| 2 | Preoperative colonoscopy through the colonic stent in patients with colorectal cancer obstruction显示文摘AIM:To evaluate the feasibility of a preoperative colonoscopy through a self-expendable metallic stent(SEMS)and to identify the factors that affect complete colonoscopy.METHODS:A total of 48 patients who had SEMS placement because of acute malignant colonic obstruction underwent preoperative colonoscopy.After effective SEMS placement,patients who showed complete resolution of radiological findings and clinical signs of acute colon obstruction underwent a standard bowel preparation.Preoperative colonoscopy was then performed using a standard colonoscope.If the passage of colonoscope was not feasible gastroscope was used.After colonoscopy,cecal intubation time,grade of bowel preparation,tumor location,stent location,presence of synchronous polyps or cancer,damage to colonoscopy and bleeding,and stent migration after colonoscopy were recorded.RESULTS:Complete evaluation with colonoscope was possible in 30 patients(62.5%).In this group,adenoma was detected in 13 patients(43.3%).The factors that affected complete colonoscopy were also analyzed:Tumor location at an angle;stent placement at an angle;and stent expansion diameter,which affected complete colonoscopy significantly.However in multivariate analysis,stent expansion diameter was the only significant factor that affected complete colonoscopy.Complete evaluation using additional gastroscope was feasible in 42 patients(87.5%).CONCLUSION:Preoperative colonoscopy through the colonic stent using only conventional colonoscope was unfavorable.The narrow expansion diameter of the stent may predict unfavorable outcome.In such a case,using small caliber scope should be considered and may expect successful outcome. | Jin Su Kim Kang Moon Lee Sang Woo Kim Eun Jung Kim Chul Hyun Lim Seong Taek Oh Myung Gyu Choi Kyu Yong Choi | 2014 | World Journal of Gastroenterology2014,20,30: | 11 |
| 3 | What is the best strategy for successful bowel preparation under special conditions?显示文摘Adequate bowel preparation is important for successful colonoscopic examination.Several effective colonic cleansing agents are available and routinely prescribed,but each carries its own limitations and benefits from particular dosing regimens.The most frequently prescribed colonic cleansing agent,the polyethylene glycol(PEG)cathartic solution,suffers from low patient compliance in general,due to its unpalatable taste and smell coupled with the large ingested volumes required.However,PEG is preferred over other cathartics for use in individuals of advanced age,sufferers of chronic kidney disease,heart failure and inflammatory bowel disease,and women who are pregnant or lactating.The laxative agents sodium phosphate(NaP)and sodium picosulfate plus magnesium citrate have been applied and have improved patient compliance and tolerance.NaP,however,should be avoided in individuals with impaired renal function or plasma clearance,such as those with chronic kidney disease,who are taking drugs that affect renal function,or who suffer from heart failure.Other special conditions that may affect an individual’s tolerance of the cathartic agent or ability to complete the administration routine include stroke,severe constipation,hematochezia,suspicious lower gastrointestinal bleeding,and mental disorders such as dementia.All ingestible bowel preparation solutions can be instilled into the stomach and duodenum through nasogastric tube or esophagogastroduodenoscope with the aid of a water irrigation pump for patients with difficulties swallowing or ingesting the large volumes of fluid required.In addition,dietary regimens based on clear liquids and low-residue foods for 1-4 d prior to the colonoscopy may be supplemental bowel preparation strategies.Achieving an effective and safe cleansing of the bowel is important for successful colonoscopy in all patients,so full knowledge of the individual’s condition and capabilities is necessary to select the most appropriate colonic cleansing agent and delivery regimen. | Yun Jeong Lim Su Jin Hong | 2014 | World Journal of Gastroenterology2014,20,11: | 5 |

| 6 | Effects of Salvia miltiorrhiza Bunge extract and its single components on monosodium urate-induced pain in vivo and lipopolysaccharide-induced inflammation in vitro显示文摘OBJECTIVE:To investigate the possible antinociceptive effects of Salvia(S.)miltiorrhiza Bunge and its single components in monosodium urate(MSU)-induced pain model in mice and lipopolysaccharide(LPS)-induced inflammation model in RAW264.7 cells.METHODS:Pretreatment of S.miltiorrhiza Bunge extract(from 1 to 50μg/mL)concentration-dependently attenuated LPS-induced nitric oxide(NO)release.The extract of S.miltiorrhiza Bunge(50 or 100 mg/kg)also caused reversals of decreased threshold for pain in the MSU-treated group as measured by Von-Frey test.Furthermore,we assessed the antinociceptive and anti-inflammatory properties of the active single components from S.miltiorrhiza Bunge such as 15,16-dihydrotanshinoneⅠ,tanshinoneⅠ,cryptotanshinone,miltirone,tanshinoneⅡA,and salvianolic acid B.Some of them showed an anti-inflammatory effect in LPS-induced NO release model and an antinociceptive effect in MSU-treated pain model.RESULTS:Our results suggest that S.miltiorrhiza Bunge extract may exert anti-inflammatory effect by reducing LPS-induced NO release and an antinociceptive property in MSU-treated pain model.Especially,tanshinoneⅡA,miltirone,cryptotanshinone,and 15,16-dihydrotanshinoneⅠ,not only appear to be responsible for LPS-induced NO release induced by S.miltiorrhiza Bunge,but also in the production of S.miltiorrhiza Bunge extract-induced antinociception in MSU-treated pain model.CONCLUSION:Therefore,the analgesic and anti-inflammatory property of S.miltiorrhiza Bunge indicate it as a therapeutic potential candidate for the treatment of pain and inflammation. | FENG Jinghui Set Byeol Kim Hee Jung Lee Su Min Sim Soon Sung Lim Hong Won Suh | 2021 | Journal of Traditional Chinese Medicine2021,41,2: | 4 |

| 9 | Is a split-dose regimen of 2 L polyethylene glycol plus ascorbic acid tolerable for colonoscopy in an early morning visit to a comprehensive medical check-up?显示文摘AIM To evaluate the effectiveness and tolerability of a split-dose 2L polyethylene glycol (PEG)/ascorbic acid(AA) regimen for healthy examinees who visited for comprehensive medical check-up in the early morning.METHODS From February 2015 to March 2015, examinees of average risk who were scheduled for a colonoscopy in the morning were retrospectively enrolled.RESULTS The 189 examinees were divided into split-dose and non-split-dose groups. The adequacy of bowel preparation for the split-dose group vs the non-splitdose group was 96.8% vs 85.2%, respectively, P<0.001, and the compliance of the last meal restrictionwas 74.6% vs 58.2%, respectively, P <0.001. The sleep disturbance (P<0.001) was more prevalent in the split-dose group, however the willingness to repeat the same preparation method (P=0.243) was not different in both groups. The split-dose regimen was the most important factor influencing adequate bowel preparation in multivariate analysis(HR=10.89, 95%CI: 6.53-18.17, P<0.001).CONCLUSION A split-dose regimen of 2L PEG/AA for an early morning colonoscopy was more effective and showed better compliance for diet restriction without any difference in satisfaction and discomfort. | 10 | Automatic Discontinuity Classification of Wind-turbine Blades Using A-scan-based Convolutional Neural Network显示文摘Recent development trends in wind power generation have increased the importance of the safe operation of wind-turbine blades(WTBs). To realize this objective, it is essential to inspect WTBs for any defects before they are placed into operation. However, conventional methods of fault inspection in WTBs can be rather difficult to implement, since complex curvatures that characterize the WTB structures must ensure accurate and reliable inspection. Moreover, it is considered useful if inspection results can be objectively and consistently classified and analyzed by an automated system and not by the subjective judgment of an inspector. To address this concern,the construction of a pressure-and shape-adaptive phased-array ultrasonic testing platform, which is controlled by a nanoengine operation system to inspect WTBs for internal defects, has been presented in this paper. An automatic classifier has been designed to detect discontinuities in WTBs by using an A-scanimaging-based convolutional neural network(CNN). The proposed CNN classifier design demonstrates a classification accuracy of nearly 99%. | 11 | Cancer-associated fibroblast-derived secreted phosphoprotein 1 contributes to resistance of hepatocellular carcinoma to sorafenib and lenvatinib显示文摘Background:Cancer-associated fibroblasts(CAFs)play an important role in the induction of chemo-resistance.This study aimed to clarify the mechanism underlying CAF-mediated resistance to two tyrosine kinase inhibitors(TKIs),sorafenib and lenvatinib,and to identify a novel therapeutic target for overcoming TKI resistance in hepatocellular carcinoma(HCC).Methods:We performed a systematic integrative analysis of publicly available gene expression datasets and whole-transcriptome sequencing data from 9 pairs of CAFs and para-cancer fibroblasts isolated from human HCC and para-tumor tissues,respectively,to identify key molecules that might induce resistance to TKIs.We then performed in vitro and in vivo experiments to validate selected targets and related mechanisms.The associations of plasma secreted phosphoprotein 1(SPP1)expression levels before sorafenib/lenvatinib treatment with progression-free survival(PFS)and overall survival(OS)of 54 patients with advanced HCC were evaluated using Kaplan-Meier and Cox regression analysis.Results:Bioinformatic analysis identified CAF-derived SPP1 as a candidate molecule driving TKI resistance.SPP1 inhibitors reversed CAF-induced TKI resistance in vitro and in vivo.CAF-derived SPP1 activated rapidly accelerated fibrosarcoma(RAF)/mitogen-activated protein kinase(MAPK)and phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/mammalian target of rapamycin(mTOR)through the integrin-protein kinase C-alpha(PKCα)signaling pathway and promoted epithelial-to-mesenchymal transition(EMT).A high plasma SPP1 level before TKI treatment was identified as an independent predictor of poor PFS(P=0.026)and OS(P=0.047)in patients with advanced HCC after TKI treatment.Conclusions:CAF-derived SPP1 enhances TKI resistance in HCC via bypass activation of oncogenic signals and EMT promotion.Its inhibition represents a promising therapeutic strategy against TKI resistance inHCC.Moreover,plasma SPP1 level before TKI treatment represents a potential biomarker for treatment response prediction. | JungWoo Eun Jung Hwan Yoon Hye Ri Ahn Seokhwi Kim Young Bae Kim Su Bin Lim Won Park TaeWook Kang Geum Ok Baek Moon Gyeong Yoon Ju A Son Ji HyangWeon Soon Sun Kim Hyo Jung Cho Jae Youn Cheong | 2023 | Cancer Communications2023,43,4: | 2 |
