1Neglected tropical diseases in the People’s Republic of China: progress towards elimination显示文摘Since the founding of the People's Republic of China in 1949,considerable progress has been made in the control and elimination of the country's initial set of 11 neglected tropical diseases.Indeed,elimination as a public health problem has been declared for lymphatic filariasis in 2007 and for trachoma in 2015.The remaining numbers of people affected by soil-transmitted helminth infection,clonorchiasis,taeniasis,and echinococcosis in 2015 were 29.1 million,6.0 million,366200,and 166100,respectively.In 2017,after more than 60 years of uninterrupted,multifaceted schistosomiasis control,has seen the number of cases dwindling from more than 10 million to 37600.Meanwhile,about 6000 dengue cases are reported,while the incidence of leishmaniasis,leprosy,and rabies are down at 600 or fewer per year.Sustained social and economic development,going hand-in-hand with improvement of water,sanitation,and hygiene provide the foundation for continued progress,while rigorous surveillance and specific public health responses will consolidate achievements and shape the elimination agenda.Targets for poverty elimination and strategic plans and intervention packages post-2020 are important opportunities for further control and elimination,when remaining challenges call for sustainable efforts.Men-Bao Qian Jin Chen Robert Bergquist Zhong-Jie Li Shi-Zhu Li Ning Xiao Jurg Utzinger Xiao-Nong Zhou 2019Infectious Diseases of Poverty2019,8,5:23

3蒿甲醚对曼氏血吸虫的作用 :剂量与效应的关系和虫的形态学和组织病理学的变化(英文)显示文摘目的 应用感染曼氏血吸虫 (利比里亚株 )的小鼠观察蒿甲醚单剂量与效应的关系 ,虫体肝移及蒿甲醚所引起的虫的形态学和组织病理学变化。 方法 感染 2 1d童虫的小鼠一次口服蒿甲醚 12 .5mg/kg至 60 0mg/kg不同剂量 ,治后 2 8d剖检观察各组虫数。感染 46d或 70d成虫的小鼠一次口服蒿甲醚 40 0mg/kg后 8~ 14d ,观察虫体肝移及其形态和组织病理学变化。 结果 蒿甲醚对 2 1d童虫的最低有效剂量为 2 0 0mg/kg ,减虫率为 81%。用蒿甲醚治疗后 8h成虫开始肝移 ,3~ 7d全部肝移 ,14d有 3 1%的虫返回肠系膜静脉。成虫虫体萎缩 ,咽部扩大 ,肠管膨胀及其色素减少。雌虫局部体表受损 ,白细胞附着 ,卵巢及卵黄腺变性退化 ,以及雄虫睾丸萎缩等。在肝内的虫体被嗜酸粒细胞为主的炎细胞包围和浸润。 结论 蒿甲醚对小鼠曼氏血吸虫 2 1d童虫的最低有效剂量为 2 0 0mg/kg ,可引起曼氏血吸虫成虫萎缩、退化或死亡。在肝内受损的虫体主要是被嗜酸粒细胞包围和侵袭所致。肖树华 郭俭 Jacques Chollet 吴嘉彤 Marcel Tanner Jurg Utzinger 2004中国寄生虫学与寄生虫病杂志2004,22,3:2
4The impact of pastoralist mobility on tuberculosis control in Ethiopia:a systematic review and meta-synthesis显示文摘Background:Directly observed treatment,short-course(DOTS)is the current mainstay to control tuberculosis(TB)worldwide.Context-specific adaptations of DOTS have impending implications in the fight against TB.In Ethiopia,there is a national TB control programme with the goal to eliminate TB,but uneven distribution across lifestyle gradients remains a challenge.Notably,the mobile pastoralist communities in the country are disproportionately left uncovered.The aim of this study was to summarize the evidence base from published literature to guide TB control strategy for mobile pastoralist communities in Ethiopia.Main text:We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines and systematically reviewed articles in seven electronic databases:Excerptra Medical Database,African Journal Online,PubMed,Google Scholar,Centre for Agriculture and Bioscience International Direct,Cochrane Library and Web of Science.The databases were searched from inception to December 31,2018,with no language restriction.We screened 692 items of which 19 met our inclusion criteria.Using a meta-ethnographic method,we identified six themes:(i)pastoralism in Ethiopia;(ii)pastoralists’livelihood profile;(iii)pastoralists’service utilisation;(iv)pastoralists’knowledge and awareness on TB control services;(v)challenges of TB control in pastoral settings;and(vi)equity disparities affecting pastoralists.Our interpretation triangulates the results across all included studies and shows that TB control activities observed in pastoralist regions of Ethiopia are far fewer than elsewhere in the country.Conclusions:This systematic review and meta-synthesis shows that TB control in Ethiopia does not align well with the pastoralist lifestyle.Inaccessibility and lack of acceptability of TB care are the key bottlenecks to pastoralist TB service provision.Targeting these two parameters holds promise to enhance effectiveness of an intervention.Faisal Nooh Lisa Crump Abdiwahab Hashi Rea Tschopp Esther Schelling Klaus Reither Jan Hattendorf Seid M.Ali Brigit Obrist Jurg Utzinger Jakob Zinsstag 2019Infectious Diseases of Poverty2019,8,5:2
5The genetic variation of Angiostrongylus cantonensis in the People’s Republic of China显示文摘Background:The People’s Republic of China(P.R.China)is the presumptive home range of the rat lungworm Angiostrongylus cantonensis,a major aetiological agent of human eosinophilic meningitis.We present a study of the genetic variation of A.cantonensis in P.R.China.Our aim was to deepen the current knowledge pertaining to its origin and global spread from a molecular perspective.Methods:Adult A.cantonensis were collected in the frame of a national survey and identified based on morphological criteria.Polymerase chain reaction(PCR)was employed to amplify the target DNA sequences(cytochrome c oxidase subunit I(cox1),nicotinamide adenine dinucleotide dehydrogenase subunit 1(nad1)and internal transcribed spacer(ITS)).The PCR product of cox1 was directly submitted to sequencing,while clone sequencing was used for nad1 and ITS.The identity of the samples was verified by comparing the sequences to those of accepted A.cantonensis specimens.The specific composition of substitutions in each gene was analysed,and the genotypes were compared based on the complete cox1,nad1 and ITS genes.Results:We characterised the complete mitochondrial genes cox1 and nad1 of 130 specimens and obtained 357 nuclear sequences containing two complete ITS(ITS1 and ITS2)and 5.8S rRNA of the same samples.All specimens were genetically confirmed as A.cantonensis.Two major groups(i.e.I and II)were identified according to the phylogeny of cox1 sequences.Group I could be further categorised into six distinct clades.Almost half of the specimens(47.7%)belong to the clade Ia and 22.3%to the group II.The former was widely distributed across the study region.A variable number of repeat units in three microsatellites was observed,resulting in considerable length variation in ITS.Intragenomic variation of ITS sequences was found in a large proportion of the samples.Genotyping showed a striking difference between mitochondrial DNA and ITS.Conclusions:Our results demonstrate that A.cantonensis is the only rat lungworm species in P.R.China and shows high genetic diversity.Results of diversity and genotyping of A.cantonensis can be impacted by the sequencing strategy and biomarker.Although ITS may be a valuable marker for interspecific identification,it is not suitable for studying the intraspecific variation of A.cantonensis due to its high intragenomic variation and current challenges for direct sequencing.Shan Lv Yi Zhang Peter Steinmann Jurg Utzinger Xiao-Nong Zhou 2017Infectious Diseases of Poverty2017,6,1:2
6Efforts to mitigate the economic impact of the COVID-19 pandemic:potential entry points for neglected tropical diseases显示文摘Background:The damage inflicted by the coronavirus diseases 2019(COVID-19)pandemic upon humanity is and will continue to be considerable.Unprecedented progress made in global health over the past 20 years has reverted and economic growth has already evaporated,giving rise to a global recession,the likes of which we may not have experienced since the Second World War.Our aim is to draw the attention of the neglected tropical disease(NTD)community towards some of the major emerging economic opportunities which are quickly appearing on the horizon as a result of COVID-19.Main text:This scoping review relied on a literature search comprised of a sample of articles,statements,and press releases on initiatives aimed at mitigating the impact of COVID-19,while supporting economic recovery.Of note,the donor scenario and economic development agendas are highly dynamic and expected to change rapidly as the COVID-19 pandemic unfolds,as are donor and lender priorities.Con elusions:The NTD community,particularly in low-and middle-income countries(LMICs),will need to work quickly,diligently,and in close collaboration with decision-makers and key stakeholders,across sectors at national and international level to secure its position.Doing so might enhance the odds of grasping potential opportunities to access some of the massive resources that are now available in the form of contributions from corporate foundations,trust funds,loans,debt relieve schemes,and other financial mechanisms,as part of the ongoing and future economic development agendas and public health priorities driven by the COVID-19 pandemic.This paper should serve as a starting point for the NTD community to seek much needed financial support in order to sustain and revitalize control and elimination efforts pertaining to NTDs in LMICs.John P.Ehrenberg Jurg Utzinger Gilberto Fontes Eliana Maria Mauricio da Rocha Nieves Ehrenberg Xiao-Nong Zhou Peter Steinmann 2021Infectious Diseases of Poverty2021,10,1:1

8Distribution of Plasmodium spp.infection in asymptomatic carriers in perennial and low seasonal malaria transmission settings in West Africa显示文摘countries in West Africa remain a hotspot for malaria with all age groups at risk.Asymptomatic carriers of Plasmodium spp.are important sources of infections for malaria vectors and thus contribute to the anchoring of the disease in favourable eco-epidemiological settings.The objective of this study was to assess the asymptomatic malaria case rates in Korhogo and Kaedi,two urban areas in northern Côte d’Ivoire and southern Mauritania,respectively.Methods:Cross-sectional surveys were carried out during the rainy season in 2014 and the dry season in 2015 in both settings.During each season,728 households were randomly selected and a household-based questionnaire was implemented to collect demographic and epidemiological data,including of malaria preventive methods used in communities.Finger-prick blood samples were obtained for biological examination using microscopy and rapid diagnostic tests(RDTs).Results:Overall,2672 households and 15858 consenting participants were surveyed.Plasmodium spp.infection was confirmed in 12.4%(n=832)and 0.3%(n=22)of the assessed individuals in Korhogo and Kaedi,respectively.In Korhogo,the prevalence of asymptomatic malaria was 10.5%(95%CI:9.7-11.2)as determined by microscopy and 9.3%(95%CI:8.6-10.0%)when assessed by RDT.In Kaedi,asymptomatic malaria prevalence was 0.2%(95%CI:0.1-0.4%)according to microscopy,while all RDTs performed were negative(n=8372).In Korhogo,asymptomatic malaria infection was significantly associated with age and season,with higher risk within the 5-14 years-old,and during the rainy season.In Kaedi,the risk of asymptomatic malaria infection was associated with season only(higher during the dry season;crude OR(cOR):6.37,95%CI:1.87-21.63).P.falciparum was the predominant species identified in both study sites representing 99.2%(n=825)in Korhogo and 59.1%(n=13)in Kaedi.Gametocytes were observed only in Korhogo and only during the rainy season at 1.3%(95%CI:0.7-2.4%).Conclusions:Our findings show a low prevalence of clinical malaria episodes with a significant proportion of asymptomatic carriers in both urban areas.National policies for malaria infections are focused on treatment of symptomatic cases.Malaria control strategies should be designed for monitoring and managing malaria infections in asymptomatic carriers.Additional measures,including indoor residual spraying,effective use of long-lasting insecticidal nets is strongly needed to reduce the number of Plasmodium spp.infections in Korhogo and Kaedi.Constant G.N.Gbalegba Hampate Ba Kigbafori D.Silue Ousmane Ba Emmanuel Tia Mouhamadou Chouaibou Nathan T.Y.Tian-Bi Gre goire Y.Yapi Brama Kone Jurg Utzinger Benjamin G.Koudou 2018Infectious Diseases of Poverty2018,7,1:1

10Prevalence of diarrhoea and risk factors among children under five years old in Mbour,Senegal:a cross-sectional study显示文摘Background:Diarrhoeal diseases remain an important cause of mortality and morbidity among children,particularly in low-and middle-income countries.In Senegal,diarrhoea is responsible for 15%of all deaths in children under the age of five and is the third leading cause of childhood deaths.For targeted planning and implementation of prevention strategies,a context-specific understanding of the determinants of diarrhoeal diseases is needed.The aim of this study was to identify risk factors of diarrhoeal diseases in children under the age of five in Mbour,Senegal.Methods:Between February and March 2014,a cross-sectional survey was conducted in four zones of Mbour to estimate the burden of diarrhoeal diseases(i.e.diarrhoea episodes in the 2 weeks preceding the survey)and associated risk factors.The zones covered urban central,peri-central,north peripheral and south peripheral areas.Overall,596 households were surveyed by a questionnaire,yielding information on sociodemographic,environmental and hygiene behavioural factors.Univariable and multivariable logistic regression analyses were used to identify risk factors associated with the occurrence of diarrhoea.Results:The reported prevalence of diarrhoea among children under the age of five during the 2 weeks preceding the survey was 26%.Without adjustment,the highest diarrhoea prevalence rates were observed in the peri-central(44.8%)and urban central zones(36.3%).Multivariable regression revealed significant associations between diarrhoeal diseases and unemployment of mothers(adjusted odds ratio[aOR]=1.62,95%confidence interval[CI]:1.18-2.23),use of open bags for storing household waste(aOR=1.75,95%CI:1.00-3.02),evacuation of household waste in public streets(aOR=2.07,95%CI:1.20-3.55),no treatment of stored drinking water(aOR=1.69,95%CI:1.11-2.56)and use of shared toilets(aOR=1.69,95%CI:1.11-2.56).Conclusion:We found a high prevalence of diarrhoea in children under the age of five in Mbour,with the highest prevalence occurring in the central and peri-central areas.These findings underscore the need for public health interventions to alleviate the burden of diarrhoea among vulnerable groups.Promotion of solid waste disposal and reduction of wastewater exposure should be implemented without delay.Sokhna Thiam Aminata N.Diene Samuel Fuhrimann Mirko S.Winkler Ibrahima Sy Jacques A.Ndione Christian Schindler Penelope Vounatsou Jurg Utzinger Ousmane Faye Gueladio Cisse 2017Infectious Diseases of Poverty2017,6,1:1
11Potential factors influencing lymphatic filariasis transmission in'hotspot'and'control'areas in Ghana:the importance of vectors显示文摘Background:Mass drug administration(MDA)programmes for the control of lymphatic filariasis in Ghana,have been ongoing in some endemic districts for 16 years.The current study aimed to assess factors that govern the success of MDA programmes for breaking transmission of lymphatic filariasis in Ghana.Methods:The study was undertaken in two'hotspot'districts(Ahanta West and Kassena Nankana West)and two control districts(Mpohor and Bongo)in Ghana.Mosquitoes were collected and identified using morphological and molecular tools.A proportion of the cibarial armatures of each species was examined.Dissections were performed onAnopheles gambiae for filarial worm detection.A questionnaire was administered to obtain information on MDA compliance and vector control activities.Data were compared between districts to determine factors that might explain persistent transmission of lymphatic filariasis.Results:High numbers of mosquitoes were sampled in Ahanta West district compared to Mpohor district(F=16.09,P=0.002).There was no significant difference between the numbers of mosquitoes collected in Kassena Nankana West and Bongo districts(F=2.16,P=0.185).Mansonia species were predominant in Ahanta West district.An.coluzzii mosquitoes were prevalent in all districts.An.melas with infected and infective filarial worms was found only in Ahanta West district.No differences were found in cibarial teeth numbers and shape for mosquito species in the surveyed districts.Reported MDA coverage was high in all districts.The average use of bednet and indoor residual spraying was 82.4 and 66.2%,respectively.There was high compliance in the five preceding MDA rounds in Ahanta West and Kassena Nankana West districts,both considered hotspots of lymphatic filariasis transmission.Conclusions:The study on persistent transmission of lymphatic filariasis in the two areas in Ghana present information that shows the importance of local understanding of factors affecting control and elimination of lymphatic filariasis.Unlike Kassena Nankana West district where transmission dynamics could be explained by initial infection prevalence and low vector densities,ongoing lymphatic filariasis transmission in Ahanta West district might be explained by high biting rates ofAn.gambiae and initial infection prevalence,coupled with high densities ofAn.melas andMansonia vector species that have low or no teeth and exhibiting limitation.Sellase Pi-Bansa Joseph Harold Nyarko Osei Kwadwo Kyeremeh Frempong Elizabeth Elhassan Osei Kweku Akuoko David Agyemang Collins Ahorlu Maxwell Alexander Appawu Benjamin Guibehi Koudou Michael David Wilson Dziedzom Komi de Souza Samuel Kweku Dadzie Jurg Utzinger Daniel Adjei Boakye 2019Infectious Diseases of Poverty2019,8,1:0
12Distinct clinical characteristics and helminth co-infections in adult tuberculosis patients from urban compared to rural Tanzania显示文摘Background:Differences in rural and urban settings could account for distinct characteristics in the epidemiology of tuberculosis(TB).We comparatively studied epidemiological features of TB and helminth co-infections in adult patients from rural and urban settings of Tanzania.Methods:Adult patients(≥18 years)with microbiologically confirmed pulmonary TB were consecutively enrolled into two cohorts in Dar es Salaam,with~4.4 million inhabitants(urban),and Ifakara in the sparsely populated Kilombero District with~400000 inhabitants(rural).Clinical data were obtained at recruitment.Stool and urine samples were subjected to diagnose helminthiases using Kato-Katz,Baermann,urine filtration,and circulating cathodic antigen tests.Differences between groups were assessed byχ2,Fisher’s exact,and Wilcoxon rank sum tests.Logistic regression models were used to determine associations.Results:Between August 2015 and February 2017,668 patients were enrolled,460(68.9%)at the urban and 208(31.1%)at the rural site.Median patient age was 35 years(interquartile range[IQR]:27-41.5 years),and 454(68%)were males.Patients from the rural setting were older(median age 37 years vs.34 years,P=0.003),had a lower median body mass index(17.5 kg/m2 vs.18.5 kg/m2,P<0.001),a higher proportion of recurrent TB cases(9%vs.1%,P<0.001),and in HIV/TB co-infected patients a lower median CD4 cell counts(147 cells/μl vs.249 cells/μl,P=0.02)compared to those from urban Tanzania.There was no significant difference in frequencies of HIV infection,diabetes mellitus,and haemoglobin concentration levels between the two settings.The overall prevalence of helminth co-infections was 22.9%(95%confidence interval[CI]:20.4-27.0%).The significantly higher prevalence of helminth infections at the urban site(25.7%vs.17.3%,P=0.018)was predominantly driven by Strongyloides stercoralis(17.0%vs.4.8%,P<0.001)and Schistosoma mansoni infection(4.1%vs.16.4%,P<0.001).Recurrent TB was associated with living in a rural setting(adjusted odds ratio[aOR]:3.97,95%CI:1.16-13.67)and increasing age(aOR:1.06,95%CI:1.02-1.10).Conclusions:Clinical characteristics and helminth co-infections pattern differ in TB patients in urban and rural Tanzania.The differences underline the need for setting-specific,tailored public health interventions to improve clinical management of TB and comorbidities.George Sikalengo Jerry Hella Francis Mhimbira Liliana K.Rutaihwa Farida Bani Robert Ndege Mohamed Sasamalo Lujeko Kamwela Khadija Said Grace Mhalu Yeromin Mlacha Christoph Hatz Stefanie Knopp Sebastien Gagneux Klaus Reither Jurg Utzinger Marcel Tanner Emilio Letang Maja Weisser Lukas Fenner 2018Infectious Diseases of Poverty2018,7,1:0
13Risk factors for schistosomiasis in an urban area in northern Cote d’Ivoire显示文摘Background:Schistosomiasis is a water-based disease transmitted by trematodes belonging to the genus Schistosoma.The aim of this study was to assess the relationship between the prevalence of schistosomiasis and access to water,sanitation and hygiene(WASH)and environmental and socioeconomic factors in the city of Korhogo,northern Côte d'Ivoire.Methods:A cross-sectional study including 728 randomly selected households was conducted in Korhogo in March 2015.The heads of the households were interviewed about access to WASH and environmental and socioeconomic factors.All children abed between 5 and 15 years living in the households were selected to provide stool and urine samples for parasitological diagnosis of Schistosoma mansoni and Schistosoma haematobium infection.The relationship between infection with S.mansoni and potential risk factors was analysed by a mixed logistic regression model with‘household’as a random factor.Likelihood ratio tests were used to identify factors that were significantly associated with a Schistosoma spp.infection.Results:The overall prevalence of schistosomiasis among school-aged children in Korhogo was 1.9%(45/2341)composed of 0.3%(3/1248)S.haematobium and 3.5%(42/1202)S.mansoni.Due to the low prevalence of S.haematobium infection,risk factor analysis was limited to S.mansoni.Boys were 7.8 times more likely to be infected with S.mansoni than girls.Children between 10 and 15 years of age were 3.8 times more likely to be infected than their younger counterparts aged 5-10 years.Moreover,living in a house further away from a water access point(odds ratio[OR]=0.29,95%confidence interval[CI]:0.13-0.70)and abstaining from swimming in open freshwater bodies(OR=0.16,95%CI:0.04-0.56)were significantly associated with decreased odds of S.mansoni infection.The socioeconomic status did not appear to influence the prevalence of S.mansoni.Conclusions:A strategy to reduce the incidence of schistosomiasis should focus on health education to change the behaviour of populations at risk and encourage communities to improve sanitation and infrastructure in order to reduce contact with surface water.Richard K.M’Bra Brama Kone Yapi G.Yapi Kigbafori D.Silue Ibrahima Sy Danielle Vienneau Nagnin Soro Gueladio Cisse Jurg Utzinger 2018Infectious Diseases of Poverty2018,7,1:0
