1Conservative management of perforated duodenal diverticulum: A case report and review of the literature显示文摘Duodenal diverticula are a relatively common condition. They are asymptomatic, unless they become complicated, with perforation being the rarest but most severe complication. Surgical treatment is the most frequently performed approach. We report the case of a patient with a perforated duodenal diverticulum, which was diagnosed early and treated conservatively with antibiotics and percutaneous drainage of secondary retroperitoneal abscesses. 2A scoping review on the field validation and implementation of rapid diagnostic tests for vector-borne and other infectious diseases of poverty in urban areas显示文摘Background:Health personnel face challenges in diagnosing vector-borne and other diseases of poverty in urban settings.There is a need to know what rapid diagnostic technologies are available,have been properly assessed,and are being implemented to improve control of these diseases in the urban context.This paper characterizes evidence on the field validation and implementation in urban areas of rapid diagnostics for vector-borne diseases and other diseases of poverty.Main body:A scoping review was conducted.Peer-reviewed and grey literature were searched using terms describing the targeted infectious diseases,diagnostics evaluations,rapid tests,and urban setting.The review was limited to studies published between 2000 and 2016 in English,Spanish,French,and Portuguese.Inclusion and exclusion criteria were refined post hoc to identify relevant literature regardless of study design and geography.A total of 179 documents of the 7806 initially screened were included in the analysis.Malaria(n=100)and tuberculosis(n=47)accounted for the majority of studies that reported diagnostics performance,impact,and implementation outcomes.Fewer studies,assessing mainly performance,were identified for visceral leishmaniasis(n=9),filariasis and leptospirosis(each n=5),enteric fever and schistosomiasis(each n=3),dengue and leprosy(each n=2),and Chagas disease,human African trypanosomiasis,and cholera(each n=1).Reported sensitivity of rapid tests was variable depending on several factors.Overall,specificities were high(>80%),except for schistosomiasis and cholera.Impact and implementation outcomes,mainly acceptability and cost,followed by adoption,feasibility,and sustainability of rapid tests are being evaluated in the field.Challenges to implementing rapid tests range from cultural to technical and administrative issues.Conclusions:Rapid diagnostic tests for vector-borne and other diseases of poverty are being used in the urban context with demonstrated impact on case detection.However,most evidence comes from malaria rapid diagnostics,with variable results.While rapid tests for tuberculosis and visceral leishmaniasis require further implementation studies,more evidence on performance of current tests or development of new alternatives is needed for dengue,Chagas disease,filariasis,leptospirosis,enteric fever,human African trypanosomiasis,schistosomiasis and cholera.Lyda Osorio Jonny Alejandro Garcia Luis Gabriel Parra Victor Garcia Laura Torres Stephanie Degroote Valery Ridde 2018Infectious Diseases of Poverty2018,7,1:2

10Ecological succession of benthic organisms on niche-type artificial reefs显示文摘As part of efforts to restore fishery resources and recover damaged coastal ecosystems,artificial reefs are often anchored on the seafloor in coastal zones,to provide new habitats for marine organisms.The aim of the study was to describe the structure of a community of benthic invertebrates colonising a niche-type artificial reef(AATN in Spanish).Nine structures were anchored at depths of 16±1.5 m for 99 weeks(22 months)in the Area of Benthic Resource Management and Exploitation(AMERB)in coastal waters of the Region of Bio Bio,Chile.The results showed that,at 3 months from submersion of the NTAR,much of the artificial substrate remained bare and there were only low levels of specimens of Balanus sp.barnacles,showing mean coverage of 11.26%,and even lower proportions of Rhodophyta,with mean coverage of 0.34%.At 6 months,the presence of hydrozoans was seen,decreasing the coverage of the barnacles,which was aided further at 8 months with the arrival of barnacle predatory invertebrates.At 8 months,new benthic invertebrates appeared and competed for the substrate.These included sponges and algae.At 99 weeks,the hydrozoans dominated the substrate,followed by barnacles and Rhodophyta,the first colonising organisms,leading to colonisation by motile macro-invertebrates,mainly consisting of crustaceans,echinoderms and molluscs.The AATN artificial system provides an ideal substrate for the development of early ecological succession and the use of this technology should be feasible in the recovery process of habitat damaged by anthropogenic actions and climate change.María-Isabel Toledo Pamela Torres Cristina Díaz Victor Zamora Jesús López Germán Olivares 2020Ecological Processes2020,9,1:1

12Effect of homogenization process on the hardness of Zn-Al-Cu alloys显示文摘The effect of a homogenizing treatment on the hardness of as-cast Zn–Al–Cu alloys was investigated. Eight alloy compositions were prepared and homogenized at 350 °C for 180 h, and their Rockwell 'B' hardness was subsequently measured. All the specimens were analyzed by X-ray diffraction and metallographically prepared for observation by optical microscopy and scanning electron microscopy. The results of the present work indicated that the hardness of both alloys(as-cast and homogenized) increased with increasing Al and Cu contents; this increased hardness is likely related to the presence of the θ and τ′ phases. 15How do we manage post-OLT redundant bile duct?显示文摘AIM: To address endoscopic outcomes of post-Orthotopic liver transplantation (OLT) patients diagnosed with a 'redundant bile duct' (RBD). METHODS: Medical records of patients who underwent OLT at the Liver Transplant Center, University Texas Health Science Center at San Antonio Texas were retrospectively analyzed. Patients with suspected biliary tract complications (BTC) underwent endoscopic retrograde cholangiopancreatography (ERCP). All ERCP were performed by experienced biliary endoscopist. RBD was defined as a looped, sigmoid-shaped bile duct on cholangiogram with associated cholestatic liver biomarkers. Patients with biliary T-tube placement, biliary anastomotic strictures, bile leaks, bile-duct stonessludge and suspected sphincter of oddi dysfunction were excluded. Therapy included single or multiple biliary stents with or without sphincterotomy. The incidence of RBD, the number of ERCP corrective sessions, and the type of endoscopic interventions were recorded. Successful response to endoscopic therapy was defined as resolution of RBD with normalization of associated cholestasis. Laboratory data and pertinent radiographic imaging noted included the pre-ERCP period and a follow up period of 6-12 mo after the last ERCP intervention. RESULTS: One thousand two hundred and eighty-two patient records who received OLT from 1992 through 2011 were reviewed. Two hundred and twenty-four patients underwent ERCP for suspected BTC. RBD was reported in each of the initial cholangiograms. Twentyone out of 1282 (1.6%) were identified as having RBD. There were 12 men and 9 women, average age of 59.6 years. Primary indication for ERCP was cholestatic pattern of liver associated biomarkers. Nineteen out of 21 patients underwent endoscopic therapy and 2/21 required immediate surgical intervention. In the endoscopically managed group: 65 ERCP procedures were performed with an average of 3.4 per patient and 1.1 stent per session. Fifteen out of 19 (78.9%) patients were successfully managed with biliary stenting. All stents were plastic. Selection of stent size and length were based upon endoscopist preference. Stent size ranged from 7 to 11.5 Fr (average stent size 10 Fr); Stent length ranged from 6 to 15 cm (average length 9 cm). Concurrent biliary sphincterotomy was performed in 10/19 patients. Single ERCP session was sufficient in 6/15 (40.0%) patients, whereas 4/15 (26.7%) patients needed two ERCP sessions and 5/15 (33.3%) patients required more than two (average of 5.4 ERCP procedures). Single biliary stent was sufficient in 5 patients; the remaining patients required an average of 4.9 stents. Four out of 19 (21.1%) patients failed endotherapy (lack of resolution of RBD and recurrent cholestasis in the absence of biliary stent) and required either choledocojejunostomy (2/4) or percutaneous biliary drainage (2/4). Endoscopic complications included: 2/65 (3%) post-ERCP pancreatitis and 2/10 (20%)non-complicated post-sphincterotomy bleeding. No endoscopic related mortality was found. The medical records of the 15 successful endoscopically managed patients were reviewed for a period of one year after removal of all biliary stents. Eleven patients had continued resolution of cholestatic biomarkers (73%). One patient had recurrent hepatitis C, 2 patients suffered septic shock which was not associated with ERCP and 1 patient was transferred care to an outside provider and records were not available for our review. CONCLUSION: Although surgical biliary reconstruction techniques have improved, RBD represents a postOLT complication. These include flaring stars, supernova remnants, accreting white dwarfs, low and high mass X-ray binaries, radio quiet and radio loud active galactic nuclei, tidal disruption events, and gamma-ray bursts. eXTP will be excellently suited to study one common aspect of these objects: their often transient nature.
