| 1 | Liver stiffness in the hepatitis B virus carrier:A non-invasive marker of liver disease influenced by the pattern of transaminases显示文摘AIM: To investigate the usefulness of transient elastography by Fibroscan (FS),a rapid non-invasive technique to evaluate liver fibrosis,in the management of chronic hepatitis B virus (HBV) carriers.METHODS: In 297 consecutive HBV carriers,we studied the correlation between liver stiffness (LS),stage of liver disease and other factors potentially influencing FS measurements.In 87 chronic hepatitis B (CHB) patients,we monitored the FS variations according to the spontaneous or treatment-induced variations of biochemical activity during follow-up.RESULTS: FS values were 12.3 ± 3.3 kPa in acute hepatitis,10.3 ± 8.8 kPa in chronic hepatitis,4.3 ± 1.0 kPa in inactive carriers and 4.6 ± 1.2 kPa in blood donors.We identified the cut-offs of 7.5 and 11.8 kPa for the diagnosis of fibrosis ≥ S3 and cirrhosis respectively,showing 93.9% and 86.5% sensitivity,88.5% and 96.3% specificity,76.7% and 86.7% positive predictive value (PPV),97.3% and 96.3% negative predictive value (NPV) and 90.1% and 94.2% diagnostic accuracy.At multivariate analysis in 171 untreated carriers,fibrosis stage (t = 13.187,P < 0.001),active vs inactive HBV infection (t = 6.437,P < 0.001),alanine aminotransferase (ALT) (t = 4.740,P < 0.001) and HBV-DNA levels (t = -2.046,P = 0.042) were independently associated with FS.Necroinflammation score (t = 2.158,> 10/18 vs ≤ 10/18,P = 0.035) and ALT levels (t = 3.566,P = 0.001) were independently associated with LS in 83 untreated patients without cirrhosis and long-term biochemical remission (t = 4.662,P < 0.001) in 80 treated patients.During FS monitoring (mean follow-up 19.9 ± 7.1 mo) FS values paralleled those of ALT in patients with hepatitis exacerbation (with 1.2 to 4.4-fold increases in CHB patients) and showed a progressive decrease during antiviral therapy.CONCLUSION: FS is a non-invasive tool to monitor liver disease in chronic HBV carriers,provided that the pattern of biochemical activity is taken into account.In the inactive carrier,it identifies non-HBV-related causes of liver damage and transient reactivations.In CHB patients,it may warrant a more appropriate timing of control liver biopsies. | Filippo Oliveri Barbara Coco Pietro Ciccorossi Piero Colombatto Veronica Romagnoli Beatrice Cherubini Ferruccio Bonino Maurizia Rossana Brunetto | 2008 | World Journal of Gastroenterology2008,14,40: | 43 |

Native to India, it is now widely distributed throughout tropical and subtropical regions of the world. Its different parts are sources of proteins, vitamins and minerals and present different pharmacological and biotechnological potential. Moreover, M. oleifera seeds are widely used in water and effluent treatment, for their coagulation, flocculation and sedimentation properties, their ability of improving water quality, by reducing organic matter and microbial load, with special applicability in intensive animal production systems, such as aquaculture. In addition, due to its high nutritional value and several medicinal properties, this tree may act as a nutritional and medical alternative for socially neglected populations. | 5 | Alternate Wetting and Drying of Rice Reduced CH4 Emissions but Triggered N2O Peaks in a Clayey Soil of Central Italy显示文摘Reducing CH_4 and N_2O emissions from rice cropping systems while sustaining production levels with less water requires a better understanding of the key processes involved. Alternate wetting and drying(AWD) irrigation is one promising practice that has been shown to reduce CH_4 emissions. However, little is known about the impact of this practice on N_2O emissions, in particular under Mediterranean climate. To close this knowledge gap, we assessed how AWD influenced grain yield, fluxes and annual budgets of CH_4 and N_2O emissions, and global warming potential(GWP) in Italian rice systems over a 2-year period. Overall, a larger GWP was observed under AWD, as a result of high N_2O emissions which offset reductions in CH_4 emissions. In the first year, with 70% water reduction, the yields were reduced by 33%, CH_4 emissions decreased by 97%, while N_2O emissions increased by more than 5-fold under AWD as compared to PF; in the second year, with a 40% water saving, the reductions of rice yields and CH_4 emissions(13% and11%, respectively) were not significant, but N_2O fluxes more than doubled. The transition from anaerobic to aerobic soil conditions resulted in the highest N_2O fluxes under AWD. | 12 | Metronomic chemotherapy for non-metastatic triple negative breast cancer: Selection is the key显示文摘Triple negative breast cancer(TNBC) accounts for 15%-20% of all breast cancer, and is still defined as what it is not. Currently, TNBC is the only type of breast cancer for which there are no approved targeted therapies and maximum tolerated dose chemotherapy with taxanes and anthracycline-containing regimens is still the standard of care in both the neoadjuvant and adjuvant settings. In the last years, metronomic chemotherapy(MC) is being explored as an alternative to improve outcomes in TNBC. In the neoadjuvant setting, purely metronomic and hybrid approaches have been developed with the objective of increasing complete pathologic response(p CR) and prolonging disease free survival. These regimens proved to be very effective achieving pC R rates between 47%-60%, but at the cost of great toxicity. In the adjuvant setting, MC is used to intensify adjuvant chemotherapy and, more promisingly, as maintenance therapy for high-risk patients, especially those with no pC R after neoadjuvant chemotherapy. Considering the dismal prognosis of TNBC, any strategy that potentially improves outcomes, specially being the oral agents broadly available and inexpensive, should be considered and certainly warrants further exploration. | 13 | Levels and trends of poly-and perfluoroalkyl substances in the Arctic environment-An update显示文摘Poly-and perfluoroalkyl substances(PFASs)are important environmental contaminants globally and in the early 2000s they were shown to be ubiquitous contaminants in Arctic wildlife.Previous reviews by Butt et al.and Letcher et al.have covered studies on levels and trends of PFASs in the Arctic that were available to 2009.The purpose of this review is to focus on more recent work,generally published between 2009 and 2018,with emphasis on PFASs of emerging concern such as perfluoroalkyl carboxylates(PFCAs)and short-chain perfluoroalkyl sulfonates(PFSAs)and their precursors.Atmospheric measurements over the period 2006e2014 have shown that fluorotelomer alcohols(FTOHs)as well as perfluorobutanoic acid(PFBA)and perfluoroctanoic acid(PFOA)are the most prominent PFASs in the arctic atmosphere,all with increasing concentrations at Alert although PFOA concentrations declined at the Zeppelin Station(Svalbard).Results from ice cores show generally increasing deposition of PFCAs on the Devon Ice cap in the Canadian arctic while declining fluxes were found in a glacier on Svalbard.An extensive dataset exists for long-term trends of long-chain PFCAs that have been reported in Arctic biota with some datasets including archived samples from the 1970s and 1980s.Trends in PFCAs over time vary among the same species across the North American Arctic,East and West Greenland,and Svalbard.Most long term time series show a decline from higher concentrations in the early 2000s.However there have been recent(post 2010)increasing trends of PFCAs in ringed seals in the Canadian Arctic,East Greenland polar bears and in arctic foxes in Svalbard.Annual biological sampling is helping to determine these relatively short term changes.Rising levels of some PFCAs have been explained by continued emissions of long-chain PFCAs and/or their precursors and inflows to the Arctic Ocean,especially from the North Atlantic.While the effectiveness of biological sampling for temporal trends in long-chain PFCAs and PFSAs has been demonstrated,this does not apply to the C4eC8ePFCAs,perfluorobutane sulfonamide(FBSA),or perfluorobutane sulfonate(PFBS)which are generally present at low concentrations in biota.In addition to air sampling,sampling abiotic media such as glacial cores,and annual sampling of lake waters and seawater would appear to be the best approaches for investigating trends in the less bioaccumulative PFASs. | Derek Muir Rossana Bossi Pernilla Carlsson Marlene Evans Amila De Silva Crispin Halsall Cassandra Rauert Dorte Herzke Hayley Hung Robert Letcher Frank Riget Anna Roos | 2019 | Emerging Contaminants2019,5,1: | 3 |
| 14 | Clinical epidemiology of chronic viral hepatitis B:A Tuscany real-world large-scale cohort study显示文摘AIM To build a regional database of chronic patients to define the clinical epidemiology of hepatitis B virus(HBV)-infected patients in the Tuscan public health care system.METHODS This study used a cross-sectional cohort design. We evaluated chronic viral hepatitis patients with HBV referred to the outpatient services of 16 hospital units. Information in the case report forms included main demographic data, blood chemistry data, viral hepatitis markers, instrumental evaluations, and eligibility for treatment or ongoing therapy and liver transplantation. RESULTS Of 4015 chronic viral hepatitis patients, 1096(27.3%) were HBV infected. The case report form was correctly completed for only 833 patients(64% males, 36% females; mean age 50.1 ± 15.4). Of these HBV-infected patients, 73% were Caucasian, 21% Asian, 4% Central African, 1% North African and 1% American. Stratifying patients by age and nationality, we found that 21.7% of HBV-infected patients were aged < 34 years(only 2.8% were Italian). The most represented routes of transmission were nosocomial/dental procedures(23%), mother-to-child(17%) and sexual transmission(12%). The most represented HBV genotypes were D(72%) and A(14%). Of the patients, 24.7% of patients were HBe Ag positive, and 75.3% were HBe Ag negative. Of the HBV patients 7% were anti-HDV positive. In the whole cohort, 26.9% were cirrhotic(35.8% aged < 45 years), and 47% were eligible for or currently undergoing treatment, of whom 41.9 % were cirrhotic. CONCLUSION Only 27.3% of chronic viral hepatitis patients were HBV infected. Our results provide evidence of HBV infection in people aged < 34 years, especially in the foreign population not protected by vaccination. Surgery is the first choice in GEP-NENs patients with localized disease whilst in the metastatic setting many other treatment options are available. Somatostatin analogues are indicated for symptoms control in functioning tumours. Furthermore they may be effective to inhibit tumour progression. GEP-NENs pathogenesis has been extensively studied in the last years therefore several driver mutations pathway genes have been identified as crucial factors in their tumourigenesis. GEP-NENs can over-express vascular endothelial growth factor(VEGF), basic-fibroblastic growth factor, transforming growth factor(TGF-α and-β), platelet derived growth factor(PDGF), insulin-like growth factor-1(IGF-1) and their receptors PDGF receptor, IGF-1 receptor, epidermal growth factor receptor, VEGF receptor, and c-kit(stem cell factor receptor) that can be considered as potential targets. The availability of new targeted agents, such as everolimus and sunitinib that are effective in advanced and metastatic pancreatic neuroendocrine tumours, has provided new treatment opportunities. | 18 | Brain-gut-liver interactions across the spectrum of insulin resistance in metabolic fatty liver disease显示文摘Metabolic associated fatty liver disease(MAFLD),formerly named“nonalcoholic fatty liver disease”occurs in about one-third of the general population of developed countries worldwide and behaves as a major morbidity and mortality risk factor for major causes of death,such as cardio-vascular,digestive,metabolic,neoplastic and neuro-degenerative diseases.However,progression of MAFLD and its associated systemic complications occur almost invariably in patients who experience the additional burden of intrahepatic and/or systemic inflammation,which acts as disease accelerator.Our review is focused on the new knowledge about the brain-gut-liver axis in the context of metabolic dysregulations associated with fatty liver,where insulin resistance has been assumed to play an important role.Special emphasis has been given to digital imaging studies and in particular to positron emission tomography,as it represents a unique opportunity for the noninvasive in vivo study of tissue metabolism.An exhaustive revision of targeted animal models is also provided in order to clarify what the available preclinical evidence suggests for the causal interactions between fatty liver,dysregulated endogenous glucose production and insulin resistance. | Eleni Rebelos Patricia Iozzo Maria Angela Guzzardi Maurizia Rossana Brunetto Ferruccio Bonino | 2021 | World Journal of Gastroenterology2021,27,30: | 2 |
| 19 | Gastro-entero-pancreatic neuroendocrine tumors:Is now time for a new approach?显示文摘Gastro-entero-pancreatic tumors(GEP-NETs) are rare neoplasms often characterized by an overexpression of somatostatin receptors.Thus,radiolabeled somatostatin analogues have showed an increasing relevance both in diagnosis and treatment,especially in low-and intermediate-differentiated GEP-NETs.These evidences have led to a growing development of new functional imaging techniques as 68 Ga-DOTATATE positron emission tomography/computed tomography(PET/CT) proved useful in the management of these neoplasms.However these tumors have a heterogeneous behavior also modifying their aggressiveness through time.Therefore sometimes 18 F-fluorodeoxyglucose PET/CT appears to be more appropriate to obtain a better assessment of the disease.According to these considerations,the combination of different functional imaging techniques should be considered in the management of GEP-NETs patients allowing clinicians to choose the tailored therapeutic approach among available options. | Rossana Berardi Mariangela Torniai Agnese Savini Silvia Rinaldi Stefano Cascinu | 2016 | World Journal of Clinical Oncology2016,7,2: | 2 |
