32018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 442,573 cases of hepatectomy in China: a multicenter retrospective investigation显示文摘Hepatectomy is currently routinely performed in most hospitals in China. China owns the largest population of liver diseases and the biggest number of liver resection cases. A nationwide multicenter retrospective investigation involving 112 hospitals was performed, and focused on liver resection for patients with hepatocellular carcinoma(HCC). 42,573 cases of hepatectomy were enrolled, and 18,275 valid cases of liver resection for HCC patients were selected for statistical analysis. The epidemiology of HCC, distribution of hepatectomy, postoperative complications and prognosis were finally analyzed. In the 18,275 HCC patients,81% had hepatitis B virus infection and 10% had hepatitis C virus infection. 38% of the HCC patients had normal Alphafetoprotein(AFP) level, and other 35% had an AFP level lower than 400 ng mL^(-1). In the study period, 97% of the hepatectomy for HCC were treated with open surgery, and 23.81% had vascular exclusion techniques. The operation time was(191.7±105.6) min,the blood loss was(546.0±562.8) m L, and blood transfusion was(543.0±1,035.2) m L. The median survival for HCC patients was 631 days, with 1-, 3-, and 5-year overall survival of 73.2%, 28.8% and 19.6%, respectively. 5Morphological study on colonic pathology in ulcerative colitis treated by moxibustion显示文摘AIM To observe the therapeutic effect ofmoxibustion on ulcerative colitis and itsinfluence on the colonic mucosal morphology.METHODS Forty-six patients with ulcerativecolitis were randomly divided into themoxibustion with herbal medicine underneathgroup and the western medicine group.Thirtypatients were treated with the abovemoxibustion and 16 patients with Salicylayefapyridine(SASP).The colonic mucosa of 13patients in the moxibustion group was observedby colonoscopy before and after the treatment.Mucin was also analyzed by H.E and AB-PASstaining.RESULTS Seventeen patients were clinicallycured,12 were improved and 1 unchanged in themoxibustion group.In the control group,5patients were clinically cured,7 improved and 4unchanged.Thirteen patients with active UCwere taken as the subjects for histopathologicanalysis in this study.The colonic mucosallesions were remarkably improved and thecharacteristic of the mucin also changed.Inmost sections,the chronic inflammation ofmucosa was geatly ameliorated(P<0.01).Theinflammatory cell infiltratation much decreased and neutrophils,disapeared in most sections(P<0.001).The goblet cells significantlyincreased(P<0.001);crypt paracrypt abscessor mucosal ulceration was seen(P<0.001).CONCLUSION The rate of cure of ulcerative colitis by moxibustion with herbal medicine beneath is superior to that by SASP. 6Efficacy and safety of Xinfeng capsule in patients with rheumatoid arthritis:a multi-center parallel-group double-blind randomized controlled trial显示文摘OBJECTIVE:To evaluate the efficacy and safety of Xinfeng capsule in patients suffering rheumatoid arthritis(RA).METHODS:A multi-center parallel-group designed,double-blind,randomized,controlled trial was conducted.Totally 304 RA patients were assigned to two groups:one group was administered Xinfeng capsule(XFC) plus the placebo of leflunomide and the other given leflunomide(LEF) plus the placebo of XFC for twelve weeks.The clinical and laboratory parameters were compared at baseline and fourth,eighth,and twelfth weeks.RESULTS:After twelve-week treatment,patients in two groups all showed some trend of effectiveness when compared in terms of American Rheumatism Association(ACR) recommended 20%,50%,70%improvement criteria,but it was insignificant.The validity in ameliorate modified disease activity score(DAS28) and laboratory indexes as erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF) were also found no difference.The score of health assessment questionnaire(HAQ),self-rating anxiety scale(SAS),self-rating depression scale(SDS) and quality of life questionnaire with rheumatoid arthritis(RAQOL)both lower than the first week and the changes showed no difference.However,the score of SDS dropped more in XFC group than in the other.A total of 147 adverse reaction cases were reported,which shows no difference between the two groups.The most common adverse reactions were hepatic impairment,anemia,leukocytopenia,epigastric discomfort and phalacrosis.CONCLUSION:XFC demonstrated better improvement in the scores of SDS and compared with those of LEF group.Liu Jian Wang Yuan Huang Chuanbing Xu Jianhua Li Zhijun Xu Liang He Liyun Sun Yue Wang Yali Xu Shengqian Zhao Ping Mao Tongjun Tan Bin Zhu Fubing Zhang Pingheng Fang Li 2015Journal of Traditional Chinese Medicine2015,35,5:27

9New classification of the anatomic variations of cystic artery during laparoscopic cholecystectomy显示文摘AIM:To investigate the anatomic variations in the cystic artery by laparoscopy,and to provide a new classification system for the guidance of laparoscopic surgeons.METHODS:Six hundred patients treated with laparoscopic cholecystectomy from June 2005 to May 2006 were studied retrospectively.The laparoscope of 30?(Stryker,American)was applied.Anatomic structures of cystic artery and conditions of Calot's triangle under laparoscope were recorded respectively.RESULTS:Laparoscopy has revealed there are many anatomic variations of the cystic artery that occur frequently.Based on our experience with 600 laparoscopic cholecystectomies,we present a new classification of anatomic variations of the cystic artery,which can be divided into three groups:(1)Calot's triangle type,found in 513 patients(85.5%);(2)outside Calot's triangle,found in 78 patients(13%);(3)compound type,observed in 9 patients(1.5%).CONCLUSION:Our classification of the anatomic variations of the cystic artery will be useful for decreasing uncontrollable cystic artery hemorrhage,and avoiding extrahepatic bile duct injury.You-Ming Ding Bin Wang Wei-Xing Wang Ping Wang Ji-Shen Yan 2007World Journal of Gastroenterology2007,13,42:16
10Removal of metals by sorghum plants from contaminated land显示文摘The growth of high biomass crops facilitated by optimal of agronomic practices has been considered as an alternative to phytoremediation of soils contaminated by heavy metals. A field trial was carried out to evaluate the phytoextraction effciency of heavy metals by three varieties of sweet sorghum (Sorghum biocolor L.), a high biomass energy plant. Ethylene diamine tetraacetate (EDTA), ammonium nitrate (NH4NO3) and ammonium sulphate ((NH4)2SO4) were tested for their abilities to enhance the removal of heavy metals Pb, Cd, Zn, and Cu by sweet sorghum from a contaminated agricultural soil. Sorghum plants always achieved the greatest removal of Pb by leaves and the greatest removal of Cd, Zn and Cu by stems. There was no significant difference among the Keller, Rio and Mray varieties of sweet sorghums in accumulating heavy metals. EDTA treatment was more effcient than ammonium nitrate and ammonium sulphate in promoting Pb accumulation in sweet sorghum from the contaminated agricultural soil. The application of ammonium nitrate and ammonium sulphate increased the accumulation of both Zn and Cd in roots of sorghum plants. 11Microwave Exposure Impairs Synaptic Plasticity in the Rat Hippocampus and PC12 Cells through Over-activation of the NMDA Receptor Signaling Pathway显示文摘Objective The aim of this study is to investigate whether microwave exposure would affect the N-methyl-D-aspartate receptor(NMDAR) signaling pathway to establish whether this plays a role in synaptic plasticity impairment. Methods 48 male Wistar rats were exposed to 30 m W/cm2 microwave for 10 min every other day for three times. Hippocampal structure was observed through H&E staining and transmission electron microscope. PC12 cells were exposed to 30 m W/cm2 microwave for 5 min and the synapse morphology was visualized with scanning electron microscope and atomic force microscope. The release of amino acid neurotransmitters and calcium influx were detected. The expressions of several key NMDAR signaling molecules were evaluated. Results Microwave exposure caused injury in rat hippocampal structure and PC12 cells, especially the structure and quantity of synapses. The ratio of glutamic acid and gamma-aminobutyric acid neurotransmitters was increased and the intracellular calcium level was elevated in PC12 cells. A significant change in NMDAR subunits(NR1, NR2 A, and NR2B) and related signaling molecules(Ca2+/calmodulin-dependent kinase II gamma and phosphorylated c AMP-response element binding protein) were examined. Conclusion 30 m W/cm2 microwave exposure resulted in alterations of synaptic structure, amino acid neurotransmitter release and calcium influx. 16Prognostic evaluation of postoperative adjuvant therapy for operable cervical cancer:10 years'experience of National Cancer Center in China显示文摘Objective: The aim of this study was to investigate the prognostic factors and to evaluate the impact of adjuvant therapy on clinical outcome for early-stage cervical cancer.Methods: The clinical-pathological data of all 1,335 patients with the International Federation of Gynecology and Obstetrics(FIGO) Ib–IIa cervical cancer treated with primary radical surgery at the Chinese National Cancer Center between May 2007 and Dec 2013 were retrospectively reviewed.The median follow-up was 70 months.Results: Of all the patients,61.6% of the cases received adjuvant therapy,with 5-year disease-free survival(DFS)of 92.1% and 5-year overall survival(OS) of 95.0%.In multivariate analysis,differentiation of G3(P<0.05),lymph node metastasis(LNM,P<0.05) and lymphovascular space invasion(LVSI,P<0.05) were independent predictors for OS,while LNM(P<0.05),deep stroma invasion(DSI,P<0.05) and LVSI(P<0.05) were independent factors for DFS.The samples were stratified by histologic type,and cervical squamous cell carcinoma(SCC) was found to share the same independent factors except for differentiation of OS.As to patients with cervical adenocarcinoma/adenosquamous carcinoma(AC/ASC),differentiation was the independent predictor of OS(P<0.05); and LVSI of DFS(P<0.05).Of 236 patients with high-risk factors,there was no significant difference in survival between concurrent chemoradiotherapy(CCRT,n=195),radiotherapy(RT,n=24),and chemotherapy(CT,n=17).Among the 190 patients with LNM who underwent CCRT,124 cases showed improved DFS after sequential CT(P=0.118),with a recurrence rate decrease of 14%,though the difference was not statistically significant.Patients with single intermediate-risk factors like DSI or LVSI were found to partially benefit from adjuvant therapy,but the difference was not statistically significant.Conclusions: LNM,LVSI,DSI and differentiation were found to be independent prognostic factors for operable cervical cancer.Aggressive postoperative adjuvant therapy based on single risk factors in Chinese National Cancer Center could benefit survival.CCRT+CT outperformed CCRT in high-risk patients.For patients with single non-high-risk factor,the role of adjuvant therapy needs to be further discussed.Tong Shu Dan Zhao Bin Li Yating Wang Shuanghuan Liu Pingping Li Jing Zuo Ping Ba i RongZhang Lingying Wu 2017Chinese Journal of Cancer Research2017,29,6:12
17Excitation and Detection of Shear Horizontal Waves with Electromagnetic Acoustic Transducers for Nondestructive Testing of Plates显示文摘The fundamental shear horizontal(SH0) wave has several unique features that are attractive for long-range nondestructive testing(NDT). By a careful design of the geometric configuration, electromagnetic acoustic transducers(EMATs) have the capability to generate a wide range of guided wave modes, such as Lamb waves and shear-horizontal(SH) waves in plates. However, the performance of EMATs is influenced by their parameters. To evaluate the performance of periodic permanent magnet(PPM) EMATs, a distributed-line-source model is developed to calculate the angular acoustic field cross-section in the far-field. Numerical analysis is conducted to investigate the performance of such EMATs with different geometric parameters, such as period and number of magnet arrays, and inner and outer coil widths. Such parameters have a great influence on the directivity of the generated SH0 waves that arises mainly in the amplitude and width of both main and side lobes. According to the numerical analysis, these parameters are optimized to obtain better directivity. Optimized PPM EMATs are designed and used for NDT of strip plates. Experimental results show that the lateral boundary of the strip plate has no perceivable influence on SH0-wave propagation, thus validating their used in NDT.
