| 1 | Clinicopathologic characteristics and prognosis of gastroenteropancreatic neuroendocrine neoplasms: a multicenter study in South China显示文摘Background: Gastroenteropancreatic neuroendocrine neoplasms(GEP-NENs) are a heterogeneous group of rare tumors. Many issues in terms of epidemiologic features, pathogenesis, and treatment of GEP-NENs are still under discussion. Our study aimed to analyze the clinicopathologic characteristics and prognosis of Chinese patients with GEP-NENs.Methods: Complete clinicopathologic data and survival information of 1183 patients with GEP-NENs treated between 2005 and 2015 were collected from five medical centers in Guangdong Province, China. Patient survival was estimated using the Kaplan–Meier method and analyzed using the log-rank test; prognostic factors were analyzed using the Cox proportional hazards model.Results: The most common tumor location was the rectum(37.4%), followed by the pancreas(28.1%), stomach(20.7%), small intestine(7.2%), appendix(3.4%), and colon(3.3%). After initial definitive diagnosis, 1016(85.9%) patients underwent surgery. The 1-, 3-, and 5-year overall survival(OS) rates for the entire cohort were 87.9%, 78.5%, and 72.8%, respectively. The 3-year OS rates of patients with G1, G2, and G3 tumors were 93.1%, 82.7%, and 43.1%, respectively(P < 0.001). The 3-year OS rates of patients with stage I, II, III, and IV tumors were 96.0%, 87.3%, 64.0%, and 46.8%, respectively(P < 0.001). Patients with distant metastasis who underwent palliative surgery had a longer survival than those who did not(P = 0.003). Similar survival benefits of palliative surgery were observed in patients with neuroendocrine tumor(P y, M category, and sur= 0.031) or neuroendocrine carcinoma(P gery were found to be independent prog= 0.046). In multivariate analysis, age, grade, N categornostic factors.Conclusions: Patients with GEP-NENs who are women, younger than 50 years old, have smaller tumor size, have lower tumor grade, have lower T/N/M category, and who undergo surgery can have potentially longer survival time. Our data showed that surgery can improve the prognosis of GEP-NEN patients with distant metastasis. | 3 | Filtering of long-term dependent fractal noise in fiber optic gyroscope显示文摘Stochastic noises of fiber optic gyroscope (FOG) mainly contain white noise and fractal noise whose long-term dependent component causes FOG a rather slow drift. In order to eliminate this component, a two-step filtering methodology is proposed. Firstly, fractional differencing (FD) method is introduced to trans-form fractal noise into fractional white noise based on the estima-tion of Hurst exponent for long-term dependent fractal process, which together with the existing white noise make up of a gener-alized white noise. Further, an improved denoising algorithm of wavelet maxima is developed to suppress the generalized white noise. Experimental results show that the basic noise terms of FOG greatly decrease, and especially the slow drift is restrained effectively. | 14 | Trajectory of fecal lactoferrin for predicting prognosis in ulcerative colitis显示文摘Objectives:To investigate the characteristics and prognostic value of fecal lactoferrin trajectories in ulcerative colitis(UC).Methods:This study used data from the UNIFI trial(ClinicalTrials.gov,NCT02407236)and included patients who received ustekinumab during induction for trajectory modeling(n=637).Patients who received ustekinumab during maintenance therapy were used for 1-year outcome analyses(n=403).The levels of fecal lactoferrin,fecal calprotectin,and serum C-reactive protein were measured at weeks O,2,4,and 8.The trajectories of these biomarkers were developed using a latent class growth mixed model.Results:The trajectories of fecal lactoferrin,fecal calprotectin,and serum C-reactive protein were distinct,but all were associated with prior exposure to anti-tumor necrosis factor agents and vedolizumab.Furthermore,the fecal lactoferrin trajectory was the most valuable predictor of endoscopic,clinical,and histological remission.Compared to the high/moderate-rapid decrease trajectory group,the moderate-slow decrease,high-slow decrease,and high-stable groups had adjusted odds ratios(95%confidence interval)of o.38(0.18,0.78;P=0.010),0.47(0.23,0.93;P=0.032),and 0.33(0.17,0.63;P=0.001),respectively,of 1-year endoscopic remission.Patients with high/moderate-rapid decrease trajectories also had the highest likelihood of achieving clinical and histological remission.Finally,we developed a patient-stratification scheme based on fecal lactoferrin trajectories and concentrations.Patients with good,moderate,and poor prognoses in the scheme had a distinct probability of achieving 1-year endoscopic remission(52.7%,30.9%,and 12.8%,respectively).Conclusions:The trajectory of fecal lactoferrin is a valuable prognostic factor for 1-year remission in UC. | Rirong Chen Li Li Yizhe Tie Minhu Chen Shenghong Zhang | 2023 | Precision Clinical Medicine2023,6,3: | 0 |
| 15 | Serum exosomal microRNA-144-3p:a promising biomarker for monitoring Crohn’s disease显示文摘Background:Crohn’s disease(CD)has a tendency for recurrence and requires adequate monitoring and personalized treatment.Since endoscopy is considerably invasive,serum biomarkers are required as alternatives for CD monitoring.Toward this,exosomal microRNAs(miRNAs)may serve as promising candidates.In this study,we aimed to assess the role of serum exosomal microRNA-144-3p(miR-144-3p)as a biomarker for CD monitoring.Methods:We prospectively recruited 154 patients without a history of surgery(Cohort 1)and 75 patients who were to undergo intestinal resection(Cohort 2).Serum samples were collected from Cohort 1 before colonoscopy and from Cohort 2 before surgery and during post-operative colonoscopic examination.The serum levels of exosomal miR-144-3p were measured using quantitative reverse-transcription polymerase chain reaction(PCR).Correlations between relative exosomal miR-144-3p levels,disease activity,and disease behavior were analysed.The area under the receiver-operating characteristic curve(AUC)was used to assess the predictive value of exosomal miR-144-3p regarding mucosal activity and postoperative recurrence.Results:A 3.33-fold increase in serum exosomal miR-144-3p levels was recorded in patients with CD compared with those in healthy controls(P<0.001).The exosomalmiR-144-3p levels were positively correlated with the simple endoscopic score of CD(q=0.547,P<0.001)as well as the Rutgeerts score(q=0.478,P<0.001).Elevated exosomalmiR-144-3p levels were correlated with the penetrating disease with high specificity(100%[95%confidence interval,95.1%–100%]).The accuracy of exosomalmiR-144-3p for identifying post-operative recurrence was higher than that of C-reactive protein(CRP)(AUC,0.775 vs 0.639;P<0.001).Conclusions:Serum exosomal miR-144-3p is a reliable biomarker of mucosal inflammation and penetrating CD.It may identify endoscopic CD recurrence after intestinal resection with higher accuracy than CRP testing. | Peng Chen Shanshan Huang Qiao Yu Kang Chao Ying Wang Gaoshi Zhou Xiaojun Zhuang Zhirong Zeng Minhu Chen Shenghong Zhang | 2022 | Gastroenterology Report2022,10,1: | 0 |
| 16 | Development and validation of a nomogram to predict indolent course in patients with ulcerative colitis:a single-center retrospective study显示文摘Background:The natural disease course for patients with ulcerative colitis(UC)is heterogeneous and few data are available on the indolent course of UC and its related factors.We aimed to develop and validate a nomogramto predict indolent course in patients with UC.Methods:Data of patients diagnosed with UC in the First Affiliated Hospital of Sun Yat-sen University(Guangzhou,China)between April 2007 and February 2021 were retrospectively analysed.Indolent course was defined as a disease course without need for strict interventions(steroids,immunomodulators,biological agents,hospitalization,or surgery therapy)during the follow-up period.The whole cohort was randomly divided into training set and validation set.The nomogram was constructed in the training set based on the results of univariate and multivariate Cox regression analyses.The performance of the nomogram was assessed by the concordance index(C-index),area under the receiver-operating characteristic curve(AUC),and calibration plots.In addition,we internally validated the nomogramvia the bootstrap method and the validation set.Results:Of 969 treatment-naive patients with UC,771(79.6%)had an indolent course after diagnosis.Of these,313 patients were included in the development and validation of the nomogram.The nomogram incorporating age,disease activity,C-reactive protein,and platelet count showed good calibration and discrimination.The C-index was 0.759(0.741 in bootstrap validation)and the AUC at 2,4,and 6 years was 0.767,0.782,and 0.775,respectively.The nomogramperformed well when applied to the validation set.Conclusion:A majority of patients with UC had an indolent course after diagnosis.The nomogram developed in this study might be useful in therapeutic decision-making and follow-up management for patients with UC. | Na Li Shukai Zhan Caiguang Liu Tong Li Tong Tu Baili Chen Yao He Minhu Chen Zhirong Zeng Xiaojun Zhuang | 2022 | Gastroenterology Report2022,10,1: | 0 |
