| 1 | Perioperative intensive insulin therapy using artificial endocrine pancreas in patients undergoing pancreatectomy显示文摘Perioperative glycemic control is important for reducing postoperative infectious complications. However, clinical trials have shown that efforts to maintain normoglycemia in intensive care unit patients result in deviation of glucose levels from the optimal range, and frequent attacks of hypoglycemia. Tight glycemic control is even more challenging in those undergoing pancreatic resection. Removal of lesions and surrounding normal pancreatic tissue often cause hormone deficiencies that lead to the destruction of glucose homeostasis, which is termed pancreatogenic diabetes. Pancreatogenic diabetes is characterized by the occurrence of hyperglycemia and iatrogenic severe hypoglycemia, which adversely effects patient recovery. Postoperatively, a variety of factors including surgical stress, inflammatory cytokines, sympathomimetic drug therapy, and aggressive nutritional support can also affect glycemic control. This review discusses the endocrine aspects of pancreatic resection and highlights postoperative glycemic control using a closed-loop system or artificial pancreas. In previous experiments, we have demonstrated the reliability of the artificial pancreas in dogs with total pancreatectomy, and its postoperative clinical use has been shown to be effectiveand safe, without the occurrence of hypoglycemic episodes, even in patients after total pancreatectomy. | 3 | A study of local amplification effect of soil layers on ground motion in the Kathmandu Valley using microtremor analysis显示文摘Past researchers have anticipated the occurrence of a great earthquake in the central Himalayas in the near future.This may cause serious damage in the Kathmandu Valley,which sits on an ancient lake bed zone,with lacustrine sediments of more than 500 m depth.In this study,the predominant frequency of ground motion is evaluated using the Horizontal-to-Vertical(H/V) spectral ratio technique and recordings of ambient noise.The results of the H/V ratio show two peaks in about 20 percent of the locations,which are distributed mainly in and around the center and northern part of the Kathmandu Valley.The predominant frequencies vary from 0.5 Hz to 8.9 Hz in the study area,whereas the second resonance frequency varies from 4 Hz to 6 Hz in the center and northern part of the valley.This indicates that the center and northern part of the valley have a wide range of resonance frequency due to two levels of impedance contrast one may be from the surface layer and the other may be from the layer underneath.These two levels of resonance indicate the importance of considering the effects of surface and lower layers during the planning and designing of infrastructures in the Kathmandu Valley. | Paudyal Y.R. | 4 | Clinical relevance of fluorodeoxyglucose positron emission tomography/computed tomography and magnifying endoscopy with narrow band imaging in decision-making regarding the treatment strategy for esophageal squamous cell carcinoma显示文摘BACKGROUND Recent advances in endoscopic technology,especially magnifying endoscopy with narrow band imaging(ME-NBI)enable us to detect superficial esophageal squamous cell carcinoma(ESCC),but determining the appropriate method of resection,endoscopic resection(ER)vs surgical resection,is often challenging.Recently,several studies have reported that 18F-fluorodeoxyglucose positron emission tomography(FDG-PET)is a useful indicator for decision-making regarding treatment for superficial ESCC.Although,there are not enough reports on association between FDG-PET uptake and clinicopathological characteristics of superficial ESCC.And,there are not enough reports on evaluating the usefulness of combination of FDG-PET and ME-NBI for determining the treatment strategy for superficial ESCC.This study evaluated clinical relevance of FDG-PET and ME-NBI in decision-making regarding the treatment strategy for ESCC.AIM To investigate the association between FDG uptake and the clinicopathological characteristics of superficial ESCC and its usefulness of combination of FDG-PET and ME-NBI for determining the treatment strategy for superficial ESCC.METHODS A database of all patients with superficial ESCC who had undergone both MENBI and FDG-PET for pre-treatment staging at Aichi Cancer Center Hospital between January 2008 and November 2018 was retrospectively analyzed.FDG uptake was defined positive or negative whether the primary lesion was visualized or could be distinguished from the background,or not.The invasion depth of ESCC was classified according to the Japan Esophageal Society.Primary endpoint is to evaluate the association between FDG uptake and clinicopathological characteristics of superficial ESCC.Secondary endpoint is to investigate the efficacy of combination of FDG-PET and ME-NBI for determining the treatment strategy for superficial ESCC.RESULTS A total of 82 lesions in 82 patients were included.FDG-PET showed positive uptake in 29(35.4%)lesions.Univariate analysis showed that uptake of FDG-PET had significant correlations with circumferential extension(P=0.014),pathological depth of tumor invasion(P<0.001),infiltrative growth pattern(P<0.001),histological grade(P=0.002),vascular invasion(P=0.001),and lymphatic invasion(P<0.001).On multivariate analysis,only depth of tumor invasion was independently correlated with FDG-PET/computed tomography visibility(P=0.018).The sensitivity,specificity,positive predictive value(PPV),negative predictive value(NPV),and accuracy of Type B2 in ME-NBI for the invasion depth of T1a muscularis mucosae and T1b upper submucosal layer were 68.4%/79.4%/50.0%/89.3%/76.8%,respectively,and those of Type B3 for the depth of T1b middle and deeper submucosal layers(SM2 and SM3)were 46.7%/100%/100%/89.3%/90.2%,respectively.On the other hand,those of FDGPET for SM2 and SM3 were 93.3%/77.6%/48.2%/98.1%/80.5%,respectively,whereas,if the combination of positive FDG uptake and type B2 and B3 was defined as an indicator for radical esophagectomy or definitive chemoradiotherapy,the sensitivity,specificity,PPV,NPV,and accuracy were 78.3%/91.5%/78.3%/91.5%/87.8%,respectively.CONCLUSION FDG uptake was correlated with the invasion depth of superficial ESCC.Combined use of FDG-PET and ME-NBI,especially with the microvascular findings of Type B2 and B3,is useful to determine whether ER is indicated for the lesion. | Kazuhiro Toriyama Masahiro Tajika Tsutomu Tanaka Makoto Ishihara Yutaka Hirayama Sachiyo Onishi Nobumasa Mizuno Takamichi Kuwahara Nozomi Okuno Shinpei Matsumoto Eiichi Sasaki Tetsuya Abe Yasushi Yatabe Kazuo Hara Keitaro Matsuo Tsuneo Tamaki Yasumasa Niwa | 2019 | World Journal of Gastroenterology2019,25,46: | 7 |
| 5 | Clinical applications of 5-aminolevulinic acid-mediated fluorescence for gastric cancer显示文摘5-aminolevulinic acid(ALA) is a naturally occurring amino acid that is a protoporphyrin IX(Pp IX) precursor and a next-generation photosensitive substance. After exogenous administration of ALA, Pp IX specifically accumulates in cancer cells owing to the impaired metabolism of ALA to Pp IX in mitochondria, which results in a red fluorescence following irradiation with blue light and the formation of singlet oxygen. Fluorescence navigation by photodynamic diagnosis(PDD) using ALA provides good visualization and detection of gastric cancer lesions and is a potentially valuable diagnostic tool for gastric cancer for evaluating both the surgical resection margins and extension of the lesion. Furthermore, PDD using ALA might be used to detect peritoneal metastases during preoperative staging laparoscopy, where it could provide useful information for the selection of a therapeutic approach. Another promising application for this modality is in the evaluation of lymph node metastases. Photodynamic therapy(PDT) using ALA to cause selective damage based on the accumulation of a photosensitizer in malignant tissue is expected to be a non-invasive endoscopic treatment for superficial early gastric cancer. ALA has the potential to be used not only as a diagnostic agent but also as a therapeutic drug, resulting in a new strategy for cancer diagnosis and therapy. | 6 | GIS-based Frequency Ratio and Logistic Regression Modelling for Landslide Susceptibility Mapping of Debre Sina Area in Central Ethiopia显示文摘Ethiopia has a mountainous landscape which can be divided into the Northwestern and Southeastern plateaus by the Main Ethiopian Rift and Afar Depression. Debre Sina area is located in Central Ethiopia along the escarpment where landslide problem is frequent due to steep slope, complex geology, rift tectonics, heavy rainfall and seismicity. In order to tackle this problem, preparing a landslide susceptibility map is very important. For this, GISbased frequency ratio(FR) and logistic regression(LR) models have been applied using landslide inventory and the nine landslide factors(i.e. lithology, land use, distance from river & fault, slope, aspect, elevation, curvature and annual rainfall). Database construction, weighting each factor classes or factors, preparing susceptibility map and validation were the major steps to be undertaken. Both models require a rasterized landslide inventory and landslide factor maps. The former was classified into training and validation landslides. Using FR model, weights for each factor classes were calculated and assigned so that all the weighted factor maps can be added to produce a landslide susceptibility map. In the case of LR model, the entire study area is firstly divided into landslide and non-landslide areas using the training landslides. Then, these areas are changed into landslide and non-landslide points so as to extract the FR maps of the nine landslide factors. Then a linear relationship is established between training landslides and landslide factors in SPSS. Based on this relationship, the final landslide susceptibility map is prepared using LR equation. The success-rate and prediction-rate of FR model were 74.8% and 73.5%, while in case of LR model these were 75.7% and 74.5% respectively. A close similarity in the prediction and validation rates showed that the model is acceptable.
