1帕博利珠单抗单用或与放疗联用治疗转移性非小细胞肺癌:两个随机试验的汇总分析显示文摘背景放疗可以提高整个机体对免疫治疗的应答。在Ⅱ期PEMBRO-RT研究和Ⅰ/Ⅱ期MDACC研究中,患有转移性非小细胞肺癌(NSCLC)的患者被随机分配入组,接受免疫治疗(帕博利珠单抗)+放疗联合疗法,或免疫治疗单一疗法。当上述2个研究单独分析时,联合疗法组显示出潜在获益。由于每个研究的样本量较小,缓解率和结局并未显示出统计学意义,然而却有显著的临床获益。因此,本研究进行汇总分析,来判断放疗是否会改善转移性NSCLC患者的免疫治疗应答。方法PEMBRO-RT和MDACC研究纳入标准:患者年龄≥18岁,患有转移性NSCLC,且有≥1处未经放疗照射的病灶,以便进行射野外应答监测。PEMBRO-RT研究纳入曾接受过化疗患者,MDACC研究纳入曾接受过治疗或新诊断患者。2个研究中的患者均未接受过免疫治疗。在PEMBRO-RT研究中患者被等比例随机分配入组,并根据吸烟状态进行分层(分为<10年组和≥10年组)。MDACC研究的患者根据放疗计划可行性被等比例随机分配入2个受试组。由于联合治疗组的干预本质,每个研究中的放疗均不适用盲法。在2个研究中,不论是否进行放疗,均静脉滴入帕博利珠单抗(每3周200 mg)。在PEMBRO-RT研究中,在放疗(24 Gy 3次分割照射)结束后1周给予第1剂帕博利珠单抗。在MDACC研究中,在第1次放疗(50 Gy 4次分割照射或45 Gy 15次分割照射)同时给予帕博利珠单抗。仅检测未经照射病灶的应答。本研究的终点为最佳射野外(远隔)应答率(ARR)、最佳射野外疾病控制率(ACR)、12周时ARR、12周时ACR、无进展生存期(PFS)和总生存期(OS)。2个研究的意向治疗(ITT)人群均纳入分析。PEMBRO-RT研究(NCT02492568)和MDACC研究(NCT02444741)均在ClinicalTrials.gov上注册。发现纳入148例患者,76例接受帕博利珠单抗治疗,72例接受帕博利珠单抗+放疗治疗。所有患者随访时间中位数为33个月[四分位距(IQR):32.4~33.6]。148例患者中124例(84%)组织学特征为非鳞癌,111例(75%)患者曾经接受过化疗。组间没有基线特征差异,包括PD-L1表达状态和转移灶体积。最常见的照射部位为肺转移灶(39%,28/72)、胸腔内淋巴结(21%,15/72)和非原发灶(17%,12/72)。帕博利珠单抗组和联合治疗组的最佳ARR分别为19.7%(15/76)和41.7%(30/72),OR=2.96,95%CI:1.42~6.20,P=0.0039;最佳ACR分别为43.4%(33/76)和65.3%(47/72),OR=2.51,95%CI:1.28~4.91,P=0.0071;PFS中位数分别为4.4(IQR:2.9~5.9)和9.0个月(IQR:6.8~11.2),HR=0.67,95%CI:0.45~0.99,P=0.045;OS中位数分别为8.7(IQR:6.4~11.0)和19.2个月(IQR:14.6~23.8),OR=0.67,95%CI:0.54~0.84,P=0.0004。在汇总分析中没有发现新的安全问题。解读帕博利珠单抗免疫疗法+放疗显著提高转移性NSCLC患者的应答和改善治疗结局。这些结果需要在三期临床试验中进行验证。陈大卫(翻译) 于金明(校对) Willemijn S M E Theelen Vivek Verma Brian P Hobbs Heike M U Peulen Joachim G J V Aerts Idris Bahce Anna Larissa N Niemeijer Joe Y Chang Patricia M de Groot Quynh-Nhu Nguyen Nathan I Comeaux George R Simon Ferdinandos Skoulidis Steven H Lin Kewen He Roshal Patel John Heymach Paul Baas James W Welsh 2021中华肿瘤防治杂志2021,28,24:49
2土壤保持耕作——全球农业可持续发展优先领域显示文摘从农业活动对全球变化以及对农业可持续发展的影响进而导致全球性土壤保持需求的迫切性增加出发,追踪了国外土壤保持耕作领域的研究和发展动态;对中国该领域研究发展现状和研究水平,从时间、深度、方法、生产应用等方面与国外进行了比较分析。中国是一个水土流失和耕地退化严重的农业大国,而从全球统计数字来看,中国土壤保持耕作面积目前仅占全球保持耕作面积的0.2%,仅占全国耕地面积的0.1%,其现状与中国耕地资源和环境的继续退化以及对土壤保持耕作需求的增加极不相符。加强中国土壤保持耕作系统研究和土壤保持耕作“节能固碳”长期建设不仅对中国而且对全球变化及农业可持续发展具有重大意义。鉴于中国土壤保持耕作领域研究和发展所存在的不足和所面临的更大挑战,以及该领域研究涉及到复杂的农业系统“耕作管理-生物过程-环境变化”相互作用及其系统能流-碳流循环过程,尤其农业生态系统碳汇/源估量尚存在诸多不确定性因素等科学问题,有必要运用综合的系统性研究分析方法,借助国外长期试验和理论研究经验,以及系统模型模拟研究手段等多学科和交叉学科知识,加速提升中国土壤保持耕作领域的科研实力和水平,为实现碳汇/源科学调控管理、减缓农业对温室效应贡献、促进农业可持续发展提供科学依据。王小彬 蔡典雄 华珞 Hoogmoed W B Oenema O Perdok U D 2006中国农业科学2006,39,4:35
3北京首钢工业区大气重金属污染树叶的磁学响应显示文摘在北京西郊首钢工业区及附近混杂的居民区、公园绿地和农村耕作区采集了55个一年生的树叶样品,并对其进行岩石磁学和元素含量分析.结果表明:磁性矿物的含量和粒径由污染源区向周边递减.研究区域树叶的磁性由低矫顽力的磁铁矿主导.进一步研究表明样品的主要磁性参数(磁化率、非磁滞剩磁以及饱和等温剩磁)与元素Fe,Pb,Cr,V和Zn呈显著相关关系(0.73≤R≤0.88).因此树叶的磁参数可以作为大气重金属污染研究的理想替代指标.胡守云 段雪梅 沈明洁 Blaha U Rosler W 闫海涛 Appel E Hoffmann V 2008科学通报2008,53,4:19
4经菱形窝切除脑干海绵状血管瘤显示文摘目的探讨28例经菱形窝入路切除的脑干海绵状血管瘤的适应证、手术技巧及术中注意点。方法术中采取坐位,枕下正中开颅,广泛打开小脑延髓裂;在脑干表面有色素沉淀或膨隆处,或在B超、导航引导、术中颅神经监护下切开脑干,病变暴露后清除血肿,然后切除病变,保留含铁血黄素沉淀的胶质组织。结果28例中有21例位于桥脑;术中发现脑干表面有含铁血黄素沉淀21例,其中脑干表面膨隆9例;所有病变位于脑干背侧;随访期间:症状和体征较术前改善的19例,保持术前水平的5例,加重、出现新的颅神经症状、或其他神经功能障碍(共济失调、运动障碍、感觉障碍)共4例。MRI随访未见病变残留或复发。结论经菱形窝入路主要适于桥脑部、且位于背侧的海绵状血管瘤;脑干表面的含铁血黄素沉淀和(或)膨隆是脑干的安全进入区,术中结合神经电生理监测、B超以及导航是手术成功的关键;术中尽可能全切病变,但保留含铁血黄素沉淀的胶质组织。卞留贯 Bertalanffy H 孙青芳 Sure U Tirakotai W 沈建康 2007中华神经外科杂志2007,23,12:12

6Relationship between magnetic parameters and heavy element contents of arable soil around a steel company,Nanjing显示文摘Magnetic parameters and element contents were determined in core NJ008 collected from the uppermost ca. 40 cm in a steel company in southwest Nanjing. The results showed that magnetic susceptibility (χ), saturation isothermal remanent magnetization (SIRM) and anhysteretic remanent magnetization (ARM) were enhanced in the uppermost 20 cm, with a mean magnetic susceptibility value of 112.5×10-8 m3 kg-1. Below 20 cm, χ decreased sharply with a mean value of 27.8×10-8m3 kg-1. Low-coercivity minerals such as magnetite dominate in arable soils, while the relative content of antiferromagnetic minerals increases below 20 cm. Heavy metals (Ni, Cu, Fe, Pb, V, and Zn) have similar vertical trends as χ. Principal component analysis reveals common high loadings of the same factor for magnetic concentration parameters (χ, ARM, and SIRM) and elements (Ni, Cu, Fe, Pb, V, and Zn) with an excellent linear correlation (0.69≤R≤0.98) between them. Magnetic susceptibility of paddy soil core NJ013, which had the same parent material as NJ008 but was far from pollution sources, showed stable values of magnetic concentration parameters along the whole core. Absolute values correspond to the so-called magnetic background value (below 20 cm) of NJ008. This indicates that pesticide and fertilizer had little effect on magnetic signals of the upper part of core NJ008 and the extremely enhanced magnetic concentration parameters originate from the steel company emission. Although, the arable soil does not reveal the pollution history and transportation due to annual ploughing, the significant linear relationship between magnetic concentration parameters and heavy metal contents suggests that magnetic parameters can serve as a proxy for quickly detecting soil metallic pollution and estimating the extent of contamination.BLAHA U ROESLER W APPEL E 2010Science China Earth Sciences2010,53,3:10
7Magnetic response to atmospheric heavy metal pollution recorded by dust-loaded leaves in Shougang industrial area, western Beijing显示文摘Fifty-five evergreen tree's leaves growing less than one year were collected from Shougang industrial area in western suburb of Beijing, including steel plants and its ambient residential areas, recreational parks and farmlands. Rock magnetic properties and heavy metal contents were studied. The results show that the magnetic properties of leaf samples are predominated by low-coercivity magnetite, and both the concentration and grain size of magnetite particles gradually decreased with the distance from the main pollution source increases. Moreover, there is a significant linear relationship between magnetic parameters (the low-field magnetic susceptibility, saturation isothermal remanent magnetization and anhysteretic remanent magnetization) and heavy metals contents (Fe, Pb, V, Cr and Zn) (0.73≤R≤ 0.88).
