| 11 | 艾曲波帕治疗慢性特发性血小板减少性紫癜对血小板计数和出血的效果:一项随机、双盲、安慰剂对照试验显示文摘背景 艾曲波帕是一种口服、非肽类、血小板生成素(TPO)受体激动剂,可以刺激血小板生成而导致血小板产生增加。本研究评价单次口服艾曲波帕50mg/d的疗效、安全性和耐受性,同时探索剂量增加到75mg/d的效果。
方法本次Ⅲ期随机、双盲、安慰剂对照研究中的慢性特发性血小板减少性紫癜(ITP)成人受试者来自23个国家的63个地区,外周血血小板计数〈30000/μl,在接受一种或一种以上的标准ITP治疗方案的基础上加用单次口服艾曲波帕50mg/d(n=76)或安慰剂(n=38)治疗6周。患者通过有效的随机化分配系统按照艾曲波帕:安慰剂为2:1的比例随机分配进入试验。治疗3周后,如果患者小板计数仍然低于50000/μ1,艾曲波帕剂量增加至75mg。主要终点是在治疗第43天外周血血小板计数≥50000/μ1的患者比例。所有接受至少一种治疗方案的试验纳入者均进行统计学分析。本试验已在Clinical Trials.gov登记注册,注册号为NCT00102739。
结果艾曲波帕组的73例患者和安慰剂组的37例患者为有效研究人群,对其43d的试验进行统计学分析。艾曲波帕组的43例(59%)患者和安慰剂组的6例(16%)患者治疗有反应[血小板计数≥50000/μ1.OR9.61(95%CI3.31~27.86),P〈0.0001]。和安慰剂组相比,接受艾曲波帕治疗的反应并不受到预先定义的各种变量(基线血小板计数,合用治疗ITP的药物,是否接受脾切除术)或者先前接受ITP治疗次数的影响:在有效分析的34例患者中增加了艾曲波帕的治疗剂量,10例(29%)有反应。通常在治疗结束2周后血小板计数返回基线水平。和安慰剂组相比,艾曲波帕组在试验中的任何时段出血均明显减少[0R 0.49(95%C10.26—0.89),P=0.021]。治疗中两组3~4级治疗副反应(艾曲波帕组2例(3%)vs安慰剂组1例(3%)]和由于副作用而终止治疗[艾曲波帕组3例(4%)vs安慰剂组2例(5%)1的发生率相似。
