Cerebral small vessel disease or intracranial large vessel atherosclerosis may carry different risk for future strokes显示文摘Background The effect of cerebral small vessel disease(CSVD)and intracranial arterial stenosis(ICAS)on stroke outcomes remains unclear. Methods Data of 1045 patients with minor stroke or transient ischaemic attack(TIA)were obtained from 45 sites of the Clopidogrel in High-Risk Patients with Acute Non-disabling Cerebrovascular Events(CHANCE)trial. We assessed the associations of burdens of CSVD and ICAS with new strokes and bleeding events using multivariate Cox regression models and those with modified Rankin Scale(mRS)scores using ordinal logistic regression models. Results Among the 1045 patients,CSVD was present in 830 cases(79.4%)and ICAS in 460(44.0%).Patients with>1 ICAS segment showed the highest risk of new strokes(HR 2.03,95%CI 1.15 to 3.56,p=0.01).No association between CSVD and the occurrence of new strokes was found. The presence of severe CSVD(common OR(cOR)2.01,95%CI 1.40 to 2.89,p<0.001)and>1 ICAS segment(cOR 2.15,95%CI 1.57 to 2.93,p<0.001)was associated with higher mRS scores.Severe CSVD(HR 10.70,95%CI 1.16 to 99.04,p=0.04),but not ICAS,was associated with a higher risk of bleeding events.Six-point modified CSVD score improved the predictive power for bleeding events and disability.Interpretation CSVD is associated with more disability and bleeding events, and ICAS is associated with an increased risk of stroke and disability in patients with minor stroke and TIA at 3 months.CSVD and ICAS may represent different vascular pathologies and play distinct roles in stroke outcomes. | Huimin Chen Yuesong Pan Lixia Zong Jing Jing Xia Meng Yuyuan Xu Hongyi Yan Xingquan Zhao Liping Liu Hao Li S Claiborne Johnston Yongjun Wang Yilong Wang | 2020 | Stroke & Vascular Neurology2020,5,2: | 13
Association of elevated hs-CRP and multiple infarctions with outcomes of minor stroke or TIA: subgroup analysis of CHANCE randomised clinical trial显示文摘Background and purpose The relationship of high-sensitive C-reactive protein(hs-CRP)levels and infarction numbers with the prognosis of stroke is uncertain. This study evaluated the association of different hs-CRP levels and infarction numbers with the prognosis of acute minor ischaemic stroke or transient ischaemic attack(TIA).Methods A subset of 807 patients with both hs-CRP measurement and baseline MRI was included from the Clopidogrel in High-risk Patients with Acute Non-disabling Cerebrovascular Events trial. The primary efficacy outcome was the occurrence of an ischaemic stroke at the 1-year follow-up.Infarction numbers were classified as multiple acute infarctions(MAIs),single acute infarction and no acute infarction(NAI).The association between different hs-CRP levels with different infarction numbers and the risk of any outcome was analysed using multivariable Cox regression models. Results Among the 807 patients,84(10.4%)patients had a recurrent ischaemic stroke within 1 year. After adjustment for conventional confounding factors, patients with both elevated hs-CRP levels and MAIs were associated with approximately 4.7-fold of risk of ischaemic stroke within 1 year(16.7% vs 3.5%,HR 4.68,95% CI 1.54 to 14.23,p=0.007),compared with those with non-elevated hs-CRP levels and NAI.Similar results were observed for the composite events. Conclusions Combined elevated hs-CRP levels and MAIs may increase 1-year stroke risk stratification efficiency in patients with minor ischaemic stroke or TIA compared with using those markers alone, which indicated that the combination of inflammatory and imaging markers might improve the effectiveness of risk stratification concerning minor ischaemic stroke or TIA. | Guangyao Wang Jing Jing Jiejie Li Yuesong Pan Hongyi Yan Xia Meng Xingquan Zhao Liping Liu Hao Li David Z Wang Yongjun Wang Yilong Wang | 2021 | Stroke & Vascular Neurology2021,6,1: | 11
Concurrent intracranial and extracranial artery stenosis and the prognosis of transient ischaemic symptoms or imaging-negative ischaemic stroke显示文摘Background and purpose Transient ischaemic attack(TIA),transient symptoms with infarction(TSI)and diffusion-weighted imaging(DWI)-negative acute ischaemic stroke(AIS)share similar aetiologies but are considered to have a rather benign prognosis. We intended to investigate the association between intracranial atherosclerotic stenosis(ICAS),extracranial atherosclerotic stenosis(ECAS)and the prognosis of patients with TIA,TSI and DWI-negative AIS.Methods Clinical and imaging data of eligible participants were derived from the Chinese Intracranial Atherosclerosis study, according to symptom duration, acute infarction on DWI and discharge diagnosis.Based on the severity and location of arterial atherosclerosis,we categorised the study population into four groups:no or<50% ICAS and no ECAS;≥50% ICAS but no ECAS;no or<50%ICAS with ECAS;and concurrent≥50% ICAS and ECAS.Using multivariable Cox regression models,we analysed the relationship between the severity and distribution of large artery atherosclerosis and the prognosis of TIA,TSI and DWI-negative AIS.Results A total of 806 patients were included,67.3% of whom were male. The median age of the study participants was 63 years.Patients in the concurrent≥50% ICAS and ECAS subgroup had both a significantly higher 1-year recurrence rate(adjusted HR 3.4(95%CI 1.15 to 10.04),p=0.027)and a higher risk of composite vascular events(adjusted HR 3.82(95%CI 1.50 to 9.72),p=0.005).Conclusions Concurrent ICAS and ECAS is associated with a higher possibility of 1-year recurrent stroke or composite vascular events.Large artery evaluation is necessary to assess patients with transient ischaemic symptoms or DWI-negative AIS.Progress in shortening the time interval between symptom onset and large vessel evaluation is needed. | Yue Suo Jing Jing Yuesong Pan Weiqi Chen Hongyu Zhou Hao Li Yuehua Pu Liping Liu Xingquan Zhao Yilong Wang Xia Meng Yongjun Wang | 2021 | Stroke & Vascular Neurology2021,6,1: | 10
Clopidogrel with aspirin in High- risk patients with Acute Non- disabling Cerebrovascular Events II (CHANCE-2): rationale and design of a multicentre randomised trial显示文摘Background In patients with a minor ischaemic stroke or transient ischaemic attack(TIA),separate trials have shown that dual antiplatelet therapy with clopidogrel plus aspirin(clopidogrel-aspirin)or ticagrelor plus aspirin(ticagrelor-aspirin)are more effective than aspirin alone in stroke secondary prevention. However, these two sets of combination have not been directly compared.Since clopidogrel was less effective in stroke patients who were CYP2C19 loss-of function(LOF)allele carriers, whether ticagrelor-aspirin is clinically superior to clopidogrel-aspirin in this subgroup of patients with stroke is unclear.Aim To describe the rationale and design considerations of the Clopidogrel in High-risk patients with Acute Non-disabling Cerebrovascular Events(CHANCE-2)trial.Design CHANCE-2 is a randomised,double-blind,double-dummy,placebo-controlled,multicentre trial that compares two dual antiplatelet strategies for minor stroke or TIA patients who are CYP2C19 LOF allele carriers:ticagrelor(180 mg loading dose on day 1 followed by 90 mg twice daily on days 2-90)or clopidogrel(300 mg loading dose on day 1 followed by 75 mg daily on days 2-90),plus open-label aspirin with a dose of 75-300 mg on day 1 followed by 75 mg daily on day 2-21.All will be followed for 1 year.Study outcomes The primary efficacy outcome is any stroke(ischaemic or haemorrhagic)within 3 months and the primary safety outcome is any severe or moderate bleeding event within 3 months.Discussion The CHANCE-2 trial will evaluate whether ticagrelor-aspirin is superior to clopidogrel-aspirin for minor stroke or TIA patients who are CYP2C19 LOF allele carriers. | Yongjun Wang Claiborne Johnston Philip M Bath Xia Meng Jing Jing Xuewei Xie Anxin Wang Yuesong Pan Anding Xu Qiang Dong Yilong Wang Xingquan Zhao Zixiao Li Hao Li | 2021 | Stroke & Vascular Neurology2021,6,2: | 10
Redox-sensitive prodrug nanoassemblies based on linoleic acid-modified docetaxel to resist breast cancers显示文摘Prodrug nanoassemblies, which can refrain from large excipients, achieve higher drug loading and control drug release, have been placed as the priority in drug delivery system. Reasoning that glutathione(GSH) and reactive oxygen species(ROS) are highly upgraded in tumor tissues which makes them attractive targets for drug delivery system, we designed and synthetized a novel prodrug which utilized mono thioether bond as a linker to bridge linoleic acid(LA) and docetaxel(DTX). This mono thioether-linked conjugates(DTX-S-LA) could self-assemble into nanoparticles without the aid of much excipients. The mono thioether endowed the nanoparticles redox sensitivity resulting in specific release at the tumor tissue. Our studies demonstrated that the nanoassemblies had uniform particle size, high stability and fast release behavior. DTX-S-LA nanoassemblies outperformed DTX solution in pharmacokinetic profiles for it had longer circulation time and higher area under curve(AUC). Compared with DTX solution, the redox dual-responsive nanoassemblies had comparable cytotoxic activity. Besides, the antitumor efficacy was evaluated in mice bearing 4 T1 xenograft. It turned out this nanoassemblies couldenhance anticancer efficacy by increasing the dose because of higher tolerance. Overall, these results indicated that the redox sensitivity nanoassemblies may have a great potential to cancer therapy. | Meng Li Liwen Zhao Tao Zhang Yue Shu Zhonggui He Yan Ma Dan Liu Yongjun Wang | 2019 | Acta Pharmaceutica Sinica B2019,9,2: | 10
Epidemiological and etiological investigation of dengue fever in the Fujian province of China during 2004–2014显示文摘Dengue fever(DF) is a vector-borne disease and a tremendous socioeconomic burden on tropical and subtropical countries worldwide. To explore the characteristics of DF epidemic in the Fujian province, information of DF cases in Fujian during 2004–2014 was collected and analyzed. The complete E genes of 48 viral isolates were amplified and sequenced for phylogenetic analysis. A total of 733 cases was reported, of which 612(83.5%) occurred during the peak period from August to October. Additionally, 76%(190/250) of imported cases originated from Southeast Asia countries, by the epidemiological investigation. Phylogenetic analysis of the 48 viral isolates revealed that three genotypes(I, IV, V) of DENV1, and one genotype each of DENV2(cosmopolitan) and DENV3(I) circulated in Fujian during 2004–2014. Similar to the results of the epidemiological investigations, the source of most of the viral isolates, including imported and indigenous cases, may be Southeast Asia countries; however, importation from adjacent provinces was also observed in recent years. Overall, DF is considered an imported epidemic disease in Fujian. Increasing diversity of the viral source and geographic expansion of the area affected by DF in recent years highlights the necessity for strengthening surveillance of the DF epidemic and developing strategies for DF prevention and control in Fujian. | Jinzhang Wang Hongbing Chen Meng Huang Yongjun Zhang Jianfeng Xie Yansheng Yan Kuicheng Zheng Yuwei Weng | 2017 | Science China(Life Sciences)2017,60,1: | 9
Endovascular therapy for Acute ischemic Stroke Trial (EAST): study protocol for a prospective, multicentre control trial in China显示文摘Background and purpose:5 recent trials have shown the benefit of endovascular treatment for acute ischaemic stroke(AIS)due to large vessel occlusion of the anterior circulation. This study aims to evaluate the safety and efficacy of Solitaire thrombectomy in patients with moderate-to-severe stroke in the Chinese population, which has a high prevalence of intracranial atherosclerosis. Methods and analysis:This multicentre prospective control study will involve 17 stroke centres in China, and plans to recruit 150 patients in the intervention group, and 150 patients in the medical group,in which patients meet enrolment criteria but refuse intervention.Patients with AIS due to large vessel occlusion indicated for treatment with Solitaire stent retriever within 12 hours of symptom onset, and who meet the inclusion and exclusion criteria, will be enrolled in this study. The primary efficacy endpoint is functional independence as defined by a modified Rankin Scale(mRS)score≤2 at 90 days or by functional improvement as defined by mRS,using shift analysis. The procedural efficacy endpoint is arterial recanalisation of the occluded target vessel measured by a modified Thrombolysis in Cerebral Infarction(mTICI)score equal or superior to 2b right following the use of the study device. The primary safety endpoint is symptomatic intracranial haemorrhage(sICH)within 24±3 hours postprocedure.Ethics and dissemination:The protocol was approved by the Ethics Committee at the coordinating centre and by the local Institutional Review Board of each participating centre.Trial registration number:NCT02350283. | Zhongrong Miao Xiaochuan Huo Feng Gao Xiaoling Liao Chunjuan Wang Ya Peng Yibin Cao Shengli Chen Meng Zhang Changchun Jiang Xiaoxiang Peng Cunfeng Song Liping Wei Qiyi Zhu Zaiyu Guo Li Liu Hang Lin Hua Yang Wei Wu Hui Liang Anding Xu Kangning Chen Xingquan Zhao Yuesong Pan Hao Li Liping Liu Yilong Wang Yongjun Wang EAST investigators | 2016 | Stroke & Vascular Neurology2016,1,2: | 8
A novel two-layered optical satellite network of LEO/MEO with zero phase factor显示文摘Optical satellite communication has advantages over the RF satellite communication in capacity, speed, secrecy and anti-interference.Space information superhighway composed of GEO/MEO/LEO satellites which are linked by optical inter-satellite links(OISL) would play important roles in communication, navigation, martial recon, remote sensing and deep space exploration.A novel LEO/MEO two-layered optical satellite network with global coverage is designed with the street-of-coverage technique. The layer of LEO satellites is the walker-delta constellation with zero phase factor and has the quasi-stable logical structure of Mesh.Equatorial and polar orbit satellites constitute the layer of MEO satellite, which can cover LEO layer effectively.Rising satellites of the system can provide 99.9% of coverage for the whole globe and 100% for China averagely. The network can provide a minimum elevation angle at 21 ? with single-satellite visibility for China, and at 16 ? with the dual-satellite visibility.Simulation results of the inter-satellite links show LEO constellation of zero phase factor outperforms non-zero phase factor constellations in azimuth angle, elevation angle and distance, which is very fit for the OISL.Simulation results of the communication performance indicate a little difference between short wave band and long wave band at low communication speed(several Gbit/s);however, the wave band at 1550 nm is a better choice at high communication speed(100 Gbit/s). | LI YongJun , WU JiLi, ZHAO ShangHong, MENG Wen, MA LiHua, SHI Lei, CHU XingChun, HOU Rui & LI Tian Telecommunication Engineering Institute, Air Force Engineering University, Xi’an 710077, China | 2010 | Science China(Information Sciences)2010,53,6: | 8
Whole genome sequencing of 10K patients with acute ischaemic stroke or transient ischaemic attack: design, methods and baseline patient characteristics显示文摘Background and purpose Stroke is the second leading cause of death worldwide and the leading cause of mortality and long-term disability in China, but its underlying risk genes and pathways are far from being comprehensively understood. We here describe the design and methods of whole genome sequencing(WGS)for 10914 patients with acute ischaemic stroke or transient ischaemic attack from the Third China National Stroke Registry(CNSR-III).Methods Baseline clinical characteristics of the included patients in this study were reported.DNA was extracted from white blood cells of participants.Libraries are constructed using qualified DNA,and WGS is conducted on BGISEQ-500 platform. The average depth is intended to be greater than 30×for each subject.Afterwards,Sentieon software is applied to process the sequencing data under the Genome Analysis Toolkit best practice guidance to call genotypes of single nucleotide variants(SNVs)and insertion-deletions.For each included subject,21 fingerprint SNVs are genotyped by MassARRAY assays to verify that DNA sample and sequencing data originate from the same individual. The copy number variations and structural variations are also called for each patient. All of the genetic variants are annotated and predicted by bioinformatics software or by reviewing public databases. Results The average age of the included 10914 patients was 62.2±11.3 years, and 31.4%patients were women. Most of the baseline clinical characteristics of the 10914 and the excluded patients were balanced. Conclusions The WGS data together with abundant clinical and imaging data of CNSR-III could provide opportunity to elucidate the molecular mechanisms and discover novel therapeutic targets for stroke. | Si Cheng Zhe Xu Yang Liu Jinxi Lin Yong Jiang Yilong Wang Xia Meng Anxin Wang Xinying Huang Zhimin Wang Guohua Chen Songdi Wu Zhengchang Jia Yongming Chen Xuerong Qiu Jun Wu Binbin Song Weizhong Ji Zhongping An Wenjun Xue Lili Zhao Yu Geng Hongyan Li Hao Li Yongjun Wang | 2021 | Stroke & Vascular Neurology2021,6,2: | 8
Analytical validation of GMEX rapid point- of care CYP2C19 genotyping system for the CHANCE-2 trial显示文摘Background and purpose Rapid genotyping is useful for guiding early antiplatelet therapy in patients with high-risk nondisabling ischaemic cerebrovascular events(HR-NICE).Conventional genetic testing methods used in CYP2C19 genotype-guided antiplatelet therapy for patients with HR-NICE did not satisfy the needs of the Clopidogrel in High-Risk Patients with Acute Nondisabling Cerebrovascular Events(CHANCE)-2 trial. Therefore,we developed the rapid-genotyping GMEX(point-of care)system to meet the needs of the CHANCE-2 trial. Methods Healthy individuals and patients with history of cardiovascular diseases(n=408)were enrolled from six centres of the CHANCE-2 trial. We compared the laboratory-based genomic test results with Sanger sequencing test results for accuracy verification.Next,we demonstrated the accuracy, timeliness and clinical operability of the GMEX system compared with laboratory-based technology(YZY Kit)to verify whether the GMEX system satisfies the needs of the CHANCE-2 trial. Results Genotypes reported by the GMEX system showed 100%agreement with those determined by using the YZY Kit and Sanger sequencing for all three CYP2C19 alleles(*2,*3 and*17)tested.The average result’s turnaround times for the GMEX and YZY Kit methods were 85.0(IQR:85.0-86.0)and 1630.0(IQR:354.0-7594.0)min(p<0.001),respectively.Conclusions Our data suggest that the GMEX system is a reliable and feasible point-of care system for rapid CYP2C19 genotyping for the CHANCE-2 trial or related clinical and research applications. | Xia Meng Anxin Wang Guojun Zhang Siying Niu Wei Li Sifei Han Fang Fang Xingquan Zhao Kehui Dong Zening Jin Huaguang Zheng Kelin Chen Hao Li Chengyuan Yang Yongjun Wang | 2021 | Stroke & Vascular Neurology2021,6,2: | 7
Low serum albumin levels predict poor outcome in patients with acute ischaemic stroke or transient ischaemic attack显示文摘Background To examine the relationship of serum albumin with poor functional outcome and mortality in patients with acute ischaemic stroke(AIS)or transient ischaemic attack(TIA),and perform a meta-analysis to summarise the association. Methods We analysed data from the Third China National Stroke Registry(CNSR-Ⅲ).Patients were divided into four groups based on serum albumin levels at admission. The outcomes included poor functional outcome(modified Rankin Scale(mRS)score of 3 to 6)and mortality at 3 months and 1 year.Multiple logistic regression models and Cox regression models were used to evaluate the association, respectively. We used a fixed-effect model to calculate the risk ratio for poor functional outcome and a random-effect model for mortality in the meta-analysis.Results A total of 13618 patients were enrolled.During the 3-month follow-up period, compared with 40 to 44.9 g/L group, patients in<35 g/L group had an increased risk of poor functional outcome and mortality(adjusted OR 1.37(95%CI 1.12 to 1.67);adjusted HR 2.13(95%CI 1.41 to 3.23)).The relationship in per 10 g/L decreased serum albumin with prognosis was consistently inversed(adjusted OR 1.17(95%CI 1.01 to 1.35);adjusted HR 1.86(95%CI 1.30 to 2.64)).Also,low serum albumin levels were independently correlated with clinical outcomes at 1 year.In the meta-analysis,the OR for poor functional outcome pooled 3 studies per 1 g/L decrease was 1.03(95%CI 1.02 to 1.05),and the HR for mortality pooled 5 studies was 1.07(95%CI 1.03 to 1.11).Conclusions Low serum albumin levels predict poor functional outcome and mortality in patients with AIS or TIA. | Hongyu Zhou Anxin Wang Xia Meng Jinxi Lin Yong Jiang Jing Jing Yingting Zuo Yilong Wang Xingquan Zhao Hao Li Yongjun Wang | 2021 | Stroke & Vascular Neurology2021,6,3: | 7
Estimating significant wave height from SAR imagery based on an SVM regression model显示文摘A new method for estimating significant wave height(SWH) from advanced synthetic aperture radar(ASAR) wave mode data based on a support vector machine(SVM) regression model is presented. The model is established based on a nonlinear relationship between σ0, the variance of the normalized SAR image, SAR image spectrum spectral decomposition parameters and ocean wave SWH. The feature parameters of the SAR images are the input parameters of the SVM regression model, and the SWH provided by the European Centre for Medium-range Weather Forecasts(ECMWF) is the output parameter. On the basis of ASAR matching data set, a particle swarm optimization(PSO) algorithm is used to optimize the input kernel parameters of the SVM regression model and to establish the SVM model. The SWH estimation results yielded by this model are compared with the ECMWF reanalysis data and the buoy data. The RMSE values of the SWH are 0.34 and 0.48 m, and the correlation coefficient is 0.94 and 0.81, respectively. The results show that the SVM regression model is an effective method for estimating the SWH from the SAR data. The advantage of this model is that SAR data may serve as an independent data source for retrieving the SWH, which can avoid the complicated solution process associated with wave spectra. | GAO Dong LIU Yongxin MENG Junmin JIA Yongjun FAN Chenqing | 2018 | Acta Oceanologica Sinica2018,37,3: | 7
It is safe to use transdermal glyceryl trinitrate to lower blood pressure in patients with acute ischaemic stroke with carotid stenosis显示文摘Background There is concern that blood pressure(BP)lowering in acute stroke may compromise cerebral perfusion and worsen outcome in the presence of carotid stenosis. We assessed the effect of glyceryl trinitrate(GTN)in patients with carotid stenosis using data from the Efficacy of Nitric Oxide in Stroke(ENOS)Trial. Methods ENOS randomised 4011 patients with acute stroke and raised systolic BP(140-220 mm Hg)to transdermal GTN or no GTN within 48 hours of onset. Those on prestroke antihypertensives were also randomised to stop or continue their medication for 7 days. The primary outcome was the modified Rankin Scale(mRS)at day 90.Ipsilateral carotid stenosis was split:<30%;30-<50%;50-<70%;≥70%.Data are ORs with 95%CIs adjusted for baseline prognostic factors.results 2023(60.5%)ischaemic stroke participants had carotid imaging. As compared with<30%,≥70%ipsilateral stenosis was associated with an unfavourable shift in mRS(worse outcome)at 90 days(OR 1.88,95%CI 1.44 to 2.44,p<0.001).Those with≥70%stenosis who received GTN versus no GTN had a favourable shift in mRS(OR 0.56,95%CI 0.34 to 0.93,p=0.024).In those with 50-<70%stenosis,continuing versus stopping prestroke antihypertensives was associated with worse disability, mood, quality of life and cognition at 90 days.Clinical outcomes did not differ across bilateral stenosis groups. Conclusions Following ischaemic stroke, severe ipsilateral carotid stenosis is associated with worse functional outcome at 90 days.GTN appears safe in ipsilateral or bilateral carotid stenosis, and might improve outcome in severe ipsilateral carotid stenosis. | Jason P Appleton Lisa J Woodhouse Andrew Belcher Daniel Bereczki Eivind Berge Valeria Caso Hui Meng Chang Hanne K Christensen Ronan Collins John Gommans Ann C Laska George Ntaios Serefnur Ozturk Gillian M Sare Szabolcs Szatmari Yongjun Wang Joanna M Wardlaw Nikola Sprigg Philip M Bath for the ENOS investigators | 2019 | Stroke & Vascular Neurology2019,4,1: | 6
Effects of redox potential on soil cadmium solubility:Insight into microbial community显示文摘Understanding the role of microbes in the solubility of cadmium(Cd) is of fundamental importance for remediation of Cd toxicity. The present study aimed to identify the microbes that involved in regulating Cd solubility and to reveal possible mechanisms. Therefore, microbial communities were investigated through high-throughput sequencing approach, the molecular ecological network was constructed and metagenomes were predicted. Our results indicated that redox conditions affected both the solubility of soil Cd and the microbial communities. Anaerobic microbes, such as Anaerolineaceae, did not only play important roles in shaping the microbial community in soils, but might also be involved in regulating the Cd solubility. Two possible mechanisms that how Anaerolineaceae involved in Cd solubility are(1) Anaerolineaceae are important organic matter degraders under anoxic conditions and(2) Anaerolineaceae can co-exist with methane metabolism microbes, while methane metabolism promotes the precipitation of soluble Cd. Thus, application of Anaerolineaceae in bioremediation of soil Cadmium contamination is a potential approach. The study provided a novel insight into the role of microbial community in the regulation of Cd solubility under different redox conditions, and suggested a potential approach for the remediation of soil Cd contamination. | Delong Meng Juan Li Tianbo Liu Yongjun Liu Mingli Yan Jin Hu Xiaoqi Li Xueduan Liu Yili Liang Hongwei Liu Huaqun Yin | 2019 | Journal of Environmental Sciences2019,31,1: | 6
Different contribution of SBP and DBP variability to vascular events in patients with stroke显示文摘Background High blood pressure variability(BPV)is a novel risk factor for cardiovascular disease. However, the heterogeneity of systolic blood pressure variability(SBPV)and diastolic blood pressure variability(DBPV)for different vascular events remains unclear. This study aims to investigate whether SBPV or DBPV has different contribution to vascular events in patients with acute ischaemic stroke(IS)or transient ischaemic attack(TIA).Methods Data from the BOSS(blood pressure and clinical outcome in TIA or IS)study were examined for vascular events at 3-month and 1-year follow-up.BPV was defined as the SD and coefficient of variation(CV)of day-to day measurements within 3 months after IS/TIA.Vascular events include cardiovascular events(myocardial infarction, unstable angina, cardiac death and congestive heart failure)and cerebrovascular events(ischaemic or haemorrhagic stroke).Logistic regression model was used to test the associations between BPV and vascular events.results Of 2325 patients with IS or TIA,103(4.43%)experienced a recurrent stroke and 64(2.75%)had cardiovascular events within 3 months.Day-to day SBPV was only associated with stroke recurrence(BPVSD:OR,1.72,95%CI 1.09 to 2.71;BPVCV:1.86,95%CI 1.19 to 2.92),but not cardiovascular events(BPVSD:1.67,95%CI 0.94 to 2.94;BPVCV:1.51,95%CI 0.86 to 2.64).However,DBPV seems to be related to both stroke(BPVSD:1.60,95%CI 1.02 to 2.49;BPVCV:1.53,95%CI 0.99 to 2.37)and cardiovascular events(BPVSD:2.48,95%CI 1.37 to 4.48;BPVCV:1.92,95%CI 1.09 to 3.36).Similar results were found at 1 year. Conclusions For patients with IS/TIA,stroke recurrence was associated with both SBPV and DBPV;however, cardiovascular events seem to be only related to DBPV. | Liye Dai Aichun Cheng Xiwa Hao Jie Xu Yingting Zuo Anxin Wang Xia Meng Hao Li Yilong Wang Xingquan Zhao Yongjun Wang | 2020 | Stroke & Vascular Neurology2020,5,2: | 6
Risk factors associated with 90-day recurrent stroke in patients on dual antiplatelet therapy for minor stroke or high-risk TIA:a subgroup analysis of the CHANCE trial显示文摘Objective Many patients receiving dual antiplatelet therapy still had recurrent strokes. We aimed to identify factors associated with recurrent stroke at 90 days in patients receiving dual antiplatelet therapy in Clopidogrel in High-risk patients with Acute Non-disabling Cerebrovascular Events trial. Methods Patients with transient ischaemic attack or minor stroke receiving clopidogrel and aspirin in the trial were analysed in the study. The primary outcome was recurrent stroke within 90 days after the index event.Cox proportional hazard model with backward selection was used to identify factors associated with stroke.results Among 2584 patients,212(8.2%)had a recurrent stroke,216(8.4%)had a composite of stroke, myocardial infarction,or vascular death and 204(7.9%)had ischaemic stroke within 90 days.Multivariate analysis identified the following factors associated with stroke:history of hypertension with poor blood pressure control(HR,1.92;95%CI 1.22 to 3.03),the high baseline National Institute of Health Stroke Scale(NIHSS)score of 2 and 3(2.12(1.07 to 4.21)and 4.11(2.05 to 8.22),respectively),time from onset to randomisation of<12 hours(1.47(1.12 to 1.94)),the lipid-lowering therapy(0.61(0.47 to 0.83)),the open-label aspirin dose at day 1 of≥300 mg(1.98(1.45 to 2.69)).Intracranial arterial stenosis(ICAS)was significantly associated with stroke in the sensitivity analysis(2.17(1.16 to 4.04)).conclusions The high baseline NIHSS score, hypertension with poor blood pressure control,ICAS,time from onset to randomisation of less than 12 hours and no lipid-lowering therapy were associated with stroke, suggesting that patients with identified predictors still remain to be at high risk of recurrent stroke although being under the dual antiplatelet therapy. | Runqi Wangqin Xianwei Wang Yilong Wang Ying Xian Xingquan Zhao Liping Liu Hao Li Xia Meng Yongjun Wang | 2017 | Stroke & Vascular Neurology2017,2,4: | 6
The efficacy and safety of nimodipine in acute ischemic stroke patients with mild cognitive impairment: a double-blind, randomized,placebo-controlled trial显示文摘Nimodipine might be effective in subcortical vascular dementia(VaD). Its benefit in preventing further cognitive decline in patients with acute ischemic stroke(AIS) and vascular mild cognitive impairment(VaMCI) remains to be established. In this multicenter, double-blind trial, we randomly assigned 654 eligible patients to nimodipine 30 mg three times a day or placebo. The primary outcome was any cognitive decline defined by the changes on the Mini-Mental State Examination(DMMSE à3) or vascular AD assessment scale cognitive subscale(DADAS-cog ! 4) at 6 months. Secondary outcomes included any distribution shift of DADAS-cog, DMMSE or cognitive improvement defined by DADAS-cog à2, or DMMSE ! 0. The primary outcome in the nimodipine group and placebo group were similar for DMMSE à3(4.18% and 7.22%, respectively, P = 0.15) and DADAS-cog ! 4(8.36% and 8.93% respectively,P = 0.88). The distribution shift of DADAS-cog and DMMSE differed significantly between the two groups(P = 0.03 and P = 0.05 respectively). Cognitive improvement occurred in 55.4% in the nimodipine group and 43.6% in the placebo group measured by DADAS-cog à2(Odds Ratio, 1.54; 95% confidence interval[CI] 1.10–2.14, P < 0.01) or 84.0% and 74.6% respectively by DMMSE ! 0(Odds Ratio, 1.79; 95% CI 1.18–2.70, P < 0.01). Nimodipine was associated with better cognitive function in the memory domain. The adverse events rate was similar in two groups. This study is registered with ClinicalTrials.gov,NCT01220622. Nimodipine did not show benefit to prevent cognitive decline in AIS patients with VaMCI, but improved cognition moderately, especially measured in the memory domain. | Huaguang Zheng Yilong Wang Anxin Wang Hao Li David Wang Xingquan Zhao Penglian Wang Haipeng Shen Lijun Zuo Yuesong Pan Zixiao Li Xia Meng Xianwei Wang Weixiong Shi Yi Ju Liping Liu Kehui Dong Chunxue Wang Rubo Sui Rong Xue Xiaoping Pan Xiaoyua Niu Benyan Luo Yi Sui Huali Wang Tao Feng Yongjun Wang | 2019 | Science Bulletin2019,64,2: | 5
Impairment of cognition and sleep after acute ischaemic stroke or transient ischaemic attack in Chinese patients: design, rationale and baseline patient characteristics of a nationwide multicentre prospective registry显示文摘background and aim Cognitive impairment and sleep disorder are both common poststroke conditions and are closely related to the prognosis of patients who had a stroke. The Impairment of CognitiON and Sleep after acute ischemic stroke or transient ischemic attack in Chinese patients(ICONS)study is a nationwide multicentre prospective registry to investigate the occurrence and associated factors of cognitive impairment and sleep disorder after acute ischaemic stroke(AIS)or transient ischaemic attack(TIA).Methods Consecutive AIS or TIA in-hospital patients within 7 days after onset were enrolled from 40 participating sites in China.Comprehensive baseline clinical and imaging data were collected prospectively.Blood and urine samples were also collected on admission and follow-up visits.Patients were interviewed face to face for cognition and sleep related outcomes at 2 weeks,3,6 and 12 months after AIS/TIA and followed up for clinical outcomes by telephone annually over 5 years.results Between August 2015 and January 2018,a total of 2625 patients were enrolled.92.65% patients had AIS and 7.35% patients had TIA.Overall, the average age was 61.04 years, and 72.38% patients were male.Median National Institutes of Health Stroke Scale score was 3 in AIS patients. Conclusions The ICONS study is a large-scale nationwide prospective registry to investigate occurrence and the longitudinal changes of cognitive impairment and sleep disorder after AIS or TIA.Data from this registry may also provide opportunity to evaluate associated factors of cognitive impairment or sleep disorder after AIS or TIA and their impact on clinical outcome. | Yongjun Wang Xiaoling Liao Chunxue Wang Ning Zhang Lijun Zuo Yang Yang Yuesong Pan Xianglong Xiang Jing Jing Xia Meng Xingquan Zhao Yilong Wang Jiong Shi Hao Li | 2021 | Stroke & Vascular Neurology2021,6,1: | 4
Trends and predictors of myocardial infarction or vascular death after ischaemic stroke or TIA in China, 2007-2018: insights from China National Stroke Registries显示文摘Background Although stroke management, primary and secondary preventions have been improved in China last decades, the trends and predictors of major vascular events after ischaemic stroke or transient ischaemic attack(TIA)at national scale are less known. Methods Data were obtained from the three phases of China National Stroke Registry(CNSR),including CNSR-Ⅰ(years 2007-2008),CNSR-Ⅱ(years 2012-2013)and CNSR-III(years 2015-2018).For comparison, patients who were diagnosed as ischaemic stroke or TIA were included.Kaplan-Meier estimates of myocardial infarction(MI)or vascular death were calculated at 1 year.Independent predictors were further assessed with a Cox proportional hazards regression. Results From 2007 to 2018,a total of 50284 patients with ischaemic stroke or TIA were enrolled in this study.A declining trend was found in 1-year MI or vascular death(p for trend<0.001),while recurrent stroke depicted a U-shape curve with a nadir in 2012-2013 cohort.A similar trend was also observed in patients who were admitted to 26 hospitals in all three CNSRs.In 2015-2018 cohort, only 251(1.7%;95%CI 1.5%to 1.9%)MI or vascular death had occurred at 1 year.Older age, previous stroke or TIA,history of coronary artery disease and the National Institutes of Health Stroke Scale>6 were associated with both an increased risk of MI or vascular death and recurrent stroke. While early antiplatelet therapy and lipid-lowering agents at discharge predicted a reduced risk. Conclusion A declining trend and current low incidence of MI or vascular death, rather than recurrent stroke, after ischaemic stroke or TIA were observed in China.Traditional factors were found as independent predictors. These findings suggested there is still much room to improve for stroke management. | Long Li Yuesong Pan Mengxing Wang Jing Jing Xia Meng Yong Jiang Caixia Guo Zening Jin Yongjun Wang | 2021 | Stroke & Vascular Neurology2021,6,2: | 4
Chromosome doubling of Sedum alfredii Hance: A novel approach for improving phytoremediation efficiency显示文摘Sedum alfredii Hance is a cadmium(Cd)/zinc(Zn)hyperaccumulator native to China. However, its relatively low biomass restricted the large-scale application for heavy metal contamination remediation. The chromosome set doubling of S.alfredii in vitro was achieved by 0.1%–0.2%(W/V)colchicine treatment. The plant DNA ploidy was analyzed by flow cytometry and chromosome set doubling plants(CSD)were identified based on the obvious different sharp peak.A tissue culture experiment with different Cd treated levels and a field trial with natural polluted mined soil were conducted to study the effects of chromosome doubling on plant biomass and Cd accumulation in shoots. The results suggested that S.alfredii is a mixoploid.Compared with the wild type plants(WT),CSD exhibited typical'gigas'characteristics in morphology including stem thickness, root hair production, number of leaves and size of stoma guard cell.Fresh weight and dry weight of CSD were increased to 1.62–2.03-fold and 2.26–3.25-fold of WT.And Cd content of CSD showed a17.49%–42.82%increase and 59%increase under tissue culture and field condition, accordingly. In addition, the TF and in BCF of CSD were 2.37-and 1.59-fold of WT,respectively. These results proved that it is feasible to promote phytoextraction efficiency of S.alfredii in Cd contaminated soils through chromosomal engineering, which provides a novel approach for hyperaccumulator application in phytoremediation. | Ying Feng Qiong Wang Qian Meng Yongjun Liu Fengshan Pan Sha Luo Yingjie Wu Luyao Ma Xiaoe Yang | 2019 | Journal of Environmental Sciences2019,31,12: | 4
