1Human cytomegalovirus and Epstein-Barr virus infection in inflammatory bowel disease:Need for mucosal viral load measurement显示文摘AIM:To evaluate the best diagnostic technique and risk factors of the human Cytomegalovirus(HCMV)and Epstein-Barr virus(EBV)infection in inflammatory bowel disease(IBD).METHODS:A cohort of 40 IBD patients(17 refractory)and 40 controls underwent peripheral blood and endoscopic colonic mucosal sample harvest.Viral infection was assessed by quantitative real-time polymerase chain reaction and immunohistochemistry,and correlations with clinical and endoscopic indexes of activity,and risk factors were investigated.RESULTS:All refractory patients carried detectable levels of HCMV and/or EBV mucosal load as comparedto 13/23(56.5%)non-refractory and 13/40(32.5%)controls.The median DNA value was significantly higher in refractory(HCMV 286 and EBV 5.440 copies/105cells)than in non-refractory(HCMV 0 and EBV 6copies/105 cells;P<0.05 and<0.001)IBD patients and controls(HCMV and EBV 0 copies/105 cells;P<0.001 for both).Refractory patients showed DNA peak values≥103 copies/105 cells in diseased mucosa in comparison to non-diseased mucosa(P<0.0121 for HCMV and<0.0004 for EBV),while non-refractory patients and controls invariably displayed levels below this threshold,thus allowing us to differentiate viral colitis from mucosal infection.Moreover,the mucosal load positively correlated with the values found in the peripheral blood,whilst no correlation with the number of positive cells at immunohistochemistry was found.Steroid use was identified as a significant risk factor for both HCMV(P=0.018)and EBV(P=0.002)colitis.Finally,a course of specific antiviral therapy with ganciclovir was successful in all refractory patients with HCMV colitis,whilst refractory patients with EBV colitis did not show any improvement despite steroid tapering and discontinuation of the other medications.CONCLUSION:Viral colitis appeared to contribute to mucosal lesions in refractory IBD,and its correct diagnosis and management require quantitative real-time polymerase chain reaction assay of mucosal specimens.Rachele Ciccocioppo Francesca Racca Stefania Paolucci Giulia Campanini Lodovica Pozzi Elena Betti Roberta Riboni Alessandro Vanoli Fausto Baldanti Gino Roberto Corazza 2015World Journal of Gastroenterology2015,21,6:12
2Prognostication and response assessment in liver and pancreatic tumors:The new imaging显示文摘Diffusion-weighted imaging(DWI), dynamic contrastenhanced magnetic resonance imaging(DCE-MRI) and perfusion computed tomography(CT) are technical improvements of morphologic imaging that can evaluate functional properties of hepato-bilio-pancreatic tumors during conventional MRI or CT examinations.Nevertheless, the term 'functional imaging' is commonly used to describe molecular imaging techniques, as positron emission tomography(PET)CT/MRI, which still represent the most widely used methods for the evaluation of functional properties of solid neoplasms; unlike PET or single photon emission computed tomography, functional imaging techniques applied to conventional MRI/CT examinations do not require the administration of radiolabeled drugs or specific equipments. Moreover, DWI and DCE-MRI can be performed during the same session, thus providing a comprehensive 'one-step' morphological and functional evaluation of hepato-bilio-pancreatic tumors. Literature data reveal that functional imaging techniques could be proposed for the evaluation of these tumors before treatment, given that they may improve staging and predict prognosis or clinical outcome. Microscopic changes within neoplastic tissues induced by treatments can be detected and quantified with functional imaging,therefore these techniques could be used also for posttreatment assessment, even at an early stage. 3Prediction of hepatocellular carcinoma biological behaviorin patient selection for liver transplantation显示文摘Morphological criteria have always been considered the benchmark for selecting hepatocellular carcinoma(HCC)patients for liver transplantation(LT).These criteria,which are often inappropriate to express the tumor’s biological behavior and aggressiveness,offer only a static view of the disease burden and are frequently unable to correctly stratify the tumor recurrence risk after LT.Alpha-fetoprotein(AFP)and its progression as well as AFP-m RNA,AFP-L3%,des-γ-carboxyprothrombin,inflammatory markers and other serological tests appear to be correlated with post-transplant outcomes.Several other markers for patient selection including functional imaging studies such as18F-FDG-PET imaging,histological evaluation of tumor grade,tissue-specific biomarkers,and molecular signatures have been outlined in the literature.HCC growth rate and response to pre-transplant therapies can further contribute to the transplant evaluation process of HCC patients.While AFP,its progression,and HCC response to pretransplant therapy have already been used as a part of an integrated prognostic model for selecting patients,the utility of other markers in the transplant setting is still under investigation.This article intends to review the data in the literature concerning predictors that could be included in an integrated LT selection model and to evaluate the importance of biological aggressiveness in the evaluation process of these patients.Umberto Cillo Tommaso Giuliani Marina Polacco Luz Maria Herrero Manley Gino Crivellari Alessandro Vitale 2016World Journal of Gastroenterology2016,22,1:7

6Methanol extract of Desmodium gangeticum DC root mimetic post-conditioning effect in isolated perfused rat heart by stimulating muscarinic receptors显示文摘Objective:To evaluate pharmacological mimetic action of herbal extract Desmodium gangeticum (DC) roots on ischemia reperfusion injury.Methods:With the help of Langendroff perfusion technique,ischemic post condition(POC) mimetic action of DG methanol root extract was evaluated and compared by using standard drugs that acts as muscarinic receptor agonist and antagonist,namely acetylcholine(Ach) and atropine(Atr) respectively in an isolated rat heart. Results:The physiological parameters like left ventricular developed pressure,end diastolic pressure and working index of isolated rat heart showed significant recovery in DG root extract administrated rat heart,similar to the recovery by POC.Kymogram results showed muscarinic receptor agonist like action for DG methanol root extract,confirmed in rat heart by muscarnic receptor agonist(acetylcholine) and anatoginst(atropine).Administration of DC root extract prior to reperfusion showed better antioxidant status in myocardial tissue homogenate and mitochondrial,complemented by the levels of cardiac specific marker proteins in myocardial tissue and perfusate.Even though DG methanol root extract mimics its action similar to that of Ach,the myocardial protection mediated by the extract was superior to Ach,due to the presence of antioxidants in the crude extract.Conclusions:DG methanol root extract provides myocardial protection towards IRI by stimulating muscarinic receptors.Gino A Kurian Jose Paddikkala 2012Asian Pacific Journal of Tropical Medicine2012,5,6:3
7Diagnosing, planning and evaluating osteochondral ankle defects with imaging modalities显示文摘This current concepts review outlines the role of diffe-rent imaging modalities in the diagnosis, preoperative planning, and follow-up of osteochondral ankle defects. An osteochondral ankle defect involves the articular cartilage and subchondral bone(usually of the talus) and is mostly caused by an ankle supination trauma. Conventional radiographs are useful as an initial imaging tool in the diagnostic process, but have only moderate sensitivity for the detection of osteochondral defects. Computed tomography(CT) and magnetic resonance imaging(MRI) are more accurate imaging modalities. Recently, ultrasonography and single photon emission CT have been described for the evaluation of osteochondral talar defects. CT is the most valuable modality for assessing the exact location and size of bony lesions. Cartilage and subchondral bone damage can be visualized using MRI, but the defect size tends to be overestimated due to bone edema. CT with the ankle in full plantar flexion has been shown a reliable tool for preoperative planning of the surgical approach. Postoperative imaging is useful for objective assessment of repair tissue or degenerative changes of the ankle joint. 8Enhanced characterization of solid solitary pulmonary nodules with Bayesian analysis-based computer-aided diagnosis显示文摘The aim of this study was to prospectively assess the accuracy gain of Bayesian analysis-based computeraided diagnosis(CAD) vs human judgment alone in characterizing solitary pulmonary nodules(SPNs) at computed tomography(CT). The study included 100 randomly selected SPNs with a definitive diagnosis. Nodule features at first and follow-up CT scans as well as clinical data were evaluated individually on a 1 to 5 points risk chart by 7 radiologists, firstly blinded then aware of Bayesian Inference Malignancy Calculator(BIMC) model predictions. Raters' predictions were evaluated by means of receiver operating characteristic(ROC) curve analysis and decision analysis. Overall ROC area under the curve was 0.758 before and 0.803 after the disclosure of CAD predictions(P = 0.003). A net gain in diagnostic accuracy was found in 6 out of 7 readers. Mean risk class of benign nodules dropped from 2.48 to 2.29, while mean risk class of malignancies rose from 3.66 to 3.92. Awareness of CAD predictions also determined a significant drop on mean indeterminate SPNs(15 vs 23.86 SPNs) and raised the mean number of correct and confident diagnoses(mean 39.57 vs 25.71 SPNs). 9Ultrathin tunable terahertz absorber based on MEMS-driven metamaterial显示文摘The realization of high-performance tunable absorbers for terahertz frequencies is crucial for advancing applications such as single-pixel imaging and spectroscopy.Based on the strong position sensitivity of metamaterials’electromagnetic response,we combine meta-atoms that support strongly localized modes with suspended flat membranes that can be driven electrostatically.This design maximizes the tunability range for small mechanical displacements of the membranes.We employ a micro-electromechanical system technology and successfully fabricate the devices.Our prototype devices are among the best-performing tunable THz absorbers demonstrated to date,with an ultrathin device thickness(~1/50 of the working wavelength),absorption varying between 60%and 80%in the initial state when the membranes remain suspended,and fast switching speed(~27μs).The absorption is tuned by an applied voltage,with the most marked results achieved when the structure reaches the snap-down state.In this case,the resonance shifts by >200% of the linewidth(14% of the initial resonance frequency),and the absolute absorption modulation measured at the initial resonance can reach 65%.The demonstrated approach can be further optimized and extended to benefit numerous applications in THz technology.Mingkai Liu Mohamad Susli Dilusha Silva Gino Putrino Hemendra Kala Shuting Fan Michael Cole Lorenzo Faraone Vincent P.Wallace Willie J.Padilla David A.Powell Ilya V.Shadrivov Mariusz Martyniuk 2017Microsystems & Nanoengineering2017,3,1:2

11Role of the advanced glycation end products receptor in Crohn's disease inflammation显示文摘AIM:To investigate the level of mucosal expression and the involvement of the receptor for the advanced glycation end products(RAGE)in delayed apoptosis and tumor necrosis factor(TNF)-αproduction in Crohn’s disease(CD).METHODS:Surgical and endoscopic specimens from both inflamed and non-inflamed areas of the ileum and/or colon were collected from 20 and 14 adult CD patients,respectively,and used for the assessment of RAGE expression by means of immunohistochemistry and western blotting analysis.Normal tissues from 21 control subjects were used for comparison.The same polyclonal anti-human RAGE antibody(R and D System)was used in all experimental conditions.RAGE staining was quantized by a score including both the amount of positive cells and intensity of immunoreactivity;cellular pattern was also described.The effects of RAGE blocking on apoptotic rate and TNF-αproduction were investigated on immune cells freshly isolated from CD mucosa and incubated both with and without the muramyl dipeptide used as antigenic stimulus.Statistical analysis was performed via the test for trend,with regression models to account for intra-patient correlations.A 2-sided P<0.05 was considered significant.RESULTS:In inflamed areas,RAGE expression in both the epithelial and lamina propria compartments was higher than control tissues(P=0.001 and 0.021,respectively),and a cluster of positive cells were usually found in proximity of ulcerative lesions.Similar results were obtained in the lamina propria compartment of non-inflamed areas(P=0.025).The pattern of staining was membranous and granular cytosolic at the epithelial level,while in the lamina propria it was diffuse cytosolic.When evaluating the amount of protein expression by immunoblotting,a significant increase of both surface area and band intensity(P<0.0001 for both)was observed in CD inflamed areas compared to control tissue,while in non-inflamed areas a significant increase was found only for band intensity(P<0.005).Moreover,a significantly lower expression in noninflamed areas in comparison with inflamed areas was found for both surface area and band intensity(P<0.0006 for both).Finally,RAGE blocking largely affects both the apoptotic rate of mucosal cells(towards an increase in both non-inflamed and inflamed areas of P<0.001 and<0.0001,respectively)and TNF-αsecretion(towards a decrease in both non-inflamed and inflamed areas of P<0.05 and<0.01,respectively),mainly in the presence of antigenic stimulation.CONCLUSION:RAGE is up-regulated in CD,especially in inflamed areas,and it appears to play a role in the mechanisms involved in chronic inflammation.Rachele Ciccocioppo Alessandro Vanoli Catherine Klersy Venerina Imbesi Vincenzo Boccaccio Rachele Manca Elena Betti Giuseppina Cristina Cangemi Elena Strada Roberta Besio Antonio Rossi Colomba Falcone Sandro Ardizzone Paolo Fociani Piergiorgio Danelli Gino Roberto Corazza 2013World Journal of Gastroenterology2013,19,45:2
