4Copy number variations are progressively associated with the pathogenesis of colorectal cancer in ulcerative colitis显示文摘AIM: To evaluate the association of known copy number variations(CNVs) in ulcerative colitis(UC) progressing to colorectal cancer.METHODS: Microsatellite instability analysis using the National Cancer Institute's panel of markers, and CNV association studies using Agilent 2 × 105 k arrays were done in tissue samples from four patient groups with UC: those at low risk(LR) or high risk of developing colorectal cancer, those with premalignant dysplastic lesions, and those with colitis-associated colorectal cancer(CAC). DNA from tissue samples of these groups were independently hybridized on arrays and analyzed. The data obtained were further subjected to downstream bioinformatics enrichment analysis to examine the correlation with CAC progression.RESULTS: Microarray analysis highlighted a progressive increase in the total number of CNVs [LR(n = 178) vs CAC(n = 958), 5.3-fold], gains and losses [LR(n = 37 and 141) vs CAC(n = 495 and 463), 13.4- and 3.3-fold, respectively], size [LR(964.2 kb) vs CAC(10540 kb), 10.9-fold] and the number of genes in such regions [LR(n = 119) vs CAC(n = 455), 3.8-fold]. Chromosomewise analysis of CNVs also showed an increase in the number of CNVs across each chromosome. There were 38 genes common to all four groups in the study; 13 of these were common to cancer genes from the Genetic Disease Association dataset. The gene set enrichment analysis and ontology analysis highlighted many cancerassociated genes. 5Transcranial magnetic stimulation for geriatric depression: Promises and pitfalls显示文摘As the global population gets older, depression in the elderly is emerging as an important health issue. A major challenge in treating geriatric depression is the lack of robust efficacy for many treatments that are of significant benefit to depressed working age adults. Repetitive transcranial magnetic stimulation(r TMS) is a novel physical treatment approach used mostly in working age adults with depression. Many TMS trials and clinics continue to exclude the elderly from treatment citing lack of evidence in this age group. In this review, we appraise the evidence regarding the safety and efficacy of rT MS in the elderly. A consistent observation supporting a high degree of tolerability and safety among the elderly patients emerged across the Randomised Controlled Trials and the uncontrolled trials. Further, there is no reliable evidence negating the utility of rT MS in the elderly with depression. We also identified several factors other than age that moderate the observed variations in the efficacy of rT MS in the elderly. These factors include but not limited to:(1) brain atrophy;(2) intensity and number of pulses(dose-response relationship); and(3) clinical profile of patients. 7Role of nutrients in the utilization of polycyclic aromatic hydrocarbons by halotolerant bacterial strain显示文摘A halotolerant bacterial strain VA1 isolated from marine environment was studied for its ability to utilize polycylic aromatic hydrocarbons (PAHs) under saline condition.Anthracene and pyrene were used as representatives for the utilization of PAH by the bacterial strain.Glucose and sodium citrate were used as additional carbon sources to enhance the PAH utilization.The strain VA1 was able to utilize anthracene (73%) and pyrene (66%) without any additional substrate.In the presence of additional carbon sources (glucose/sodium citrate) the utilization of PAH was faster.PAH was utilized faster by VA1 in the presence of glucose than sodium citrate.The stain utilized 87% and 83% of anthracene and pyrene with glucose as carbon source and with sodium citrate the strain utilized 81% and 76% respectively in 4 days.Urea as an alternative source of nitrogen also enhanced the utilization of PAHs (anthracene and pyrene) by the bacterial strain up to 88% and 84% in 4 days.Sodium nitrate as nitrogen source was not able to enhance the PAH utilization rate.Phenotypic and phlyogenetic analysis proved that the PAHs utilizing halotolerant strain VA1 belongs to Ochrobactrum sp.Pugazhendi Arulazhagan Namsivayam Vasudevan 2011Journal of Environmental Sciences2011,23,2:6

10Time to clinical response and remission for therapeutics in inflammatory bowel diseases: what should the clinician expect, what should patients be told?显示文摘An awareness of the expected time for therapies to induce symptomatic improvement and remission is necessary for determining the timing of follow-up, disease(re)assessment, and the duration to persist with therapies, yet this is seldom reported as an outcome in clinical trials. In this review, we explore the time to clinical response and remission of current therapies for inflammatory bowel disease(IBD) as well as medication, patient and disease related factors that may influence the time to clinical response. It appears that the time to therapeutic response varies depending on the indication for therapy(Crohn's disease or ulcerative colitis). Agents with the most rapid time to clinical response included corticosteroids, calcineurin inhibitors, exclusive enteral nutrition, aminosalicylates and anti-tumor necrosis factor therapy which will work in most patients within the first 2 mo. Vedolizumab,methotrexate and thiopurines had a longer time to clinical response and can take several months to achieve maximal efficacy. Factors affecting the time to clinical response of therapies included use of concomitant therapy, disease duration, smoking status, disease phenotype and advanced age. There appears to be marked variation in time to clinical response for therapies used in IBD which is further influenced by disease and patient related factors. 11Addressing quality of life in the patient with interstitial cystitis/bladder pain syndrome显示文摘Interstitial cystitis/bladder pain syndrome(IC/BPS)is a debilitating,chronic condition characterized by chronic pelvic pain,urinary urgency,and frequency and is well-known to be associated with a decrease in work productivity,emotional changes,sleep,sexual dysfunction,and mobility.Many metrics of quality of life(QoL)in this patient population have been developed;however,a unified,standardized approach to QoL in these patients has not been determined.The effects of IC/BPS and co-morbid conditions on QoL are described using current validated metrics.Next,data regarding successful treatment of IC/BPS in terms of QoL improvement are reviewed.While QoL is the single most important clinical measure of success in the treatment of patients suffering from IC/BPS,addressing QoL in this patient population remains a significant challenge,as its effects on QoL are highly variable and unable to be differentiated from the effects of comorbid conditions on QoL,including depression,poor sleep,and inability to work.Future studies will need to address treatment efficacy on the basis of IC/BPS specific QoL metrics,and multi-modal assessment and therapy to address comorbid disease will also play an important role in the future to ensure comprehensive management of these patients.Vinaya Vasudevan Robert Moldwin 2017Asian Journal of Urology2017,4,1:4
12印度南部2型糖尿病患者糖尿病视网膜病变相关危险因素的研究(英文)显示文摘目的:探究2型糖尿病患者糖尿病视网膜病变(DR)的严重度及受试者中糖尿病视网膜病变严重程度与其病程、糖化血红蛋白水平、高血压史、年龄和性别的相关性。方法:横断面研究。2012-05/06,采取简单随机抽样方法进行结构化问卷调查,收集100例年龄≥35岁糖尿病视网膜病变患者。进行描述性统计、单变量和多变量有序逻辑回归分析。用excel表进行数据录入,并用SPSS 21.0进行数据分析(P<0.05)。结果:100例患者,平均年龄53.16±10.81(35~78)岁。通过单因素分析发现,糖尿病视网膜病变严重度与年龄(P<0.01),糖尿病病程(P<0.001),糖化血红蛋白水平(P<0.001),糖尿病家族史(P<0.01),高血压病史(P<0.05),均呈正相关,高密度脂蛋白(HDL)及年龄与DR严重程度无相关性。除年龄因素外,上述所有因素均为影响糖尿病视网膜病变严重度的独立风险因素。结论:研究表明,糖尿病病程、糖化血红蛋白水平、糖尿病家族史、高血压病史以及性别与糖尿病视网膜病变严重程度密切相关。但多因素分析中,年龄、HDL因素与DR严重程度无关。因此,利用现有治疗手段的有效性和及时控制,可以预防和消除视力威胁性疾病,延缓DR的发生。Senthilvel Vasudevan Sumathi Senthilvel Jayanthi Sureshbabu 2017国际眼科杂志2017,17,9:4

15A New Species of the Genus Tylototriton(Amphibia: Urodela: Salamandridae) from Eastern Himalaya显示文摘A new species of the genus Tylototriton is described from eastern Himalaya based on molecular and morphological comparisons. The new species is diagnosable from the closely-related species by having light brown colouration in dorsal region in life, flat and blunt snout, greatly separated dorsolateral bony ridges on head and straightthick tailfin. 17Occurrence of Clostridium perfringens contamination in poultry feed ingredients:Isolation, identification and its antibiotic sensitivity pattern显示文摘This work has been undertaken to study the occurrence of Clostridium perfringens contamination in the poultry feed ingredients and find out its in-vitro antibiotic sensitivity pattern to various antimicrobial drugs. Two hundred and ninety-eight poultry feed ingredient samples received at Poultry Disease Diagnosis and Surveillance Laboratory, Namakkal, Tamil Nadu in South India were screened for the presence of C. perfringens. The organisms were isolated in Perfringens agar under anaerobic condition and subjected to standard biochemical tests for confirmation. In vitro antibiogram assay has been carried out to determine the sensitivity pattern of the isolates to various antimicrobial drugs. One hundred and one isolates of C. perfringens were obtained from a total of 298 poultry feed ingredient samples. Overall positivity of 33.89% could be made from the poultry feed ingredients. Highest level of C. perfringens contamination was detected in fish meal followed by bone meal, meat and bone meal and dry fish.Antibiogram assay indicated that the organisms are highly sensitive to gentamicin(100%), chlortetracycline(96.67%), gatifloxacin(93.33%), ciprofloxacin(86.67%), ofloxacin(86.67%) and lincomycin(86.67%). All the isolates were resistant to penicillin-G. 18Deep learning vs conventional learning algorithms for clinical prediction in Crohn's disease: A proof-of-concept study显示文摘BACKGROUND Traditional methods of developing predictive models in inflammatory bowel diseases(IBD)rely on using statistical regression approaches to deriving clinical scores such as the Crohn's disease(CD)activity index.However,traditional approaches are unable to take advantage of more complex data structures such as repeated measurements.Deep learning methods have the potential ability to automatically find and learn complex,hidden relationships between predictive markers and outcomes,but their application to clinical prediction in CD and IBD has not been explored previously.AIM To determine and compare the utility of deep learning with conventional algorithms in predicting response to anti-tumor necrosis factor(anti-TNF)therapy in CD.METHODS This was a retrospective single-center cohort study of all CD patients who commenced anti-TNF therapy(either adalimumab or infliximab)from January 1,2010 to December 31,2015.Remission was defined as a C-reactive protein(CRP)<5 mg/L at 12 mo after anti-TNF commencement.Three supervised learning algorithms were compared:(1)A conventional statistical learning algorithm using multivariable logistic regression on baseline data only;(2)A deep learning algorithm using a feed-forward artificial neural network on baseline data only;and(3)A deep learning algorithm using a recurrent neural network on repeated data.Predictive performance was assessed using area under the receiver operator characteristic curve(AUC)after 10×repeated 5-fold cross-validation.RESULTS A total of 146 patients were included(median age 36 years,48%male).Concomitant therapy at anti-TNF commencement included thiopurines(68%),methotrexate(18%),corticosteroids(44%)and aminosalicylates(33%).After 12 mo,64%had CRP<5 mg/L.The conventional learning algorithm selected the following baseline variables for the predictive model:Complex disease behavior,albumin,monocytes,lymphocytes,mean corpuscular hemoglobin concentration and gamma-glutamyl transferase,and had a cross-validated AUC of 0.659,95%confidence interval(CI):0.562-0.756.A feed-forward artificial neural network using only baseline data demonstrated an AUC of 0.710(95%CI:0.622-0.799;P=0.25 vs conventional).A recurrent neural network using repeated biomarker measurements demonstrated significantly higher AUC compared to the conventional algorithm(0.754,95%CI:0.674-0.834;P=0.036).CONCLUSION Deep learning methods are feasible and have the potential for stronger predictive performance compared to conventional model building methods when applied to predicting remission after anti-TNF therapy in CD.Danny Con Daniel R van Langenberg Abhinav Vasudevan 2021World Journal of Gastroenterology2021,27,38:2

20Structural and luminescence enhancement properties of Eu^(3+)/Ag nanocrystallites doped SiO_2-TiO_2 matrices显示文摘We reported here the structural and optical characterisation of silver nanocrystallites/Eu3+ :SiO2-TiO2 matrices synthesised through sol-gel route. Structural characterisations were done by using energy dispersive spectroscopy (EDX), X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FTIR), atomic force microscope (AFM) and transmission electron microscopy (TEM) measurements and optical characterisations were performed by absorption and emission spectroscopy. The TEM and XRD measurements confirmed the presence of nanocrystals. A broad absorption band was observed due to surface plasmon resonance of silver nanocrystals. The effect of silver nanocrystals on the emission spectrum of Eu3+ doped SiO2-TiO2 matrices was discussed. An attempt was made to explain this fluorescence enhancement by invoking phenomena such as energy transfer, asymmetry ratio, surface plasmon, surface roughness, crystallinity and grain boundary. Our analysis, based on the experimental results, suggested that all the phenomena except crystallinity and grain boundary had favourable effects on fluorescence enhancement. We also estimated the relevant parameters associated with the phenomena that affected the fluorescence emission from the Eu3+ ions in the matrix.
