| 5 | Robotic assisted radical prostatectomy accelerates postoperative stress recovery: Final results of a contemporary prospective study assessing pathophysiology of cortisol peri-operative kinetics in prostate cancer surgery显示文摘Objective:To investigate the effects of prostate cancer(PCa)surgery on the stress system and to identify potential independent factors associating with stress recovery.Methods:The design of the study was prospective and PCa surgery included robot assisted radical prostatectomy(RARP)or retropubic radical prostatectomy(RRP).Between February 2013 to December 2014,315 consecutive patients were evaluated.The effects of PCa surgery on the stress system were measured by cortisol serum levels before and after surgery on postoperative day(POD)0,1,3,5 and 45.Cortisol variations in the population and subpopulation(RARP vs.RRP)of patients were investigated by statistical methods.Factors associating with stress recovery were assessed by simple linear regression(SLR)and multiple linear regression(MLR)analysis.Results:RARP was performed in 75.9%of cases.In the patient population,there were wide serum cortisol perioperative variations.PCa surgery triggered the stress system which immediately(POD 0)responded by cortisol overproduction which induced the negative feedback mechanism that started on POD 1,continued on POD 3,was still ongoing on POD 5 and completely settled on POD 45(stress recovery).In the subpopulation of patients,significantly lower cortisol serum levels were detected on POD 3e5 in RARP cases inwhomcortisol levels were close to preoperative levels(stress recovery)on POD 5.Independent predictive factors of serum cortisol on POD 5(stress recovery)were preoperative cortisol(p Z 0.02),cortisol levels on POD 3(p<0.0001)and RARP(p Z 0.03)in which the association was negative(stress recovery faster than RRP).Conclusion:Our study shows that PCa surgery immediately(POD 0)triggers the stress system which respond by overproduction of cortisol which induces the negative feedback mechanism that starts on POD 1,is still ongoing on POD 5,but is completely settled on POD 45.Moreover,after surgical trauma,our study gives evidence that the RARP procedure associates with stress recovery faster than RRP.Further confirmatory studies are required. | Antonio B.Porcaro Nicolo`de Luyk Paolo Corsi Marco Sebben Alessandro Tafuri Davide Inverardi Davide De Marchi Irene Tamanini Matteo Brunelli Maria Angela Cerruto Gian Luca Salvagno Gian Cesare Guidi Walter Artibani | 2016 | Asian Journal of Urology2016,3,2: | 1 |

| 13 | GIS tools for preliminary debris-flow assessment at regional scale显示文摘The assessment of the areas endangered by debris flows is a major issue in the context of mountain watershed management. Depending on the scale of analysis, different methods are required for the assessment of the areas exposed to debris flows.While 2-D numerical models are advised for detailed mapping of inundation areas on individual alluvial fans, preliminary recognition of hazard areas at the regional scale can be adequately performed by less data-demanding methods, which enable priority ranking of channels and alluvial fans at risk by debris flows. This contribution focuses on a simple and fast procedure that has been implemented for regionalscale identification of debris-flow prone channels and prioritization of the related alluvial fans. The methodology is based on the analysis of morphometric parameters derived from Digital Elevation Models(DEMs). Potential initiation sites of debris flows are identified as the DEM cells that exceed a threshold of slope-dependent contributing area. Channel reaches corresponding to debris flows propagation, deceleration and stopping conditions are derived from thresholds of local slope. An analysis of longitudinal profiles is used for the computation of the runout distance of debris flows. Information on erosion-resistant bedrock channels and sediment availability surveyed in the field are taken into account in the applications. A set of software tools was developed and made available(http://gffzz188fe103f8f1460ascqoncb5oquku6fuo.ffgz.tsg.suse.edu.cn/Hydrogeomorphology Tools) to facilitate the application of the procedure. This approach, which has been extensively validated by means of field checks, has been extensively applied in the eastern Italian Alps. | 15 | Importance of computed tomography in posterior malleolar fractures:Added information to preoperative X-ray studies显示文摘BACKGROUND Ankle fractures are common lesions of the lower limbs.Approximately 40%of ankle fractures affect the posterior malleolus(PM).Historically,PM osteosynthesis was recommended when PM size in X-ray images was greater than 25%of the joint.Currently,computed tomography(CT)has been gaining traction in the preoperative evaluation of ankle fractures.AIM To elucidate the similarity in dimensions and to correlate PM size in X-ray images with the articular surface of the affected tibial plafond in the axial view on CT(AXCT)of a PM fracture.METHODS Eighty-one patients(mean age:39.4±13.5 years)were evaluated(54.3%were male).Two independent examiners measured PM size in profile X-ray images(PMXR)and sagittal CT(SAGCT)slices.The correlation of the measurements between the examiners and the difference in the PM fragment sizes between the two images were compared.Next,the PM size in PMXR was compared with the surface of the tibial plafond involved in the fracture in AXCT according to the Haraguchi classification.RESULTS The correlation rates between the examiners were 0.93 and 0.94 for PMXR and SAGCT,respectively(P<0.001).Fragments were 2.12%larger in SAGCT than in PMXR(P=0.018).In PMXR,there were 56 cases<25%and 25 cases≥25%.When PMXR was<25%,AXCT corresponded to 10.13%of the tibial plafond.When PMXR was≥25%,AXCT was 24.52%(P<0.001).According to the Haraguchi classification,fracture types I and II had similar PMXR measurements that were greater than those of type III.When analyzing AXCT,a significant difference was found between the three types,with II>I>III(P<0.001).CONCLUSION PM fractures show different sizes using X-ray or CT images.CT showed a larger PM in the sagittal plane and allowed the visualization of the real dimensions of the tibial plafond surface. | NoéDe Marchi Neto Pietro Felice Tomazini Nesello Jordanna Maria Bergamasco Marco Tulio Costa Ralph Walter Christian Nilson Roberto Severino | 2023 | World Journal of Orthopedics2023,14,12: | 0 |
