| 1 | Comparative evaluation of microscopy,OptiMAL® and 18S rRNA gene based multiplex PCR for detection of Plasmodium falciparum & Plasmodium vivax from field isolates of Bikaner,India显示文摘Objective:To evaluate microscopy,OptiMAL? and multiplex PCR for the identification of Plasmodium falciparum(P.falciparum) and Plasmodium vivax(P.vivax) from the field isolates of Bikaner,Rajasthan(Northwest India).Methods:In this study,a multiplex PCR(P.falciparum and P.vivax) was further developed with the incorporation of Plasmodium malariae(P.malariae) specific primer and also a positive control.The performance of microscopy,plasmodium lactate dehydrogenase(pLDH) based malaria rapid diagnostic test OptiMAL? and 18S rRNA gene based multiplex PCR for the diagnosis of P.falciparum and P.vivax was compared.Results:The three species multiplex PCR if.falciparum,P.vivax and P.malariae) with an inbuilt positive control was developed and evaluated.In comparison with multiplex PCR,which showed the sensitivity and specificity of 99.36%(95%CI,98.11%-100.00%) and 100.00%(95%CI,100.00%-100.00%),the sensitivity and specificity of microscopy was 90.44%(95%CI,88.849-95.04%) and 99.22%(95% CI,97.71%-100.00%),and OptiMAL? was 93.58%(95%CI,89.75%-97.42%) and 97.69%(95%CI, 95.10%-100.00%).The efficiencies were 99.65%,95.10%and 95.45%for multiplex PCK.microscopy and OptiMAL?.respectively.Conclusions:Our results raise concerns over the overall sensitivities of microscopy and OptiMAL?,when compared to the multiplex PCR and thus stress the need for new molecular interventions in the accurate detection of the malarial parasites.This further highlights the fact that further developments are needed to improve the performance of rapid diagnostic tests at field level. | Deepak Pakalapati Shilpi Garg Sheetal Middha Abhishek Kochar Amit Kumar Subudhi Boopathi Pon Arunachalam Sanjay Kumar Kochar Vishal Saxena Pareek RP Jyoti Acharya Dhanpat Kumar Kochar Ashis Das | 2013 | Asian Pacific Journal of Tropical Medicine2013,6,5: | 3 |

| 15 | Unusual presentation of extramedullary blast crisis in chronic myeloid leukemia:A case report显示文摘BACKGROUND Extramedullary blast crisis in chronic myeloid leukemia(CML)is an uncommon occurrence of leukemic blast infiltration in regions other than the bone marrow.Malignant infiltration of the serosal membranes should be considered in cases where CML presents with ascites or pleural effusion.CASE SUMMARY A 23-year-old female with CML presented with progressively worsening ascites and pleural effusion despite first-line tyrosine kinase inhibitor treatment.Her blood work indicated leukocytosis with myelocyte bulge and 2%blasts.Analysis of the patient’s bone marrow confirmed the chronic phase of CML.Abdominal ultrasound revealed hepatosplenomegaly with ascites.The fluid investigation of both ascites and pleural effusion revealed a predominance of neutrophils with exudate.However,no acid-fast bacilli or growth was observed after culturing.Although hydroxyurea reduced cell counts,there was no observed effect on ascites or pleural effusion.Repeat investigation of the ascitic and pleural fluid revealed a polymorphous myeloid cell population consisting of myelocytes,metamyelocytes,band forms,neutrophils and a few myeloblasts.Extramedullary blast crisis was suspected,and mutation analysis was performed.We switched the patient to dasatinib.The patient’s symptoms were relieved,and ascites and pleural effusion diminished.CONCLUSION Serosal membrane involvement in CML is extremely rare.In this case,the patient responded well to dasatinib treatment. | Rashmi Mishra Sandeep Garg Praveen Bharti Deepak Ranjan Malla Ishan Rohatgi Sachin Gautam | 2023 | World Journal of Hematology2023,10,4: | 0 |

| 17 | Real-world five-year outcomes of FlexyRap®cobalt-chromium rapamycin-eluting stents with biodegradable polymer in patients with de-novo coronary artery disease显示文摘BACKGROUND The use of biodegradable polymer drug-eluting stents(BP-DES)has been proven to minimize restenosis and stent thrombosis.The current post-marketing monitoring was observed at the 5-year clinical outcomes of individuals who had been treated with FlexyRap®DES in the real world.AIM To assess the safety and effectiveness of FlexyRap®DES at the 5-year follow-up in real-world settings.METHODS Findings from a retrospective,multi-center,observational,post-market clinical follow-up study of patients treated with FlexyRap®DES for de novo coronary artery disease(CAD)were reported.During the 12-mo follow-up,the primary endpoint was target lesion failure,which was defined as the composite of cardiovascular death, target vessel myocardial infarction(TV-MI), and clinically driven target lesion revascularization.RESULTS The data of 500 patients received with FlexyRap®DES was obtained at the completion of the surveillance timeline of 5-year.After the implantation of FlexyRap®DES,the device success rate was 100%.Adverse events that led to major bleeding,permanent disability,or death were not experienced in the patients.The major adverse cardiac event rate at 12-mo,3-year,and 5-year follow-up was 1(0.2%),0(0%),and 1(0.2%)respectively with 0(0%)cardiovascular death,2(0.4%)TV-MI,and 0(0%)TLR compositely.Furthermore,late stent thrombosis was found in 2(0.4%)patients at the follow-up of 12-mo,very late stent thrombosis was observed in 2 patients(0.4%)at 3-year follow-up.CONCLUSION FlexyRap®DES was proved to be safe and efficacious in real-world patients with de novo CAD,indicating a lowered rate of cardiac events and stent thrombosis at 5-year follow-up. | Nitish Garg Raman Chawla Vivek Tandon Deepak Garg Nilesh Parshottam Preeti Vani Malte Neuss | 2023 | World Journal of Cardiology2023,15,3: | 0 |
