1Design of a new dynamical core for global atmospheric models based on some efficient numerical methods显示文摘A careful study on the integral properties of the primitive hydrostatic balance equations for baroclinic atmosphere is carried out, and a new scheme todesign the global adiabatic model of atmospheric dynamics ispresented. This scheme includes a method of weighted equal-areamesh and a fully discrete finite difference method with quadraticand linear conservations for solving the primitive equationsystem. Using this scheme, we established a new dynamical corewith adjustable high resolution acceptable to the availablecomputer capability, which can be very stable without anyfiltering and smoothing. Especially, some important integralproperties are kept unchanged, such as the anti-symmetries of thehorizontal advection operators and the vertical convectionoperator, the mass conservation, the effective energy conservationunder the standard stratification approximation, and so on. 3A complete sequence and comparative analysis of a SARS-associated virus(Isolate BJ01)显示文摘The genome sequence of the Severe Acute Respiratory Syndrome (SARS)-associated virus provides essential information for the identification of pathogen(s), exploration of etiology and evolution, interpretation of transmission and pathogenesis, development of diagnostics, prevention by future vaccination, and treatment by developing new drugs. We report the complete genome sequence and comparative analysis of an isolate (BJ01) of the coronavirus that has been recognized as a pathogen for SARS. The genome is 29725 nt in size and has 11 ORFs (Open Reading Frames). It is composed of a stable region encoding an RNA-dependent RNA polymerase (composed of 2 ORFs) and a variable region representing 4 CDSs (coding sequences) for viral structural genes (the S, E, M, N proteins) and 5 PUPs (putative uncharacterized proteins). Its gene order is identical to that of other known coronaviruses. The sequence alignment with all known RNA viruses places this virus as a member in the family of Coronaviridae. Thirty putative substitutions have been identified by comparative analysis of the 5 SARS- associated virus genome sequences in GenBank. Fifteen of them lead to possible amino acid changes (non-synonymous mutations) in the proteins. Three amino acid changes, with predicted alteration of physical and chemical features, have been detected in the S protein that is postulated to beinvolved in the immunoreactions between the virus and its host. Two amino acid changes have been detected in the Mprotein, which could be related to viral envelope formation. Phylogenetic analysis suggests the possibility of non-human origin of the SARS-associated viruses but provides noevidence that they are man-made. 4Effect of early enteral nutrition on postoperative nutritional status and immune function in elderly patients with esophageal cancer or cardiac cancer显示文摘Objective: To explore the effect of early enteral nutrition (EN) on postoperative nutritional status, intestinal permeability, and immune function in elderly patients with esophageal cancer or cardiac cancer. Methods: A total of 96 patients with esophageal cancer or cardiac cancer who underwent surgical treatment in our hospital from June 2007 to December 2010 were enrolled in this study. They were divided into EN group (n=50) and parenteral nutrition (PN) group (n=46) based on the nutrition support modes. The body weight, time to first flatus/defecation, average hospital stay, complications and mortality after the surgery as well as the liver function indicators were recorded and analyzed. Peripheral blood samples were collected on the days 1, 4 and 7 after surgery. The plasma diamine oxidase (DAO) activity and D-lactate level were determined to assess the intestinal permeability. The plasma endotoxin levels were determined using dynamic turbidimetric assay to assess the protective effect of EN on intestinal mucosal barrier. The postoperative blood levels of inflammatory cytokines and immunoglobulins were determined using enzyme- linked immunosorbent assay (ELISA). Results: After the surgery, the time to first flatus/defecation, average hospital stay, and complications were significantly less in the EN group than those in the PN group (P<0.05), whereas the EN group had significantly higher albumin levels than the PN group (P<0.05). On the 7th postoperative day, the DAO activity, D-lactate level and endotoxin contents were significantly lower in the EN group than those in the PN group (all P<0.05). In addition, the EN group had significantly higher IgA, IgG, IgM, and CD4 levels than the PN group (P<0.05) but significantly lower IL-2, IL-6, and TNF-α levels (P<0.05). 62018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 9Double contrast-enhanced two-dimensional and three-dimensional ultrasonography for evaluation of gastric lesions显示文摘AIM:To investigate the value of two-dimensional(2D) and three-dimensional(3D) double contrast-enhanced ultrasonography(DCUS) imaging for evaluation of gastric lesions.METHODS:2D and 3D DCUS imaging with both oral and intravenous administrations of contrast agents was used to assess gastroscopiclly-confirmed gastric lesions in 46 patients with benign and malignant diseases.Initially,liquid-based ultrasound contrast agent(Xinzhang) was given orally at dose of 500-600 mL for conventional ultrasound examination of the gastric lesions,and then a microbubble-based contrast agent(SonoVue) was injected intravenously at dose of 1.2-2.4 mL in bolus fashion to assess the perfusion pattern of the lesions using contrast imaging modes.The parameters derived from time-intensity curves including the arrival time(AT),time to peak(TTP),peak intensity(PI) and enhanced intensity(EI) were measured on the 2D DCUS imaging.3D DCUS of the lesions was acquired to demonstrate the value of this imaging mode.RESULTS:There were 22 cases with benign lesions including chronic gastritis(n = 5),gastric ulcer(n = 9),gastric polyps(n = 3),gastric stromal tumors(n = 5),and 24 cases with malignant lesions including gastric cancer(n = 20),gastric cardia carcinoma(n = 3) and post-operative recurrent gastric cancer(n = 1) in the study.The oral contrast-enhanced ultrasonography(CEUS) imaging of the stomach clearly demonstrated the anatomy of the stomach and morphologic features of gastric lesions.With optimal scanning window and imaging display under oral CEUS,intravenous CEUS clearly showed the perfusion of gastric lesions with various characteristic manifestations.Both 2D and 3D DCUS images clearly demonstrated normal gastric wall as a three-layer structure,from the inside out,hyperechoic mucosa,hypoechoic muscularis and hyperechoic serosa,respectively.There were statistical significant differences of AT(8.68 ± 2.06 vs 10.43 ± 2.75,P = 0.017),PI(34.64 ± 6.63 vs 29.58 ± 8.22,P = 0.023) and EI(29.72 ± 6.69 vs 22.66 ± 7.01,P = 0.001) between malignant lesions and normal gastric wall.However,no differences of AT,PI and EI between benign lesions and normal gastric wall tissue were found.3D DCUS could intuitively display morphological features and vascularities of the lesions with multiplanar and volume views.3D DCUS imaging provided comprehensive information complementary to 2D imaging.The crater or wellhead appearances and feeding vessels as well as distorted nourishing vasculature of gastric carcinoma were better seen with 3D imaging than 2D imaging.CONCLUSION:DCUS imaging can simultaneously display the anatomic and perfusion features of gastric lesions.3D DCUS can provide additional information to 2D DCUS for evaluation of gastric lesions.Hong Shi Xiu-Hua Yu Xin-Zhang Guo Yuan Guo Hong Zhang Bin Qian Zhang-Rui Wei Li Li Xian-Chen Wang Zi-Xiang Kong 2012World Journal of Gastroenterology2012,18,31:36

12A probiotic treatment containing Lactobacillus,Bifidobacterium and Enterococcus improves IBS symptoms in an open label trial显示文摘Objective: To evaluate the efficacy and safety of live combined Bifidobacterium, Lactobacillus and Enterococcus capsules in treatment of irritable bowel syndrome. Methods: Eighty-five patients [male 32, female 53; age (45.31±11.72) years] were given live combined Bifidobacterium, Lactobacillus and Enterococcus capsules 1260 mg/d t.i.d.×4 weeks. Syndrome scales were used to evaluate the efficacy in gastrointestinal syndrome. Fecal flora was also measured before and after the treatment. Six bacteria were cultured and the colony forming units were counted in stool. SPSS was used for data analysis. Results: Seventy-four patients finished the follow-up. No side-effect was found. For treatment of irritable bowel syndrome, the effective rate of live combined Bifidobacterium, Lactobacillus and Enterococcus capsules was 56.8% in the second week, 74.3% in the fourth week and 73.0% in the sixth week. Single symptom was improved, especially in abdominal pain and stool character. The probiotica con- taining live combined Bifidobacterium, Lactobacillus and Enterococcus could increase bifidobacterium count (P<0.01) and lac- tobacillus count (P<0.05); decrease bacteroides count (P<0.05) and enterococci count (P<0.01); No obvious changes were ob- served in clostridium difficile colonitis and enterobacteriaceae (P>0.05). Conclusion: The result of the study indicated that the administration of live combined Bifidobacterium, Lactobacillus and Enterococcus improved the symptom of irritable bowel syndrome and that there was a gradual increase of this effect. Thereafter conditions remained stable for 2 weeks. 14Geochronology and significance of volcanic rocks in the Ning-Wu Basin of China显示文摘The Ning-Wu(Nanjing-Wuhu) Basin is one of the most important volcanic basins in the Middle-Lower Yangtze River Valley,eastern China.It consists of four volcanic units,i.e.,the Longwangshan,Dawangshan,Gushan,and Niangniangshan Formations.Their LA-ICP MS U-Pb zircon ages are 134.8±1.8,132.2±1.6,129.5±0.8,and 126.8±0.6 Ma,respectively.Results of this study indicate that all volcanic rocks in the Ning-Wu Basin were formed in the Early Cretaceous from 135 to 127 Ma,lasting 8-10 m.y.No Jurassic volcanic activities occurred in any of the volcanic basins of the Middle-Lower Yangtze River Valley,including the Ning-Wu Basin.These new chronological results provide significant evidence for further study of this region to improve our understanding of Mesozoic tectonic,magmatic,and metallogenic processes of eastern China.ZHOU TaoFa FAN Yu YUAN Feng ZHANG LeJun QIAN Bin MA Liang YANG XiFei David R COOKE 2011Science China Earth Sciences2011,54,2:33

With the pump meridional section fixed, ten variables along the shroud and hub are selected to control the blade load by using a three-dimensional inverse design method. Hydraulic efficiency, along with impeller head, is applied as an optimization objective; and a radial basis neural network(RBNN) is adopted to approximate the objective function with 82 training samples. Local sensitivity analysis shows that decision variables have different impacts on the optimization objectives. Instead of randomly selecting one solution to implement, a technique for ordering preferences by similarity to ideal solution(TOPSIS) is introduced to select the best compromise solution(BCS) from pareto-front sets. The proposed method is applied to optimize the baseline model, i.e. a mixed- flow waterjet pump whose specific speed is 508 min?1?m3s?1?m. The performance of the waterjet pump was experimentally tested. Compared with the baseline model, the optimized impeller has a better hydraulic efficiency of 92% as well as a higher impeller head at the design operation point. Furthermore, the off-design performance is improved with a wider highefficiency operation range. After optimization, velocity gradients on the suction surface are smoother and flow separations are eliminated at the blade inlet part.
