoreactivity was evaluated semiquantitatively and statistical analyses including χ2 tests, uni- and multi-variate survival analyses were performed.RESULTS: CDX2 was mostly expressed in a nuclear or supranuclear pattern, whereas MUC2 showed an almost exclusive supranuclear reactivity. Both antigens were present in >80% of areas exhibiting intestinal metaplasia. An immunoreactivity in >5% of the tumor area was observed in 57% (CDX2) or in 21% (MUC2) of the carcinomas. The presence of both molecules did not correlate with WHO,significantly stronger vlUC2 expression in mucinous tumors). CDX2 correlated with a lower pT and pN stage in the subgroups of intestinal and stage I cancers and was associated with MUC2 positivity. A prognostic impact of CDX2 or MUC2 was not observed. CONCLUSION: CDX2 and MUC2 play an important role in the differentiation of normal, inflamed, and neoplastic gastric tissues. | 5 | Correlation analysis of dual-energy CT iodine maps with quantitative pulmonary perfusion MRI显示文摘AIM:To correlate dual-energy computed tomography(DECT) pulmonary angiography derived iodine maps with parameter maps of quantitative pulmonary perfusion magnetic resonance imaging(MRI).METHODS:Eighteen patients with pulmonary perfusion defects detected on DECT derived iodine maps were included in this prospective study and additionally underwent time-resolved contrast-enhanced pulmonary MRI [dynamic contrast enhanced(DCE)-MRI].DCE-MRI data were quantitatively analyzed using a pixel-by-pixel deconvolution analysis calculating regional pulmonary blood flow(PBF),pulmonary blood volume(PBV) and mean transit time(MTT) in visually normal lung parenchyma and perfusion defects.Perfusion parameterswere correlated to mean attenuation values of normal lung and perfusion defects on DECT iodine maps.Two readers rated the concordance of perfusion defects in a visual analysis using a 5-point Likert-scale(1 = no correlation,5 = excellent correlation).RESULTS:In visually normal pulmonary tissue mean DECT and MRI values were:22.6 ± 8.3 Hounsfield units(HU);PBF:58.8 ± 36.0 mL/100 mL per minute;PBV:16.6 ± 8.5 mL;MTT:17.1 ± 10.3 s.In areas with restricted perfusion mean DECT and MRI values were:4.0 ± 3.9 HU;PBF:10.3 ± 5.5 mL/100 mL per minute,PBV:5 ± 4 mL,MTT:21.6 ± 14.0 s.The differences between visually normal parenchyma and areas of restricted perfusion were statistically significant for PBF,PBV and DECT(P < 0.0001).No linear correlation was found between MRI perfusion parameters and attenuation values of DECT iodine maps(PBF:r = 0.35,P = 0.15;PBV:r = 0.34,P = 0.16;MTT:r = 0.41,P = 0.08).Visual analysis revealed a moderate correlation between perfusion defects on DECT iodine maps and the parameter maps of DCE-MRI(mean score 3.6,k 0.45).CONCLUSION:There is a moderate visual but not statistically significant correlation between DECT iodine maps and perfusion parameter maps of DCE-MRI. | Jan Hansmann Paul Apfaltrer Frank G Zoellner Thomas Henzler Mathias Meyer Gerald Weisser Stefan O Schoenberg Ulrike I Attenberger | 2013 | World Journal of Radiology2013，5，5： | 6 |

| 8 | Archaea and the human gut:New beginning of an old story显示文摘Methanogenic archaea are known as human gut inhabitants since more than 30 years ago through the detection of methane in the breath and isolation of two methanogenic species belonging to the order Methanobacteriales,Methanobrevibacter smithii and Methanosphaera stadtmanae.During the last decade,diversity of archaea encountered in the human gastrointestinal tract(GIT)has been extended by sequence identification and culturing of new strains.Here we provide an updated census of the archaeal diversity associatedwith the human GIT and their possible role in the gut physiology and health.We particularly focus on the still poorly characterized 7th order of methanogens,the Methanomassiliicoccales,associated to aged population.While also largely distributed in non-GIT environments,our actual knowledge on this novel order of methanogens has been mainly revealed through GIT inhabitants.They enlarge the number of final electron acceptors of the gut metabolites to mono-di-and trimethylamine.Trimethylamine is exclusively a microbiota-derived product of nutrients(lecithin,choline,TMAO,L-carnitine)from normal diet,from which seems originate two diseases,trimethylaminuria(or Fish-Odor Syndrome)and cardiovascular disease through the proatherogenic property of its oxidized liver-derived form.This therefore supports interest on these methanogenic species and its use as archaebiotics,a term coined from the notion of archaea-derived probiotics. | Nadia Gaci Guillaume Borrel William Tottey Paul William O’Toole Jean-Fran?ois Brugère | 2014 | World Journal of Gastroenterology2014，20，43： | 5 |

| 16 | Appropriate prescribing in the elderly: Current perspectives显示文摘Advances in medical therapeutics have undoubtedly contributed to health gains and increases in life expectancy over the last century. However, there is growing evidence to suggest that therapeutic decisions in older patients are frequently suboptimal or potentially inappropriate and often result in negative outcomes such as adverse drug events, hospitalisation and increased healthcare resource utilisation. Several factors influence the appropriateness of medication selectionin older patients including age-related changes in pharmacokinetics and pharmacodynamics, high numbers of concurrent medications, functional status and burden of co-morbid illness. With ever-increasing therapeutic options, escalating proportions of older patients worldwide, and varying degrees of prescriber education in geriatric pharmacotherapy, strategies to assist physicians in choosing appropriate pharmacotherapy for older patients may be helpful. In this paper, we describe important age-related pharmacological changes as well as the principal domains of prescribing appropriateness in older people. We highlight common examples of drugdrug and drug-disease interactions in older people. We present a clinical case in which the appropriateness of prescription medications is reviewed and corrective strategies suggested. | 18 | Three-dimensional simulation of liquid injection,film formation and transport,in fluidized beds显示文摘Liquid injection,and film formation and transport in dense-phase gas-solids fluidized beds are numerically simulated in three dimensions using a collisional exchange model that is based on the mechanism that collisions cause transfer of liquid mass,momentum,and energy between particles. In the model,each of the particles is represented by a solid core and a liquid film surrounding the core. The model is incorporated in the framework of the commercial code Barracuda? developed by CPFD Software. The commercial software is an advanced CFD-based computational tool where the particles are treated as discrete entities,calculated by the MP-PIC method,and tracked using the Lagrangian method. Details of the collisional liquid transfer model have been previously presented in O'Rourke,Zhao,and Snider (2009); this paper presents new capabilities and proof-testing of the collision model and a new method to better quantify the penetration length. Example calculations of a fluidized bed without liquid injection show the expected effect of collisions on the reduction of granular temperature (fluctuational kinetic energy) of the bed. When applied to liquid injection into a dense-phase fluidized bed under different conditions,the model predicts liquid penetration lengths comparable to the experiments. In addition,the simulation reveals for the first time the dynamic mixing of the liquid droplets with the bed particles and the transient distribution of the droplets inside the bed. | Paul Zhao Peter J.
