1Pediatricians lack knowledge for the diagnosis and management of functional constipation in children over 6 mo of age显示文摘AIM To assess the knowledge of general pediatricians througout Indonesia about the diagnosis and treatment of childhood constipation.METHODS A comprehensive questionnaire was distributed to general pediatricians from several teaching hospitals and government hospitals all over Indonesia.RESULTS Data were obtained from 100 pediatricians, with a mean of 78.34 ± 18.00 mo clinical practice, from 20 cities throughout Indonesia. Suspicion of constipation in a child over 6 mo of age arises when the child presents with a decreased frequency of bowel movements(according to 87% of participants) with a mean of one bowel movement per 3.59 ± 1.0 d, hard stools(83%), blood in the stools(36%), fecal incontinence(33%), and/or difficulty in defecating(47%). Only 26 pediatricians prescribe pharmacologic treatment as first therapeutic approach, while the vast majority prefers nonpharmacologic treatment, mostly(according to 68%) The preferred nonpharmacologic treatment are high-fiber diet(96%), increased fluid intake(90%), toilet training(74%), and abdominal massage(49%). Duration of non-pharmacological treatment was limited to 1 to 2 wk. Seventy percent of the pediatricians recommending toilet training could only mention some elements of the technique, and only 15% was able to explain it fully and correctly. Lactulose is the most frequent pharmacologic intervention used(87% of the participants), and rectal treatment with sodium citrate, sodium lauryl sulfo acetate, and sorbitol is the most frequent rectal treatment(85%). Only 51% will prescribe rectal treatment for fecal impaction. The majority of the pediatricians(69%) expect a positive response during the first week with a mean(± SD) of 4.1(± 2.56) d. Most participants(86%) treat during one month or even less. And the majority(67%) stops treatment when the frequency and/or consistency of the stools have become normal, or if the patient had no longer complaints.CONCLUSION These data provide an insight on the diagnosis and management of constipation in childhood in Indonesia. Although general pediatricians are aware of some important aspects of the diagnosis and mangement of constipation, overall knowledge is limited. Efforts should be made to improve the distribution of existing guidelines. 4Investigation of regurgitation and other symptoms of gastroesophageal reflux in Indonesian infants显示文摘AIM: To evaluate the incidence of regurgitation and other symptoms of gastroesophageal reflux in Indonesian infants. METHODS: In a cross-sectional study at the University Outpatient Clinic for vaccination in Jakarta, 138 mothers of healthy infants less than 12-mo old were prospectively asked to report the frequency of regurgitation. RESULTS: Whatever the age was, some infants did not regurgitate (from 10% during the first month of life to 67% in 1-year-old infants). Regurgitation of at least once a day was reported in 77% of infants younger than 3 mo. Daily regurgitation decreased to 12% in the 9-12 mo old group. Reported peak prevalence was 81%(26/32) during the firs tmonth of life. Regurgitation decreased sharply between the 4-6 and 7-9 mo old groups (from 44% to 9%). The longer the regurgitation persisted, the more frequently the mother perceived regurgitation as a problem. Volume and frequency of regurgitation, back arching, irritability, crying and refusal of feeding were the symptoms causing maternal anxiety. The longer the regurgitation persisted, the more frequently the mothers viewed it as a health problem. 5印度尼西亚儿童牛奶摄入与乳糖吸收不良之间的关系显示文摘目的探讨3~12岁印度尼西亚儿童牛奶摄入与乳糖吸收不良之间的关系。方法该研究为横断面研究。研究对象为在印度尼西亚雅加达中部随机选取的营养状况良好的174名3~12岁健康儿童,包括72名3~5岁儿童和102名6~12岁儿童。结果3~5岁儿童乳糖吸收不良患病率为20.8%(15/72),6~12岁儿童乳糖吸收不良患病率为35.3%(36/102)。牛奶或乳制品摄入与乳糖吸收不良之间无显著关联性(P>0.05)。在氢呼气试验中,51例乳糖吸收不良儿童主要表现为腹泻(62%)、腹痛(52%)和恶心(5%)。结论3~12岁印度尼西亚儿童牛奶摄入和乳糖吸收不良无关联,这提示遗传易感性可能比乳糖摄入的适应机制更重要。Syarimonitha Munadzilah Badriul Hegar Rini Sekartini Yvan Vandenplas 2021中国当代儿科杂志2021,23,10:0
