1Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases显示文摘AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary tumors.METHODS This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the periods.CONCLUSION Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. 2基于区间多阶段随机规划模型的灌区多水源优化配置显示文摘灌区多水源灌溉系统中存在许多不确定性因素,随着系统环境的变化及不确定性因素的影响,导致其配水过程具有动态特征。针对灌区多水源灌溉系统的配水特点,该文建立基于区间多阶段随机规划的灌区多水源优化配置模型。同时,考虑灌溉水对作物产量的影响,引入水分敏感指数权重系数,并以黑龙江省和平灌区水稻不同生育阶段灌溉水资源优化配置进行实例研究。结果表明,在不同来水情境下,管理者可根据各个生育阶段水分敏感指数权重系数,判断作物不同生育阶段的需水敏感程度,当来水情境的来水量多时,会产生余水量,可调配给下一生育阶段;当来水情境的来水量少时,管理者可在减少灌溉水量与增加外调水之间进行权衡,并根据需水关键期与需水非关键期做出决策,使水资源在作物生育阶段间及作物生育阶段内进行分配,实现灌区多水源灌溉系统的动态配水。该模型的应用在确保作物产量的同时,使灌溉水资源在作物各个生育阶段进行合理配置,有效地避免了水资源浪费,对提高灌溉水利用效率、保证水资源的可持续利用具有重要意义。付强 刘银凤 刘东 李天霄 刘巍 Amgad Osman 2016农业工程学报2016,32,1:14
3Clinical correlates of common corneal neovascular diseases: a literature review显示文摘A large subset of corneal pathologies involves the formation of new vessels(neovascularization), leading to compromised visual acuity. This article aims to review the clinical causes and presentations of corneal neovascularization(CNV) by examining the mechanisms behind common CNV-related corneal pathologies, with a particular focus on herpes simplex stromal keratitis,contact lenses-induced keratitis and CNV secondary to keratoplasty. 4Differences in neuroplasticity after spinal cord injury in varying animal models and humans显示文摘Rats have been the primary model to study the process and underlying mechanisms of recovery after spinal cord injury. Two weeks after a severe spinal cord contusion, rats can regain weight-bearing abilities without therapeutic interventions, as assessed by the Basso, Beattie and Bresnahan locomotor scale. However, many human patients suffer from permanent loss of motor function following spinal cord injury. While rats are the most understood animal model, major differences in sensorimotor pathways between quadrupeds and bipeds need to be considered. Understanding the major differences between the sensorimotor pathways of rats, non-human primates, and humans is a start to improving targets for treatments of human spinal cord injury. This review will discuss the neuroplasticity of the brain and spinal cord after spinal cord injury in rats, non-human primates, and humans. 5Establishing proof of concept:Platelet-rich plasma and bone marrow aspirate concentrate may improve cartilage repair following surgical treatment for osteochondral lesions of the talus显示文摘Osteochondral lesions of the talus are common injuries in the athletic patient. They present a challenging clinical problem as cartilage has a poor potential for healing. Current surgical treatments consist of reparative(microfracture) or replacement(autologous osteochondral graft) strategies and demonstrate good clinical outcomes at the short and medium term follow-up. Radiological findings and second-look arthroscopy however, indicate possible poor cartilage repair with evidence of fibrous infill and fissuring of the regenerative tissue following microfracture. Longer-term follow-up echoes these findings as it demonstrates a decline in clinical outcome. The nature of the cartilage repair that occurs for an osteochondral graft to become integrated with the native surround tissue is also of concern. Studies have shown evidence of poor cartilage integration,with chondrocyte death at the periphery of the graft, possibly causing cyst formation due to synovial fluid ingress. Biological adjuncts, in the form of platelet-rich plasma(PRP) and bone marrow aspirate concentrate(BMAC), have been investigated with regard to their potential in improving cartilage repair in both in vitro and in vitro settings. The in vitro literature indicates that these biological adjuncts may increase chondrocyte proliferation as well as synthetic capability, while limiting the catabolic effects of an inflammatory joint environment. These findings have been extrapolated to in vitro animal models, with results showing that both PRP and BMAC improve cartilage repair. 6Development of an Arabic version of the National Eye Institute Visual Function Questionnaire as a tool to study eye diseases patients in Egypt显示文摘AIM:To develop and test an Arabic version of the National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).METHODS:NEI-VFQ-25 was translated into Arabic according to WHO translation guidelines. We enrolled adult consenting patients with bilateral chronic eye diseases who presented to 14 hospitals across Egypt from October to December 2012, and documented their clinical findings. Psychometric properties were then tested using STATA.RESULTS:We recruited 379 patients, whose mean age was(54.5±15)y. Of 46.2% were males, 227 had cataract,31 had glaucoma, 23 had retinal detachment, 37 had diabetic retinopathy, and 61 had miscellaneous visual defects. Non-response rate and the floor and ceiling numbers of the Arabic version(ARB-VFQ-25) were calculated. Internal consistency was high in all subscales(except general health), with Cronbach-α ranging from0.702-0.911. Test-retest reliability was high(intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. However, some questions had high non-response rates and should be substituted by available alternatives. 7Complex ankle arthrodesis: Review of the literature显示文摘Complex ankle arthrodesis is defined as an ankle fusion that is at high risk of delayed and nonunion secondary to patient comorbidities and/or local ankle/hindfoot factors. Risk factors that contribute to defining this group of patients can be divided into systemic factors and local factors pertaining to co-existing ankle or hindfoot pathology. Orthopaedic surgeons should be aware of these risk factors and their association with patients' outcomes after complex ankle fusions. Both external and internal fixations have demonstrated positive outcomes with regards to achieving stable fixation and minimizing infection. Recent innovations in the application of biophysical agents and devices have shown promising results as adjuncts for healing. Both osteoconductive and osteoinductive agents have been effectively utilized as biological adjuncts for bone healing with low complication rates. Devices such as pulsed electromagnetic field bone stimulators, internal direct current stimulators and low-intensity pulsed ultrasound bone stimulators have been associated with faster bone healing and improved outcomes scores when compared with controls. 8Platelet-rich plasma increases transforming growth factor-beta1 expression at graft-host interface following autologous osteochondral transplantation in a rabbit model显示文摘AIM: To explore the effect of platelet-rich plasma on protein expression patterns of transforming growth factor-beta1(TGF-β1) in cartilage following autologous osteochondral transplantation(AOT) in a rabbit knee cartilage defect model.METHODS: Twelve New Zealand white rabbits received bilateral AOT. In each rabbit, one knee was randomized to receive an autologous platelet rich plasma(PRP) injection and the contralateral knee received saline injection. Rabbits were euthanized at 3, 6 and 12 wk post-operatively. Articular cartilage sections were stained with TGF-β1 antibody. Histological regions of interest(ROI)(left, right and center of the autologous grafts interfaces) were evaluated using Meta Morph. Percentage of chondrocytes positive for TGF-β1 was then assessed.RESULTS: Percentage of chondrocytes positive for TGF-β1 was higher in PRP treated knees for selected ROIs(left; P = 0.03, center; P = 0.05) compared to control and was also higher in the PRP group at each post-operative time point(P = 6.6 × 10^(-4), 3.1 × 10^(-4) and 7.3 × 10^(-3) for 3, 6 and 12 wk, respectively). TGF-β1 expression was higher in chondrocytes of PRP-treated knees(36% ± 29% vs 15% ± 18%)(P = 1.8 × 10^(-6)) overall for each post-operative time point and ROI. CONCLUSION: Articular cartilage of rabbits treated with AOT and PRP exhibit increased TGF-β1 expression compared to those treated with AOT and saline. 9Molecular underpinnings of corneal angiogenesis:advances over the past decade显示文摘The cornea is maintained in an avascular state by maintaining an environment whereby anti-angiogenic factors take the upper hand over factors promoting angiogenesis. Many of the common pathologies affecting the cornea involve the disruption of such equilibrium and the shift towards new vessel formation, leading to corneal opacity and eventually-vision loss. Therefore it is of paramount importance that the molecular underpinnings of corneal neovascularization(CNV) be clearly understood, in order to develop better targeted treatments. This article is a review of the literature on the recent discoveries regarding pro-angiogenic factors of the cornea(such as vascular endothelial growth factors,fibroblast growth factor and matrix metalloproteinases)and anti-angiogenic factors of the cornea(such as endostatins and neostatins). 10Host immune cellular reactions in corneal neovascularization显示文摘Corneal neovascularization(CNV) is a global important cause of visual impairment. The immune mechanisms leading to corneal heme- and lymphangiogenesis have been extensively studied over the past years as more attempts were made to develop better prophylactic and therapeutic measures. This article aims to discuss immune cells of particular relevance to CNV, with a focus on macrophages, Th17 cells, dendritic cells and the underlying immunology of common pathologies involving neovascularization of the cornea. 11沙库巴曲/缬沙坦治疗扩张型心肌病合并心力衰竭一例显示文摘血管紧张素受体-脑啡肽酶抑制剂(ARNI)沙库巴曲/缬沙坦对扩张型心肌病(DCM)合并射血分数减低心力衰竭(HFrEF)及复杂心律失常治疗效果仍不明确。该文报道了1例52岁男性,因发现心率慢8年余,胸闷头晕3个月入院,诊断为DCM、心力衰竭、室性心动过速、心房颤动、心房扑动。行植入型体内自动除颤器植入术,术后予β受体拮抗剂、ACEI、醛固酮拮抗剂、利尿剂等药物治疗,效果不佳,后更换ACEI为ARNI(沙库巴曲/缬沙坦)治疗3个月,起始剂量为100 mg,每日2次,服药3个月后患者LVEF从42%升至54%,N-端脑钠肽前体降低,6 min步行距离增加,室性期前收缩及室性心动过速减少,说明起始剂量100 mg的沙库巴曲/缬沙坦治疗DCM合并HFrEF及复杂心律失常安全,效果或优于ACEI类药物。Khaled Abdulbaqi Baggash Nasr Amgad Qaid Abdu Al-gaobahi 朱世杰 赵海玉 彭健 2020新医学2020,51,2:5

14Numerical simulation and analysis of confined turbulent buoyant jet with variable source显示文摘In this work, experimental and numerical investigations are undertaken for confined buoyant turbulent jet with varying inlet temperatures. Results of the experimental work and numerical simulations for the problem under consideration are presented. Four cases of different variable inlet temperatures and different flow rates are considered. The realizable k-ε turbulence model is used to model the turbulent flow. Comparisons show good agreements between simulated and measured results. The average deviation of the simulated temperature by realizable k-ε turbulent model and the measured temperature is within 2%. The results indicate that temperatures along the vertical axis vary, generally, in nonlinear fashion as opposed to the approximately linear variation that was observed for the constant inlet temperature that was done in a previous work. Furthermore, thermal stratification exits, particularly closer to the entrance region. Further away from the entrance region the variation in temperatures becomes relatively smaller. The stratification is observed since the start of the experiment and continues during the whole course. Numerical experiments for constant, monotone increasing and monotone decreasing of inlet temperature are done to show its effect on the buoyancy force in terms of Richardson number.EL-AMIN Mohamed F.
