1Delayed diagnosis of alpha-1-antitrypsin deficiency following post-hepatectomy liver failure: A case report显示文摘Post-hepatectomy liver failure(PHLF) is a leading cause of morbidity and mortality following major liver resection. The development of PHLF is dependent on the volume of the remaining liver tissue and hepatocyte function. Without effective pre-operative assessment, patients with undiagnosed liver disease could be at increased risk of PHLF. We report a case of a 60-year-old male patient with PHLF secondary to undiagnosed alpha-1-antitrypsin deficiency(AATD) following major liver resection. He initially presented with acute large bowel obstruction secondary to a colorectal adenocarcinoma, which had metastasized to the liver. There was no significant past medical history apart from mild chronic obstructive pulmonary disease. After colonic surgery and liver directed neo-adjuvant chemotherapy, he underwent a laparoscopic partially extended right hepatectomy and radio-frequency ablation. Post-operatively he developed PHLF. The cause of PHLF remained unknown, prompting reanalysis of the histology, which showed evidence of AATD. He subsequently developed progressive liver dysfunction, portal hypertension, and eventually an extensive parastomal bleed, which led to his death; this was ultimately due to a combination of AATD and chemotherapy. 7Keeping it simple: the value of an irreducibly simple climate model显示文摘Richardson et al.(Sci Bull, 2015. doi:10.1007/s11434-015-0806-z) suggest that the irreducibly simple climate model described in Monckton of Brenchley et al.(Sci Bull 60:122–135, 2015. doi:10.1007/s11434-014-0699-2) was not validated against observations, relying instead on synthetic test data based on underestimated global warming, illogical parameter choice and near-instantaneous response at odds with ocean warming and other observations. However, the simple model, informed by its authors' choice of parameters, usually hindcasts observed temperature change more closely than the general-circulation models, and finds high climate sensitivity implausible. With IPCC's choice of parameters, the model is further validated in that it duly replicates IPCC's sensitivity interval. Also, fast climate system response is consistent with near-zero or net-negative temperature feedback.Given the large uncertainties in the initial conditions and evolutionary processes determinative of climate sensitivity,subject to obvious caveats a simple sensitivity-focusedmodel need not, and the present model does not, exhibit significantly less predictive skill than the general-circulation models.Christopher Monckton of Brenchley Willie W.-H. 8Generation of Rate-of-Injection (ROI) profile for Computational Fluid Dynamics (CFD) model of Internal Combustion Engine (ICE) using machine learning显示文摘Injector configuration and spray characteristics are important parameters that define diesel combustion and emissions performance. One of the critical spray inputs is the Rate-of-Injection (ROI) profile. The ROI profile depends on the spray’s operating conditions, including nozzle geometry (e.g., nozzle diameter), injection pressure, and injection duration. Besides, the internal nozzle flow phenomenon and external ambient conditions can further impact fuel introduction characteristics. This study measured the ROI profile of a heavy-duty (multi-hole) diesel injector using the Bosch tube technique. Injection pressure and injection duration were varied from 600 to 2600 bar and 0.5–3.0 ms, respectively. After post-processing, measurement data were then used to train numerical models, including a developed machine learning (ML) model that can create very similar ROI profiles with experimental data. Next, a Computational Fluid Dynamics (CFD) simulation used the ROI profile generated by ML model. For comparison, there are other simplified ROI profiles used in similar CFD simulation configuration. Results showed that the any difference in ROI profiles could affect the combustion and emissions significantly. This further emphasizes the need to provide high-fidelity spray input in terms of ROI profile for CFD simulation.
