2Compared efficacy of preservation solutions on the outcome of liver transplantation:Meta-analysis显示文摘AIM To compare the effects of the four most commonly used preservation solutions on the outcome of liver transplantations.METHODS A systematic literature search was performed using MEDLINE, Scopus, EMBASE and the Cochrane Library databases up to January 31^(st), 2017. The inclusion criteria were comparative, randomized controlled trials(RCTs) for deceased donor liver(DDL) allografts with adult and pediatric donors using the gold standard University of Wisconsin(UW) solution or histidinetryptophan-ketoglutarate(HTK), Celsior(CS) and Institut Georges Lopez(IGL-1) solutions. Fifteen RCTs(1830 livers) were included; the primary outcomes were primary non-function(PNF) and one-year posttransplant graft survival(OGS-1). RESULTS All trials were homogenous with respect to donor and recipient characteristics. There was no statistical difference in the incidence of PNF with the use of UW, HTK, CS and IGL-1(RR = 0.02, 95%CI: 0.01-0.03, P = 0.356). Comparing OGS-1 also failed to reveal any difference between UW, HTK, CS and IGL-1(RR = 0.80, 95%CI: 0.80-0.80, P = 0.369). Two trials demonstrated higher PNF levels for UW in comparison with the HTK group, and individual studies described higher rates of biliary complications where HTK and CS were used compared to the UW and IGL-1 solutions. 3New insights into the vitamin D requirements during pregnancy显示文摘Pregnancy represents a dynamic period with physical and physiological changes in both the mother and her developing fetus. The dramatic 2–3 fold increase in the active hormone 1,25(OH)2 D concentrations during the early weeks of pregnancy despite minimal increased calcium demands during that time of gestation and which are sustained throughout pregnancy in both the mother and fetus suggests an immunomodulatory role in preventing fetal rejection by the mother. While there have been numerous observational studies that support the premise of vitamin D's role in maintaining maternal and fetal well-being, until recently, there have been few randomized clinical trials with vitamin D supplementation. One has to exhibit caution,however, even with RCTs, whose results can be problematic when analyzed on an intent-to-treat basis and when there is high non-adherence to protocol(as if often the case), thereby diluting the potential good or harm of a given treatment at higher doses. As such, a biomarker of a drug or in this case 'vitamin' or pre-prohormone is better served. For these reasons, the effect of vitamin D therapies using the biomarker circulating 25(OH)D is a far better indicator of true 'effect.' When pregnancy outcomes are analyzed using the biomarker 25(OH)D instead of treatment dose, there are notable differences in maternal and fetal outcomes across diverse racial/ethnic groups, with improved health in those women who attain a circulating25(OH)D concentration of at least 100 nmol · L-1(40 ng · m L-1). Because an important issue is the timing or initiation of vitamin D treatment/supplementation, and given the potential effect of vitamin D on placental gene expression and its effects on inflammation within the placenta, it appears crucial to start vitamin D treatment before placentation(and trophoblast invasion); however, this question remains unanswered.Additional work is needed to decipher the vitamin D requirements of pregnant women and the optimal timing of supplementation, taking into account a variety of lifestyles, body types, baseline vitamin D status,and maternal and fetal vitamin D receptor(VDR) and vitamin D binding protein(VDBP) genotypes.Determining the role of vitamin D in nonclassical, immune pathways continues to be a challenge that once answered will substantiate recommendations and public health policies.Bruce W Hollis Carol L Wagner 2017Bone Research2017,5,3:5
4Minimization vs tailoring:Where do we stand with personalized immunosuppression during renal transplantation in 2015?显示文摘The introduction of novel immunosuppressive agents over the last two decades and the improvement of our diagnostic tools for early detection of antibodymediated injury offer us an opportunity, if not a mandate, to better match the immunosuppression needs of the individual patients with side effects of the therapy. However, immunosuppressive regimens in the majority of programs remain mostly protocol-driven, with relatively little inter-program heterogeneity in certain areas of the world. Emerging data showing different outcomes with a particular immunosuppressive strategy in populations with varying immunological risks underscore a real potential for 'personalized medicine' in renal transplantation. Studies demonstrating marked differences in the adverse-effect profiles of individual drugs including the risk for viral infections, malignancy and renal toxicity call for a paradigm shift away from a 'one size fits all' approach to an individually tailored immunosuppressive therapy for renal transplant recipients, assisted by both screening for predictors of graft loss and paying close attention to dose or class-related adverse effects. Our paper explores some of the opportunities during the care of these patients. Potential areas of improvements may include:(1) a thorough assessment of immunological and metabolic risk profile of each renal transplant recipient;(2) screening for predictors of graft loss and early signs of antibody-mediated rejection with donor-specific antibodies, protocol biopsies and proteinuria(including close follow up of adverse effects with dose adjustments or conversions as necessary); and(3) increased awareness of the possible link between poor tolerance of a given drug at a given dose and non-adherence with the prescribed regimen. 5Sex-specific effects of Eugenia punicifolia extract on gastric ulcer healing in rats显示文摘AIM To evaluate the sex-specific effects of a hydroalcoholic extract from Eugenia punicifolia(HEEP) leaves on gastric ulcer healing.METHODS In this rat study involving males, intact(cycling) females, and ovariectomized females, gastric ulcers were induced using acetic acid. A vehicle, lansoprazole, or HEEP was administered for 14 d after ulcer induction. Body weight was monitored throughout the treatment period. At the end of treatment, the rats were euthanized and the following in vivo and in vitro investigations were performed: macroscopic examination of the lesion area and organ weights, biochemical analysis, zymography, and evaluation of protein expression levels. Additionally, the concentration-dependent effect of HEEP was evaluated in terms of subacute toxicity and cytotoxicity.RESULTS Compared to the vehicle, HEEP demonstrated a great healing capacity by substantially reducing the ulcerative lesion area in males(52.44%), intact females(85.22%), and ovariectomized females(65.47%), confirming that HEEP accelerates the healing of acetic acidinduced gastric lesions and suggesting that this effect is modulated by female sex hormones. The antiulcer effect of HEEP was mediated by prostaglandin E2 only in male rats. Overall, the beneficial effect of HEEP was the highest in intact females. Notably, HEEP promoted the expression of vascular endothelial growth factor(intact vs ovariectomized females) and decreased the expression of Caspase-8 and Bcl-2(intact female vs male or ovariectomized female). Additionally, HEEP enhanced fibroblast proliferation and migration into a wounded area in vitro, confirming its healing effect. 6多种代用资料和模型模拟得到的亚洲季风在8.2ka显著气候影响的证据(英文)显示文摘Given the likelihood of future reductions in the strength of the Atlantic Meridional Overturning Circulation(AMOC),it is important to document how changes in the AMOC have altered climate patterns in the past and to assess the skill of coupled climate models in reproducing these teleconnections.Of past abrupt changes in the AMOC,the 8.2 ka event provides a particularly useful case study because its duration,magnitude of AMOC reduction and background climate state are closest to conditions expected in the future.In this research,we present an expanded proxy synthesis of the 8.2 ka event in monsoonal Asia,including new high-resolution lake and bog records,more sites from the East Asia monsoon region and proxies of winter monsoon strength.We compare proxy evidence with a new simulation of the 8.2 ka event using the Community Climate System Model version 3(CCSM3) and prescribing North Atlantic freshwater forcing according to the latest reconstructions.We find clear and objectively-determined evidence for 8.2 ka climate anomalies at nearly all of the fourteen proxy sites,emphasizing the strong and widespread impacts of the event in monsoonal Asia during both summer and winter seasons.The model simulation corroborates that these anomalies,described generally as a weakening of the summer monsoon and strengthening of the winter monsoon,were likely caused by a reduction of the AMOC.Examination of regional anomalies in East Asia reveals some spatial heterogeneity,however,that in the model simulation is caused by contraction of the seasonal migration of the subtropical monsoon front.The duration of climate anomalies at 8.2 ka in monsoonal Asia,both in proxy records and the model simulation,generally matches the duration of the event in Greenland ice core δ^(18)O,further supporting a tight connection to the North Atlantic.MORRILL Carrie WAGNER Amy J OTTO-BLIESNER Bette L ROSENBLOOM Nan 2011地球环境学报2011,2,3:3
