| 1 | Benefit of everolimus as a monotherapy for a refractory breast cancer patient bearing multiple genetic mutations in the PI3K/AKT/mTOR signaling pathway显示文摘A postmenopausal patient with a diagnosis of estrogen receptor(ER)(+), progesterone receptor(PR)(+), and human epidermal growth factor receptor-2(HER2)(-) breast cancer was reported. The patient refused surgery and was resistant to conventional chemotherapy regimens. Computed tomography and the circulating tumor cell test indicated that the patient's tumor burden increased rapidly even after several chemotherapy sessions. Multiple genetic aberrances in the phosphatidylinositol3-kinases(PI3 K) signaling pathway were detected via next-generation sequencing(NGS)-based liquid biopsy, including a p. G1007 R missense mutation in exon 21 of PIK3 CA(33.61%), a p.L70 fs frameshift mutation in exon 3 of phosphatase and tension homolog deleted on chromosome ten(PTEN)(49.14%), and a p. D1542 Y missense mutation in exon 32 of mammalian target of rapamycin(m TOR)(1.66%). Therefore, only the m TOR inhibitor everolimus was administered to the patient. Partial remission(PR) was observed after 2 months, and sustained stable disease(SD) was observed after a year and a half. Subsequent sequencing showed that the mutation ratio of PIK3 CA decreased to 4.17%, and that the PTEN and m TOR mutations disappeared, which revealed the significant curative effect of everolimus. We report the first case of successful monotherapy treatment using everolimus in a patient with advanced breast cancer bearing mutations in genes involved in the PI3 K/ARK/m TOR signaling pathway. | 2 | Rapid response of brain metastasis to crizotinib in a patient with KLC1-ALK fusion and MET gene amplification positive non-small cell lung cancer:a case report显示文摘Non-small cell lung cancer(NSCLC) ranks as the leading cause of cancer-related death in the world. Brain metastasis(BM) is a common complication of NSCLC, with 25%–40% of patients developing BM during the course of the disease. A significant strategy of local disease control in the central nervous system is radiation therapy. With the development of precision medicine,the concept of treating lung cancer BM has gradually changed. In this case, we performed a surgical procedure to obtain enough tumor tissue for the detection of the target gene and other related experiments after the patient was informed. Finally, we found that the patient had both hepatocyte growth factor receptor(MET) gene amplification and kinesin light chain 1-anaplastic lymphoma kinase fusion(KLC1-ALK) through next-generation sequencing and showed sensitivity to the targeted therapy of crizotinib. The patient exhibited good response. | 3 | Magnetic resonance imaging characteristics of postoperative intracranial dissemination of recurrent gliomas显示文摘Ten intracranial gliomas cases, that had postoperative intracranial dissemination, underwent magnetic resonance imaging (MRI) examinations, including T1 weighted imaging, fat-suppressed T1 weighted imaging, T2 weighted imaging and fluid attenuated inversion recovery (FLAIR). Results showed that tumor metastasis had occurred via the cerebrospinal fluid, the brain white matter fibers and the surgical access site alone. On the plain MRI scans, 1/7 cases were linearly thickened with isointensity and 5/7 cases exhibited nodular foci on T1 weighted imaging; the cerebral sulci and cisterns in 2/7 cases had become shallow and five cases had nodular foci on T2 weighted imaging. FLAIR imaging revealed that the cerebral sulci and cisterns in 2/7 cases had become shallow and that six cases had affected nodular foci. The contrast-enhanced MRI scans revealed linear thickening in seven cases, nodules in seven cases, similarities to 'mould-like' signs in six cases and hydrocephalus in six cases. | 5 | Coal-Biomass Preheating Combustion Characteristics in Cement Precalciner.Part 1:Preheating of Coal/Biomass显示文摘The co-combustion of biomass and coal can positively impact the environment and reduce the cost of power generation.However,biomass fuels have many limitations.Circulating fluidized bed(CFB)preheating combustion is suitable for co-combusting coal and biomass because of better fuel adaptability.In the cement industry,fuel combustion and raw meal decomposition in precalciners affect cement quality and cause pollutant emissions.The preheating combustion method used in precalciners can improve combustion performance and reduce NO_(x)emissions.This study investigated the preheating characteristics of a coal-biomass mixed fuel in a cement precalciner.The effects of load,biomass type,and biomass proportion on the preheated fuel and the conditions of the CFB were investigated.The results indicated that a lower load reduces the combustible components in gaseous and solid preheated fuels.However,due to the gas volume remains constant under different loads,a lower load also increases temperature and intensifies the reaction.The carbon chain and microscopic structural activities of preheated fuels are considerably enhanced,facilitating their combustion in precalciners and reducing nitrogen oxides in rotary kilns.Furthermore,adding biomass can improve the reactivity of a fuel subjected to preheating.Thus,biomass fuels(e.g.,rice husks)exhibit high combustion efficiency,and thus high energy utilization.The present study achieved better pore structure and molecular activity using preheated fuel from a CFB preheater.In addition,the improvement of pore structure and molecular activity increases with the proportion of the biomass. | LI Pengpeng REN Qiangqiang HAN Shaobo ZHANG Chi LYU Qinggang | 2023 | Journal of Thermal Science2023,32,5: | 0 |

| 8 | Sealing functional ionic liquids in conjugated microporous polymer membrane by solvent-assisted micropore tightening显示文摘Porous organic polymers hold great promise for molecular sieving membrane separation.Although the inclusion of functional ionic liquid(IL)in the pores offers a facile way to manipulate their separation properties,the IL leaching during the separation process is difficult to avoid.Herein,we report a strategy to in-situ encapsulate ILs into the micropores of the conjugated microporous polymer membrane via a 6-min electropolymerization and further seal the aperture of the pores to prevent ILs leaching by solvent-assisted micropore tightening(SAMT).Upon screening the binding energy between different ILs and gas molecules,two ILs were selected to be incorporated into the membrane for CO_(2)/CH_(4) and O_(2)/N_(2) gas separations.The resultant separation performances surpass the 2008 Robeson upper bound.Notably,the ILs can be locked in the micropores by a facile high surface tension solvent treatment process to improve their separation stability,as evidenced by a 7-day continuous test.This simple and controllable process not only enables efficient and steady separation performance but also provides an effective strategy for confining and sealing functional guest molecules in the porous solids for various applications. | Mengxi Zhang Anke Yu Xiangyu Wu Pengpeng Shao Xin Huang Dou Ma Xianghao Han Jing Xie Xiao Feng Bo Wang | 2022 | Nano Research2022,15,3: | 0 |
| 9 | Intravenous immunoglobulin accompanied with high-dose methylprednisolone therapy for 17 children with anti-N-methyl-D-aspartate receptor encephalitis:Clinic and nursing显示文摘Objective:An increasing number of pediatric patients are being diagnosed with anti-N-methyl-Daspartate receptor(NMDAR)encephalitis,whose treatment requires immunotherapy through nursing interventions.This study aimed to analyze the clinical features and long-term prognosis of pediatric anti-NMDAR encephalitis and to gather nursing experiences of immunotherapy.Methods:Seventeen children diagnosed with anti-NMDAR encephalitis were admitted to the pediatric department.They were subjected to a therapy of intravenous immunoglobulin(IVIG)accompanied with high-dose methylprednisolone(HDMP).Multidisciplinary cooperation and intensive care were used to manage them.The effects of nursing intervention and therapy were repeatedly assessed and analyzed throughout the course of treatment and recovery.Results:None of the patients manifested adverse drug reaction(ADR)during IVIG administration.At the first administration of HDMP,ADRs were promptly and efficiently treated in four patients(24%;i.e.,one case each of hyperglycosemia,hypertension,aggravated symptoms,and gastrointestinal bleed).Two patients underwent rehabilitation,and six patients received hyperbaric oxygenation during hospitalization.Nine patients with indwelling gastric tubes experienced four times of unplanned extubation.Hospital stay ranged from 11 days to 59 days,with the mean duration of 26 days.Discharge evaluation revealed that 16 patients who scored 0e2 on the modified Rankin scale presented obvious remission,and one patient who had a mRS score of 4 exhibited less improvement.The mRS scores of hospitalization,discharge,and six-month follow-up displayed statistically significant differences.Conclusions:Nursing interventions of immunotherapy ensures the security of IVIG administration.Multidisciplinary cooperation promotes remission.Our findings can serve as reference for healthcare teams. | Huihan Zhao Yunli Han Yu He Huiqiao Huang Qin Wei Pengpeng Wang Yanping Ying | 2016 | International Journal of Nursing Sciences2016,3,4: | 0 |
