| 1 | 北祁连早古生代花岗质岩浆作用及构造演化显示文摘北祁连山中段花岗岩锆石SHRIMP定年结果表明,柯柯里岩体的斜长花岗岩和石英闪长岩的年龄分别为512Ma和501 Ma,野马咀和金佛寺花岗岩的年龄分别为508Ma和424Ma。结合区内其它花岗岩体的定年资料,根据花岗岩的岩石地球化学特征及岩体产出的构造位置、区域地质资料等,我们认为,早古生代北祁连洋板块向南俯冲,至少引发了两期花岗质岩浆作用,第一次岩浆作用形成柯柯里斜长花岗岩(512Ma)、野马咀花岗岩(508Ma)和柯柯里石英闪长岩(501 Ma),第二次花岗质岩浆作用形成牛心山花岗岩(477Ma)。由于往南俯冲的板块受到柴达木板块向北俯冲的影响,俯冲受阻,继而俯冲极性发生变化,转向北俯冲,形成了民乐窑沟(463Ma)等花岗岩侵入体。大约440Ma之后,洋盆闭合,柴达木陆块和阿拉善陆块对接碰撞,形成北祁连造山带。由于造山带根部岩石圈发生折沉作用,造山带上不同的块体伸展、滑塌,形成一系列碰撞后花岗岩如金佛寺花岗岩(424Ma)及牛心山岩体的石英闪长岩(435Ma)等。 | 吴才来 徐学义 高前明 李向民 雷敏 郜源红 Ronald B FROST Joseph L WOODEN | 2010 | 岩石学报2010,26,4: | 132 |

| 5 | Can tranexamic acid change preoperative anemia management during total joint arthroplasty?显示文摘AIM: To investigate the postoperative transfusion and complication rates of anemic and nonanemic total joint arthroplasty patients given tranexamic acid(TXA).METHODS: A cross-sectional prospective study was conducted of primary hip and knee arthroplasty cases performed from 11/2012 to 6/2014. Exclusion criteria included revision arthroplasty, bilateral arthroplasty, acute arthroplasty after fracture, and contraindication to TXA. Patients were screened prior to surgery, with anemia was defined as hemoglobin of less than 12 g/d L for females and of less than 13 g/d L for males. Patients were divided into four different groups, based on the type of arthroplasty(total hip or total knee) and hemoglobin status(anemic or nonanemic). Intraoperatively, all patients received 2 g of intravenous TXA during surgery. Postoperatively, allogeneic blood transfusion(ABT) was directed by both clinical symptomsand relative hemoglobin change. Complications were recorded within the first two weeks after surgery and included thromboembolism, infection, and wound breakdown. The differences in transfusion and complication rates, as well as the relative hemoglobin change, were compared between anemic and nonanemic groups.RESULTS: A total of 232 patients undergoing primary joint arthroplasty were included in the study. For the total hip arthroplasty cohort, 21%(18/84) of patients presented with preoperative anemia. Two patients in the anemic group and two patients in the nonanemic group needed ABTs; this was not significantly different(P = 0.20). One patient in the anemic group presented with a deep venous thromboembolism while no patients in the nonanemic group had an acute complication; this was not significantly different(P = 0.21). For nonanemic patients, the average change in hemoglobin was 2.73 ± 1.17 g/d L. For anemic patients, the average change in hemoglobin was 2.28 ± 0.96 g/d L. Between the two groups, the hemoglobin difference of 0.45 g/d L was not significant(P = 0.13). For the total knee arthroplasty cohort, 18%(26/148) of patients presented with preoperative anemia. No patients in either group required a blood transfusion or had an acute postoperative complication. For nonanemic patients, the average change in hemoglobin was 1.85 ± 0.79 g/d L. For anemic patients, the average change in hemoglobin was 1.09 ± 0.58 g/d L. Between the two groups, the hemoglobin difference of 0.76 g/d L was significant(P < 0.001). | 9 | Thermal hypersensitivity in a subset of irritable bowel syndrome patients显示文摘AIM:To characterize thermal hypersensitivity in patients with constipation-and diarrhea-predominant irritable bowel syndrome (IBS).METHODS:Thermal pain sensitivity was tested among patients with diarrhea-predominant IBS (D-IBS) and constipation-predominant IBS (C-IBS) compared to healthy subjects.A total of 42 patients (29 female and 13 male;mean age 27.0±6.4 years) with D-IBS;24 patients (16 female and eight male;mean age 32.5±8.8 years) with C-IBS;and 52 control subjects (34 female and 18 male;mean age 27.3±8.0 years) participated in the study.Thermal stimuli were delivered using a Medoc Thermal Sensory Analyzer with a 3 cm × 3 cm surface area.Heat pain threshold (HPTh) and heat pain tolerance (HPTo) were assessed on the left ventral forearm and left calf using an ascending method of limits.The Functional Bowel Disease Severity Index (FBDSI) was also obtained for all subjects.RESULTS:Controls were less sensitive than C-IBS and D-IBS (both at P < 0.001) with no differences between C-IBS and D-IBS for HPTh and HPTo.Thermal hyperalgesia was present in both groups of IBS patients relative to controls,with IBS patients reporting significantly lower pain threshold and pain tolerance at both test sites.Cluster analysis revealed the presence of subgroups of IBS patients based on thermal hyperalgesia.One cluster (17% of the sample) showed a profile of heat pain sensitivity very similar to that of healthy controls;a second cluster (47% of the sample) showed moderate heat pain sensitivity;and a third cluster (36% of the sample) showed a very high degree of thermal hyperalgesia.CONCLUSION:A subset of IBS patients had thermal hypersensitivity compared to controls,who reported significantly lower HPTh and HPTo.All IBS patients had a higher score on the FBDSI than controls.Interestingly,the subset of IBS patients with high thermal sensitivity (36%) had the highest FBDSI score compared to the other two groups of IBS patients. | QiQi Zhou Roger B Fillingim Joseph L Riley Ⅲ G Nicholas Verne | 2009 | World Journal of Gastroenterology2009,15,26: | 2 |

| 11 | Effect of weekend admission on geriatric hip fractures显示文摘BACKGROUND The care discrepancy for patients presenting to a hospital on the weekend relative to the work week is well documented.With respect to hip fractures,however,there is no consensus about the presence of a so-called“weekend effect”.This study sought to determine the effects,if any,of weekend admission on care of geriatric hip fractures admitted to a large tertiary care hospital.It was hypothesized that geriatric hip fracture patients admitted on a weekend would have longer times to medical optimization and surgery and increased complication and mortality rates relative to those admitted on a weekday.AIM To determine if weekend admission of geriatric hip fractures is associated with poor outcome measures and surgical delay.METHODS A retrospective chart review of operative geriatric hip fractures treated from 2015-2017 at a large tertiary care hospital was conducted.Two cohorts were compared:patients who arrived at the emergency department on a weekend,and those that arrived at the emergency department on a weekday.Primary outcome measures included mortality rate,complication rate,transfusion rate,and length of stay.Secondary outcome measures included time from emergency department arrival to surgery,time from emergency department arrival to medical optimization,and time from medical optimization to surgery.RESULTS There were no statistically significant differences in length of stay(P=0.2734),transfusion rate(P=0.9325),or mortality rate(P=0.3460)between the weekend and weekday cohorts.Complication rate was higher in patients who presented on a weekend compared to patients who presented on a weekday(13.3%vs 8.3%;P=0.044).Time from emergency department arrival to medical optimization(22.7 h vs 20.0 h;P=0.0015),time from medical optimization to surgery(13.9 h vs 10.8 h;P=0.0172),and time from emergency department arrival to surgery(42.7 h vs 32.5 h;P<0.0001)were all significantly longer in patients who presented to the hospital on a weekend compared to patients who presented to the hospital on a weekday.CONCLUSION This study provided insight into the“weekend effect”for geriatric hip fractures and found that day of presentation has a clinically significant impact on delivered care. | Jordan B Pasternack Matthew L Ciminero Michael Silver Joseph Chang Ronald J Simon Kevin K Kang | 2020 | World Journal of Orthopedics2020,11,9: | 2 |
