| 1 | Focus on emerging drugs for the treatment of patients with non-alcoholic fatty liver disease显示文摘Non-alcoholic fatty liver disease(NAFLD)has become the most common liver disorder in Western countries and is increasingly being recognized in developing nations.Fatty liver disease encompasses a spectrum of hepatic pathology,ranging from simple steatosis to non-alcoholic steatohepatitis,cirrhosis,hepatocellular carcinoma and end-stage liver disease.Moreover,NAFLD is often associated with other metabolic conditions,such as diabetes mellitus type 2,dyslipidemia and visceral obesity.The most recent guidelines suggest the management and treatment of patients with NAFLD considering both the liver disease and the associated metabolic co-morbidities.Diet and physical exercise are considered the first line of treatment for patients with NAFLD,but their results on therapeutic efficacy are often contrasting.Behavior therapy is necessary most of the time to achieve a sufficient result.Pharmacological therapy includes a wide variety of classes of molecules with different therapeutic targets and,often,little evidence supporting the real efficacy.Despite the abundance of clinical trials,NAFLD therapy remains a challenge for the scientific community,and there are no licensed therapies for NAFLD.Urgently,new pharmacological approaches are needed.Here,we will focus on the challenges facing actual therapeutic strategies and the most recent investigated molecules. | Alessandro Federico Claudio Zulli Ilario de Sio Anna Del Prete Marcello Dallio Mario Masarone Carmela Loguercio | 2014 | World Journal of Gastroenterology2014,20,45: | 10 |

| 3 | Liver failure in an obese middle-aged woman after biliointestinal bypass显示文摘Obesity is considered an emerging epidemic that is often associated with non-alcoholic fatty liver disease. Among the therapeutic options for morbid obesity, bariatric surgery plays an important role when conventional therapies fail. The effects of bariatric surgery on liver function and morphology are controversial in the literature. Liver failure has been reported after jejunoileal bypass(JIB), biliopancreatic diversion and gastric bypass. Biliointestinal bypass(BIB) is considered an effective procedure among recently introduced bariatric surgery techniques. It is a clinically safe, purely malabsorptive operation in which the blind intestinal loop of the JIB is anastomosed to the gallbladder, allowing a portion of bile to transit into excluded intestinal tract. BIB is the only procedure, to our knowledge, to have no liver side effects reported in the literature. We report the case of a young obese woman who developed liver failure 8 mo after BIB. She had a rapid weight loss(70 kg) with a reduction in body mass index of 41% from January to September 2012. Because of a severe hepatic decompensation, she was referred to a transplantation centre. | 6 | Systemic treatment of advanced, metastatic, medullary thyroid carcinoma显示文摘Medullary thyroid carcinoma(MTC)is a rare endocrine tumor,which arises from thyroid parafollicular C cells.Through its ability to metastasize by blood and lymphatic vessels,it can show a more aggressive clinical behavior than differentiated thyroid cancers.Mutation of RET gene is the main molecular alteration involved in MTC origin.In the case of germline RET mutation,MTC can be inherited in an autosomal dominant way and show three different phenotypes:familial medullary thyroid carcinoma and multiple endocrine neoplasia types IIA and IIB.In addition,in sporadic cases,somatic RET mutation remains the key molecular alteration in most of cases.Total thyroidectomy with prophylactic or therapeutic central compartment lymph nodes dissection is the surgical treatment of choice.Further surgical treatments and local therapies should be used in the case of single or few local or distant metastasis.However,in cases with large metastatic spread of the disease,particularly in those with significant tumor progression,additional systemic treatments are needed.In this review,we discuss the key points of systemic treatment in advanced,metastatic MTC.We provide an update on the main aspects(from biological rationale to clinical experience)of each treatment,focusing our attention on the drugs used in clinical practice in the last years.Finally,we give insights about the emerging treatments from highly selective RET inhibitors to new radionuclide therapy. | Antonio Matrone Carla Gambale Alessandro Prete Virginia Cappagli Loredana Lorusso Valeria Bottici Rossella Elisei | 2021 | Journal of Cancer Metastasis and Treatment2021,7,1: | 0 |
| 7 | Unilateral post-chemotherapy robot-assisted retroperitoneal lymph node dissection in Stage II non-seminomatous germ cell tumor:A tertiary care experience显示文摘Objective Post-chemotherapy retroperitoneal lymph node dissection(PC-RPLND)represents an integral component of the management of patients with non-seminomatous germ cell tumor(NSGCT).Modified templates have been proposed to minimize the surgical morbidity of the procedure.Moreover,the implementation of robotic surgery in this setting has been explored.We report our experience with unilateral post-chemotherapy robot-assisted retroperitoneal lymph node dissection(PC-rRPLND)for clinical Stages IIA and IIB NSGCTs.Methods A retrospective single institution review was performed including 33 patients undergoing PC-rRPLND for Stages IIA and IIB NSGCTs between January 2015 and February 2019.Following orchiectomy,patients were scheduled for chemotherapy with three cycles of bleomycin-etoposide-cisplatin.Patients with a residual tumor of<5 cm and an ipsilateral metastatic disease on pre-and post-chemotherapy CT scans were eligible for a unilateral template in absence of rising tumor markers.Descriptive statistics were provided for demographics,clinical characteristics,intraoperative and postoperative parameters.Perioperative,oncological,and functional outcomes were recorded.Results Overall,7(21.2%)patients exhibited necrosis or fibrosis;14(42.4%)had mature teratoma;and 12(36.4%)had viable tumor at final histology.The median lymph node size at surgery was 25(interquartile range[IQR]21-36)mm.Median operative time was 180(IQR 165-215)min and no major postoperative complications were observed.Anterograde ejaculation was preserved in 75.8%of patients.Median follow-up was 26(IQR 19-30)months and a total of three recurrences were recorded.Conclusion PC-rRPLND is a reliable and technically reproducible procedure with safe oncological outcomes and acceptable postoperative ejaculatory function in well selected patients with NSGCTs. | Dario Franzese Antonio Tufano Alessandro Izzo Raffaele Muscariello Giovanni Grimaldi Giuseppe Quarto Luigi Castaldo Sabrina Rossetti Savio Domenico Pandolfo Sonia Desicato Paola Del Prete Matteo Ferro Sandro Pignata Sisto Perdonà | 2023 | Asian Journal of Urology2023,10,4: | 0 |
