1Numerical modeling of centrifuge cyclic lateral pile load experiments显示文摘To gain insight into the inelastic behavior of piles, the response of a vertical pile embedded in dry sand and subjected to cyclic lateral loading was studied experimentally in centrifuge tests conducted in Laboratoire Central des Ponts et Chaussées. Three types of cyclic loading were applied, two asymmetric and one symmetric with respect to the unloaded pile. An approximately square-root variation of soil stiffness with depth was obtained from indirect in-flight density measurements, laboratory tests on reconstituted samples, and well-established empirical correlations. The tests were simulated using a cyclic nonlinear Winkler spring model, which describes the full range of inelastic phenomena, including separation and re-attachment of the pile from and to the soil. The model consists of three mathematical expressions capable of reproducing a wide variety of monotonic and cyclic experimental p-y curves. The physical meaning of key model parameters is graphically explained and related to soil behavior. Comparisons with the centrifuge test results demonstrate the general validity of the model and its ability to capture several features of pile-soil interaction, including: soil plastifi cation at an early stage of loading, 'pinching' behavior due to the formation of a relaxation zone around the upper part of the pile, and stiffness and strength changes due to cyclic loading. A comparison of the p–y curves derived from the test results and the proposed model, as well as those from the classical curves of Reese et al. 5Organ sparing to cure stage IV rectal cancer: A case report and review of literature显示文摘BACKGROUND Rectal sparing is an option for some rectal cancers with complete or good response after chemoradiotherapy(CRT);however,it has never been evaluated in patients with metastases.We assessed long-term outcomes of a rectal-sparing approach in a liver-first strategy for patients with rectal cancer with resectable liver metastases.CASE SUMMARY We examined patients who underwent an organ-sparing approach for rectal cancer with synchronous liver metastases using a liver-first strategy during 2010-2015(n=8).Patients received primary chemotherapy and pelvic CRT.Liver surgery was performed during the interval between CRT completion and rectal tumor re-evaluation.Clinical and oncological characteristics and long-term outcomes were assessed.CASE SUMMARY All patients underwent liver metastatic resection with curative intent.The R0 rate was 100%.Six and two patients underwent local excision and a watch-and-wait(WW)approach,respectively.All patients had T3N1 tumors at diagnosis and had good clinical response after CRT.The median survival time was 60(range,14-127)mo.Three patients were disease free for 5,8,and 10 years after the procedure.Five patients developed metastatic recurrence in the liver(n=5)and/or lungs(n=2).Only one patient developed local recurrence concurrent with metastatic recurrence 24 mo after the WW approach.Two patients died during follow-up.CONCLUSION The results suggest good local control in patients undergoing organ-sparing strategies for rectal cancer with synchronous liver metastasis.Prospective trials are required to validate these data and identify good candidates for these strategies.Hélène Meillat Jonathan Garnier Anais Palen Jacques Ewald Cécile de Chaisemartin Marguerite Tyran Emmanuel Mitry Bernard Lelong 2023World Journal of Gastrointestinal Surgery2023,15,11:0
