| 1 | Neonatal gastric perforation: A single center experience显示文摘AIM: To determine the etiology and prognostic factors for neonatal gastric perforation(NGP), a rare but life-threatening disease.METHODS: Between 1980 and 2011, nine patients un-derwent surgical intervention for NGP at Seoul National University Children's Hospital. The characteristics and prognosis of the patients were retrospectively analyzed.RESULTS: Among the nine patients, three(33.3%) were preterm babies and five(55.5%) had associated anomalies, which included diaphragmatic eventration(n = 2), congenital diaphragmatic hernia, esophageal atresia with tracheoesophageal fistula, and antral web. Three(33.3%) patients were born before 1990 and three(33.3%) had a birth weight < 2500 g. Pneumo-peritoneum was found on preoperative images in six(66.7%) patients, and incidentally in the other three(33.3%) patients. Surgery was performed within 24 h after the onset of symptoms in seven(77.8%) patients. The overall mortality rate was 22.2%(2/9). | 11 | Characteristics and outcomes of chronic liver disease patients with acute deteriorated liver function by severity of underlying liver disease显示文摘AIM: To analyze characteristics and outcome of patients with acute-on-chronic liver failure(ACLF) according to the severity of underlying liver disease. METHODS: One hundred and sixty-seven adult patients with chronic liver disease and acute deteriorated liver function, defined by jaundice and coagulopathy, were analyzed. Predisposition, type of injury, response, organ failure, and survival were analyzed and compared between patients with non-cirrhosis(type A), cirrhosis(type B) and cirrhosis with previous decompensation(type C).RESULTS: The predisposition was mostly hepatitis B in type A, while it was alcoholic liver disease in typesB and C. Injury was mostly hepatic in type A, but was non-hepatic in type C. Liver failure, defined by CLIF-SOFA, was more frequent in types A and B, and circulatory failure was more frequent in type C. The 30-d overall survival rate(85.3%, 81.1% and 83.7% for types A, B and C, respectively, P = 0.31) and the 30-d transplant-free survival rate(55.9%, 65.5% and 62.5% for types A, B and C, respectively P = 0.33) were not different by ACLF subtype, but 1-year overall survival rate were different(85.3%, 71.7% and 58.7% for types A, B and C, respectively, P = 0.02).CONCLUSION: There were clear differences in predisposition, type of injury, accompanying organ failure and long-term mortality according to spectrum of chronic liver disease, implying classifying subtype according to the severity of underlying liver disease is useful for defining, clarifying and comparing ACLF. | Yun Soo Hong Dong Hyun Sinn Geum-Youn Gwak Juhee Cho Danbee Kang Yong-Han Paik Moon Seok Choi Joon Hyeok Lee Kwang Cheol Koh Seung Woon Paik | 2016 | World Journal of Gastroenterology2016，22，14： | 1 |

| 16 | Effect of music on colonoscopy performance: A propensity scorematched analysis显示文摘BACKGROUND Music has been used to reduce stress and improve task performance during medical therapy.AIM To assess the effects of music on colonoscopy performance outcomes.METHODS We retrospectively reviewed patients who underwent colonoscopy performed by four endoscopists with popular music.Colonoscopy performance outcomes,such as insertion time,adenoma detection rate(ADR),and polyp detection rate(PDR),were compared between the music and non-music groups.To reduce selection bias,propensity score matching was used.RESULTS After one-to-one propensity score matching,169 colonoscopies were selected from each group.No significant differences in insertion time(4.97 vs 5.17 min,P=0.795)and ADR(39.1%vs 46.2%,P=0.226)were found between the two groups.Subgroup analysis showed that the insertion time(3.6 vs 3.8 min,P=0.852)and ADR(51.1%vs 44.7%,P=0.488)did not significantly differ between the two groups in experts.However,in trainees,PDR(46.9%vs 66.7%,P=0.016)and ADR(25.9%vs 47.6%,P=0.006)were significantly lower in the music than in the nonmusic group.CONCLUSION The current study found that listening to music during colonoscopy did not affect procedure performance.Moreover,it suggested that music may distract trainees from appropriately detecting adenomas and polyps. | Eun Jeong Choi Sam Ryong Jee Sang Heon Lee Jun Sik Yoon Seung Jung Yu Jong Hyun Lee Han Byul Lee Sang Wook Yi Myeong Pyo Kim Byung Cheol Chung Hong Sub Lee | 2023 | World Journal of Gastrointestinal Endoscopy2023，15，5： | 0 |
| 17 | Sprayable nanomicelle hydrogels and inflammatory bowel disease patient cell chips for development of intestinal lesion-specific therapy显示文摘All-in-one treatments represent a paradigm shift in future medicine.For example,inflammatory bowel disease(IBD)is mainly diagnosed by endoscopy,which could be applied for not only on-site monitoring but also the intestinal lesion-targeted spray of injectable hydrogels.Furthermore,molecular conjugation to the hydrogels would program both lesion-specific adhesion and drug-free therapy.This study validated this concept of all-in-one treatment by first utilizing a well-known injectable hydrogel that underwent efficient solution-to-gel transition and nanomicelle formation as a translatable component.These properties enabled spraying of the hydrogel onto the intestinal walls during endoscopy.Next,peptide conjugation to the hydrogel guided endoscopic monitoring of IBD progress upon adhesive gelation with subsequent moisturization of inflammatory lesions,specifically by nanomicelles.The peptide was designed to mimic the major component that mediates intestinal interaction with Bacillus subtilis flagellin during IBD initiation.Hence,the peptide-guided efficient adhesion of the hydrogel nanomicelles onto Toll-like receptor 5(TLR5)as the main target of flagellin binding and Notch-1.The peptide binding potently suppressed inflammatory signaling without drug loading,where TLR5 and Notch-1 operated collaboratively through downstream actions of tumor necrosis factor-alpha.The results were produced using a human colorectal cell line,clinical IBD patient cells,gut-on-a-chip,a mouse IBD model,and pig experiments to validate the translational utility. | Hyo-Jin Yoon Songhyun Lee Tae Young Kim Seung Eun Yu Hye-Seon Kim Young Shin Chung Seyong Chung Suji Park Yong Cheol Shin Eun Kyung Wang Jihye Noh Hyun Jung Kim Cheol Ryong Ku Hong Koh Chang-Soo Kim Joon-Sang Park Young Min Shin Hak-Joon Sung | 2022 | Bioactive Materials2022，7，12： | 0 |
