| 5 | Autonomic Nervous Function in Vasovagal Syncope of Children and Adolescents显示文摘Syncope is defined as a transient loss of consciousness due to global cerebral hypoperfusion, accompanied by loss of muscle tone and failure to maintain an active position. Vasovagal syncope (VVS) is the most common presentation of syncope, and its diagnostic criteria include:(1) absence of any other evident etiology for syncope or presyncope,(2) positive response to head-up tilt test with evident vasovagal reaction (hypotension and/or bradycardia), and (3) no concomitant chronic or acute disease [1, 2]. The onset of VVS peaks initially in childhood and adolescence, and accounts for 60%–70% of all syncopal cases. Clinicians pay great attention to syncope among children and adolescents, due to its high prevalence and its impact on patients’ quality of life. Affected individuals often experience mental stress, economic burdens, and accidental bodily injuries related to syncope [2]. While the pathogenesis of VVS is not fully understood, autonomic nervous dysfunction has been identified as a contributing mechanism. | 6 | IL-17A-dependent gut microbiota is essential for regulating diet-induced disorders in mice显示文摘The gut microbiota plays a key role in obesity and related metabolic disorders, and multiple factors including diet, host genotype, and age regulate it. Many studies have examined the contribution of extrinsic factors to the regulation of the gut microbiota, but the importance of the host genetic constitution cannot be ignored. Interleukin 17A(IL-17A), a pro-inflammatory cytokine, is important in the defense against infection and diseases. Here, we investigated the association among IL-17, a high-fat diet(HFD), and the gut microbiota. Mice deficient in IL-17A were resistant to diet-induced obesity and related diseases. Compared with the Ⅱ-17^(-/-)mice, wild-type(WT) mice challenged with HFD showed obvious weight fluctuations, such as those seen in type 2 diabetes, and hematological changes similar to those associated with metabolic syndrome. However, housing WT mice and Ⅱ-17a^(-/-)mice together significantly alleviated these symptoms in the WT mice. A metagenomic analysis of the mouse feces indicated that the microbial community compositions of these two groups differed before HFD feeding. The HFD mediated shifts in the gut microbial compositions, which were associated with the mouse phenotypes.We also identified potentially beneficial and harmful species present during this period, and drew networks of the most abundant species. A functional analysis indicated pathway changes in the WT and Ⅱ-17a^(-/-)mice when fed the HFD. | 10 | Serum uric acid and risk of incident diabetes in middle-aged and elderly Chinese adults: prospective cohort study显示文摘The association between serum uric acid and the risk of incident diabetes in Chinese adults remains unknown.This study aimed to investigate this association in a community-dwelling population aged≥40 years in Shanghai,China.Oral glucose tole3rance test was conducted during baseline and follow-up visits.Relative risk regression was utilized to examine the associations between baseline gender-specific serum uric acid levels and incident diabetes risk.A total of 613(10.3%)incident diabetes cases were identified during the follow-up visit after 4.5 years.Fasting plasma glucose,postload glucose,and glycated hemoglobin A1c during the follow-up visit progressively increased across the sex-specific quartiles of serum uric acid(all Ps<0.05).The incidence rate of diabetes increased across the quartiles of serum uric acid(7.43%,8.77%,11.47%,and 13.43%).Multivariate adjusted regression analysis revealed that individuals in the highest quartile had 1.36-fold increased risk of diabetes compared with those in the lowest quartile of serum uric acid(odds ratio(95%confidence interval)=1.36(1.06−1.73)).Stratified analysis indicated that the association was only observed in women.Accordingly,serum uric acid was associated with the increased risk of incident diabetes among middle-aged and elderly Chinese women. | Di Cheng Chunyan Hu Rui Du Hongyan Qi Lin Lin Xueyan Wu Lina Ma Kui Peng Mian Li Min Xu Yu Xu Yufang Bi Weiqing Wang Yuhong Chen Jieli Lu | 2020 | Frontiers of Medicine2020，14，6： | 3 |
