| 1 | Predictive factors for early aspiration in liver abscess显示文摘AIM: To determine the predictive factors for early aspiration in liver abscess. METHODS: A retrospective analysis of all patients with liver abscess from 1995 to 2004 was performed. Abscess was diagnosed as amebic in 661 (68%) patients, pyogenic in 200 (21%), indeterminate in 73 (8%) and mixed in 32 (3%). Multiple logistic regression analysis was performed to determine predictive factors for aspiration of liver abscess. RESULTS: A total of 966 patients, 738 (76%) male, mean age 43 ± 17 years, were evaluated: 540 patients responded to medical therapy while adjunctive percutaneous aspiration was performed in 426 patients. Predictive factors for aspiration of liver abscess were: age ≥ 55 years, size of abscess ≥ 5 cm, involvement of both lobes of the liver and duration of symptoms ≥ 7 d. Hospital stay in the aspiration group was relatively longer than in the non aspiration group. Twelve patients died in the aspiration group and this mortality was not statistically significant when compared to the non aspiration group. | 4 | Exploitation of Gene Expression and Cancer Biomarkers in Paving the Path to Era of Personalized Medicine显示文摘Cancer therapy agents have been used extensively as cytotoxic drugs against tissue or organ of a specific type of cancer. With the better understanding of molecular mechanisms underlying carcinogenesis and cellular events during cancer progression and metastasis, it is now possible to use targeted therapy for these molecular events. Targeted therapy is able to identify cancer patients with dissimilar genetic defects at cellular level for the same cancer type and consequently requires individualized approach for treatment. Cancer therapy begins to shift steadily from the traditional approach of ‘‘one regimen for all patients' to a more individualized approach, through which each patient will be treated specifically according to their specific genetic defects. Personalized medicine accordingly requires identification of indicators or markers that guide in the decision making of such therapy to the chosen patients for more effective therapy. Cancer biomarkers are frequently used in clinical practice for diagnosis and prognosis, as well as identification of responsive patients and prediction of treatment response of cancer patient. The rapid breakthrough and development of microarray and sequencing technologies is probably the main tool for paving the way toward ‘‘individualized biomarker-driven cancer therapy' or ‘‘personalized medicine'. | 6 | Productive performance, egg quality, hematological parameters and serum chemistry of laying hens fed diets supplemented with certain fat-soluble vitamins, individually or combined, during summer season显示文摘This present study aimed to determine the efficacy of supplementing layer diets with vitamin A(0, 8,000 and16,000 IU/kg diet) and vitamin E(0, 250 and 500 mg/kg diet) either individually or in combination on egg production and quality, and blood hematology and chemistry of birds reared under summer conditions. A total of 135 Bovans Brown laying hens were distributed to 9 treatment groups with 5 replicates of 3 hens/pen in a 3 × 3 factorial design. A significant improvement in feed conversion ratio(FCR) was observed as supplementary vitamin A or E increased(P 0.01). Hens fed diets supplemented with 16,000 IU vitamin A plus 500 mg vitamin E/kg diet had the best FCR among all groups. Egg quality traits were not significantly affected by the interaction of vitamin A and vitamin E levels. There was a significant increase in monocytes(P ≤ 0.01)and a decrease in basophils counts(P ≤ 0.05) in response to vitamin E. Significant decreases were observed in packed cell volume(PCV), thyroxine(T_4), alanine transferase(ALT), albumin, total cholesterol and total lipids((P ≤ 0.05 or P ≤ 0.01) P 0.01), and increases were observed in serum concentrations of globulin(P ≤ 0.05)and calcium(P ≤ 0.01) due to vitamin A. The combination of 0 IU vitamin A and 500 mg vitamin E/kg diet had the highest values of PCV(40.09%) and hemoglobin(Hb)(10.33 mg/100 mL) among all groups. Vitamin E raised serum values of total protein, total cholesterol and total lipids(P ≤ 0.05 or P≤ 0.01). Feed intake, FCR,PCV, Hb, lymphocytes, monocytes, eosinophils, T_4, ALT and total protein were significantly affected by the interaction of vitamins A and E(P ≤ 0.05 or P ≤ 0.01). The interaction of vitamins A and E was only significant with respect to serum total protein(P ≤ 0.05). It can be concluded that layer diets supplemented with vitamins A and E had good results in alleviating the harmful impacts of high ambient temperature. | 17 | Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients显示文摘AIM: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. METHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (ALT) levels at baseline, 78.9% had their ALT normalized (including the virological non responders). ALT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. All patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects.
