3Intraductal neoplasm of the intrahepatic bile duct: Clinicopathological study of 24 cases显示文摘AIM: To investigate the clinicopathological features of intraductal neoplasm of the intrahepatic bile duct (INihB). METHODS: Clinicopathological features of 24 cases of INihB, which were previously diagnosed as biliary papillomatosis or intraductal growth of intrahepatic biliary neoplasm, were reviewed. Mucin immunohistochemistry was performed for mucin (MUC)1, MUC2, MUC5AC and MUC6. Ki-67, P53 and β-catenin immunoreactivity were also examined. We categorized each tumor as adenoma (low grade), borderline (intermediate grade), and malignant (carcinoma in situ , high grade including tumors with microinvasion). RESULTS: Among 24 cases of INihB, we identified 24 tumors. Twenty of 24 tumors (83%) were composed of a papillary structure; the same feature observed in intraductal papillary neoplasm of the bile duct (IPNB). In contrast, the remaining four tumors (17%) showed both tubular and papillary structures. In three of the four tumors (75%), macroscopic mucin secretion was limited but microscopic intracellular mucin was evident. Histologically, 16 tumors (67%) were malignant, three (12%) were borderline, and five (21%) were adenoma. Microinvasion was found in four cases (17%). Immunohistochemical analysis revealed that MUC1 was not expressed in the borderline/adenoma group but was expressed only in malignant lesions (P = 0.0095). Ki-67 labeling index (LI) was significantly higher in the malignant group than in the borderline/adenoma group (22.2 ± 15.5 vs 7.5 ± 6.3, P < 0.01). In the 16 malignant cases, expression of MUC5AC showed borderline significant association with high Ki-67 LI (P = 0.0622). Nuclear expression of β-catenin was observed in two (8%) of the 24 tumors, and these two tumors also showed MUC1 expression. P53 was negative in all tumors. CONCLUSION: Some cases of INihB have a tubular structure, and are subcategorized as IPNB with tubular structure. 4Peripheral portal vein-oriented non-dilated bile duct puncture for percutaneous transhepatic biliary drainage显示文摘AIM: To evaluate the efficacy of peripheral portal vein(PV)-oriented non-dilated bile duct(BD) puncture for percutaneous transhepatic biliary drainage(PTBD).METHODS: Thirty-five patients with non-dilated BDs underwent PTBD for the management of various biliary disorders, including benign bilioenteric anastomotic stricture(n = 24), BD stricture(n = 5) associated with iatrogenic BD injury, and postoperative biliary leakage(n = 6). Under ultrasonographic guidance, percutaneous transhepatic puncture using a 21-G needle was performed along the running course of the peripheral targeted non-dilated BD(preferably B6 for right-sided approach, and B3 for left-sided approach) or along the accompanying PV when the BD was not well visualized. This technique could provide an appropriate insertion angle of less than 30° between the puncture needle and BD running course. The puncture needle was then advanced slightly beyond the accompanying PV. The needle tip was moved slightly backward while injecting a small amount of contrast agent to obtain the BD image, followed by insertion of a 0.018-inch guide wire(GW). A drainage catheter was then placed usinga two-step GW method. RESULTS: PTBD was successful in 33(94.3%) of the 35 patients with non-dilated intrahepatic BDs. A rightsided approach was performed in 25 cases, while a left-sided approach was performed in 10 cases. In 31 patients, the first PTBD attempt proved successful. Four cases required a second attempt a few days later to place a drainage catheter. PTBD was successful in two cases, but the second attempt also failed in the other two cases, probably due to poor breath-holding ability. Although most patients(n = 26) had been experiencing cholangitis with fever(including septic condition in 8 cases) before PTBD, only 5(14.3%) patients encountered PTBD procedure-related complications, such as transient hemobilia and cholangitis. No major complications such as bilioarterial fistula or portal thrombosis were observed. 6Shared and distinct roles of T peripheral helper and T follicular helper cells in human diseases显示文摘The interactions of CD4^(+)T cells and B cells are fundamental for the generation of protective antibody responses,as well as for the development of harmful autoimmune diseases.Recent studies of human tissues and blood samples have established a new subset of CD4^(+)B helper T cells named peripheral helper T(Tph)cells.Unlike T follicular helper(Tfh)cells,which interact with B cells within lymphoid organs,Tph cells provide help to B cells within inflamed tissues.Tph cells share many B helper-associated functions with Tfh cells and induce B cell differentiation toward antibody-producing cells.The differentiation mechanism is also partly shared between Tph and Tfh cells in humans,and both Tfh and Tph cells can be found within the same tissues,including cancer tissues.However,Tph cells display features distinct from those of Tfh cells,such as the expression of chemokine receptors associated with Tph cell localization within inflamed tissues and a low Bd-6/Blimp1 ratio.Unlike that of Tfh cells,current evidence shows that the target of Tph cells is limited to memory B cells.In this review,we first summarize recent findings on human Tph cells and discuss how Tph and Tfh cells play shared and distinct roles in human diseases.Hiroyuki Yoshitomi Hideki Ueno 2021Cellular & Molecular Immunology2021,18,3:7
7Tangnaikang improves insulin resistance and β-cell apoptosis by ameliorating metabolic inflammation in SHR.Cg-Lepr^(cp)/NDmcr rats显示文摘OBJECTIVE: To investigate the effects of Tangnaikang(TNK), a mixture of five herbal plant extracts,on inflammation-mediated insulin resistance andβ-cell apoptosis in SHR.Cg-Leprcp/NDmcr(SHR-cp)rats.METHODS: Seven-week-old SHR-cp rats were randomly divided into a control(CON) group and a TNK(3.24 g/kg) group. Wistar-Kyoto rats at the same age were used as the normal control group.After 7 weeks of continuous intragastric administration of TNK, the glucose metabolic status and insulin sensitivity of the rats were evaluated by assessing fasting serum glucose(FBG), the oral glucose tolerance test(OGTT), fasting serum insulin(FINS),and the insulin sensitivity index(ISI). Serum tumor necrosis factor-α(TNF-α), interleukin-6(IL-6), C-reactive protein(CRP) and adiponectin were measured via enzyme-linked immunosorbent assays.Macrophage infiltration and apoptosis in adipose tissues were detected through F4/80 immunohistochemistry and the terminal deoxynucleotidyl transferase d UTP nick end labeling(TUNEL) assay. Islet morphology and β-cell apoptosis were investigated using double immunofluorescence staining and the TUNEL assay. The expression of cytokine genes in adipose tissue, the liver, and the pancreas was detected in real-time polymerase chain reaction assays. The expression and phosphorylation levels of insulin signaling-, inflammation-, and β-cell survival-related proteins in the liver and pancreas of SHR-cp rats were detected by western blotting.RESULTS: TNK(3.24 g/kg) treatment significantly decreased body weight, FBG and FINS; improved impaired glucose tolerance; increased the ISI; reduced serum levels of TNF-α, CRP and IL-6; and increased serum adiponectin. The m RNA expression of inflammatory markers was markedly reduced in the liver, pancreas, and adipose tissue. F4/80-and TUNEL-positive cells in adipose tissues were decreased, as was the number of TUNEL-positiveβ-cells. 10Trans-cinnamaldehyde promotes nitric oxide release via the protein kinase-B/v-Akt murine thymoma viral oncogene-endothelial nitric oxide synthase pathway to alleviate hypertension in SHR.Cg-Lepr^(cp)/NDmcr rats显示文摘OBJECTIVE:To evaluate whether endothelial dysfunction and hypertension are prevented by trans-cinnamaldehyde(t CA) through the activation of endothelial nitric oxide synthase(e NOS).METHODS:Human umbilical vein endothelial cells(HUVECs) were cultured in vitro and stimulated with t CA to determine cell viability using the methyl thiazolyl tetrazolium assay.The effect of t CA on nitric oxide(NO) production was determined by diaminofluorescein-dyes in the absence or presence of inhibitors of e NOS,AMPK,PKA,and AKT.The effect of t CA on blood pressure was determined by the tail-cuff method in obesity spontaneous hypertension(SHR.Cg-Leprcp/NDmcr) rats.The phosphorylation of e NOS and protein expression of the insulin-signaling pathway(Ins R-IRS1-PI3 K-AKT)were measured by western blot.RESULTS:t CA at concentrations less than 100 μM did not affect cell viability in cultured HUVECs.Stimulation with t CA promoted NO release in a time-dependent manner compared with the control group.t CA-treated HUVECs also significantly increased AKT-Ser473 and e NOS-Ser1177 phosphorylation.In SHR-CP rats,treatment with t CA at a dose of 40 mg/kg/day for 6 weeks markedly reduced the systolic blood pressure and diastolic blood pressure,increased the phosphorylation of AKT and e NOS,and increased urinary nitric oxidation.CONCLUSION:t CA attenuated endothelial dysfunction and reduced blood pressure in SHR-CP rats.The underlying mechanisms may involve the increase in AKT and e NOS phosphorylation and the release of e NOS-derived NO.Zhang Lu Wu Lili Liu Ximing Yoshitomi Hisae Ikeda Katsumi Negishi Hiroko Pan Yajing Sun Wen Qin Lingling Li Juan-E Xu Tunhai Liu Tonghua Gao Ming 2018Journal of Traditional Chinese Medicine2018,38,4:2
