| 1 | Controversies in the treatment of Crohn’s disease: The case for an accelerated step-up treatment approach显示文摘The ideal treatment strategy for Crohn’s disease (CD) remains uncertain, as does the optimal endpoint of therapy. Top-down versus step-up describes two different approaches: early use of immunomodulators and biological agents in the former versus initial treatment with steroids in the latter, with escalation to immunomodulators or biological drugs in patients proven to be steroid refractory or steroid dependent. Top-down therapy has been associated with higher rates of mucosal healing. If mucosal healing proves to be associated with better long-term outcomes, such as a decreased need for hospitalization and surgery, top-down therapy may be the better approach for many patients. The main concern with the top-down approach is the toxicity of the immunomodulators and biological agents, which have been linked with infectious complications as well as an increased risk of lymphoma. It is unlikely that one strategy will be best for all patients given the underlying heterogeneity of CD presentation and severity. Ultimately, we must weigh the safety and efficacy of the therapies with the risks of the disease itself. Unfortunately our ability to risk stratify patients at diagnosis remains rudimentary. | 2 | Effect of Ni^(7+) Ion Irradiation on Structure and Ammonia Sensing Properties of Thermally Oxidized Zinc and Indium Films显示文摘ZnO and In2O3 films were prepared by thermal oxidation of vacuum deposited zinc and indium films, respectively onto the glass substrate at 30 C. The fabricated films have been irradiated with 100-MeV Ni7+ ions at different fluences ranging from 5×1011 to 5×1013 ions/cm2. The structural and gas sensing properties of pristine and irradiated films have been discussed. X-ray diffraction (XRD) pattern of pristine and irradiated films reveal that the films are polycrystalline in nature and crystallinity increases after irradiation. In this study, highly porous In2O3 nanorods evolved when being irradiated at a fluence of 5×1013 ions/cm2 while ZnO film shows decrease in number of nanowires. | 3 | Surface Treatment of Inorganic CsPbI_(3) Nanocrystals with Guanidinium Iodide for Efficient Perovskite Light-Emitting Diodes with High Brightness显示文摘The remarkable evolution of metal halide perovskites in the past decade makes them promise for next-generation optoelectronic material.In particular,nanocrystals(NCs)of inorganic perovskites have demonstrated excellent performance for light-emitting and display applications.However,the presence of surface defects on the NCs negatively impacts their performance in devices.Herein,we report a compatible facial post-treatment of CsPbI_(3) nanocrystals using guanidinium iodide(GuI).It is found that the GuI treatment effectively passivated the halide vacancy defects on the surface of the NCs while offering effective surface protection and exciton confinement thanks to the beneficial contribution of iodide and guanidinium cation.As a consequence,the film of treated CsPbI_(3) nanocrystals exhibited significantly enhanced luminescence and charge transport properties,leading to high-performance light-emitting diode with maximum external quantum efficiency of 13.8%with high brightness(peak luminance of 7039 cd m^(−2) and a peak current density of 10.8 cd A^(−1)).The EQE is over threefold higher than performance of untreated device(EQE:3.8%).The operational half-lifetime of the treated devices also was significantly improved with T50 of 20 min(at current density of 25 mA cm^(−2)),outperforming the untreated devices(T50~6 min). | Minh Tam Hoang Amandeep Singh Pannu Yang Yang Sepideh Madani Paul Shaw Prashant Sonar Tuquabo Tesfamichael Hongxia Wang | 2022 | Nano-Micro Letters2022,14,4: | 2 |
| 4 | 糖尿病酮症酸中毒患者发生胃肠道出血的预测因素:一项回顾性的观察研究显示文摘背景酮症酸中毒(DKA)是糖尿病一种常见的急性并发症,需要积极的药物治疗。我们旨在研究DKA患者急性胃肠道出血(AGIB)和急性上消化道出血(AUGIB)的发生率及其相关危险因素。方法:我们对本中心2010年1月至2015年12月间收治的DKA病例进行回顾性分析,通过电子病历库收集其人口统计学、临床、实验室、内镜及住院资料。根据有无发生胃肠道出血将患者分为两组。结果:共计234例DKA患者纳入研究,其中27例(11.5%)有AGIB的记录。除2例直肠出血和3例隐匿性出血外,22例(9.4%)表现为呕血。AGIB组与非AGIB组患者年龄、性别和种族的分布无显著差异。两组患者电解质水平、阴离子间隙、pH值及糖化血红蛋白HbA1c的差异亦无统计学意义。然而,AGIB组患者初始血糖水平显著高于非AGIB组(738 mg/dL vs 613 mg/dL,P=0.014)。AGIB组患者病死率也有所升高(7.4%vs 4.8%),尽管差异未达到统计学意义。结论:初始高血糖水平会增加DKA患者发生AGIB的风险。我们还注意到发生AGIB的DKA患者病死率增加,尽管这个增加未达到统计学意义。临床上应更加积极地纠正DKA患者的血糖水平以降低AGIB发生率,从而降低住院病死率。 | Kanthi Rekha Badipatla Preeti Jadhav Sushma Vaddigiri Bharat Bajantri Amandeep Singh Chaitanya Chandrala Vijay S Are Suresh Kumar Nayudu | 2017 | Gastroenterology Report2017,5,4: | 2 |

| 9 | Analysis of Protein-Ligand Interactions of SARS-CoV-2 Against Selective Drug Using Deep Neural Networks显示文摘In recent time, data analysis using machine learning accelerates optimized solutions on clinical healthcare systems. The machine learning methods greatly offer an efficient prediction ability in diagnosis system alternative with the clinicians. Most of the systems operate on the extracted features from the patients and most of the predicted cases are accurate. However, in recent time, the prevalence of COVID-19 has emerged the global healthcare industry to find a new drug that suppresses the pandemic outbreak. In this paper, we design a Deep Neural Network(DNN)model that accurately finds the protein-ligand interactions with the drug used. The DNN senses the response of protein-ligand interactions for a specific drug and identifies which drug makes the interaction that combats effectively the virus. | 13 | Medications in type-2 diabetics and their association with liver fibrosis显示文摘BACKGROUND The prevalence of nonalcoholic fatty liver disease(NAFLD) is significantly rising worldwide. Type-2 diabetes(T2D) is a major risk factor for NAFLD progression.AIM To assess the association of commonly used medications to advanced fibrosis(AF) in patients with biopsy-proven NAFLD and T2D.METHODS We used the International Classification of Disease 9 th Revision Clinical Modification coding system to identify patients with T2D and included patients who underwent liver biopsy for suspected NAFLD between January 1, 2000 to December 31, 2015. We compared demographics, clinical characteristics, and differences in pattern of medication use in patients who had biopsy-proven AF to those without it. A univariate and multivariate analysis was performed to assess the association of different classes of medication with the presence of AF.RESULTS A total of 1183 patients were included in the final analysis, out of which 32%(n =381) had AF on liver biopsy. Mean age of entire cohort was 52 years and majority were females(65%) and Caucasians(85%). Among patients with AF, 51% were on oral hypoglycemics, 30% were on insulin, 66% were on antihypertensives and 27% were on lipid lowering agents for the median duration of 19 mo, 10 mo, 26 mo, and 24 mo respectively. Medications associated with decreased risk of AF included metformin, liraglutide, lisinopril, hydrochlorothiazide, atorvastatin and simvastatin while the use of furosemide and spironolactone were associated with higher prevalence of AF.CONCLUSION In our cohort of T2D with biopsy proven NAFLD, the patients who were receiving metformin, liraglutide, lisinopril, hydrochlorothiazide, atorvastatin and simvastatin were less likely to have AF on biopsy, while patients who were receiving furosemide and spironolactone had a higher likelihood of having AF when they underwent liver biopsy.
