| 1 | Factors that mediate colonization of the human stomach by Helicobacter pylori显示文摘Helicobacter pylori(H.pylori)colonizes the stomach of humans and causes chronic infection.The majority of bacteria live in the mucus layer overlying the gastric epithelial cells and only a small proportion of bacteria are found interacting with the epithelial cells.The bacteria living in the gastric mucus may act as a reservoir of infection for the underlying cells which is essential for the development of disease.Colonization of gastric mucus is likely to be key to the establishment of chronic infection.How H.pylori manages to colonise and survive in the hostile environment of the human stomach and avoid removal by mucus flow and killing by gastric acid is the subject of this review.We also discuss how bacterial and host factors may together go some way to explaining the susceptibility to colonization and the outcome of infection in different individuals.H.pylori infection of the gastric mucosa has become a paradigm for chronic infection.Understanding of why H.pylori is such a successful pathogen may help us understand how other bacterial species colonise mucosal surfaces and cause disease. | Ciara Dunne Brendan Dolan Marguerite Clyne | 2014 | World Journal of Gastroenterology2014,20,19: | 8 |
| 2 | Quantitative analysis of ruminal methanogenic microbial populations in beef cattle divergent in phenotypic residual feed intake(RFI)offered contrasting diets显示文摘Background:Methane(CH_4) emissions in cattle are an undesirable end product of rumen methanogenic fermentative activity as they are associated not only with negative environmental impacts but also with reduced host feed efficiency.The aim of this study was to quantify total and specific rumen microbial methanogenic populations in beef cattle divergently selected for residual feed intake(RFI) while offered(i) a low energy high forage(HF) diet followed by(ii) a high energy low forage(LF) diet.Ruminal fluid was collected from 14 high(H)and 14 low(L) RFI animals across both dietary periods.Quantitative real time PCR(qRT-PCR) analysis was conducted to quantify the abundance of total and specific rumen methanogenic microbes.Spearman correlation analysis was used to investigate the association between the relative abundance of methanogens and animal performance,rumen fermentation variables and diet digestibility.Results:Abundance of methanogens,did not differ between RFI phenotypes.However,relative abundance of total and specific methanogen species was affected(P< 0.05) by diet type,with greater abundance observed while animals were offered the LF compared to the HF diet.Conclusions:These findings suggest that differences in abundance of specific rumen methanogen species may not contribute to variation in CH_4 emissions between efficient and inefficient animals,however dietary manipulation can influence the abundance of total and specific methanogen species. | Ciara A Carberry David A Kenny Alan K Kelly Sinead M Waters | 2014 | Journal of Animal Science and Biotechnology2014,5,4: | 6 |
| 3 | Identifying more reliable parameters for the detection of change during the follow-up of mild to moderate keratoconus patients显示文摘Background:Reaching a consensus on which parameters are most reliable at detecting progressive keratoconus patients with serial topography imaging is not evident.The aim of the study was to isolate the parameters best positioned to detect keratoconus progression using the Pentacam HR^(®)measures based on the respective limits of repeatability and range of measurement.Method:Using the Pentacam HR^(®),a tolerance index was calculated on anterior segment parameters in healthy and keratoconic eyes.The tolerance index provides a scale from least to most affected parameters in terms of measurement noise relative to that observed in healthy eyes.Then,based on the“number of increments”from no disease to advanced disease,a relative utility(RU)score was also calculated.RU values close to 1 indicate parameters best positioned to detect a change in keratoconic eyes.Results:The tolerance index values indicated that 36%of ocular parameters for keratoconic eyes had repeatability limits which were wider than normative limits(worse),but 28%of the ocular parameters were narrower than normative limits(better).Considering only those parameters with a RU greater than 0.95,a small number of parameters were within this range,such as corneal curvature and asphericity indices.Conclusions:This study demonstrates that measurement error in keratoconic eyes is significantly greater than healthy eyes.Indices implemented here provide guidance on the levels of expected precision in keratoconic eyes relative to healthy eyes to aid clinicians in distinguishing real change from noise.Importantly maximal keratometry(Kmax),central corneal thickness(CCT)and thinnest corneal thickness(TCT)were highlighted as problematic indices for the follow-up of keratoconus in terms of repeatability. | Ivo Guber Colm McAlinden François Majo Ciara Bergin | 2017 | Eye and Vision2017,4,1: | 6 |
| 4 | Randomized study comparing incidence of radial artery occlusion post-percutaneous coronary intervention between two conventional compression devices using a novel airinflation technique显示文摘AIM To compare post-percutaneous coronary intervention(PCI) radial artery occlusion(RAO) incidence between two conventional radial artery compression devices using a novel air-inflation technique.METHODS One hundred consecutive patients post-PCI were randomized 1:1 to Safeguard or TR band compression devices. Post-radial sheath removal, each compression device was inflated with additional 2 m L of air above index bleeding point during air-filled device application and gradually down-titrated accordingly. RAO was defined as absence of Doppler flow signal performed at 24 h and at 6 wk post-PCI. Patients with missing data were excluded. Statistical significance was defined as P < 0.05.RESULTS All patients had 6 F radial sheath inserted. No significant differences were observed between Safeguard Radial(n = 42) vs TR band(n = 42) in terms of age(63 ± 11 years vs 67 ± 11 years), clinical presentation(electives, n = 18 vs n = 16; acute coronary syndrome, n = 24 vs n = 26) and total procedural heparin(7778 ± 2704 IU vs 7825 ± 2450 IU). RAO incidence was not significantly different between groups at 24 h(2% vs 0%, P = 0.32) and 6 wk(0%, both).CONCLUSION Safeguard Radial and TR band did not demonstrate significant between-group differences in short-term RAO incidence. Lack of evidence of RAO in all postPCI patients at 6 wk follow-up, regardless of radial compression device indicate advantage of using the novel and pragmatic air-inflation technique. | 5 | Reducing transfusion requirements in liver transplantation显示文摘Liver transplantation(LT) was historically associated with massive blood loss and transfusion. Over the past two decades transfusion requirements have reduced dramatically and increasingly transfusionfree transplantation is a reality. Both bleeding and transfusion are associated with adverse outcomes in LT. Minimising bleeding and reducing unnecessary transfusions are therefore key goals in the perioperative period. As the understanding of the causes of bleeding has evolved so too have techniques to minimize or reduce the impact of blood loss. Surgical 'piggyback' techniques, anaesthetic low central venous pressure and haemodilution strategies and the use of autologous cell salvage, point of care monitoring and targeted correction of coagulopathy, particularly through use of factor concentrates, have all contributed to declining reliance on allogenic blood products. Pre-emptive management of preoperative anaemia and adoption of more restrictive transfusion thresholds is increasingly common as patient blood management(PBM) gains momentum. Despite progress, increasing use of marginal grafts and transplantation of sicker recipients will continue to present new challenges in bleeding and transfusion management. Variation in practice across different centres and within the literature demonstrates the current lack of clear transfusion guidance. | 6 | Predictability of refraction following immediate sequential bilateral cataract surgery(ISBCS)performed under general anaesthesia显示文摘Background:To evaluate the predictability of refraction following immediate sequential bilateral cataract surgery(ISBCS)performed under general anaesthesia.Methods:This is a retrospective review of all ISBCS performed at Kantonsspital Winterthur,Switzerland,between April 2000 and September 2013.The case notes of 250 patients were reviewed.Patients having full refraction reported(110 patients/220 eyes)were included.210(95%)eyes had a straight forward phacoemulsification with posterior chamber intraocular lens implantation,seven eyes had a planned extracapsular cataract extraction(ECCE);three eyes had an intracapsular cataract extraction.Results:Both eyes of 110 patients(64 women,46 men)with a mean age of 79.0 years,standard deviation(SD)±11.4(range 26 to 97 years)were included.Median preoperative best corrected visual acuity(BCVA)was 0.5 LogMAR in the first eye,the interquartile range(IQR)was[0.4,1.2];0.7 LogMAR in the second eye with IQR[0.4,1.8].At one month,the median BCVA was 0.2 LogMAR,IQR[0.1,0.3]in the first eye,median BCVA was 0.1 LogMAR and IQR[0.0,0.5]in the second eye.There were 3 eyes(3%)that lost 3 lines or more in BCVA at one month(control vs.pre-operatively).In all three cases,poor visual acuity had been recorded pre-operatively(>1 LogMAR).Achieved refraction was within±1.0 D of the target in 83%of eyes.There were only 5%(n=6)of cases where if delayed sequential bilateral extraction had been performed could potentially intraocular lens(IOL)choice have been adjusted,in four of these cases,target refraction was within±1.0 D in the second eye.Conclusions:ISBCS performed under general anaesthesia achieves target refraction in 83%of eyes after consideration of complications,ocular co-morbidities and systemic restrictions.In the majority of cases where IOL power calculation could be considered,the achieved refraction of the second surgical eye was within±1.0 D of intended refraction.This undermines the utility of IOL power adjustments in the second surgical eye. | Ivo Guber Laurent Rémont Ciara Bergin | 2015 | Eye and Vision2015,2,1: | 3 |
