2Enhancing the dissolution of phenylbutazone using Syloid based mesoporous silicas for oral equine applications显示文摘Three mesoporous silica excipients(Syloid? silicass AL-1 FP, XDP 3050 and XDP 3150) were formulated with a model drug known for its poor aqueous solubility, namely phenylbutazone, in an attempt to enhance the extent and rate of drug dissolution. Although other forms of mesoporous silica have been investigated in previous studies, the effect of inclusion with these specific Syloid? silicas based excipients and more interestingly, with phenylbutazone, is unknown. This work reports a significant enhancement for both the extent and rate of drug release for all three forms of Syloid? silicas at a 1:1 drug:silica ratio over a period of 30 min. An explanation for this increase was determined to be conversion to the amorphous form and an enhanced drug loading ability within the pores. Differences between the release profiles of the three silicas were concluded to be a consequence of the physicochemical differences between the three forms. Overall, this study confirms that Syloid? silicas based excipients can be used to enhance dissolution, and potentially therefore bioavailability, for compounds with poor aqueous solubility such as phenylbutazone. In addition, it has been confirmed that drug release can be carefully tailored based on the choice of Syloid? silicas and desired release profile.Laura J. Waters John P. Hanrahan Joseph M. 3Thermodynamics of clayedrug complex dispersions: Isothermal titration calorimetry and high-performance liquid chromatography显示文摘An understanding of the thermodynamics of the complexation process utilized in sustaining drug release in clay matrices is of great importance.Several characterisation techniques as well as isothermal calorimetry were utilized in investigating the adsorption process of a model cationic drug(diltiazem hydrochloride,DIL)onto a pharmaceutical clay system(magnesium aluminium silicate,MAS).X-ray powder diffraction(XRPD),attenuated total reflectance Fourier transform infrared spectroscopy(ATRFTIR)and optical microscopy confirmed the successful formation of the DIL-MAS complexes.Drug quantification from the complexes demonstrated variable behaviour in the differing media used with DIL degrading to desacetyl diltiazem hydrochloride(DC-DIL)in the 2 M HCl media.Here also,the authors report for the first time two binding processes that occurred for DIL and MAS.A competitor binding model was thus proposed and the thermodynamics obtained suggested their binding processes to be enthalpy driven and entropically unfavourable.This information is of great importance for a formulator as care and consideration should be given with appropriate media selection as well as the nature of binding in complexes.Ana-Maria Totea Juan Sabin Irina Dorin Karl Hemming Peter R.Laity Barbara R.Conway Laura Waters Kofi Asare-Addo 2020Journal of Pharmaceutical Analysis2020,10,1:0
4Surgery in (pre)malignant celiac disease显示文摘AIM: To report the outcome of surgery in patients with(pre)malignant conditions of celiac disease(CD) and the impact on survival.METHODS: A total of 40 patients with(pre)malignant conditions of CD,ulcerative jejunitis(n = 5) and enteropathy associated T-cell lymphoma(EATL)(n = 35),who underwent surgery between 2002 and 2013 were retrospectively evaluated. Data on indications,operative procedure,post-operative morbidity and mortali ty,adjuvant therapy and overal l survival(OS) were collected. Eleven patients with EATL who underwent chemotherapy without resection were included as a control group for survival analysis. Patients were followed-up every three months during the first year and at 6-mo intervals thereafter.RESULTS: Mean age at resection was 62 years. The majority of patients(63%) underwent elective laparotomy. Functional stenosis(n = 1 3) and perforation(n = 12) were the major indications for surgery. In 70% of patients radical resection wasperformed. Early postoperative complications,mainly due to leakage or sepsis,occurred in 14/40(35%) of patients. Eight patients required reoperation. More patients who underwent resection in the acute setting(n = 3,20%) died compared to patients treated in the elective setting. With a median follow-up of 20 mo,seven patients(18%) required reoperation due to long-term complications. Significantly more patients who underwent acute surgery could not be treated with adjuvant chemotherapy.
