1Helicobacter pylori:Does it add to risk of coronary artery disease显示文摘Helicobacter pylori(H. pylori) is a known pathogen implicated in genesis of gastritis, peptic ulcer disease, gastric carcinoma and gastric lymphoma. Beyond the stomach, the organism has also been implicated in the causation of immune thrombocytopenia and iron deficiency anemia. Although an area of active clinical research, the role of this gram negative organism in causation of atherosclerosis and coronary artery disease(CAD) remains enigmatic. CAD is a multifactorial disease which results from the atherosclerosis involving coronaryarteries. The major risk factors include age, diabetes mellitus, smoking, hypertension and dyslipidemia. The risk of coronary artery disease is believed to increase with chronic inflammation. Various organisms like Chlamydia and Helicobacter have been suspected to have a role in genesis of atherosclerosis via causation of chronic inflammation. This paper focuses on available evidence to ascertain if the role of H. pylori in CAD causation has been proven beyond doubt and if eradication may reduce the risk of CAD or improve outcomes in these patients.Vishal Sharma Amitesh Aggarwal 2015World Journal of Cardiology2015,7,1:7
2Dengue and its effects on liver显示文摘Dengue has emerged as an important arboviral disease with significant impact on the disease burden in population residing in tropical countries. Dengue is spread by the bite of Aedes mosquito. The virus seems to have some hepatotoxic effects. Affliction of liver in form of derangements in the liver function tests is common and may include mild elevations in serum bilirubin, elevated transaminases and derangements in serum albumin. Although asymptomatic in most cases, clinical manifestations like jaundice, and acute liver failure(ALF) may occasionally complicate the clinical picture. Indeed, dengue has been implicated as an important cause of ALF in endemic countries. 3Imaging-based algorithmic approach to gallbladder wall thickening显示文摘Gallbladder(GB) wall thickening is a frequent finding caused by a spectrum of conditions. It is observed in many extracholecystic as well as intrinsic GB conditions. GB wall thickening can either be diffuse or focal. Diffuse wall thickening is a secondary occurrence in both extrinsic and intrinsic pathologies of GB, whereas, focal wall thickening is mostly associated with intrinsic GB pathologies. In the absence of specific clinical features, accurate etiological diagnosis can be challenging. The survival rate in GB carcinoma(GBC) can be improved if it is diagnosed at an early stage, especially when the tumor is confined to the wall. The pattern of wall thickening in GBC is often confused with benign diseases, especially chronic cholecystitis, xanthogranulomatous cholecystitis, and adenomyomatosis. Early recognition and differentiation of these conditions can improve the prognosis. In this minireview, the authors describe the patterns of abnormalities on various imaging modalities(conventional as well as advanced) for the diagnosis of GB wall thickening. 6Endoscopic ultrasound guided interventional procedures显示文摘Endoscopic ultrasound(EUS) has emerged as an important diagnostic and therapeutic modality in the field of gastrointestinal endoscopy. EUS provides access to many organs and lesions which are in proximity to the gastrointestinal tract and thus giving an opportunity to target them for therapeutic and diagnostic purposes. This modality also provides a real time opportunityto target the required area while avoiding adjacent vascular and other structures. Therapeutic EUS has found role in management of pancreatic fluid collections, biliary and pancreatic duct drainage in cases of failed endoscopic retrograde cholangiopancreatography, drainage of gallbladder, celiac plexus neurolysis/blockage, drainage of mediastinal and intra-abdominal abscesses and collections and in targeted cancer chemotherapy and radiotherapy. Infact, therapeutic EUS has emerged as the therapy of choice for management of pancreatic pseudocysts and recent innovations like fully covered removable metallic stents have improved results in patients with organised necrosis. Similarly, EUS guided drainage of biliary tract and pancreatic duct helps drainage of these systems in patients with failed cannulation, inaccessible papilla as with duodenal/gastric obstruction or surgically altered anatomy. EUS guided gall bladder drainage is a useful emergent procedure in patients with acute cholecystitis who are not fit for surgery. EUS guided celiac plexus neurolysis and blockage is more effective and less morbid vis-à-vis the percutaneous technique. The field of interventional EUS is rapidly advancing and many more interventions are being continuously added. 9Arterial pH,bicarbonate levels and base deficit at presentation as markers of predicting mortality in acute pancreatitis:a single-centre prospective study显示文摘Background.Arterial blood gas(ABG)parameters such as pH form part of multi-parameter scoring systems for predicting severe acute pancreatitis;however,literature on detailed evaluation of ABG alone in this context is scarce.Methods.Patients with acute pancreatitis presenting to our unit between January 2012 and November 2013 were prospectively studied.ABG analysis was done at admission and development of organ failure,any need for intervention,and mortality were noted.The association between various parameters of ABG analysis and the development of organ failure or local complications,need for interventions(endoscopic/radiological/surgical)and mortality were analysed.Results.Two hundred and five patients(mean age:39.33-13.85 years;61.0%males)were prospectively studied.The aetiology of acute pancreatitis was alcohol in 93 patients(45.4%)and gall stone disease in 73(35.6%).Organ failure developed in 71.2%patients and 83.9%had local complications.In 18%of patients,endoscopic/radiological/surgical interventions were needed and 14.6%died.The patients(n=35)with metabolic acidosis(pH<7.35)suffered higher frequency of organ failure,need for interventions and mortality.Patients with low arterial bicarbonate levels,as well as higher base deficit,also displayed higher frequency of organ failure,need for interventions and mortality.The receiver operating characteristic(ROC)curves for pH<7.35,bicarbonate<22 meq/L and base deficit of>4 meq/L for prediction of mortality were 0.771(95%CI:0.664–0.878),0.707(95%CI:0.622–0.791)and 0.780(95%CI:0.693–0.867),respectively.Conclusion.Arterial pH,bicarbonate levels,and base deficit at presentation are useful early markers for predicting adverse outcome in acute pancreatitis.Vishal Sharma Thingbaijam Shanti Devi Ravi Sharma Puneet Chhabra Rajesh Gupta Surinder S.Rana Deepak K.Bhasin 2014Gastroenterology Report2014,2,3:1

11Small bowel bleeding:a comprehensive review显示文摘The small intestine is an uncommon site of gastro-intestinal(GI)bleeding;however it is the commonest cause of obscure GI bleeding.It may require multiple blood transfusions,diagnostic procedures and repeated hospitalizations.Angiodysplasia is the commonest cause of obscure GI bleeding,particularly in the elderly.Inflammatory lesions and tumours are the usual causes of small intestinal bleeding in younger patients.Capsule endoscopy and deep enteroscopy have improved our ability to investigate small bowel bleeds.Deep enteroscopy has also an added advantage of therapeutic potential.Computed tomography is helpful in identifying extra-intestinal lesions.In cases of difficult diagnosis,surgery and intra-operative enteroscopy can help with diagnosis and management.The treatment is dependent upon the aetiology of the bleed.An overt bleed requires aggressive resuscitation and immediate localisation of the lesion for institution of appropriate therapy.Small bowel bleeding can be managed by conservative,radiological,pharmacological,endoscopic and surgical methods,depending upon indications,expertise and availability.Some patients,especially those with multiple vascular lesions,can re-bleed even after appropriate treatment and pose difficult challenge to the treating physician.Deepak Gunjan Vishal Sharma Surinder S Rana Deepak K Bhasin 2014Gastroenterology Report2014,2,4:1

17Value of neutrophil-lymphocyte ratio in evaluating response to percutaneous catheter drainage in patients with acute pancreatitis显示文摘BACKGROUND Early prediction of response to percutaneous catheter drainage(PCD)of necrotic collections in acute pancreatitis(AP)using simple and objective tests is critical as it may determine patient prognosis.The role of white blood cell(WBC)count and neutrophil-lymphocyte ratio(NLR)has not been assessed as a tool of early prediction of PCD success and is the focus of this study.AIM To assess the value of WBC and NLR in predicting response to PCD in AP.METHODS This retrospective study comprised consecutive patients with AP who underwent PCD between June 2018 and December 2019.Severity and fluid collections were classified according to the revised Atlanta classification and organ failure was defined according to the modified Marshall Score.WBC and NLR were monitored 24 h prior PCD(WBC-0/NLR-0)and 24 h(WBC-1/NLR-1),48 h(WBC-2/NLR-2)and 72 h(WBC-3/NLR-3)after PCD.NLR was calculated by dividing the number of neutrophils by the number of lymphocytes.The association of success of PCD(defined as survival without the need for surgery)with WBC and NLR was assessed.The trend of WBC and NLR was also assessed post PCD.RESULTS One hundred fifty-five patients[median age 40±13.6(SD),64.5%males,53.5%severe AP]were included in the final analysis.PCD was done for acute necrotic collection in 99(63.8%)patients and walled-off necrosis in 56(36.1%)patients.Median pain to PCD interval was 24±69.89 d.PCD was successful in 109 patients(group 1)and 46 patients(group 2)who failed to respond.There was no significant difference in the baseline characteristics between the two groups except the severity of AP and frequency of organ failure.Both WBC and NLR showed an overall decreasing trend.There was a significant difference between WBC-0 and WBC-1(P=0.0001).WBC-1 and NLR-1 were significantly different between the two groups(P=0.048 and 0.003,respectively).The area under the curve of WBC-1 and NLR-1 for predicting the success of PCD was 0.602 and 0.682,respectively.At a cut-off value of 9.87 for NLR-1,the sensitivity and specificity for predicting the success of PCD were calculated to be 75%and 65.4%respectively.CONCLUSION WBC and NLR can be used as simple tests for predicting response to PCD in patients with acute necrotizing pancreatitis.Pankaj Gupta Gaurav Chayan Das Akash Bansal Jayanta Samanta Harshal S Mandavdhare Vishal Sharma Shano Naseem Vikas Gupta Thakur Deen Yadav Usha Dutta Neelam Varma Manavjit Singh Sandhu Rakesh Kochhar 2022World Journal of Clinical Cases2022,10,1:1
18Optimization of photovoltaic power system:a comparative study显示文摘This paper presents a comparative study of P&O,fuzzy P&O and BPSO fuzzy P&O control methods by using MATLAB software for optimizing the power output of the solar PV grid array.The voltage,power output and the duty cycle of the solar PV array are well presented and analyzed with an algorithm.The model consists of 66 PV Cells connected parallel and 5 PV cells connected in series to make solar PV array.The BPSO Fuzzy method generates 43.4820 MW output power more than P&O method and 150 KW more than P&O fuzzy method.This also shows that the time response of the photovoltaic system reduces to perturbations and insures the continuity of the operation at the time in response to the continued maximum power point.It also eliminates the fluctuations around MPPT.Simulation results also revealed that BPSO fuzzy P&O controller is more effective as compare with P&O and fuzzy P&O models.Vishal Dabra Krishan Kumar Paliwal Pardeep Sharma Narender Kumar 2017Protection and Control of Modern Power Systems2017,2,1:1
