| 2 | Biomedical Perspective of Electrochemical Nanobiosensor显示文摘Electrochemical biosensor holds great promise in the biomedical area due to its enhanced specificity, sensitivity, label-free nature and cost effectiveness for rapid point-of-care detection of diseases at bedside. In this review, we are focusing on the working principle of electrochemical biosensor and how it can be employed in detecting biomarkers of fatal diseases like cancer, AIDS, hepatitis and cardiovascular diseases. Recent advances in the development of implantable biosensors and exploration of nanomaterials in fabrication of electrodes with increasing the sensitivity of biosensor for quick and easy detection of biomolecules have been elucidated in detail. Electrochemical-based detection of heavy metal ions which cause harmful effect on human health has been discussed. EUS has the unique advantage of combining ultrasound and endoscopy to obtain detailed information of the gastrointestinal tract. Over the past decade, the use of EUS in liver diseases has been increasing. EUS, which was initially used as a diagnostic tool, is now having increasing therapeutic role as well. We provide a review of the application of EUS in the diagnostic and therapeutic aspects of liver disease. | 4 | Propofol vs midazolam sedation for elective endoscopy in patients with cirrhosis:A systematic review and meta-analysis of randomized controlled trials显示文摘BACKGROUND Patients with cirrhosis frequently require sedation for elective endoscopic procedures.Several sedation protocols are available,but choosing an appropriate sedative in patients with cirrhosis is challenging.AIM To conduct a systematic review and meta-analysis to compare propofol and midazolam for sedation in patients with cirrhosis during elective endoscopic procedures in an attempt to understand the best approach.METHODS This systematic review and meta-analysis was conducted using the PRISMA guidelines.Electronic searches were performed using MEDLINE,EMBASE,Central Cochrane,LILACS databases.Only randomized control trials(RCTs)were included.The outcomes studied were procedure time,recovery time,discharge time,and adverse events(bradycardia,hypotension,and hypoxemia).The risk of bias assessment was performed using the Revised Cochrane Risk-of-Bias tool for randomized trials(RoB-2).Quality of evidence was evaluated by GRADEpro.The meta-analysis was performed using Review Manager.RESULTS The search yielded 3,576 records.Out of these,8 RCTs with a total of 596 patients(302 in the propofol group and 294 in the midazolam group)were included for the final analysis.Procedure time was similar between midazolam and propofol groups(MD:0.25,95%CI:-0.64 to 1.13,P=0.59).Recovery time(MD:-8.19,95%CI:-10.59 to-5.79,P<0.00001).and discharge time were significantly less in the propofol group(MD:-12.98,95%CI:-18.46 to-7.50,P<0.00001).Adverse events were similar in both groups(RD:0.02,95%CI:0-0.04,P=0.58).Moreover,no significant difference was found for bradycardia(RD:0.03,95%CI:-0.01 to 0.07,P=0.16),hypotension(RD:0.03,95%CI:-0.01 to 0.07,P=0.17),and hypoxemia(RD:0.00,95%CI:-0.04 to 0.04,P=0.93).Five studies had low risk of bias,two demonstrated some concerns,and one presented high risk.The quality of the evidence was very low for procedure time,recovery time,and adverse events;while low for discharge time.CONCLUSION This systematic review and meta-analysis based on RCTs show that propofol has shorter recovery and patient discharge time as compared to midazolam with a similar rate of adverse events.These results suggest that propofol should be the preferred agent for sedation in patients with cirrhosis. | John Alexander Lata Guacho Diogo Turiani Hourneaux de Moura Igor Braga Ribeiro Alberto Machado da Ponte Neto Shailendra Singh Marina Gammaro Baldavira Tucci Wanderley Marques Bernardo Eduardo Guimarães Hourneaux de Moura | 2020 | World Journal of Gastrointestinal Endoscopy2020,12,8: | 4 |
| 5 | Antifertility effect of chronically administered Tabernaemontana divaricata leaf extract on male rats显示文摘Objective:To investigate the antifertility effect of chronically administered Tabernaemontana divarkala(T.divaricata) leaf extract on male rats.Methods:The effect of 50%ethanol extract of T.divaricata leaves on reproduction was studied on male rats.The study was divided into four groups of five animals each.The first groups(1) received vehicle alone to serve as control. The second,third and fourth groups(Ⅱ,ⅡandⅣ) of animals were administered the leaf extract daily at 50 mg/kg body weight,p.o.,100 nig/kg body weight,p.o.,and 200 mg/kg body weight, p.o.,respectively,for a period of 60 days.Results:Significant decreases in the weight of testes, epididymis,seminal vesicle and ventral prostate were observed.A dose related reduction in the testicular sperm count,epididymal sperm count and motility,number of fertile male,ratio between delivered and inseminated females and numbers of pups were observed.The testis showed a clear correlation between the dose and severity of lesions of seminiferous epithelium. In general,the seminiferous tubules appear reduced in size with a frequently filled eosinophilic material.Spermatogenesis arrested at the secondary spermatocyte stage.Pachytene spermatocytes were undergoing degeneration.Disorganisation and sloughing of immature germ cell were visible. Leydinf cells were atrophied.No morphological changes were observed in Sertoli cells.Significant reduction in serum concentration of luteinizing hormone and testosterone were observed.No distinct change in serum FSH concentration was recorded.The final body weights of all groups were elevated markedly.No alterations were recorded in any hematologiocal parameters. | 8 | Development of Tunable Silk Fibroin/Xanthan Biopolymeric Scaffold for Skin Tissue Engineering Using L929 Fibroblast Cells显示文摘Skin acts as protective barrier against a number of factors such as dust,opportunistic microbial and viral infections,regulates body temperature and waste discharge.Fibroblast cell population plays an important role in devclopment of skin architecturc.A scaffold having capability to support and enhance fibroblast growth is a viable option for wound dressing material which can shorten the time for wound to heal.In this work,Silk Fibroin(SF)and Xanthan(Xa)were blended in three ratios 80 SF:20 Xa(SFX82),60 SF:40 Xa(SFX64),and 50SF:50 Xa(SFX55)to create SF/Xa scaffold.Miscibility and other physicochemical properties of SF/Xa scaffold are functions of blending ratios and blend with the ratio 80 SF:20 Xa has the highest miscibility.Thermal properties of SF/Xa blends are a function of miscibility with SFX82 having superior thermal propertis of all fabricated scaffolds.The porosity of SF/Xa scaffolds is in the range of 67%to 50%,with pore size of 58.1 um-45.5 um,water uptake capacity of 92%-86%,and surface roughness of 49.95 nm-385 nm.SFX82 shows highest growth rate of L929 fibroblast cells indicating its superiority over other scaffolds for providing biological cues for the growth and proliferation of fibroblastic cells in natural environment.SFX82 scaffold is found to be most suitable for fibroblastic cells thereby enhancing the tissue regeneration at wound site. | Shailendra Singh Shera Rathindra Mohan Banik | 2021 | Journal of Bionic Engineering2021,18,1: | 1 |

| 12 | Invitro Biodegradability of Silk Fibroin/Xanthan Biopolymeric Composite Scaffolds显示文摘Silk fibroin/xanthan scaffolds were prepared by blending silk fibroin and xanthan in the ratios 80SF:20Xa(SFX82),60SF:40Xa(SFX64),and 50SF:50Xa(SFX55)using freeze drying method.In-vitro degradation behavior of the prepared scaffolds was studied for 37 days in phosphate buffer saline.The degradation rate was the function of silk fibroin,xanthan and^-crystallite contents in the silk fibroin/xanthan composites.SFX82 degraded extremely slowly whereas SFX55 showed faster degradation rate.Hydrophilic xanthan was the main contributor of weight loss.SFX82 and SFX64 exhibited surface degradation whereas SFX55 showed bulk degradation which indicated that higher silk fibroin ratios favor surface degradation.Due to bulk degradation,SFX55 showed maximum surface roughness among the composite scaffolds.The FTIR spectrum revealed total loss of xanthan from the composites after degradation.The broad and low-intensity peaks in the FTIR spectrum of composite scaffolds con finned reduction in 0-sheet crystallite content during degradation.XRD analysis also confirmed reduction in 0-sheet crystals and revealed that degraded composite scaffold had predominantly amorphous structure.The degraded scaffold showed higher porous structure than the non-degraded scaffold.The in vitro degradability testing gives a good approximation of degradation of scaffold in vivo and helps in designing a robust biopolymeric composite scaffold for tissue engineering. | Shailendra Singh Shera Shraddha Sahu Rathindra Mohan Banik | 2019 | Journal of Bionic Engineering2019,16,2: | 0 |

| 14 | State of the art in finite element approaches for milling process:a review显示文摘Over a century,metal cutting has been observed as a vital process in the domain of manufacturing.Among the numerous available metal-cutting processes,milling has been considered as one of the most employable processes to machine a variety of engineering materials productively.In the milling process,material removal occurs when the workpiece is fed against a rotating tool with multiple cutting edges.In order to maximize the profitability of metal cutting operations,it is essential that the various input and output variable relationships are analyzed and optimized.The experimental method of studying milling processes is costly and time demanding,particularly when a large variety of elements such as cutting tool shape,materials,cutting conditions,and so on,are included.Due to these issues,other alternatives emerged in the form of mathematical simulations that employ numerical methods.The finite element approaches have well-proven to be the most practical and commonly utilized numerical methods.The finite element model(FEM)can be used to determine the various physical interactions occurring during the machining process along with the prediction of various milling characteristics,such as cutting forces,cutting temperature,stresses,etc.,with the help of milling inputs.In the present article,various research studies in the broad milling process domain practiced with numerous finite element approaches have been critically reviewed and reported.It further highlights the several experimental and finite element approaches-based research studies that attempted to analyze and optimize the overall performance of the different milling processes.In recent years,various investigators have explored numerous ways to enhance milling performance by probing the different factors that influence the quality attributes.Some of the studies have also been found to be focused on the economic impacts of milling and various process inputs that affect milling performance.Furthermore,various milling factors’impact on the performance characteristics are presented and critically discussed.The issues related to the recent improvements in tool-work interaction modeling,experimental techniques for acquiring various milling performance measures,and the aspects of turn and micro-milling with finite element-based modeling have been further highlighted.Among the various available classifications in the milling process as employed in industries,face milling is more strongly established compared to other versions such as end milling,helical milling,gear milling,etc.The final section of this research article explores the various research aspects and outlines future research directions. | Shailendra Chauhan Rajeev Trehan Ravi Pratap Singh | 2023 | Advances in Manufacturing2023,11,4: | 0 |

| 16 | Clinical characteristics of COVID-19 patients who underwent tracheostomy and its effect on outcome:A retrospective observational study显示文摘BACKGROUND The exponential rise in Coronavirus disease 2019(COVID-19)cases has resulted in an increased number of patients requiring prolonged ventilatory support and subsequent tracheostomy.With the limited availability of literature regarding the outcomes of COVID-19 patients with tracheostomy,we attempted to study the clinical characteristics and multiple parameters affecting the outcomes in these patients.AIM To determine all-cause mortality following tracheostomy and its association with various risk factors in COVID-19 patients.METHODS This retrospective study included 73 adult COVID-19 patients admitted to the ICU between 1 April,2020 and 30 September,2021 who underwent tracheostomy as a result of acute respiratory failure due to COVID-19.The data collected included demographics(age,sex),comorbidities,type of oxygen support at admission,severity of COVID-19,complications,and other parameters such as admission to tracheostomy,intubation to tracheostomy,ICU stay,hospital stay,and outcome.RESULTS This study included 73 adult patients with an average age of 52±16.67 years,of which 52%were men.The average time for admission to tracheostomy was 18.12±12.98 days while intubation to tracheostomy was 11.97±9 days.The mortality rate was 71.2%and 28.8%of patients were discharged alive.The mean duration of ICU and hospital stay was 25±11 days and 28.21±11.60 days,respectively.Greater age,severe COVID-19,mechanical ventilation,shock and acute kidney injury were associated with poor prognosis;however,early tracheostomy in intubated patients resulted in better outcomes.CONCLUSION Patients with severe COVID-19 requiring mechanical ventilation have a poor prognosis but patients with early tracheostomy may benefit with no added risk.We recommend that the timing of tracheostomy be decided on a case-by-case basis and a well-designed randomised controlled trial should be performed to elucidate the potential benefit of early tracheostomy in such patients. | Yudhyavir Singh Kapil Dev Soni Abhishek Singh Nikita Choudhary Fahina Perveen Richa Aggarwal Nishant Patel Shailendra Kumar Anjan Trikha | 2022 | World Journal of Virology2022,11,6: | 0 |
| 17 | Numerical Analysis of Phase Change and Container Materials for Thermal Energy Storage in the Storage Tank of Solar Water Heating System显示文摘This study evaluates the effectiveness of phase change materials(PCMs) inside a storage tank of warm water for solar water heating(SWH) system through the theoretical simulation based on the experimental model of S.Canbazoglu et al.The model is explained by five fundamental equations for the calculation of various parameters like the effectiveness of PCMs,the mass of hot water,total heat content,and duration of charging.This study simulated eleven PCMs to analyze their effectiveness like Sodium hydrogen phosphate dodecahydrate(SHPD),OM 37,N-Eicosane(NE),Lauric acid(LA),Paraffin wax(PW),OM 48,Paraffin wax C_(20-33)(PW-C20-33),Sodium acetate trihydrate(SAT),Palmitic acid(PA),Myristic acid(MA),and Stearic acid(SA).Among all PCMs,the SHPD has found the highest value of effectiveness factor of 3.27.So,it is the most recommended PCM for the storage tank of the SWH system.The study also includes the melt fraction analysis of all enumerated PCMs corresponding to container materials of stainless steel,glass,aluminum mixed,tin,aluminum,and copper.This melt fraction analysis is performed by making a coding program in the FORTRAN programming language.Through the analysis,copper container material is found to have high melting rate for all PCMs so it is superior to other container materials. | SINGH Shailendra ANAND Abhishek SHUKLA Amritanshu SHARMA Atul | 2024 | Journal of Thermal Science2024,33,2: | 0 |
| 18 | Clinical characteristics and outcomes of COVID-19 in patients with autoimmune hepatitis:A population-based matched cohort study显示文摘BACKGROUND Patients with autoimmune hepatitis(AIH)require life-long immunosuppressive agents that may increase the risk of poor coronavirus disease 2019(COVID-19)outcomes.There is a paucity of large data at the population level to assess whether patients with AIH have an increased risk of severe diseases.AIM To evaluate the impact of pre-existing AIH on the clinical outcomes of patients with COVID-19.METHODS We conducted a population-based,multicenter,propensity score-matched cohort study with consecutive adult patients(≥18 years)diagnosed with COVID-19 using the TriNeTx research network platform.The outcomes of patients with AIH(main group)were compared to a propensity score-matched cohort of patients:(1)Without chronic liver disease(CLD);and(2)Patients with CLD except AIH(non-AIH CLD)control groups.Each patient in the main group was matched to a patient in the control group using 1:1 propensity score matching to reduce confounding effects.The primary outcome was all-cause mortality,and secondary outcomes were hospitalization rate,need for critical care,severe disease,mechanical ventilation,and acute kidney injury(AKI).For each outcome,the risk ratio(RR)and confidence intervals(CI)were calculated to compare the association of AIH with the outcome.RESULTS We identified 375 patients with AIH,1647915 patients with non-CLD,and 15790 patients with non-AIH CLD with COVID-19 infection.Compared to non-CLD patients,the AIH cohort had an increased risk of all-cause mortality(RR=2.22;95%CI:1.07-4.61),hospitalization rate(RR=1.78;95%CI:1.17-2.69),and severe disease(RR=1.98;95%CI:1.19-3.26).The AIH cohort had a lower risk of hospitalization rate(RR=0.72;95%CI:0.56-0.92),critical care(RR=0.50;95%CI:0.32-0.79),and AKI(RR=0.56;95%CI:0.35-0.88)compared to the non-AIH CLD patients.CONCLUSION Patients with AIH are associated with increased hospitalization risk,severe disease,and all-cause mortality compared to patients without pre-existing CLD from the diagnosis of COVID-19.However,patients with AIH were not at risk for worse outcomes with COVID-19 than other causes of CLD. | Arunkumar Krishnan Ruhee A Patel Yousaf Bashir Hadi Diptasree Mukherjee Sarah Shabih Shyam Thakkar Shailendra Singh Tinsay A Woreta Saleh A Alqahtani | 2023 | World Journal of Hepatology2023,15,1: | 0 |
| 19 | Screening of COVID-19 Patients Using Deep Learning and IoT Framework显示文摘In March 2020,the World Health Organization declared the coronavirus disease(COVID-19)outbreak as a pandemic due to its uncontrolled global spread.Reverse transcription polymerase chain reaction is a laboratory test that is widely used for the diagnosis of this deadly disease.However,the limited availability of testing kits and qualified staff and the drastically increasing number of cases have hampered massive testing.To handle COVID19 testing problems,we apply the Internet of Things and artificial intelligence to achieve self-adaptive,secure,and fast resource allocation,real-time tracking,remote screening,and patient monitoring.In addition,we implement a cloud platform for efficient spectrum utilization.Thus,we propose a cloudbased intelligent system for remote COVID-19 screening using cognitiveradio-based Internet of Things and deep learning.Specifically,a deep learning technique recognizes radiographic patterns in chest computed tomography(CT)scans.To this end,contrast-limited adaptive histogram equalization is applied to an input CT scan followed by bilateral filtering to enhance the spatial quality.The image quality assessment of the CT scan is performed using the blind/referenceless image spatial quality evaluator.Then,a deep transfer learning model,VGG-16,is trained to diagnose a suspected CT scan as either COVID-19 positive or negative.Experimental results demonstrate that the proposed VGG-16 model outperforms existing COVID-19 screening models regarding accuracy,sensitivity,and specificity.The results obtained from the proposed system can be verified by doctors and sent to remote places through the Internet. | Harshit Kaushik Dilbag Singh Shailendra Tiwari Manjit Kaur Chang-Won Jeong Yunyoung Nam Muhammad Attique Khan | 2021 | Computers, Materials & Continua2021,,12: | 0 |
| 20 | Risk of pancreatic cancer in individuals with celiac disease in the United States:A population-based matched cohort study显示文摘BACKGROUND Celiac disease(CD)has been associated with gastrointestinal malignancies.However,the magnitude of the risk of pancreatic cancer(PC)associated with CD is much less clear,and risks have not been estimated from large populations.AIM To assess the risk of PC in CD patients.METHODS We conducted a population-based,multicenter,propensity score-matched cohort study with consecutive patients diagnosed with CD using the TriNeTx research network platform.We examined the incidence of PC in patients with CD compared with a matched cohort of patients without CD(non-CD,controls).Each patient in the main group(CD)was matched to a patient in the control group using 1:1 propensity score matching to reduce confounding effects.The incidence of PC was estimated using a Cox proportional hazards model with a hazard ratio(HR)and 95%confidence interval(CI).RESULTS A total of 389980 patients were included in this study.Among them,155877 patients had a diagnosis of CD,and the remaining 234103 individuals without CD were considered a control cohort.The mean duration of follow-up for patients in the CD and control cohorts was 5.8±1.8 and 5.9±1.1 years,respectively.During the follow-up,309 patients with CD developed PC,whereas 240 patients developed PC in the control group(HR=1.29;95%CI:1.09-1.53).In the secondary analyses in the first year after diagnosis of CD,patients with CD were at a significant increase in risk for PC;151 patients with CD had an incidence of PC compared with 96 incidences of PC among the patients in the non-CD control group(HR=1.56;95%CI:1.20-2.01)and sensitivity analysis showed similar magnitude to the one generated in the primary and secondary analysis.CONCLUSION Patients with CD are at increased risk of PC.Risk elevation persists beyond the first year after diagnosis to reference individuals without CD from the general population. | Arunkumar Krishnan Yousaf Bashir Hadi Sarah Shabih Diptasree Mukherjee Ruhee A Patel Rushik Patel Shailendra Singh Shyam Thakkar | 2023 | World Journal of Gastrointestinal Oncology2023,15,3: | 0 |
