1Comparison of PPIs and H_2-receptor antagonists plus prokinetics for dysmotility-like dyspepsia显示文摘AIM:To compare efficacy of proton pump inhibitors(PPIs)with H2-receptor antagonists(H2RAs)plus prokinetics(Proks)for dysmotility-like symptoms in functional dyspepsia(FD).METHODS:Subjects were randomized to receive openlabel treatment with either rabeprazole 10 mg od(n= 57)or famotidine 10 mg bid plus mosapride 5 mg tid(n=57)for 4 wk.The primary efficacy endpoint was change(%)from baseline in total dysmotility-like dyspepsia symptom score.The secondary efficacy endpoint was patient satisfaction with treatment.RESULTS:The improvement in dysmotility-like dyspep-sia symptom score on day 28 was significantly greater in the rabeprazole group(22.5%±29.2%of baseline) than the famotidine+mosapride group(53.2%± 58.6%of baseline,P<0.0001).The superior benefit of rabeprazole treatment after 28 d was consistent regardless of Helicobacter pylori status.Significantly more subjects in the rabeprazole group were satisfied or very satisfied with treatment on day 28 than in the famotidine+mosapride group(87.7%vs 59.6%,P= 0.0012).Rabeprazole therapy was the only significant predictor of treatment response(P<0.0001),defined as a total symptom score improvement≥50%.CONCLUSION:PPI monotherapy improves dysmotility-like symptoms significantly better than H2RAs plus Proks,and should be the treatment of first choice for Japanese FD.Masahiro Sakaguchi Miyuki Takao Yasuo Ohyama Hiroshi Oka Hiroshi Yamashita Takumi Fukuchi Kiyoshi Ashida Masahiro Murotani Masuyo Murotani Kazuo Majima Hiroshi Morikawa Takashi Hashimoto Keisuke Kiyota Hirohiko Esaki Kanji Amemoto Gouhei Isowa Fumiyuki Takao 2012World Journal of Gastroenterology2012,18,13:8
2基于模糊语言变量的动态目标无线网络选择技术(英文)显示文摘Even though various wireless Network Access Technologies(NATs)with different specifications and applications have been developed in the recent years,no single wireless technology alone can satisfy the anytime,anywhere,and any service wireless-access needs of mobile users.A real seamless wireless mobile environment is only realized by considering vertical and horizontal handoffs together.One of the major design issues in heterogeneous wireless networks is the support of Vertical Handoff(VHO).VHO occurs when a multi-interface enabled mobile terminal changes its Point of Attachment(PoA)from one type of wireless access technology to another,while maintaining an active session.In this paper we present a novel multi-criteria VHO algorithm,which chooses the target NAT based on several factors such as user preferences,system parameters,and traffic-types with varying Quality of Service(QoS)requirements.Two modules i.e.,VHO Necessity Estimation(VHONE)module and target NAT selection module,are designed.Both modules utilize several'weighted'users’and system’s parameters.To improve the robustness of the proposed algorithm,the weighting system is designed based on the concept of fuzzy linguistic variables.Faisal Kaleem Abolfazl Mehbodniya Arif Islam Kang K.Yen Fumiyuki Adachi 2013China Communications2013,10,1:6
3Molecular Mechanisms Contributing to Resistance to Tyrosine Kinase-Targeted Therapy for Non-Small Cell Lung Cancer显示文摘One of the most important pathways in non-small cell lung cancer(NSCLC) is the epidermal growth factor receptor(EGFR) pathway. This pathway affects several crucial processes in tumor development and progression,including tumor cell proliferation,apoptosis regulation,angiogenesis,and metastatic invasion.Targeting EGFR is currently being intensely explored.We are witnessing the development of a number of potential molecular-inhibiting treatments for application in clinical oncology.In the last decade,the tyrosine kinase(TK) domain of the EGFR was identified in NSCLC patients,and it has responded very well with a dramatic clinical improvement to TK inhibitors such are gefitinib and erlotinib.Unfortunately,there were primary and/or secondary resistance to these treatments,as shown by clinical trials.Subsequent molecular biology studies provided some explanations for the drug resistance phenomenon.The molecular mechanisms of resistance need to be clarified.An in-depth understanding of these targeted-therapy resistance may help us explore new strategies for overcoming or reversing the resistance to these inhibitors for the future of NSCLC treatment.Fariz Nurwidya Akiko Murakami Fumiyuki Takahashi Kazuhisa Takahashi 2012Clinical oncology and cancer resexreh2012,9,1:6
4慢性头晕患者的颈肌张力和静态平衡的客观评价显示文摘目的:头晕患者常诉有颈部僵硬的症状。本研究拟探讨颈部症状在头晕患者中的病理机制。方法:26例因单侧前庭病变导致慢性头晕的患者和24例健康对照者参与本次研究,通过客观测量慢性头晕患者双侧斜方肌的肌张力,探讨其与静态姿势干扰的关系。采用颈肌张力仪客观测量双侧斜方肌张力,计算双侧颈肌张力(ANT)和双侧颈肌张力比值(右/左)(RNT)。静态平衡仪测量重心移动轨迹的总长度(LNG)。结果:头晕患者闭眼状态下,ANT和LNG呈负相关(r=-0.44,P<0.05)。头晕患者和健康对照者在闭眼状态时,RNT和LNG呈正相关。结论:ANT平均值升高提示前庭代偿导致的维持静态姿势平衡的颈肌张力增加。颈肌张力的不对称和姿势不平衡有关。张甦琳 孔维佳 YOUJI ASAMA FUMIYUKI GOTO TOMOKO TSUTSUMI KAORU OGAWA 2013临床耳鼻咽喉头颈外科杂志2013,27,3:5
