| 1 | Small sphincterotomy combined with endoscopic papillary large balloon dilation versus sphincterotomy显示文摘AIM:To compare small sphincterotomy combined with endoscopic papillary large balloon dilation(SES+ ELBD)and endoscopic sphincterotomy(EST)for large bile duct stones. METHODS:We compared prospectively SES+ELBD (group A,n=27)with conventional EST(group B, n=28)for the treatment of large bile duct stones(≥ 15 mm).When the stone could not be removed with a normal basket,mechanical lithotripsy was performed. We compared the rates of complete stone removal with one session and application of mechanical lithotripsy. | 2 | Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography显示文摘AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose. After adding the propofol, the procedure could be conducted successfully (one case in the BIS group, three cases in the control group). The endoscopist rated patient sedation as excellent for all patients in both groups. All patients in both groups rated their level of satisfaction as high (no discomfort). During the post-procedural follow-up in the recovery area, no cases of clinically significant hypoxic episodes were recorded in either group. No other postoperative side effects related to sedation were observed in either group. CONCLUSION:BIS monitoring trend to slighlty reduce the mean propofol dose. | 3 | Clinical application of traditional herbal medicine in five countries and regions: Japan;South Korea;China's Mainland;Hong Kong, China;Taiwan, China显示文摘Objective:To study the characteristics of the clinical application of traditional herbal medicine in five countries and regions where traditional medicine(TM)is widely used:Japan;South Korea;China's Mainland;Hong Kong,China;Taiwan,China.And to analyze,from multiple angles,the variations in the methods of TM used in each countries and regions.Methods:Fifty commonly used traditional herbs(TOP50)from classical formulae were selected,and descriptive statistics,correlation analyses,and Friedman test were used to compare methods of clinical application and dosages in the countries and regions of interest based on a retrospective review of randomly-selected medical records.Results:There were significant differences in sequencing TOP50 among the five countries and regions(P<.01).The adherence to the classical formulae,from strong to weak was Japan;China's Mainland;Taiwan,China;Hong Kong,China;South Korea.There were also significant differences in TOP50 dosing regimens(P<.01).Overall dosages of TOP50 from high to low were:China's Mainland;Hong Kong,China;Taiwan,China;South Korea;Japan.Finally,there were significant differences in the dosage intervals(P<.01),with dosage intervals from maximum to minimum as follows:China's Mainland;Taiwan,China;Hong Kong,China;South Korea;Japan.Conclusions:The clinical dosages of traditional herbal medicine used in the China's Mainland are relatively large compared with those prescribed in the other four countries and regions,and the range of dosages in China's Mainland is more flexible.On the other hand,clinical dosages of TOP50 are the lowest in Japan,where the dosage range is also the narrowest,supporting the idea that Doctors of Kampo medicine are highly cautious and are likely to adhere strictly to classical formulae,while Hong Kong,China and Taiwan,China are in the middle.The dosage of TOP50 in South Korea is significantly lower than that used in the China's Mainland;Hong Kong,China and Taiwan,China where the adherence to the classical formulae was the lowest,albeit slightly higher than that in Japan.The methods of clinical application of traditional herbal medicine in the five countries and regions were affected by multiple factors,including history,geography,climate,and culture. | Liming Chen Yanling Fu Lin Zhang Sijia Zhao Qing Feng Yihua Cheng Tosha Yanai Daji Xu Mingyu Luo Sang Woo An Woong Seok Lee Seong Hoon Cho Byung Wook Lee | 2015 | Journal of Traditional Chinese Medical Sciences2015,2,3: | 1 |

| 13 | Incidental focal colorectal ^(18)F-fluorodeoxyglucose uptake on positron emission tomography/computed tomography显示文摘AIM: To assess the clinical significance of incidental focal colorectal 18 F-fluorodeoxyglucose ( 18 F-FDG) uptake on 18 F-FDG-positron emission tomography/computed tomography (PET/CT). METHODS: The records of all the cases which had undergone colonoscopy after PET/CT within a two weeks interval were reviewed. Adenomas were considered advanced when they were villous, ≥ 10 mm in size, or had high-grade dysplasia. Colorectal cancers and advanced adenomas are collectively referred to as advanced colorectal neoplasms. Receiver-operating characteristic curve analysis was used to determine thesignificant predictive maximum standardized uptake value (SUVmax) cutoff value for advanced colorectal neoplasms and cancer. RESULTS: Ninety-five colorectal lesions matched the site of incidental focal colorectal 18 F-FDG uptake on PET/CT and 146 did not. Colonoscopy showed advanced colorectal neoplasms corresponding to the site of 18 F-FDG uptake in 49 of the 95 (51.5%) lesions with incidental uptake. Of the lesions without incidental uptake, only 6 of 146 (4.1%) had advanced colorectal neoplasms on colonoscopy, indicating a statistically significant difference between the two groups (P < 0.001). The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of incidental focal 18 F-FDG uptake in identifying advanced colorectal neoplasms were 89.1%, 75.3%, 51.6%, 95.9%, and 78.4%, respectively. In detecting only CRC, these values were 89.2%, 69.6%, 34.7%, 97.3%, and 72.6%, respectively. The significant SUVmax cutoff value for advanced colorectal neoplasms (area under the curve 0.755, P < 0.001) was 4.35, with a sensitivity, specificity, PPV, NPV, and accuracy of 75.5%, 65.2%, 69.8%, 71.4% and 70.5%, respectively. For CRC, 5.05 was the significant SUVmax cutoff value (area under the curve 0.817, P < 0.001), with a sensitivity, specificity, PPV, NPV, and accuracy of 84.8%, 71.0%, 80.9%, 89.8%, and 75.8%, respectively. The SIS mixers are used in both bands. The constructed dewar for receiver has the performance with the vacuum of 10 -8 Torr and the temperature of 4 2K. The frequency independent matching of Gaussian beam is applied to design the quasioptical system with two focusing elements for each band, respectively. The beam measurement system is also developed to measure the quasioptical system and their components as well as to align the quasioptical elements. We have measured the intensity distributions of the Gaussian beam in the transverse plane and the variations of the beam widths along the propagation axis near the Cassegrain focus.
