| 1 | Adjuvant therapy sparing in rectal cancer achieving complete response after chemoradiation显示文摘AIM:To evaluate the long-term results of conventional chemoradiotherapy and laparoscopic mesorectal excision in rectal adenocarcinoma patients without adjuvant therapy.METHODS:Patients with biopsy-proven adenocarcinoma of the rectum staged cT3-T4 by endoscopic ultrasound or magnetic resonance imaging received neoadjuvant continuous infusion of 5-fluorouracil for five weeks and concomitant radiotherapy.Laparoscopic surgery was planned after 5-8 wk.Patients diagnosed with ypT0N0 stage cancer were not treated with adjuvant therapy according to the protocol.Patients with ypT1-2N0 or ypT3-4 or N+were offered 5-fluorouracil-based adjuvant treatment on an individual basis.An external cohort was used as a reference for the findings.RESULTS:One hundred and seventy six patients were treated with induction chemoradiotherapy and 170underwent total mesorectal excision.Cancer staging of ypT0N0 was achieved in 26/170(15.3%)patients.After a median follow-up of 58.3 mo,patients withypT0N0 had five-year disease-free and overall survival rates of 96%(95%CI:77-99)and 100%,respectively.We provide evidence about the natural history of patients with localized rectal cancer achieving a complete response after preoperative chemoradiation.The inherent good prognosis of these patients will have implications for clinical trial design and care of patients.CONCLUSION:Withholding adjuvant chemotherapy after complete response following standard neoadjuvant chemoradiotherapy and laparoscopic mesorectal excision might be safe within an experienced multidisciplinary team. | Xabier García-Albéniz Rosa Gallego Ralf Dieter Hofheinz Gloria Fernández-Esparrach Juan Ramón Ayuso-Colella Josep Antoni Bombí Carles Conill Miriam Cuatrecasas Salvadora Delgado Angels Ginés Rosa Miquel Mario Pagés Estela Pineda Verónica Pereira Aarón Sosa Oscar Reig Iván Victoria Luis Feliz Antonio María de Lacy Antoni Castells Iris Burkholder Andreas Hochhaus Joan Maurel | 2014 | World Journal of Gastroenterology2014，20，42： | 5 |
| 2 | Pyonephrosis as a sign of sarcomatoid carcinoma of the renal pelvis显示文摘We report the case of an urgent nephrectomy because of a pyonephrosis and sepsis due to an unsuspected sarcomatoid transitional cell carcinoma, an infrequent subtype with a bad oncological prognosis. We present a 58-year-old man assessed by internal medicine for a general syndrome and weakness many months previously. A pyonephrotic kidney was observed at abdominal computed tomography in the context of septic shock, without suspecting the underlying cause. The pathology report described a sarcomatoid transitional cell carcinoma. Sarcomatoid transitional cell carcinoma is an invasive and infrequent subtype of urothelial tumors. | 3 | Impact of global climate change on the health,welfare and productivity of intensively housed livestock显示文摘Major scientific studies have shown that global warming(i.e.increasing average temperature of the Earth)is now a reality.The aims of this paper are to broadly review the underlining causes of global warming,the general effects of global warming on social and environmental systems and the specific effects of resulting from global warming phenomena severe fluctuations in weather patterns,particularly heat waves on livestock health,welfare and productivity.Finally this article aims to summarise some common sense climate control methods and more importantly to highlight the required future research and development(R&D)work that is necessary to achieve a new level of building environment control capability,and thus ensure that the intensive livestock industries will be able to cope with the changed external climate.With the increasing temperatures on a global scale,the most direct effect of the high temperature on the animals is heat stress,which has been proven to have a variety of negative effects on animal health,welfare and productivity.Different potential measures could be taken in future to alleviate the increased heat stress.Some of these measures are mere adaptations or improvements of current engineering solutions.However,facing the complex challenges of global warming and particularly resulting from it the rapid increase of the number of consecutive days with significantly higher than average temperatures will probably require novel solutions,including new designs based on solid engineering judgment,development of new engineering standards and codes to guide designs,the exploration of new and superior building materials,the need for better energy management,and the development of substantially more“intelligent”control systems that will balance changing exterior disturbances,interior building loads and demands to the biological needs of the occupants of the structures. | Tadeusz Kuczynski Victoria Blanes-Vidal Baoming Li Richard S.Gates Irenilza de Alencar Nääs Daniella J.Moura Daniel Berckmans Thomas M.Banhazi | 2011 | International Journal of Agricultural and Biological Engineering2011，4，2： | 3 |
