1An App knock-in rat model for Alzheimer’s disease exhibiting Aβand tau pathologies,neuronal death and cognitive impairments显示文摘A major obstacle in Alzheimer’s disease(AD)research is the lack of predictive and translatable animal models that reflect disease progression and drug efficacy.Transgenic mice overexpressing amyloid precursor protein(App)gene manifest non-physiological and ectopic expression of APP and its fragments in the brain,which is not observed in AD patients.The App knock-in mice circumvented some of these problems,but they do not exhibit tau pathology and neuronal death.We have generated a rat model,with three familiar App mutations and humanized Aβsequence knocked into the rat App gene.Without altering the levels of full-length APP and other APP fragments,this model exhibits pathologies and disease progression resembling those in human patients:deposit of Aβplaques in relevant brain regions,microglia activation and gliosis,progressive synaptic degeneration and AD-relevant cognitive deficits.Interestingly,we have observed tau pathology,neuronal apoptosis and necroptosis and brain atrophy,phenotypes rarely seen in other APP models.This App knock-in rat model may serve as a useful tool for AD research,identifying new drug targets and biomarkers,and testing therapeutics.Keliang Pang Richeng Jiang Wei Zhang Zhengyi Yang Lin-Lin Li Makoto Shimozawa Simone Tambaro Johanna Mayer Baogui Zhang Man Li Jiesi Wang Hang Liu Ailing Yang Xi Chen Jiazheng Liu Bengt Winblad Hua Han Tianzi Jiang Weiwen Wang Per Nilsson Wei Guo Bai Lu 2022Cell Research2022,32,2:6
2Ovarian cancer standard of care: are there real alternatives?显示文摘Ovarian cancer remains a major issue for gynecological oncologists, and most patients are diagnosed when the disease is already advanced with a poor chance of survival. Debulking surgery followed by platinum-taxane chemotherapy is the current standard of care, but based on several different strategies currently under evaluation, some encouraging data have been published in the last 4 to 5 years. This review provides a state-of-the-art overview of the available alternatives to conventional treatment and the most promising new combinations. For example, neoadjuvant chemotherapy does not seem to be inferior to primary debulking. Despite its outcome improvements, intraperitoneal chemotherapy struggles for acceptance due to the heavy toxicity. Dose-dense chemotherapy, after showing an impressive efficacy in Asian populations, has not produced equal results in a European cohort, and the results of alternative platinum doublets are not superior to those of carboplatin and paclitaxel. In this setting, adherence to a maintenance therapy after first-line treatment and multiple(primarily antiangiogenic) agents appears to be effective. Although many questions, including the duration of maintenance treatment and the use of bevacizumab beyond progression, remain unanswered, new biologic agents, such as poly(ADP-ribose) polymerase(PARP) inhibitors, nintedanib, and mitogen-activated protein/extracellular signal-regulated kinase(MEK) inhibitors, have emerged as potential therapeutic options in the very near future. 3Vertical hepatitis C virus transmission:Main questions and answers显示文摘Hepatitis C virus(HCV) affects about 3% of the world's population and peaks in subjects aged over 40 years. Its prevalence in pregnant women is low(1%-2%) in most western countries but drastically increases in women in developing countries or with high risk behav-iors for blood-transmitted infections. Here we review clinical, prognostic and therapeutic aspects of HCV in-fection in pregnant women and their offspring infected through vertical transmission. Pregnancy-related im-mune weakness does not seem to affect the course of acute hepatitis C but can affect the progression of chronic hepatitis C. In fact, postpartum immune res-toration can exacerbate hepatic inflammation, thereby worsening the liver disease, particularly in patients with liver cirrhosis. HCV infection increases the risk of gestational diabetes in patients with excessive weight gain, premature rupture of membrane and caesarean delivery. Only 3%-5% of infants born to HCV-positive mothers have been infected by intrauterine or perinatal transmission. Maternal viral load, human immunode-ficiency virus coinfection, prolonged rupture of mem-branes, fetal exposure to maternal infected blood con-sequent to vaginal or perineal lacerations and invasive monitoring of fetus increase the risk of viral transmis-sion. Cesarean delivery and breastfeeding increases the transmission risk in HCV/human immunodeficiency virus coinfected women. The consensus is not to offer antivi-ral therapy to HCV-infected pregnant women because it is based on ribavirin(pregnancy category X) because of its embryocidal and teratogenic effects in animal spe-cies. In vertically infected children, chronic C hepatitis is often associated with minimal or mild liver disease and progression to liver cirrhosis and hepatocarcinoma is lower than in adults.
