| 1 | Effects of different resuscitation fluid on severe acute pancreatitis显示文摘AIM: To compare effects of different resuscitation fluid on microcirculation, inflammation, intestinal barrier and clinical results in severe acute pancreatitis (SAP). METHODS: One hundred and twenty patients with SAP were enrolled at the Pancreatic Disease Institute between January 2007 and March 2010. The patients were randomly treated with normal saline (NS group), combination of normal saline and hydroxyethyl starch (HES) (SH group), combination of normal saline, hydroxyethyl starch and glutamine (SHG group) in resuscitation. The ratio of normal saline to HES in the SH and SHG groups was 3:1. The glutamine (20% glutamine dipeptide, 100 mL/d) was supplemented into the resuscitation liquid in the SHG group. Complications and outcomes including respiratory and abdominal infection, sepsis, abdominal hemorrhage, intra-abdominal hypertension, abdominal compartment syndrome (ACS), renal failure, acute respiratory distress syndrome (ARDS), multiple organ dysfunction syndrome (MODS), operation intervention, length of intensive care unit stay, length of hospital stay, and mortality at 60 d were compared. Moreover, blood oxygen saturation (SpO 2 ), gastric intramucosal pH value (pHi), intra-abdominal pressure (IAP), inflammation cytokines, urine lactulose/mannitol (L/M) ratio, and serum endotoxin were investigated to evaluate the inflammatory reaction and gut barrier. RESULTS: Compared to the NS group, patients in the SH and SHG groups accessed the endpoint more quickly (3.9 ± 0.23 d and 4.1 ± 0.21 d vs 5.8 ± 0.25 d, P < 0.05) with less fluid volume (67.26 ± 28.53 mL/kg/d, 61.79 ± 27.61 mL/kg per day vs 85.23 ± 21.27 mL/kg per day, P < 0.05). Compared to the NS group, incidence of renal dysfunction, ARDS, MODS and ACS in the SH and SHG groups was obviously lower. Furthermore, incidence of respiratory and abdominal infection was significantly decreased in the SH and SHG groups, while no significant difference in sepsis was seen. Moreover, less operation time was needed in the SH and SHG group than the NS group, but the difference was not significant. The mortality did not differ significantly among these groups. Blood SpO 2 and gastric mucosal pHi in the SH and SHG groups increased more quickly than in the NS group, while IAP was significantly decreased in the SH and SHG group. Moreover, the serum tumor necrosis factor-α, interleukin-8 and C-reactive protein levels in the SH and SHG groups were obviously lower than in the NS group at each time point. This processing method, i.e., improved in situ polymerization, was used to synthesize flame retardant PA6. In situ formed MCA nanoparticles were supposed to be linked to PA6 chains in the e-caprolactam hydrolytic polymerization system to obtain star- type polymers for the first time. Through TEM photographs, it can be found that the in situ formed MCA nanoparticles with diametric size of less than 50 nm, are nanoscaled, highly uniformly dispersed in the PA6 matrix. | 7 | Effect and mechanism of Irbesartan on occurrence of ventricular arrhythmias in rats with myocardial ischemia through connexin43(cx43)显示文摘Objective: To explore the effect and mechanism of angiotensin Ⅱ receptor blockers-Irbesartan on occurrence of ventricular arrhythmias in rats with myocardial ischemia. Methods: Rats with embryonic cardiomyocytes-H9c2 were randomly divided into control group, ischemia group, Irbesartan group and Irbesartan+ischemia group. The cell viability of rats in each group was tested using MTT. Real-time PCR was employed to detect the expression of connexin43(Cx43) mR NA and western blot to detect the expression of Cx43 and phosphorylated Cx43. SD rats were randomly divided into the sham-operation group(SO), myocardial infarction group(MI), Irbesartan group and MI+ Irbesartan group, with 10 rats in each group. HE staining was employed to observe the change in the pathomorphology of left ventricular tissue and TUNEL method to analyze the cell apoptosis in the tissue. The immunofluorescence was adopted to observe the expression and distribution of Cx43 in the left ventricular myocardium and study the change in the expression of Cx43 in the cardiac muscular tissue at mR NA and protein level. Results: The intervention of Irbesartan in the condition of ischemia indicated the significant decrease in the number of necrotic cells. The expression of Cx43 was significantly decreased under the culture of ischemia(P<0.05), but in the presence of Irbesartan, the expression of Cx43 was increased compared with the ischemia group(P<0.01). The results of WB assay showed the similar trend of change at mRNA level. There was the significant difference in the score of ventricular arerythmia between MI group and SO group(P<0.01). The incidence of ventricular tachycardia or ventricular fibrillation was significantly increased compared with the one in SO group(P<0.05). There was the significant difference in the overall score between MI+Irbesartan group and MI group(P<0.05). The expression of Cx43 in the cardiac muscular tissue in MI group was significantly decreased(P<0.01 vs SO group). But the expression of Cx43 was increased after the treatment with Irbesartan. Conclusions: Irbesartan can inhibit the injury of H9c2 cardiomyocytes and the decreased expression of Cx43 that are induced by the ischemic myocardial infarction. | 10 | A Prospective Randomized Multicenter Controlled Trial on Salvianolate for Treatment of Unstable Angina Pectoris in A Chinese Elderly Population显示文摘Objective: To evaluate the efficacy and safety of salvianolate in elderly patients with unstable angina pectoris(UAP). Methods: A prospective double-blind randomized placebo-controlled multicenter trial in elderly patients with UAP from 13 third-grade class-A hospitals in China was performed. A total of 318 patients were randomly allocated in a 1:1 ratio to an experimental group(160 patients) and a control group(158 patients). The experimental group was treated with salvianolate for 14 days on the basis of conventional medicine, and the control group was given a placebo for 14 days with the same criteria. Follow-up was lasted 28 days in both groups. The primary endpoint was biweekly frequency of angina pectoris attacks. The secondary endpoints included biweekly dosage of nitroglycerin, the Seattle Angina Questionnaire, angina pectoris severity and duration, myocardial injury markers, high-sensitivity C-reactive protein(hs-CRP) and N-terminal pro-B-type natriuretic peptide(NT-proBNP), as well as major adverse cardiovascular events(MACEs). Safety was assessed according to adverse events and serious adverse events. Results: Baseline characteristics were similar between treatment groups. Compared with those in the control group, the frequency of biweekly angi na attacks(2.92 vs. 4.08, P =0.025), th e biweekly dosage of nitroglycerin, as well as the severity and duration of angina attacks(P <0.01) were reduced by salvianolate. The Seattle Angina Questionnaire score was also significantly improved in the experimental group than in the control group(P <0.05). No significant differences were observed between the two groups with respect to the incidence of MACEs. Salvianolate was well tolerated. | 12 | Serum Uric Acid is Associated with the Predicted Risk of Prevalent Cardiovascular Disease in a Community-dwelling Population without Diabetes显示文摘Objective To examine the association between serum uric acid levels and cardiovascular disease risk among individuals without diabetes. Methods We investigated the association between serum uric acid levels and the risk of prevalent cardiometabolic diseases, 10-year Framingham risk for coronary heart disease, and 10-year risk for atherosclerotic cardiovascular diseases(ASCVD) among 8,252 participants aged ≥ 40 years without diabetes from Jiading district, Shanghai, China. Results Body mass index, waist circumference, blood glucose, glycated hemoglobin, blood pressure, and serum lipids increased progressively across the sex-specific quartiles of uric acid(all P trend < 0.05). Compared with individuals in the lowest quartile, those in the higher quartiles had a significantly higher prevalence of obesity, hypertension, and dyslipidemia(all P trend < 0.05). A fully adjusted logistic regression analysis revealed that individuals in the highest quartile had an increased risk of predicted cardiovascular disease compared with those in the lowest quartile of uric acid. The multivariate adjusted odds ratios(ORs) [95% confidence intervals(CIs)] for the highest quartiles for high Framingham risk were 3.00(2.00-4.50) in men and 2.95(1.08-8.43) in women. The multivariate adjusted ORs(95% CIs) for the highest quartile for high ASCVD risk were 1.93(1.17-3.17) in men and 4.53(2.57-7.98) in women. | 14 | Heavy metals contents in soil profiles of typical agricultural lands in Yixing,Jiangsu Province,China显示文摘The effect of land use pattern on the heavy metals(HMs)distribution in soil profiles was investigated in Yixing City,China,for the four typical types of land utilization,i.e.vegetable fields on the plain,paddy fields,tea gardens on hilly lands and bamboo groves.The variance of HMs concentration in the depth of 0-15 cm,15-30 cm,30-60 cm of soil profiles was compared among the different tillage ages. Results showed there was a notable accumulation of Cd,Hg,Cu,Pb,Zn and Cr in the soil profiles of vegetable fields and paddy fields causing a pollution of Cu,Hg and Cd in some profiles,while no remarkable accumulation of HMs in the soil profiles of the tea gardens and bamboo groves was found. The accumulation extent of HMs in the soil profiles of the four kinds of land use followed a sequence of vegetable fields>paddy fields>tea gardens>bamboo groves.The accumulation of HMs was mainly due to the industry,transportation,other urban human activities,and intensive use of fertilizers and pesticides.The downward leaching of HMs was relatively remarkable in the soil profiles of vegetable fields. | 15 | IL-10-Producing Type 1 Regulatory T Cells and Allergy显示文摘As an important subset of regulatory T(Treg) cells,IL-10-producing type 1 regulatory T cells(Tr1) ,have some different features to thymic-derived naturally occurring CD4+CD25+Foxp3+ Treg cells(nTreg cells) . Similar to nTreg cells,Tr1 also play important roles in the control of allergic inflammation in several ways. There is a fine balance between Tr1 and Th2 responses in healthy subjects. Skewing of allergic-specific effctor T cells to a Tr1 phenotype appears to be a critical event in successful allergen-specific immunotherapy and glucocorticoids and β2-agonists treatment. Tr1 suppress Th2 cells and effector cells of allergic inflammation,such as eosinophils,mast cells,basophils,through producing IL-10,and perhaps TGF-β. | 20 | Area and volume ratios for prediction of visual outcome in idiopathic macular hole显示文摘AIM:To predict the visual outcome in patients undergoing macular hole surgery by two novel three-dimensional morphological parameters on optical coherence tomography(OCT): area ratio factor(ARF) and volume ratio factor(VRF).METHODS: A clinical case series was conducted,including 54 eyes of 54 patients with an idiopathic macular hole(IMH). Each patient had an OCT examination before and after surgery. Morphological parameters of the macular hole,such as minimum diameter,base diameter,and height were measured. Then,the macular hole index(MHI),tractional hole index(THI),and hole form factor(HFF) were calculated. Meanwhile,novel postoperative macular hole(MH) factors,ARF and VRF were calculated by three-dimensional morphology. Bivariate correlations were performed to acquire asymptotic significance values between the steady best corrected visual acuity(BCVA) after surgery and 2D/3D arguments of MH by the Pearson method with two-tailed test. All significant factors were analyzed by the receiver operating characteristic(ROC) curve analysis of SPSS software which were responsible for vision recovery. ROC curves analyses were performed to further discuss the different parameters on the prediction of visual outcome.RESULTS: The mean and standard deviation values of patients' age,symptoms duration,and follow-up time were 64.8±8.9y(range: 28-81),18.6±11.5d(range: 2-60),and 11.4±0.4mo(range: 6-24),respectively. Steady-postBCVA analyzed with bivariate correlations was found to be significantly correlated with base diameter(r=0.521,P<0.001),minimum diameter(r=0.514,P<0.001),MHI(r=-0.531,P<0.001),THI(r=-0.386,P=0.004),HFF(r=-0.508,P<0.001),and ARF(r=-0.532,P<0.001). Other characteristic parameters such as age,duration of surgery,height,diameter hole index,and VRF were not statistically significant with steady-post-BCVA. According to area under the curve(AUC) values,values of ARF,MHI,HFF,minimum diameter,THI,and base diameter are 0.806,0.772,0.750,0.705,0.690,and 0.686,respectively. However,Steady-post-BCVA analysis with bivariate correlations for VRF was no statistical significance. Results of ROC curve analysis indicated that the MHI value,HFF,and ARF was greater than 0.427,1.027 and 1.558 respectively which could correlate with better visual acuity.CONCLUSION: Compared with MHI and HFF,ARF could effectively express three-dimensional characteristics of macular hole and achieve better sensitivity and specificity.
