3Adjunctive acupuncture for pain and symptom management in the inpatient setting: protocol for a pilot hybrid effectiveness-implementation study显示文摘BACKGROUND: Effective pain management among hospitalized patients is an important aspect of providing quality care and achieving optimal clinical outcomes and patient satisfaction. Common pharmacologic approaches for pain, though effective, have serious side effects and are not appropriate for all inpatients. Findings from randomized controlled trials(RCTs) support the efficacy of acupuncture for many symptoms relevant to inpatients including postoperative pain, cancer-related pain, nausea and vomiting, and withdrawal from narcotic use. However, the extent to which findings from RCTs translate to real-world implementation of acupuncture in typical hospital settings is unknown. METHODS/DESIGN: In partnership with the launch of a clinical program offering acupuncture services to inpatients at the University of California San Francisco's Mount Zion Hospital, we are conducting a pilot study using a hybrid effectiveness-implementation design to:(1) assess the effectiveness of acupuncture to manage pain and other symptoms and improve patient satisfaction; and(2) evaluate the barriers and facilitators to implementing an on-going acupuncture service for inpatients. During a twomonth pre-randomization phase, we evaluated and adapted clinical scheduling and treatment protocols with acupuncturists and hospital providers and pretested study procedures including enrollment, consent, and data collection. During a six-month randomization phase, we used a two-tiered consent process in which inpatients were first consented into a study of symptom management, randomized to be offered acupuncture, and consented for acupuncture if they accepted. We are also conducting in-depth interviews and focus groups to assess evidence, context, and facilitators of key provider and hospital administration stakeholders.DISCUSSION: Effectiveness research in 'real-world' practice settings is needed to inform clinical decision-making and guide implementation of evidence-based acupuncture practices. To successfully provide clinical acupuncture services and maintain a rigorous research design, practice-based trials of acupuncture require careful planning and attention to setting-specific, contextual factors.TRIAL REGISTRATION: This trial has been registered in Clinical Trials.gov. 11西双版纳曼旦村人工橡胶林“退胶还林”经济补偿标准研究显示文摘热带雨林是地球的生物多样性宝库,近年人类活动导致全球热带雨林面积不断减少,使得保护雨林、保护环境,促进可持续发展成为当今世界各个国家和人民的共同愿望。从20世纪50年代至今,西双版纳森林覆盖面积减少了近30%,大部分转变为人工橡胶林。面对经济发展与不良生态效应之间的矛盾关系,以政府为主导的有偿'退胶'是在一定程度上拯救雨林的直接途径。本研究采用土地期望值理论和复利计算法,估算出西双版纳曼旦村橡胶林的土地期望值和森林价值,进而制定出'退胶还林'经济补偿标准,并采用了以20年为年限分期付款、征收/租赁的补偿方式。这一研究结果在理论和实践两方面为'退胶还林'面临的实际问题提供了科学的解决方案和参考依据。为促进'退胶还林'的实施,本文还建议在大规模'退胶还林'计划实施之前,要采取政策和措施,尽可能缓解大面积橡胶林对西双版纳当地生态环境的负面影响。张佳琦 Sun Joseph Chang Alison Wee 薛达元 2015资源科学2015,37,12:2

14Effect of weekend admission on geriatric hip fractures显示文摘BACKGROUND The care discrepancy for patients presenting to a hospital on the weekend relative to the work week is well documented.With respect to hip fractures,however,there is no consensus about the presence of a so-called“weekend effect”.This study sought to determine the effects,if any,of weekend admission on care of geriatric hip fractures admitted to a large tertiary care hospital.It was hypothesized that geriatric hip fracture patients admitted on a weekend would have longer times to medical optimization and surgery and increased complication and mortality rates relative to those admitted on a weekday.AIM To determine if weekend admission of geriatric hip fractures is associated with poor outcome measures and surgical delay.METHODS A retrospective chart review of operative geriatric hip fractures treated from 2015-2017 at a large tertiary care hospital was conducted.Two cohorts were compared:patients who arrived at the emergency department on a weekend,and those that arrived at the emergency department on a weekday.Primary outcome measures included mortality rate,complication rate,transfusion rate,and length of stay.Secondary outcome measures included time from emergency department arrival to surgery,time from emergency department arrival to medical optimization,and time from medical optimization to surgery.RESULTS There were no statistically significant differences in length of stay(P=0.2734),transfusion rate(P=0.9325),or mortality rate(P=0.3460)between the weekend and weekday cohorts.Complication rate was higher in patients who presented on a weekend compared to patients who presented on a weekday(13.3%vs 8.3%;P=0.044).Time from emergency department arrival to medical optimization(22.7 h vs 20.0 h;P=0.0015),time from medical optimization to surgery(13.9 h vs 10.8 h;P=0.0172),and time from emergency department arrival to surgery(42.7 h vs 32.5 h;P<0.0001)were all significantly longer in patients who presented to the hospital on a weekend compared to patients who presented to the hospital on a weekday.CONCLUSION This study provided insight into the“weekend effect”for geriatric hip fractures and found that day of presentation has a clinically significant impact on delivered care.Jordan B Pasternack Matthew L Ciminero Michael Silver Joseph Chang Ronald J Simon Kevin K Kang 2020World Journal of Orthopedics2020,11,9:2
