| 1 | Artificial intelligence in gastrointestinal endoscopy:The future is almost here显示文摘Artificial intelligence(AI) enables machines to provide unparalleled value in a myriad of industries and applications. In recent years, researchers have harnessed artificial intelligence to analyze large-volume, unstructured medical data and perform clinical tasks, such as the identification of diabetic retinopathy or the diagnosis of cutaneous malignancies. Applications of artificial intelligence techniques, specifically machine learning and more recently deep learning, are beginning to emerge in gastrointestinal endoscopy. The most promising of these efforts have been in computeraided detection and computer-aided diagnosis of colorectal polyps, with recent systems demonstrating high sensitivity and accuracy even when compared to expert human endoscopists. AI has also been utilized to identify gastrointestinal bleeding, to detect areas of inflammation, and even to diagnose certain gastrointestinal infections. | 7 | Overexpression of NK2 inhibits liver regeneration after partial hepatectomy in mice显示文摘AIM: To investigate the in vivo effects of NK2 on liver regeneration after partial hepatectomy (PH). METHODS: Survival after PH was observed with 21 NK2 transgenic mice and 23 wild-type (WT) mice over 10 d. Liver regeneration was analyzed using histology and immunohistochemistry. Expressions of genes were analyzed using Northern blot analysis, immunoprecipitation and immunoblotting, and reverse transcriptase polymerase chain reaction assay. KaplanMeier method and the log-rank test were used for ahalyzing the survival after PH. Differences in the resultsof immunohistochemistry and percentage of liver regeneration was determined by the Student's t-test. RESULTS: More than half of NK2 transgenic mice died within 48 h after PH. After PH, increased deposition of small lipid droplets in hepatocytes was evident and hepatic proliferation was inhibited in NK2 transgenic mice. The hepatic expression and kinase activity of HGF receptor, c-Met, were unchanged among WT mice and NK2 transgenic mice after PH. | 15 | Role of nuclear receptor CAR in carbon tetrachloride-induced hepatotoxicity显示文摘AIM: To investigate the precise roles of CAR in CCl4-induced acute hepatotoxicity.METHODS: To prepare an acute liver injury model, CCl4 was intraperitoneally injected in CAR+/+ and CAR-/- mice.RESULTS: Elevation of serum alanine aminotransferase and extension of centrilobular necrosis were slightly inhibited in CAR-/- mice compared to CAR+/+ mice without PB. Administration of a CAR inducer, PB, revealed that CCl4-induced liver toxicity was partially inhibited in CAR-/- mice compared with CAR+/+ mice. On the other hand,androstanol, an inverse agonist ligand, inhibited hepatotoxicity in CAR+/+ but not in CAR-/- mice. Thus, CAR activation caused CCl4 hepatotoxicity while CAR inhibition resulted in partial protection against CCl4-induced hepatotoxicity.There were no differences in the expression of CYP2E1, the main metabolizing enzyme for CCl4, between CAR+/+ and CAR-/- mice. However, the expression of other CCl4-metabolizing enzymes, such as CYP2B10 and 3A11, was induced by PB in CAR+/+ but not in CAR-/- mice. Although the main pathway of CCl4-induced acute liver injury is mediated by CYP2E1, CAR modulates its pathway via induction of CYP2B10 and 3A11 in the presence of activator or inhibitor.CONCLUSION: The nuclear receptor CAR modulates CCl4-induced liver injury via induction of CCl4-metabolizing enzymes in the presence of an activator. | 20 | Fulminant amebic colitis in a patient with concomitant cytomegalovirus infection after systemic steroid therapy:A case report显示文摘BACKGROUND Amebic colitis is an infection caused by Entamoeba histolytica and most commonly observed in regions with poor sanitation.It is also seen as a sexually transmitted disease in developed countries.While amebic colitis usually has a chronic course with repeated exacerbations and remissions,it may also manifest as a fulminant form that rapidly progresses and leads to severe,life-threatening complications,such as intestinal perforation,peritonitis,and sepsis,that have a high mortality rate.CASE SUMMARY A 68-year-old man was admitted to our hospital with chest pain and acute dyspnea.He was diagnosed with acute coronary syndrome,acute heart failure,and bacterial pneumonia.His respiratory condition worsened despite receiving intensive care and intravenous antibiotics.On the fifth day of hospitalization,he was diagnosed with acute respiratory distress syndrome and was started on steroid therapy.He subsequently developed bloody stools and was diagnosed with cytomegalovirus(CMV)enterocolitis based on biopsy results and a peripheral blood CMV pp65 antigenemia test result.Although we started antiviral therapy with ganciclovir,which was successful in reducing his antigen titers,he continued to have bloody diarrhea.Three weeks after initiation of ganciclovir therapy and six weeks after his admission,the patient died from intestinal perforation.We only posthumously diagnosed him with amebic colitis and CMV enterocolitis based on autopsy findings of transmural necrosis of the entire colon with massive ameba infiltration.CONCLUSION We urge clinicians to consider Entamoeba histolytica infection if severe colitis progresses after steroid therapy.Preemptive treatment is recommended then. | Naoki Shijubou Toshiyuki Sumi Koki Kamada Takeyuki Sawai Yuichi Yamada Tatsuru Ikeda HisashiNakata Yuji Mori Hirofumi Chiba | 2021 | World Journal of Clinical Cases2021,9,15: | 0 |
