| 4 | Pancreatic adenocarcinoma: A review of recent paradigms and advances in epidemiology, clinical diagnosis and management显示文摘Pancreatic cancer is one of the dreaded malignancies for both the patient and the clinician.The five-year survival rate of pancreatic adenocarcinoma(PDA)is as low as 2%despite multimodality treatment even in the best hands.As per the Global Cancer Observatory of the International Agency for Research in Cancer estimates of pancreatic cancer,by 2040,a 61.7%increase is expected in the total number of cases globally.With the widespread availability of next-generation sequencing,the entire genome of the tumors is being sequenced regularly,providing insight into their pathogenesis.As invasive PDA arises from pancreatic intraepithelial neoplasia and mucinous neoplasm and intraductal papillary neoplasm,screening for them can be beneficial as the disease is curable with resection at an early stage.Routine preoperative biliary drainage has no role in patients suffering from PDA with obstructive jaundice.If performed,metallic stents are preferred over plastic ones.Minimally invasive procedures are preferred to open procedures as they have less morbidity.The duct-to-mucosa technique for pancreaticojejunostomy is presently widely practiced.The role of intraperitoneal drains after surgery for PDA is controversial.Neoadjuvant chemoradiotherapy has been proven to have a significant role both in locally advanced as well as in resectable PDA.Many new regimens and drugs have been added in the arsenal of chemoradiotherapy for metastatic disease.The roles of immunotherapy and gene therapy in PDA are being investigated.This review article is intended to improve the understanding of the readers with respect to the latest updates of PDA,which may help to trigger new research ideas and make better management decisions. | Nikhil Gupta Raghav Yelamanchi | 2021 | World Journal of Gastroenterology2021,27,23: | 1 |

| 6 | Rationale for integration of palliative care in the medical intensive care:A narrative literature review显示文摘Despite the remarkable technological advancement in the arena of critical care expertise,the mortality of critically ill patients remains high.When the organ functions de-teriorate,goals of care are not fulfilled and life-sustaining treatment becomes a burden on the patient and caregivers,then it is the responsibility of the physician to provide a dignified end to life,control the symptoms of the patient and provide psychological support to the family members.Palliative care is the best way forward for these patients.It is a multidimensional specialty which emphasizes patient and family-based care and aims to improve the quality of life of patients and their caregivers.Although intensive care and palliative care may seem to be at two opposite ends of the spectrum,it is necessary to amalgamate the postulates of palliative care in intensive care units to provide holistic care and best benefit patients admitted to intensive care units.This review aims to highlight the need for an alliance of palliative care with intensive care in the present era,the barriers to it,and models proposed for their integration and various ethical issues. | Nishkarsh Gupta Raghav Gupta Anju Gupta | 2022 | World Journal of Critical Care Medicine2022,11,6: | 0 |
| 7 | Use of intravascular lithotripsy in non-coronary artery lesions显示文摘BACKGROUND Intravascular lithotripsy(IVL)is a novel technique increasingly used for plaque modification and endovascular revascularization in patients with severe calcification and peripheral artery disease.However,much of the available literature on IVL is focused on its use in coronary arteries,with relatively limited data on non-coronary artery use.AIM To analyze the safety and efficacy of current IVL use in non-coronary artery lesions,as reported in case reports and case series.METHODS We searched EMBASE,PubMed,and Reference Citation Analysis databases for case reports and case series on IVL use in peripheral artery disease.We then extracted variables of interest and calculated the mean and proportions of these variables.RESULTS We included 60 patients from 33 case reports/case series.Ninety-eight percent of the cases had IVL usage in only one blood vessel,while four had the IVL used in two vessels(2.0%),resulting in 64 Lesions treated with IVL.The mean age of the patients was 73.7(SD 10.9).IVL was successfully used in severe iliofemoral artery stenosis(51.6%),severe innominate,subclavian,and carotid artery stenosis(26.7%combined),and severe mesenteric vessel stenosis(9.4%).Additionally,IVL was successfully used in severe renal(7.8%)and aortic artery(4.7%)stenosis.There were complications in 12%of the cases,with dissection being the commonest.CONCLUSION IVL has successfully used in plaque modification and endovascular revascularization in severely calcified and challenging lesions in the iliofemoral,carotid,subclavian,aorta,renal,and mesenteric vessels.The most severe but transient complications were with IVL use in the aortic arch and neck arteries. | Chukwuemeka Anthony Umeh Ashley Stratton Tifani Wagner Shipra Saigal Krystal Sood Raghav Dhawan Cory Wagner Jessica Obi Sabina Kumar Tsung Han Scottie Ching Rahul Gupta | 2023 | World Journal of Cardiology2023,15,8: | 0 |
| 8 | Role of optical coherence tomography in Barrett’s esophagus显示文摘Traditional endoscopic techniques for Barrett’s esophagus(BE)surveillance relied on factor of probability as endoscopists performed cumbersome random biopsies of low yield.Optical coherence tomography(OCT)is a novel technique based on tissue light interference and is set to break conventional barriers.OCT was initially introduced in ophthalmology but was soon adopted by other areas of medicine.When applied to endoscopy,OCT can render images of the superficial layers of the gastrointestinal tract and is highly sensitive in detecting dysplasia in BE.Volumetric laser endomicroscopy is a second generation OCT endoscope device which is able to identify buried glands after ablation.Addition of artificial intelligence to OCT has rendered it more productive.The newer additions to OCT such as angiogram and laser marking will increase the accuracy of investigation.In spite of the few inevitable drawbacks associated with the technology,it presently outperforms all newer endoscopic techniques for the surveillance of BE. | Nikhil Gupta Raghav Yelamanchi Himanshu Agrawal Nitin Agarwal | 2021 | Artificial Intelligence in Gastrointestinal Endoscopy2021,2,4: | 0 |
