| 3 | Upper-gastrointestinal bleeding secondary to peptic ulcer disease:Incidence and outcomes显示文摘AIM:To evaluate the incidence,surgery,mortality,and readmission of upper gastrointestinal bleeding(UGIB)secondary to peptic ulcer disease(PUD).METHODS:Administrative databases identified all hospitalizations for UGIB secondary to PUD in Alberta,Canada from 2004 to 2010(n=7079)using the International Classification of Diseases Codes(ICD-10).A subset of the data was validated using endoscopy reports.Positive predictive value and sensitivity with 95%confidence intervals(CI)were calculated.Incidence of UGIB secondary to PUD was calculated.Logistic regression was used to evaluate surgery,in-hospital mortality,and 30-d readmission to hospital with recurrent UGIB secondary to PUD.Co-variants accounted for in our logistic regression model included:age,sex,area of residence(i.e.,urban vs rural),number of Charlson comorbidities,presence of perforated PUD,undergoing upper endoscopy,year of admission,and interventional radiological attempt at controlling bleeding.A subgroup analysis(n=6356)compared outcomes of patients with gastric ulcers to those with duodenal ulcers.Adjusted estimates are presented as odds ratios(OR)with95%CI.RESULTS:The positive predictive value and sensitivity of ICD-10 coding for UGIB secondary to PUD were85.2%(95%CI:80.2%-90.2%)and 77.1%(95%CI:69.1%-85.2%),respectively.The annual incidence between 2004 and 2010 ranged from 35.4 to 41.2 per100000.Overall risk of surgery,in-hospital mortality,and 30-d readmission to hospital for UGIB secondary to PUD were 4.3%,8.5%,and 4.7%,respectively.Interventional radiology to control bleeding was performed in 0.6%of patients and 76%of these patients avoided surgical intervention.Thirty-day readmission significantly increased from 3.1%in 2004 to 5.2%in 2010(OR=1.07;95%CI:1.01-1.14).Rural residents(OR rural vs urban:2.35;95%CI:1.83-3.01)and older individuals(OR≥65 vs<65:1.57;95%CI:1.21-2.04)were at higher odds of being readmitted to hospital.Patients with duodenal ulcers had higher odds of dying(OR=1.27;95%CI:1.05-1.53),requiring surgery(OR=1.73;95%CI:1.34-2.23),and being readmitted to hospital(OR=1.54;95%CI:1.19-1.99)when compared to gastric ulcers.CONCLUSION:UGIB secondary to PUD,particularly duodenal ulcers,was associated with significant morbidity and mortality.Early readmissions increased over time and occurred more commonly in rural areas. | Samuel Quan Alexandra Frolkis Kaylee Milne Natalie Molodecky Hong Yang Elijah Dixon Chad G Ball Robert P Myers Subrata Ghosh Robert Hilsden Sander Veldhuyzen van Zanten Gilaad G Kaplan | 2014 | World Journal of Gastroenterology2014，20，46： | 21 |
| 4 | BMI, WC, WHtR, VFI and BFI: Which Indictor is the Most Efficient Screening Index on Type 2 Diabetes in Chinese Community Population显示文摘Objective Obesity is a major risk factor for type 2 diabetes, many indexes can be used to describes obesity and predict diabetes. This research attempts to identify the best indicator of obesity to screening diabetes in Chinese population. Methods A cross-sectional data of 8121 subjects aged 35-60 years were included in this research belongs to the Diabetes Appropriate Technology Intervention Study. Anthropometric indicators including body weight, height, waist circumferences (WC), body fat index (BFI) and visceral fat index (VFI) and blood biochemical indicators after an overnight fast [fasting blood glucose, total cholesterol, high-density lipoprotein (HDL) cholesterol, and triacylglycerol] were measured. BMI (body mass index) and Weight to Height Ratio was calculated. Results Subjects with obesity had a higher risk of physician diagnosed diabetes (OR=2.50, 95% CI 1.83-3.43), new diagnosed diabetes (OR=4.23, 95% CI 2.91-6.15) and pre-diabetes (OR=1.75, 95% CI 1.31-2.34) compared to those with normal Body mass index (BMI). There was a significant trend of increased risk of all diabetes status with increased waist circumference (WC). The waist-to-height ratio (WHtR) yielded the most significant association with new diagnosed diabetes and physician diagnosed diabetes than other indices. Conclusion Central obesity is significantly correlated with diabetes. | 6 | Modeling rock specimens through 3D printing: Tentative experiments and prospects显示文摘Current developments in 3D printing (3DP) technology provide the opportunity to produce rock-like specimens and geotechnical models through additive manufacturing, that is, from a file viewed with a computer to a real object. This study investigated the serviceability of3 DP products as substitutes for rock specimens and rock-type materials in experimental analysis of deformation and failure in the laboratory. These experiments were performed on two types of materials as follows:(1) compressive experiments on printed sand-powder specimens in different shapes and structures, including intact cylinders, cylinders with small holes, and cuboids with pre-existing cracks, and(2) compressive and shearing experiments on printed polylactic acid cylinders and molded shearing blocks. These tentative tests for 3DP technology have exposed its advantages in producing complicated specimens with special external forms and internal structures, the mechanical similarity of its product to rock-type material in terms of deformation and failure, and its precision in mapping shapes from the original body to the trial sample(such as a natural rock joint). | 12 | Inactivated Sendai Virus Suppresses Murine Melanoma Growth by Inducing Host Immune Responses and Down-regulating β-catenin Expression显示文摘Objective This paper aims to investigate the anti-tumor mechanism of inactivated Sendai virus (Hemagglutinating virus of Japan envelope, HVJ-E) for murine melanoma (B16F10). Methods The murine dendritic cells (DCs) were treated with HVJ-E, and then the cytokines secreted from DCs and costimulation-related molecules on DCs were measured. Meanwhile, the expression of β-catenin in HVJ-E treated murine melanoma cells was detected. In addition, HVJ-E was intratumorally injected into the melanoma on C57BL/6 mice, and the immune cells, CTL response and tumor volume were analyzed. Results HVJ-E injected into B16F10 melanoma obviously inhibited the growth of the tumor and prolonged the survival time of the tumor-bearing mice. Profiles of cytokines secreted by dendritic cells (DCs) after HVJ-E stimulation showed that the number of cytokines released was significantly higher than that elicited by PBS (P<0.05). The co-stimulation-related molecules on DCs were comparable to those stimulated by LPS. Immunohistochemical examinations demonstrated the repression of β-catenin in B16F10 melanoma cells after HVJ-E treatment. Meanwhile, real-time reverse transcription PCR revealed that HVJ-E induced a remarkable infiltration of CD11c positive cells, chemokine ligand 10 (CXCL10) molecules, interleukin-2 (IL-2) molecule, CD4 + and CD8 + T cells into HVJ-E injected tumors. Furthermore, the mRNA expression level of β-catenin in the HVJ-E injected tumors was also down-regulated. In addition, B16F10-specific CTLs were induced significantly after HVJ-E was injected into the tumor-bearing mice. Conclusion This is the first report to show the effective inhibition of melanoma tumors by HVJ-E alone and the mechanism through which it induces antitumor immune responses and regulates important signal pathways for melanoma invasion. | 13 | Effect of moxa-burning heat stimulating Liangmen(ST 21) and Zusanli(ST 36) on proliferation and apoptosis signaling proteins in rats with stress-induced gastric ulcer显示文摘OBJECTIVE: To observe the effect of moxa-burning heat stimulating acupoints of Liangmen(ST 21)and Zusanli(ST 36) on the proliferation and apoptosis signaling proteins in rats with stress-induced gastric ulcer.METHODS: Forty rats were randomly divided into four groups: negative control(NC), ulcer control(UC), acupoints of stomach meridian(ASM), and acupoints control(AC). The acute gastric ulcer model was established by bound and water immersion.Rats in NC and UC groups didn't receive any moxa-burning heat stimulating treatment, while rats in ASM and AC groups were treated with buringmoxa heat stimulating the acupoints of Liangmen(ST 21) and Zusanli(ST 36) and their controlled points, respectively. Rats in all groups were sacrificed after 12 consecutive days treatment. | 15 | Radial propagation of magnetospheric substorm-injected energetic electrons observed using a BD-IES instrument and Van Allen Probes显示文摘In cases where substorm injections can be observed simultaneously by multiple spacecraft,they can help elucidate the potential mechanisms of particle transport and energization,of great importance to understanding and modeling the magnetosphere.In this paper,using data returned from the BeiDa-IES(BD-IES) instrument onboard a satellite in an inclined(55°) geosynchronous orbit(IGSO),in combination with two geo-transfer orbiting(GTO) satellite Van Allen Probes(A and B),we analyze a substorm injection event that occurred on the 16 th of October 2015.During this substorm injection,the IGSO onboard BD-IES was outbound,while both Van Allen Probe satellites(A and B) were inbound,a configuration of multiple trajectories that provides a unique opportunity to simultaneously investigate both the inward and outward radial propagation of substorm injection.Indicated by AE/AL indices,this substorm was closely related to an IMF/solar wind discontinuity that showed a sharp change in IMF Bz direction to the north.The innermost signature of this substorm injection was detected by Van Allen Probes A and B at L-3.7,while the outermost signature was observed by the onboard BD-IES instrument at L~\0.These data indicate that the substorm had a global,rather than just local,effect.Finally,we suggest that electric fields carried by fast-mode compressional waves around the substorm injection are the most likely candidate mechanism for the electron injection signatures observed in the inner- and outermost inner magnetosphere. | ZONG QiuGang HAO YiXin ZOU Hong FU SuiYan ZHOU XuZhi REN Jie WANG LingHua YUAN ChongJing LIU ZhiYang JIA XiangHong QUAN Lin LIU Ying WANG YongFu | 2016 | Science China Earth Sciences2016，59，7： | 8 |
