1Pancreatic necrosis:Complications and changing trend of treatment显示文摘Incidence of acute pancreatitis seems to be increasing in the Western countries and has been associated with significantly increased morbidity. Nearly 80% of the patients with acute pancreatitis undergo resolution; some develop complications including pancreatic necrosis. Infection of pancreatic necrosis is the leading cause of death in these patients. A significant portion of these patients needs surgical interventions. Traditionally, the 'gold standard' procedure has been the open surgical necrosectomy, which is now being completed by the relatively lesser invasive interventions. Minimally invasive surgical(MIS) procedures include endoscopic drainage, percutaneous image-guided catheter drainage, and retroperitoneal drainage. 2Elizabethkingia miricola :A rare non-fermenter causing urinary tract infection显示文摘Elizabethkingia miricola(E.miricola) is a gram-negative non-fermentative bacterium which is rarely encountered.It is usually misidentified or considered as a contaminant in routine microbiology laboratories due to the limitations in conventional biochemical techniques.However,with the advent of the matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOFMS),the identification of non-fermenters has become easy and this has led to enhanced understanding of the clinical significance of these uncommonly isolated microorganisms.The genus Elizabethkingia has only two species E.meningoseptica and E.miricola.Both of these organisms are known to be multi-drug resistant and therefore,their accurate identification and antimicrobial susceptibility testing are necessary prior to the initiation of appropriate therapy.In the world literature till date,only 3 cases of sepsis caused by E.miricola have been reported.We present the first case of E.miricola association with urinary tract infection.Parakriti Gupta Kamran Zaman Balvinder Mohan Neelam Taneja 2017World Journal of Clinical Cases2017,5,5:6
3Aerobic bacterial flora of the normal conjunctiva at high altitude area of Shimla Hills in India:a hospital based study显示文摘AIM:To study the normal aerobic conjunctival flora in lower to mid Himalayan region of Shimla Hills.METHODS:Samples from normal conjunctiva of 200 individuals above the age of 13 years who visited Indira Gandhi Medical College Hospital Shimla for refraction or cataract surgery were taken by anaesthetising the conjunctival sac with sterile 4% Xylocaine solution.Samples were obtained by gently rubbing lower fornix with a sterile cotton wool swab moistened with normal saline and keeping the eye lids wide apart to avoid contamination from lid margins.It was immediately inoculated in Brain Heart infusion and subjected to standard aerobic culture and identification techniques.RESULTS:Totally,72 eyes(36%) conjunctival sacs were sterile.Predominant aerobes isolated were Staphylococcus spp.in 120(60%) followed by Haemophilus in 16(8%),diphtheroids in 10(5%),and Escherichia coli(E.coli) in 4(2%).A single aerobe was isolated from 98 eyes(49%) while 30 eyes(15%) yielded more than 1 aerobe.Staphylococcus epidermidis(S.epidermidis) was the most common bacterium,found alone in 58 eyes(29%) and in combination with another aerobe in 30 eyes(15%),followed by Staphylococcus aureus(S.aureus) in 18(9%) and 10 eyes(5%) respectively.CONCLUSION:S.epidermidis is the most common commensal organism followed by Haemophilus species.diphtheroids occupied the third position which otherwise are found more abundantly in literature.Pathogens like S.aureus,Staphylococcus citreus and E.coli were also found.Therefore,preoperative administration of topical broad spectrum antibiotics is extremely important in prophylaxis against ocular infection.The variation in microflora of normal conjunctiva in this part of world can be attributed to geographical,climatic and ethnic characteristics of the population under study.Sharma Parmeshri Dass Sharma Neelam Gupta Ravinder Kumar Singh Parul 2013International Journal of Ophthalmology(English edition)2013,6,5:4

8Value of neutrophil-lymphocyte ratio in evaluating response to percutaneous catheter drainage in patients with acute pancreatitis显示文摘BACKGROUND Early prediction of response to percutaneous catheter drainage(PCD)of necrotic collections in acute pancreatitis(AP)using simple and objective tests is critical as it may determine patient prognosis.The role of white blood cell(WBC)count and neutrophil-lymphocyte ratio(NLR)has not been assessed as a tool of early prediction of PCD success and is the focus of this study.AIM To assess the value of WBC and NLR in predicting response to PCD in AP.METHODS This retrospective study comprised consecutive patients with AP who underwent PCD between June 2018 and December 2019.Severity and fluid collections were classified according to the revised Atlanta classification and organ failure was defined according to the modified Marshall Score.WBC and NLR were monitored 24 h prior PCD(WBC-0/NLR-0)and 24 h(WBC-1/NLR-1),48 h(WBC-2/NLR-2)and 72 h(WBC-3/NLR-3)after PCD.NLR was calculated by dividing the number of neutrophils by the number of lymphocytes.The association of success of PCD(defined as survival without the need for surgery)with WBC and NLR was assessed.The trend of WBC and NLR was also assessed post PCD.RESULTS One hundred fifty-five patients[median age 40±13.6(SD),64.5%males,53.5%severe AP]were included in the final analysis.PCD was done for acute necrotic collection in 99(63.8%)patients and walled-off necrosis in 56(36.1%)patients.Median pain to PCD interval was 24±69.89 d.PCD was successful in 109 patients(group 1)and 46 patients(group 2)who failed to respond.There was no significant difference in the baseline characteristics between the two groups except the severity of AP and frequency of organ failure.Both WBC and NLR showed an overall decreasing trend.There was a significant difference between WBC-0 and WBC-1(P=0.0001).WBC-1 and NLR-1 were significantly different between the two groups(P=0.048 and 0.003,respectively).The area under the curve of WBC-1 and NLR-1 for predicting the success of PCD was 0.602 and 0.682,respectively.At a cut-off value of 9.87 for NLR-1,the sensitivity and specificity for predicting the success of PCD were calculated to be 75%and 65.4%respectively.CONCLUSION WBC and NLR can be used as simple tests for predicting response to PCD in patients with acute necrotizing pancreatitis.Pankaj Gupta Gaurav Chayan Das Akash Bansal Jayanta Samanta Harshal S Mandavdhare Vishal Sharma Shano Naseem Vikas Gupta Thakur Deen Yadav Usha Dutta Neelam Varma Manavjit Singh Sandhu Rakesh Kochhar 2022World Journal of Clinical Cases2022,10,1:1

17Salmonella-Salmonellosis-Rare presentations of a common pathogen显示文摘Salmonella are most commonly associated with gastroenteritis and enteric fever in humans. Occasionally,dissemination of bacilli throughout the body results in establishment of localized foci of persistent infection especially in patients with debilitating diseases and immunosuppressive states.Infection at various aberrant sites due to Salmonella has been reported relatively seldom.It has perfected the art of intracellular survival in niches from where they can cause myriad of effects.Six cases with Salmonella infection at unusual sites without any preexisting or underlying disease diagnosed over a period of two years are presented here.Salmonella etiology was not suspected in these patients and the diagnosis was made microbiologically only after culture isolation.Neelam Kaistha Varsha Gupta Shailpreet Sidhu Jagdish Chander 2011Asian Pacific Journal of Tropical Medicine2011,4,5:0
