| 2 | Chinese Trauma Surgeon Association for management guidelines of vacuum sealing drainage application in abdominal surgeries-Update and systematic review显示文摘Vacuum sealing drainage (VSD) is frequently used in abdominal surgeries. However, relevant guidelines are rare. Chin ese Trauma Surge on Associati on orga nized a committee composed of 28 experts across China in July 2017, aiming to provide an evidence-based recommendation for the application of VSD in abdominal surgeries.Eleven questions regarding the use of VSD in abdominal surgeries were addressed:(1) which type of materials should be respectively chosen for the intraperitoneal cavity, retroperitoneal cavity and superficial incisions?(2) Can VSD be preventively used for a high-risk abdominal incision w让h primary suture?(3) Can VSD be used in severely contaminated/infected abdominal surgical sites?(4) Can VSD be used for temporary abdominal cavity closure under some special conditions such as severe abdominal trauma, infection, liver transplantation and intra-abdominal volume increment in abdominal compartment syndrome?(5) Can VSD be used in abdominal organ inflammation, injury, or postoperative drainage?(6) Can VSD be used in the treatment of intestinal fistula and pancreatic fistula?(7) Can VSD be used in the treatment of intra-abdominal and extra-peritoneal abscess?(8) Can VSD be used in the treatment of abdominal wall wounds, wound cavity, and defects?(9) Does VSD in crease the risk of bleeding?(10) Does VSD increase the risk of intestinal wail injury?(11) Does VSD increase the risk of peritoneal adhesion? Focusing on these questions, evidence-based recommendations were given accordingly. VSD was strongly recommended regarding the questions 2-4. Weak recommendations were made regarding questions 1 and 5-11. | 5 | Intraductal papillary neoplasm of the bile duct显示文摘Intraductal papillary neoplasm of the bile duct(IPNB)is a variant of bile duct carcinoma that is characterized by intraductal growth and better outcomes compared with common cholangiocarcinoma.IPNBs are mainly found in patients from Far Eastern areas,where hepatolithiasis and clonorchiasis are endemic.According to the immunohistochemical profiles of the mucin core proteins,IPNBs are classified into four types:pancreaticobiliary,intestinal,gastric,and oncocytic.Approximately 40%-80%of IPNBs contain a component of invasive carcinoma or tubular or mucinous adenocarcinoma,suggesting that IPNB is a disease with high potential for malignancy.It is difficult to make an accurate preoperative diagnosis because of IPNB’s low incidence and the lack of specificity in its clinical manifestation.The most common abnormal preoperative imaging findings of IPNB are intraductal masses and the involvement of bile duct dilation.Simultaneous proximal and distal bile duct dilation can be detected in some cases,which has diagnostic significance.Cholangiography and cholangioscopy are needed to confirm the pathology and demonstrate the extent of the lesions.However,pathologic diagnosis by biopsy cannot reflect the actual stage in many cases because different foci may be of different stages and because mixed pathologic findings may exist in the same lesion.Surgical resection is the major treatment.Systematic cholangioscopy with staged biopsies and frozen sections is recommended during resection to ensure that no minor tumors are left and that curative resection is achieved.Staging,histologic subtype,curative resection and lymph node metastasis are factors affecting long-term survival. | Xue-Shuai Wan Yi-Yao Xu Jun-Yan Qian Xiao-Bo Yang An-Qiang Wang Lian He Hai-Tao Zhao Xin-Ting Sang | 2013 | World Journal of Gastroenterology2013,19,46: | 14 |

| 7 | Prognostic impact of the red cell distribution width in esophageal cancer patients: A systematic review and meta-analysis显示文摘AIM To clarify the previous discrepant conclusions, we performed a meta-analysis to evaluate the prognostic value of red cell distribution width(RDW) in esophageal cancer(EC). METHODS We searched the PubM ed, EMBASE, Web of Science and Cochrane Library databases to identify clinical studies, followed by using STATA version 12.0 for statistical analysis. Studies that met the following criteria were considered eligible:(1) Studies including EC patients who underwent radical esophagectomy;(2) studies including patients with localized disease without distant metastasis;(3) studies including patients without preoperative neoadjuvant therapy;(4) studies including patients without previous antiinflammatory therapies and with available preoperative laboratory outcomes;(5) studies reporting association between the preoperative RDW and overall survival(OS)/disease-free survival(DFS)/cancer-specific survival(CSS); and(6) studies published in English.RESULTS A total of six articles, published between 2015 and 2017, fulfilled the selection criteria in the end. Statistical analysis showed that RDW was not associated with the prognosis of EC patients, irrespective of OS/CSS [hazard ratio(HR) = 1.27, 95% confidence interval(CI): 0.97-1.57, P = 0.000] or DFS(HR = 1.42, 95%CI: 0.96-1.88, P = 0.000). Subgroup analysis indicated that elevated RDW was significantly associated with worse OS/CSS of EC patients when RDW > 13%(HR = 1.45, 95%CI: 1.13-1.76, P = 0.000), when the patient number ≤ 400(HR = 1.45, 95%CI: 1.13-1.76, P = 0.000) and when the study type was retrospective(HR = 1.42, 95%CI : 1.16-1.69, P = 0.000).CONCLUSION Contrary to our general understanding, this meta-analysis revealed that RDW cannot serve as an indicator of poor prognosis in patients with EC. | 8 | Lipid layer thickness and tear meniscus height measurements for the differential diagnosis of evaporative dry eye subtypes显示文摘AIM: To explore a new diagnostic index for differentiating the evaporative dry eye(EDE) subtypes by analysis of their respective clinical characteristics. METHODS: A cross-sectional study of 139 patients(139 eyes) with EDE who were enrolled and classified as obstructive meibomian gland dysfunction(MGD)(n=81) and non-obstructive MGD(n=58) EDE. All patients completed a Standard Patient Evaluation of Eye Dryness(SPEED) questionnaire and were evaluated for average lipid layer thickness(LLT), tear meniscus height measurements(TMH), tear break-up time(TBUT), ocular surface staining score, Schirmer I test(SIT), lid margin abnormalities, and meibomian gland function and morphology. RESULTS: Age, average LLT, TMH, scores of lid margin abnormalities, meibum quality, meibomian gland loss(MGL)(all P≤0.001), and TBUT(P=0.03) were all significantly different between obstructive MGD EDE patients and nonobstructive MGD EDE patients. Average LLT in obstructive MGD EDE was correlated with meibomian expressibility(r=-0.541, P≤0.001), lid margin abnormalities were marginally not significant(r=0.197, P=0.077), and TMH was correlated with MGL(total MGL: r=0.552, P≤0.001; upper MGL: r=0.438, P≤0.001; lower MGL: r=0.407, P≤0.001). Average LLT in non-obstructive MGD EDE, was correlated with meibomian expressibility and Oxford staining(r=-0.396, P=0.002; r=-0.461, P≤0.001). The efficiency of combining average LLT and TMH was optimal, with a sensitivity of 80.2% and a specificity of 74.1%. Obstructive MGD EDE patients had an average LLT≥69 nm and TMH≥0.25 mm, while non-obstructive MGD EDE patients had an average LLT<69 nm and TMH<0.25 mm.CONCLUSION: Obstructive MGD EDE and nonobstructive MGD EDE have significantly different clinical characteristics. | 12 | Intercalibration of international and domestic ^(40)Ar/^(39)Ar dating standards显示文摘Four international standards, Ga1550, MMhb-1, Lp-6, Bern 4M, and one domestic standard BT-1 have been intercalibrated. The repeated measurements on MMhb-1 with different mass demonstrate that MMhb-1 is inhomogeneous in age and its average age is 519.8 Ma. The results of Bern 4M and Lp-6 reflect that they have an invariable value of 40Ar*/39Ark (F) and the ages we obtained are consensus with their K-Ar age: Lp-6=127.7Ma; Bern 4M=18.2 Ma. Analyses of BT-1 age spectra, Ca/K and Cl/K spectra as well as inverse isochrons indicate that the sample is homogeneous and invariable and keeps close chemically, with its trapped argon isotope composition close to the at- mosphere. The dating results show that age values are reproducible and steady, total fusion age, step-heating age, plateau age and isochron age are in accord with each other within the error range (2σ). Therefore, we recommend 28.7 Ma as the calibrated age of BT-1. We also discuss the variation in neutron flux gradients of Beijing 49-2 reactor. It was found that the neutron flux gradient varies considerably, and more monitors (standard samples) are needed to fix the trend of variation. The coefficient of the 49-2 reactor that transfers the ratio of production rate of 37ArCa/39ArK into Ca/K ratio is 1.78. | 13 | Genetic analysis and molecular mapping of a novel gene for zebra mutation in rice (Oryza sativa L.)显示文摘A novel zebra mutant,zebra-15,derived from the restorer line Jinhui10(Oryza sativa L. ssp. indica) treated by EMS,displayed a distinctive zebra leaf from seedling stage to jointing stage. Its chlorophyll content decreased(55.4%) and the ratio of Chla/Chlb increased(90.2%) significantly in the yellow part of the zebra-15,compared with the wild type. Net photosynthetic rate and fluorescence kinetic parameters showed that the decrease of chlorophyll content significantly influenced the photosynthetic efficiency of the mutant. Genetic analysis of F2 segregation populations derived from the cross of Xinong1A and zebra-15 indicated that the zebra leaf trait is controlled by a single recessive nuclear gene. Ninety-eight out of four hundred and eighty pairs of SSR markers showed the diversity between the Xinong1A and the zebra-15,their F2 population was then used for gene mapping. Zebra-15(Z-15) gene was primarily restricted on the short arm of chromosome 5 by 150 F2 recessive individuals,19.6 cM from marker RM3322 and 6.0 cM from marker RM6082. The labeled objects are sent into a SVM classifier to generate the pedestrian & vehicle classifier. | 15 | Cartilage-targeting peptide-modified dual-drug delivery nanoplatform with NIR laser response for osteoarthritis therapy显示文摘Cartilage-targeting delivery of therapeutic agents is still an effective strategy for osteoarthritis(OA)therapy.Recently,scavenging for reactive oxygen species(ROS)and activating autophagy have been increasingly reported to treat OA effectively.In this study,we designed,for the first time,a dual-drug delivery system based on metal organic framework(MOF)-decorated mesoporous polydopamine(MPDA)which composed of rapamycin(Rap)loaded into the mesopores and bilirubin(Br)loaded onto the shell of MOF.The collagen II-targeting peptide(WYRGRL)was then conjugated on the surface of above nanocarrier to develop a cartilage-targeting dual-drug delivery nanoplatform(RB@MPMW).Our results indicated the sequential release of two agents from RB@MPMW could be achieved via near-infrared(NIR)laser irritation.Briefly,the rapid release of Br from the MOF shell exhibited excellent ROS scavenging ability and anti-apoptosis effects,however responsively reduced autophagy activity,to a certain extent.Meanwhile,following the NIR irradiation,Rap was rapidly released from MPDA core and further enhanced autophagy activation and chondrocyte protection.RB@MPMW continuously phosphorylated AMPK and further rescued mitochondrial energy metabolism of chondrocytes following IL-1βstimulation via activating SIRT1-PGC-1αsignaling pathway.Additionally,the cartilage-targeting property of peptide-modified nanocarrier could be monitored via Magnetic Resonance(MR)and IVIS imaging.More significantly,RB@MPMW effectively delayed cartilage degeneration in ACLT rat model.Overall,our findings indicated that the as-prepared dual-drug delivery nanoplatform exerted potent anti-inflammation and anti-apoptotic effects,rescued energy metabolism of chondrocytes in vitro and prevented cartilage degeneration in vivo,which thereby showed positive performance for OA therapy. | Song Xue Xiaojun Zhou Weilin Sang Cong Wang Haiming Lu Yiming Xu Yiming Zhong Libo Zhu Chuanglong He Jinzhong Ma | 2021 | Bioactive Materials2021,6,8: | 5 |

| 17 | Low concentration of sodium hyaluronate temporarily elevates the tear film lipid layer thickness in dry eye patients with lipid deficiency显示文摘AIM: To investigate the effects of different concentrations of artificial tears on lipid layer thickness(LLT) and blink rate(BR) in dry eye patients.METHODS: This study included 106 eyes of 58 patients with dry eye. The lipid deficiency type was defined as the LLT baseline <75 nm. The LLT and BR were measured at baseline and 1, 5 and 15 min after the instillation of 0.1% or 0.3% sodium hyaluronate(SH) eye drops by using the LipiV iew ocular surface interferometer. RESULTS: In the lipid deficiency group, the LLT increased from baseline at 1 min post instillation. The LLT after the instillation of 0.1% SH was significantly higher than that after the instillation of 0.3% SH(P<0.001). The LLT returned to baseline at 15 min post instillation of 0.1% SH and at 5 min post instillation of 0.3% SH. In the non-lipid deficiency group, the LLT decreased from baseline at 1 min and returned to baseline at 5 min for both treatments. The BRs were not significantly different at different time points for both treatments.CONCLUSION: SH eye drops induce a short-term increase in LLT of patients with lipid deficiency. A low concentration of artificial tears have a stronger effect than a high concentration of artificial tears on the increase in LLT. In comparison, SH eye drops induce a transient and slight decrease in LLT of patients without lipid deficiency. | 20 | A simple and easily implemented risk model to predict 1-year ischemic stroke and systemic embolism in Chinese patients with atrial fibrillation显示文摘Background:Accurate prediction of ischemic stroke is required for deciding anticoagulation use in patients with atrial fibrillation(AF).Even though only 6%to 8%of AF patients die from stroke,about 90%are indicated for anticoagulants according to the current AF management guidelines.Therefore,we aimed to develop an accurate and easy-to-use new risk model for 1-year thromboembolic events(TEs)in Chinese AF patients.Methods:From the prospective China Atrial Fibrillation Registry cohort study,we identified 6601 AF patients who were not treated with anticoagulation or ablation at baseline.We selected the most important variables by the extreme gradient boosting(XGBoost)algorithm and developed a simplified risk model for predicting 1-year TEs.The novel risk score was internally validated using bootstrapping with 1000 replicates and compared with the CHA2DS2-VA score(excluding female sex from the CHA2DS2-VASc score).Results:Up to the follow-up of 1 year,163 TEs(ischemic stroke or systemic embolism)occurred.Using the XGBoost algorithm,we selected the three most important variables(congestive heart failure or left ventricular dysfunction,age,and prior stroke,abbreviated as CAS model)to predict 1-year TE risk.We trained a multivariate Cox regression model and assigned point scores proportional to model coefficients.The CAS scheme classified 30.8%(2033/6601)of the patients as low risk for TE(CAS score=0),with a corresponding 1-year TE risk of 0.81%(95%confidence interval[CI]:0.41%–1.19%).In our cohort,the C-statistic of CAS model was 0.69(95%CI:0.65–0.73),higher than that of CHA2DS2-VA score(0.66,95%CI:0.62–0.70,Z=2.01,P=0.045).The overall net reclassification improvement from CHA2DS2-VA categories(low=0/high≥1)to CAS categories(low=0/high≥1)was 12.2%(95%CI:8.7%–15.7%).Conclusion:In Chinese AF patients,a novel and simple CAS risk model better predicted 1-year TEs than the widely-used CHA2DS2-VA risk score and identified a large proportion of patients with low risk of TEs,which could potentially improve anticoagulation decision-making.Trial Registration:www.chictr.org.cn(Unique identifier No.ChiCTR-OCH-13003729). | Chao Jiang Tian-Ge Chen Xin Du Xiang Li Liu He Yi-Wei Lai Shi-Jun Xia Rong Liu Yi-Ying Hu Ying-Xue Li Chen-Xi Jiang Nian Liu Ri-Bo Tang Rong Bai Cai-Hua Sang De-Yong Long Guo-Tong Xie Jian-Zeng Dong Chang-Sheng Ma | 2021 | Chinese Medical Journal2021,,19: | 4 |
