| 1 | Surgical outcome after docetaxel-based neoadjuvant chemotherapy in locally-advanced gastric cancer显示文摘AIM:To investigate feasibility,morbidity and surgical mortality of a docetaxel-based chemotherapy regimen randomly administered before or after gastrectomy in patients suffering from locally-advanced resectable gastric cancer.METHODS:Patients suffering from locally-advanced(T3-4 any N M0 or any T N1-3 M0)gastric carcinoma,staged with endoscopic ultrasound,bone scan,computed tomography,and laparoscopy,were assigned to receive four 21 d/cycles of TCF(docetaxel 75 mg/m 2 day 1,cisplatin 75 mg/m 2 day 1,and fluorouracil 300 mg/m 2 per day for days 1-14),either before(Arm A)or after(Arm B)gastrectomy.Operative morbidity,overall mortality,and severe adverse events were compared by intention-to-treat analysis.RESULTS:From November 1999 to November 2005,70 patients were treated.After preoperative TCF(Arm A),thirty-two(94%)resections were performed,85% of which were R0.Pathological response was complete in 4 patients(11.7%),and partial in 18(55%).No surgical mortality and 28.5%morbidity rate were observed,similar to those of immediate surgery arm(P= 0.86).Serious chemotherapy adverse events tended to be more frequent in arm B(23%vs 11%,P=0.07),with a single death per arm.CONCLUSION:Surgery following docetaxel-based chemotherapy was safe and with similar morbidity to immediate surgery in patients with locally-advanced resectable gastric carcinoma. | Roberto Biffi Nicola Fazio Fabrizio Luca Antonio Chiappa Bruno Andreoni Maria Giulia Zampino Arnaud Roth Jan Christian Schuller Giancarla Fiori Franco Orsi Guido Bonomo Cristiano Crosta Olivier Huber | 2010 | World Journal of Gastroenterology2010，16，7： | 43 |
| 2 | Resin infiltration of deproteinised natural occlusal subsurface lesions improves initial quality of fissure sealing显示文摘The aim of this ex vivo study was to evaluate the infiltration capability and rate of microleakage of a low-viscous resin infiltrant combined with a flowable composite resin(RI/CR) when used with deproteinised and etched occlusal subsurface lesions(International Caries Detection and Assessment System code 2). This combined treatment procedure was compared with the exclusive use of flowable composite resin(CR) for fissure sealing. Twenty premolars and 20 molars revealing non-cavitated occlusal carious lesions were randomly divided into two groups and were meticulously cleaned and deproteinised using Na OCl(2%). After etching with HCl(15%), 10 premolar and 10 molar lesions were infiltrated(Icon/DMG; rhodamine B isothiocyanate(RITC)-labelled) followed by fissure sealing(G-?nial Flo/GC; experimental group, RI/CR). In the control group(CR), the carious fissures were only sealed. Specimens were cut perpendicular to the occlusal surface and through the area of the highest demineralisation(DIAGNOdent pen, Ka Vo). Using confocal laser-scanning microscopy, the specimens were assessed with regard to the percentage of caries infiltration, marginal adaption and internal integrity. Within the CR group, the carious lesions were not infiltrated. Both premolar(57.9% ± 23.1%) and molar lesions(35.3% ± 22.1%) of the RI/CR group were uniformly infiltrated to a substantial extent, albeit with significant differences(P = 0.034). Moreover, microleakage(n = 1) and the occurrence of voids(n = 2) were reduced in the RI/CR group compared with the CR group(5 and 17 specimens,respectively). | 3 | Deep Scalogram Representations for Acoustic Scene Classification显示文摘Spectrogram representations of acoustic scenes have achieved competitive performance for acoustic scene classification. Yet, the spectrogram alone does not take into account a substantial amount of time-frequency information. In this study, we present an approach for exploring the benefits of deep scalogram representations, extracted in segments from an audio stream. The approach presented firstly transforms the segmented acoustic scenes into bump and morse scalograms, as well as spectrograms; secondly, the spectrograms or scalograms are sent into pre-trained convolutional neural networks; thirdly,the features extracted from a subsequent fully connected layer are fed into(bidirectional) gated recurrent neural networks, which are followed by a single highway layer and a softmax layer;finally, predictions from these three systems are fused by a margin sampling value strategy. We then evaluate the proposed approach using the acoustic scene classification data set of 2017 IEEE AASP Challenge on Detection and Classification of Acoustic Scenes and Events(DCASE). On the evaluation set, an accuracy of 64.0 % from bidirectional gated recurrent neural networks is obtained when fusing the spectrogram and the bump scalogram, which is an improvement on the 61.0 % baseline result provided by the DCASE 2017 organisers.
