1Laparoscopic Partial Nephrectomy With Segmental Renal Artery Clamping:Technique and Clinical Outcomes显示文摘Background:Warm ischemic injury is one of the most important factors affecting renal function in partial nephrectomy(PN).The technique of segmental renal artery clamping emerges as an alternative to conventional renal artery clamping for renal hilar control.Objective:To evaluate the feasibility and efficiency of laparoscopic PN(LPN) with segmental renal artery clamping in comparison with the conventional technique.Design,setting,and participants:A total of 75 patients underwent LPN from June 2007 to November 2009.All patients had T1a or T1b tumor in one kidney and a normal contralateral kidney.Thirty-seven patients underwent surgeries with main renal artery clamping,and 38 underwent surgeries with segmental artery clamping.Intervention:All procedures were performed by the same laparoscopic surgeon.Measurements:Blood loss,operation time,warm ischemia(WI) time,and complications affected renal function before and after operation were recorded.Results and limitations:All LPNs were completed without conversion to open surgery or nephrectomy.The novel technique slightly increased WI time(p < 0.001) and intraoperative blood loss(p = 0.006),while it provided better postoperative affected renal function(p < 0.001) compared with the conventional technique.The total complication rate was 12%.Among the 38 cases where segmental renal artery clamping was performed,7 had to convert to the conventional method.Tumor size and location influenced the number of clamped segmental arteries.Long-term postoperative renal function is still awaited.Conclusions:LPN with segmental artery clamping is safe and feasible in clinical practice.It minimizes the intraoperative WI injury and improves early postoperative affected renal function compared with main renal artery clamping.(C) 2010 European Association of Urology.Published by Elsevier B.V.All rights reserved.Shao,P.F. 7ON THE TENSILE MECHANICAL PROPERTY OF Si-Mn TRIP STEELS AT HIGH STRAIN RATE显示文摘Tensile mechanical properties of 1.6Si-1.58Mn-0.195C TRIP (transformation-induced plasticity) steels under high strain rate and effects of DP (dual-phase) treatments were studied and compared to the quasi-static tensile behavior. The results show that the increasing of strain rate leads to increasing in their strengths and decreasing in the uniform elongation remarkably. Because the stable retained austenite in TRIP steel can transform to martensite during tensile testing and the material exhibits excellent characteristic of transformation induced plasticity, the plastic deformation behavior is evidently improved and the combination of strength and elongation is superior to that of dual-phase steel, although its strength is smaller than that of DP steel. However, DP treated steel shown lower elongation under dynamic tension in spite of higher strength. A model was proposed to explain the excellent elongation rate of TRIP steel compared with DP steel on the basis of SEM analysis and the strength of the components in microstructure.X.C. Wei, L. 14Common non-synonymous polymorphisms in the BRCA1 Associated RING Domain (BARD1) gene are associated with breast cancer susceptibility:a case-control analysis显示文摘The BRCA1 Associated RING Domain(BARD1) gene has been identified as a high penetrance gene for breast cancer, whose germline and somatic mutations were reported in both non-BRCA1/2 hereditary site-specific and sporadic breast cancer cases. BARD1 plays a crucial role in tumor repression, along with its heterodimeric partner BRCA1. In the current study, we tested the hypothesis that common non-synonymous polymorphisms in BARD1 are associated with breast cancer susceptibility in a case-control study of 507 patients with incident breast cancer and 539 frequency-matched cancer-free controls in Chinese women. We genotyped all three common(minor allele frequency (MAF) > 0.10) non-synonymous polymorphisms(Pro24Ser, Arg378Ser, and Val507Met) in BARD1. We found that the BARD1 Pro24Ser variant genotypes(24Pro/Ser and 24Ser/Ser) and Arg378Ser variant homozygote 378Ser/Ser were associated with a significantly decreased breast cancer risk, compared with their wild-type homozygotes, respectively. Furthermore, a significant locus-locus interaction was evident between Pro24Ser and Arg378Ser (P-int = 0.032). Among the 378Ser variant allele carriers, the 24Pro/Pro wild-type homozygote was associated with a significantly increased breast cancer risk (adjusted OR = 1.81, 95% CI = 1.11-2.95), but the subjects having 24Pro/Ser or Ser/Ser variant genotypes had a significantly decreased risk(adjusted OR = 0.74, 95% CI = 0.56-0.99). In stratified analysis, this locus-locus interaction was more evident among subjects without family cancer history, those with positive estrogen receptor (ER) and individuals with negative progesterone receptor(PR). These findings indicate that the potentially functional polymorphisms Pro24Ser and Arg378Ser in BARD1 may jointly contribute to the susceptibility of breast cancer.Huo, X Hu, Z. B Zhai, X.
