2Impact of obesity on the surgical outcome following repeat hepatic resection in Japanese patients with recurrent hepatocellular carcinoma显示文摘AIM: To evaluate the impact of obesity on the posto- perative outcome after hepatic resection in patients with hepatocellular carcinoma (HCC). METHODS: Data from 328 consecutive patients with primary HCC and 60 patients with recurrent HCC were studied. We compared the surgical outcomes between the non-obese group (body mass index: BMI < 25 kg/m2) and the obese group (BMI ≥ 25 kg/m2). RESULTS: Following curative hepatectomy in patients with primary HCC, the incidence of postoperative complications and the long-term prognosis in the non- obese group (n = 240) were comparable to those in the obese group (n = 88). Among patients with recurrent HCC, the incidence of postoperative complications after repeat hepatectomy was not significantly different between the non-obese group (n = 44) and the obese group (n = 16). However, patients in the obese group showed a significantly poorer long-term prognosis than those in the non-obese group (P < 0.05, five-yearsurvival rate; 51.9% and 92.0%, respectively). CONCLUSION: Obesity alone may not have an adverse effect on the surgical outcomes of patients with primary HCC. 3A novel negative regulatory mechanism of Smurf2 in BMP/Smad signaling in bone显示文摘Transforming growth factor-β(TGF-β)and bone morphogenetic protein(BMP)play important roles in bone metabolism.Smad ubiquitination regulatory factors(Smurfs)regulate TGF-β/BMP signaling via ubiquitination,resulting in degradation of signaling molecules to prevent excessive activation of TGF-β/BMP signaling.Though Smurf2 has been shown to negatively regulate TGF-β/Smad signaling,its involvement in BMP/Smad signaling in bone metabolism has not been thoroughly investigated.In the present study,we sought to evaluate the role of Smurf2 in BMP/Smad signaling in bone metabolism.Absorbable collagen sponges containing 3μg of recombinant human BMP2(rhBMP2)were implanted in the dorsal muscle pouches of wild type(WT)and Smurf2−/−mice.The rhBMP2-induced ectopic bone in Smurf2−/−mice showed greater bone mass,higher mineral apposition and bone formation rates,and greater osteoblast numbers than the ectopic bone in WT mice.In WT mice,the ectopic bone consisted of a thin discontinuous outer cortical shell and scant inner trabecular bone.In contrast,in Smurf2−/−mice,the induced bone consisted of a thick,continuous outer cortical shell and abundant inner trabecular bone.Additionally,rhBMP2-stimulated bone marrow stromal cells(BMSCs)from Smurf2−/−mice showed increased osteogenic differentiation.Smurf2 induced the ubiquitination of Smad1/5.BMP/Smad signaling was enhanced in Smurf2−/−BMSCs stimulated with rhBMP2,and the inhibition of BMP/Smad signaling suppressed osteogenic differentiation of these BMSCs.These findings demonstrate that Smurf2 negatively regulates BMP/Smad signaling,thereby identifying a new regulatory mechanism in bone metabolism.Junichi Kushioka Takashi Kaito Rintaro Okada Hiroyuki Ishiguro Zeynep Bal Joe Kodama Ryota Chijimatsu Melanie Pye Masahiro Narimatsu Jeffrey LWrana Yasumichi Inoue Hiroko Ninomiya Shin Yamamoto Takashi Saitou Hideki Yoshikawa Takeshi Imamura 2020Bone Research2020,8,4:6
4Effect of herbal medicine Juzentaihoto on hepatic and intestinal heat shock gene expression requires intestinal microflora in mouse显示文摘AIM: To evaluate the role of intestinal microflora in the effects of multi-herbal medicine on gene expression in the gut and liver. METHODS: The multi-herbal medicine Juzentaihoto (JTX) was administered to five germ-free mice and regular mice for 2 wk. Among the results of the comprehensive gene chip analysis of the intestine and liver, we featured heat shock proteins (HSPs) 70 and 105 because their gene expression changed only in the presence of microflora. Real-time RT-PCR was performed to confirm the expression levels of these HSP genes. To determine whether JTX acts directly on the HSP genes, sodium arsenite (SA) was used to induce the heat shock proteins directly. To examine the change of the intestinal microflora with administration of JTX, the terminal restriction fragment polymorphism (T-RFLP) method was used. To identify the changed bacteria, DNA sequencing was performed. RESULTS: Heat shock protein gene expression,documented by gene chip and real-time RT-PCR, changed with the administration of JTX in the regular mice but not in the germ-free mice. JTX did not suppress the direct induction of the HSPs by SA. T-RFLP suggested that JTX decreased unculturable bacteria and increased Lactobacillus johnsoni. These data suggested that JTX changed the intestinal microflora which, in turn, changed HSP gene expression. 6A case of eosinophilic cholangitis:Imaging findings ofcontrast-enhanced ultrasonography, cholangioscopy, andintraductal ultrasonography显示文摘A 38-year-old woman was referred to our institution due to epigastralgia. She presented with obstructive jaundice and eosinophilia. Endoscopic retrograde cholangiopancreatography showed diffuse narrowing from the distal common bile duct to the bifurcation of the hepatic ducts. An endoscopic plastic biliary stent was inserted; the specimen obtained from the common bile duct wall revealed dense infiltration by eosinophils. Treatment was started with prednisolone 60 mg daily. The patient's biliary stenosis and eosinophilia gradually improved. Eosinophilic infiltration in the lungs or stomach is relatively common, but it is rare in the common bile duct. Most of the reported cases of eosinophilic cholangitis presented with eosinophilia; our patient's eosinophil count was over 1000/mm3. 9日本三波川带别子型矿床矿石的矿物及地球化学特征显示文摘三波川带别子型矿床常见矿石的主要组分硫化物为黄铁矿、黄铜矿和闪锌矿。磁黄铁矿和斑铜矿在臬些矿床中大量产出,而其它硫化物和硫盐则极为少见。在一般块状矿石中很少见到的矿物如方铅矿、黝铜矿等,常常在主要层状矿体内和附近呈分支状、舌状和脉状产出,富铜矿石中发现有微量存在。这些矿物可能是矿床变质变形过程中及常见组分硫化物重结晶过程中形成的。 别子型矿石的显著地球化学特征是其高Co、低Pb和低Ba含量。虽然可用的硫化物硫同位素数据大多数仅来自别子矿床,但别子型矿床的硫同位素比值的变化范围很小,即δ^(34)S为+0.4‰~+3.2‰,平均值+2.1‰;在所检测的6个矿床中,黄铁矿的Se/S比值的变化范围也窄,紧挨在其平均值1.32×10^(-4)处浮动。 别子型矿石的这些矿物及地化特征可能归固于有关基性火山岩的化学性质。现代海底富铜硫化物矿床,其化学性质与别子型矿床极为相似。虽然这两种矿床彼此间在矿物学特征上有点差异,丘垄内部的海底蚀变和热液交代作用可能会把海底硫化物矿床的原生矿物组合改变为别子型矿床的矿石矿物组合。Katsuo Kase Masahiro Yamamoto 张祖海 1993地质地球化学1993,,2:2

16Conservative reconstruction using stents as salvage therapy for disruption of esophago-gastric anastomosis显示文摘Esophagectomy with extended lymphadenectomy and gastric conduit reconstruction is a radical procedure for the treatment of esophageal cancer that is associated with a high morbidity rate.Gastric conduit necrosis is a fatal complication that occurs in 2%of patients.Conventionally,two-stage salvage surgery consisting of removal of the necrotic gastric conduit followed by reconstruction has been performed;however,this procedure has a high morbidity rate.We describe a61-year-old man who underwent minimally invasive esophagectomy complicated by slowly progressive gastric conduit necrosis associated with complete neck drainage and a stable overall condition.There was a 2 cm gap in the anastomosis.Because there was no evidence of residual gastric conduit necrosis,a removable,covered self-expanding metal stent(SEMS)was inserted to bridge the anastomosis.The stent was fixed to the patient's ear with silk thread through the lasso on its proximal end to prevent migration.Eight weeks after insertion,the stent was removed easily without any associated complications.The anastomotic defect was completely bridged with granulation tissue,showing progressive epithelialization without leakage or stenosis.The patient was discharged home in good general health.This is the first report of the successful conservative management of esophago-gastric conduit anastomosis disruption with SEMS placement.Taro Oshikiri Yoshinobu Yamamoto Ikuya Miki Masahiro Tsuda Tetsu Nakamura Yasuhiro Fujino Masahiro Tominaga Yoshihiro Kakeji 2015World Journal of Gastroenterology2015,21,28:2
